한방병원에 내원한 근골격계 질환 환자중 골절 발생 현황 (Characteristics of Patients with Skeletal Fracture Admitted in Oriental Hospital)

  • Objective : The purpose of this study is to investigate the characteristics of patients with fracture admitted in oriental hospital. Methods : From october 1996 to october 2001, we retrospectively evaluated the simple radiographic proven 8441 patients with musculoskeletal pain in oriental hospital. The patients included 3899 men and 4542 women. All fractures were analyzed in terms of location, age and sex. Results : 1. The fracture rate of radiologic study was relatively high in foot, chest, upper extremity and hand. The frequent fractures were in spine (257 cases), foot (119 cases), upper extremity (83 cases). and chest (68 cases). 2. The male patients with fracture were significantly higher than female in 0-40 years group: in 0-20 years group(8.5 % vs 4.0 %, p<0.01), in 21-40 years group (4.1 % vs 2.6, p<0.05), while female patients with fracture were significantly higher than male patients in more than 60 years group( 10.1 % vs 15.4 %, p<0.01). 3. The incidence of fracture showed significant association with increased age except 0-20 years group(p<0.01). Spine and chest cage in mate, and upper extremity, pelvis, spine, ankle and chest cage in female were significantly higher on older age groups. Conclusion : The overall fracture rate of radiologic study was 6.6 % for patients with musculoskeletal problem admitted in oriental hospital. The distribution of fracture showed significant difference in terms of age, sex and site. So we believe the radiologic study is helpful to evaluate fracture in oriental hospital.

Screw Fixation without Fusion for Low Lumbar Burst Fracture : A Severe Canal Compromise But Neurologically Intact Case

  • The low lumbar spine is deeply located in flexible segments, and has a physiologic lordosis. Therefore, burst fractures of the low lumbar spine are uncommon injuries. The treatment for such injuries may either be conservative or surgical management according to canal compromise and the neurological status. However, there are no general guidelines or consensus for the treatment of low lumbar burst fractures especially in neurologically intact cases with severe canal compromise. We report a patient with a burst fracture of the fourth lumbar vertebra, who was treated surgically but without fusion because of the neurologically intact status in spite of severe canal compromise of more than 85%. It was possible to preserve motion segments by removal of screws at one year later. We also discuss why bone fusion was not necessary with review of the relevant literature.

마사지 시술 후 발생한 흉골골절 환자 증례보고 (Sternal Fracture occurred after Thai Massage : A Case Report)

  • Objectives: The purpose of this study is to investigate the safety, injury mechanism, complication and adverse reactions associated with Chuna manual theraphy(CMT), to report one case of adverse reaction with Thai Massage(TM). Methods: A 76-year-old woman presented with chest pain and dorsalgia. The patient undertook a TM. The patient's chest pain didn't improve by Korean medecine. In MRI study, sternal fracture was diagnosed. Results and Conclusions : It is supposed that this patient suffered chest pain and dorsalgia due to a TM. From this case, we can understand the etiology of sternal fracture to some extent and consider the complication of Chuna manual theraphy through TM. In addition, This study raises the awareness of the possibility of a sternal fracture caused by a low-energy CMT technique.

Usefulness of Fracture Risk Assessment Tool Using Lumbar Bone Mineral Density in Prediction of Osteoporotic Vertebral Fracture

  • Objective : To investigate the value of lumbar bone mineral density (BMD) in fracture risk assessment (FRAX) tool. Methods : One hundred and ten patients aged over 60 years were enrolled and divided into 2 groups as non-osteoporotic vertebral fracture (OVF) and OVF groups. The 10-year-risk of major osteoporotic vertebral fracture of each group was calculated by FRAX tool with femoral and lumbar spine BMDs to compare the usefulness of lumbar spine BMD in prediction of OVF. The blood level of osteocalcin and C-terminal telopeptide (CTX) as markers of activities of osteoblast and osteoclast, respectively were analyzed using the institutional database. Results : In the OVF group, the ratio of patients with previous fracture history or use of glucocorticoid was higher than those in non-OVF group (p=0.000 and 0.030, respectively). The levels of T-score of femur neck and lumbar spine in OVF group were significantly lower than those in non-OVF group (p=0.001 and 0.000, respectively). The risk of OVF in FRAX using femur BMD in non-OVF and OVF groups was $6.7{\pm}6.13$ and $11.4{\pm}10.06$, respectively (p=0.007). The risk of using lumbar BMD in the 2 groups was $6.9{\pm}8.91$ and $15.1{\pm}15.08$, respectively (p=0.002). The areas under the receiver operator characteristic curve in the FRAX risk with lumbar and femur neck BMD were 0.726 and 0.684, respectively. The comparison of osteocalcin and CTX was not significant (p=0.162 and 0.369, respectively). Conclusion : In our study, the 10-year risk of major osteoporotic fracture in the OVF group of our study was lower than the recommended threshold of intervention for osteoporosis. Hence, a lower threshold for the treatment of osteoporosis may be set for the Korean population to prevent OVF. In the prediction of symptomatic OVF, FRAX tool using lumbar spine BMD may be more useful than that using femur neck BMD.

The Retrial of Percutaneous Vertebroplasty for the Treatment of Vertebral Compression Fracture

  • Objective : For the treatment of osteoporotic vertebral compression fracture, percutaneous vertebroplasty (PVP) is currently widely used as an effective and relatively safe procedure. However, some patients do not experience pain relief after PVP. We performed several additional PVP procedures in those patients who did not have any improvement of pain after their initial PVP and we obtained good results. Our purpose is to demonstrate the effective results of an additional PVP procedure at the same previously treated level. Methods : We reviewed the medical records and the radiologic data of the PVP procedures that were performed at our hospital from November 2005 to May 2008 to determine the patients who had undergone additional PVP. We identified ten patients and we measured the clinical outcomes according to the visual analogue scale (VAS) score and the radiologic parameters, including the anterior body height and the kyphotic angulation. Results : The mean volume of polymethylmethacrylate injected into each vertebrae was 4.3 mL (range: 2-8 mL). The mean VAS score was reduced from 8 to 2.32. The anterior body height was increased from 1.7 cm to 2.32 cm. The kyphotic angulation was restored from 10.14 degrees to 2.32 degrees. There were no complications noted. Conclusion : The clinical and radiologic outcomes suggest that additional PVP is effective for relieving pain and restoring the vertebral body in patients who have unrelieved pain after their initial PVP. Our study demonstrates that additional PVP performed at the previously-treated vertebral levels could provide therapeutic benefit.

  • The compressed fracture of spine caused by osteoporosis is one of the most frequent diseases in bone fracture. Recently the vertebroplasty has drawn much attention as a medical treatment for the compressed fracture of spine, which strengthens the vertebral body and corrects deformity, and relieves pain in patients by injecting bone cement. The finite element analysis is used to investigate the vertebroplasty quantitatively. Previous works with finite element analysis have drawbacks in their simplified models geometry of vertebral body and with material properties of bone. In this paper the exact geometry of vertebral body has been constructed from medical image data and the biomechanical property changes of vertebral body in vertebroplasty have been investigate by using three dimensional finite element analysis.

Image J를 활용한 뼈의 노화도 예측법 (Prediction of Bone Aging by Adapting Image J)

  • Calcium density in human bones decreases as people are getting older due to the interior or exterior environmental factors. Bone aging forms osteoporosis. And this can bring out various spine fractures which develops a complications. Thus the prediction of seniliy is one of the important factors in spine diseases. Once spine aged, diverse fractures occur such as compression fracture and micro fracture. Side images of the spine by the digital radiography (DR) were prepared, and pixel arbitrary unit with Image J was measured from one spot in the lumbar bone part. By calculating pixel arbitrary unit of the simple contrast, it was obtained that the value of pixel arbitrary unit decreased as seniliy of bones increased. By simply applying Image J to the seniliy of patient's spine, the seniliy of bones predicts the level of danger with only digital radiography(2D) image. consequently we show that Image J value of pixel arbitrary unit index for predicts the level of precaution of osteoporosis patient.

외상 후 우연히 발견된 고리뼈 활의 선천적 결손 (증례 보고) (Congenital Defects of the Atlantal Arch Presenting Incidentally after Trauma)

  • A 55-year-old woman was seen in the emergency department with posterior neck pain and a headache after a traffic accident. Physical examination revealed tenderness on palpation over the posterior skull and a midline spinous process of the cervical spine without neurologic deficit. A plain radiograph of the cervical spine demonstrated the absence of the lateral portion of the posterior arch of the atlas and very lucent shadowing of the anterior midline of the atlas, suggesting a fracture of the anterior arch. On three-dimensional computed tomography (CT) of the cervical spine, anterior and posterior bony defects of the atlas were noted. Well-corticated defects were noted with sclerotic change and with no evidence of soft tissue swelling adjacent to the bony discontinuities, consistent with a congenital abnormality. With conservative therapy, the patient gradually showed a lessening of the midline tenderness. Careful investigation with radiography or CT is needed for these patients to avoid confusion with a fracture, because these patients seldom need surgical treatment.

  • A 3-D finite element model of human thoracolumbar spine (T12-L2) was reconstructed from CT images. Various anterior and posterior instrumentation techniques were performed with long cage after corpectomy. Six loading cases were applied up to 10 Nm, espectively. The rotations of T12 with respect to L2 were measured and the stiffnesses were calculated as the applied forces divided by the segmental rotations. The posterior fixation technique increased the stiffness of the spine the most. The addition of anterior rod from 1 to 2 increased the stiffness significantly without posterior fixation, but no effect was found with posterior fixation. We found that different fixation techniques changed the stiffness of the spine.

강직성 척수염이 있는 경수 손상 환자에서 발생한 지연성 척추주위 농양 (Delayed Postoperative Paravertebral Abscess in a Patient with Cervical Spinal Cord Injury Accompanied by Ankylosing Spondylitis)

  • Ankylosing spondylitis (AS) is a chronic inflammatory disease presenting progressive spinal stiffness and sacroiliitis. Cervical spine fracture combined with AS should be treated with operation, but it is closely related with increased rates of surgical site infection, which are associated with an elevated erythrocyte sedimentation rate and elevated C-reactive protein. We report a case of delayed postoperative infection appeared in cervical paravertebral space, which was masked by laboratory findings and clinical characteristics represented in this rheumatic disease. A 53-year-old man who had medical history of AS got operation after cervical spine fracture. During hospitalization, he experienced aching pain originating from left posterior neck to shoulder, which was revealed out to be delayed postoperative infection, diagnostically obscured by elevated values of inflammatory markers. This case emphasizes detailed evaluation considering symptoms and comorbidity of the patient should be performed to apply proper management.
