경추 일자목 변형에 대한 경추 후인과 신전 움직임이 경추 시상면의 형태학적 변화에 미치는 영향: 사례연구 (Effect on Morphological Change of Cervical Spine to Apply to Neck Retraction and Extension Regarding the Straight Deformity of Cervical Spine: Case Study)

  • Background: The purpose of this study was to identify the effects of morphological Change of cervical spine to apply to neck retraction and extension regarding the straight deformity of cervical spine. Methods: A 40-year male subject with straight deformity of cervical spine participated in this study. The study subject underwent a cervical lateral radiography on the static position, neck retraction and neck extension on standing. Measurement method were using computer-based digital radiogram on a picture achieving computer system forthe centroid method, Cobb's angle and Jackson's angle. Results: Neck retraction was increased kyphosis on the C2-4 with lordosis on the C5-7. Neck extension was increased lordosis on the C2-7. Conclusions: These findings suggest that neck retraction was increased kyphosis and neck extension was increased lordosis based on the straight deformity of cervical spine. Therefore, we should be consider that neck extension exercise when increased lordosis for the patients of straight deformity of cervical spine.

Effect of Gym-ball Exercise Program to Pelvis and Spine of the 20's Woman

  • PURPOSE: This study is to investigate the effect of 4 weeks of gym ball exercise on the pelvis and spine of women in 20s. METHODS: Randomly selected 10 of 20 subjects are allocated to experimental group who will practice gym-ball exercise while the rest 10 subjects are allocated to control group. Values for pelvic obliquity DL-DR, pelvic torsion DL-DR, pelvic rotation, kyphotic angle ICT-ITL (max), and lordotic angle ITL-ILS (max) were measured through recording using 3-dimensional image analyzer as a preliminary inspection. Gym-ball exercise was implemented 3 sessions a week for 4 weeks. Each session consisted of 10 minutes of warm up exercise, 30 minutes of main exercise, and 10 minutes of cool down exercise totaling 50 minutes. Post inspections were measured after exercise. RESULTS: Experimental group showed statistically significant difference in pelvic obliquity DL-DR, pelvic torsion DL-DR, and kyphotic angle ICT-ITL (max) (p<.05) and values of pelvic obliquity DL-DR showed statistically significant difference between two groups (p<.05). However pelvic rotation, lordotic angle ITL-ILS (max) did not show a significant difference. CONCLUSION: These results showed that gym-ball exercise has positive effect in the pelvic obliquity, torsion and spine kyphotic angle and expected to have positive effect on the body balance, body lineup, and coordination.

양반 앉은 자세에서의 SPINE-GUARD 착용이 허리부위의 코브각 및 통증 그리고 몸통 굽힘각도의 변화에 미치는 영향 (Impacts from the Wearing of a SPINE-GUARD on a Cross-Legged Sit on Variations of the Cobb's Angle, Lower Back Pain and Trunk Flexion Angle)

  • 앉은 자세를 장시간 유지하면 허리의 만곡에 변화가 오면서 통증이 발생하게 된다. 본 연구에서는 양반 앉은자세에서 SPINE-GUARD(허리 안정화 벨트) 착용이 허리뼈의 코브각과 허리통증 그리고 몸통 굽힘각도의 변화에 미치는 영향에 대해 알아보았다. 대상자 15명(남자)은 2주 동안 주 5회 매회 90분씩 벨트를 착용한 채로 영화를 감상하거나 강의를 들었다. 허리뼈의 코브각은 착용전 $48.36{\pm}14.57$에서 착용후 $28.09{\pm}11.63$로 유의한 감소를 보였다(p<0.05). 통증은 착용전 $4.53{\pm}2.36$이고, 착용후 $2.733{\pm}2.153$로 유의하게 감소하였다(p<0.05). 몸통의 굽힘각도 또한 착용전 $82.33{\pm}20.30$에서 착용 후 $70.2{\pm}19.43$로 유의한 감소를 보였다(p<0.05). 이것으로 보아 SPINE-GUARD의 착용은 허리뼈의 코브각 및 몸통굽힘각도의 변화 그리고 통증의 감소에 영향을 주는 것으로 사료된다.

  • Objectives : This study was designed to observe the effect of Chuna manual therapy on the asymmetrical alignment. Methods : To analyze static structural alignment, posterior inferior ilium deviation(PI), inflare change of pelvis were checked from full spine AP X-ray. And Cobb's angle of spine and height of shoulder girdle were analyzed. To evaluate the pain visual analogue score(VAS) was scored. Chuna therapy treated 8-10 times for 1 month. Results and Conclusions : In 3 cases, VAS was improved significantly. Full spine AP X-ray shows improvement of structural imbalace. Shoulder height difference, cobb's angle, pelvic insufficiency improved considerably. These results suggest that Chuna therapy might be effective for malalignment patients.

The Effect of the Pedicle-Facet Angle on Degenerative Cervical Spondylolisthesis

  • Objective : To measure the orientation of the facet joints of cervical spine (C-spine) segments in the sagittal plane, known as the pedicle-facet (P-F) angle, and to use these measurements to evaluate the relationship between the P-F angle and the amount of vertebral anterolisthesis in patients with degenerative cervical spondylolisthesis (DCS). Methods : A retrospective case-control study was performed including 30 age- and sex-matched patients with DCS and 30 control participants. Anterior-posterior and lateral view radiographs of the C-spine were obtained in a standing position. The P-F angle at all cervical levels and the amount of anterolisthesis at C4-5 were measured from lateral view plain radiographs. Results : The P-F angles at C4-5 were $141.14{\pm}7.14^{\circ}$ for the DCS group and $130.53{\pm}13.50^{\circ}$ (p=0.012) for the control group, and at C5-6 were $137.46{\pm}8.53^{\circ}$ for the DCS group and $128.53{\pm}16.01^{\circ}$ for the control group (p=0.001). The mean P-F angle at C4-5 did not correlate with the amount of anterolisthesis (p=0.483). The amount of anterior slippage did correlate with age (p<0.001). Conclusion : The P-F angle was intrinsically higher at C4-5, compared to C5-6, in both the DCS and control groups, which might explain the increased likelihood for anterolisthesis of C4. Higher P-F angles in the DCS group may be a predisposing factor to slippage. The P-F angle may interact with age to increase incidence of anterolisthesis with increasing age.

Dynamic Corrective Brace를 이용한 특발성 척추측만증의 치료효과에 관한 증례보고 (A Clinical Study on Idiopathic Scoliosis with the Dynamic Corrective Brace)

  • Objectives : This study is designed to evaluate the Effectiveness of dynamic corrective brace on patient of idiopathic scoliosis. Methods : We have investigated 3 patients with idiopathic scoliosis, confirmed X-ray and weared dynamic corrective brace from December 2003 to December 2004. Results : After 12 months of application with dynamic corrective brace, One case, the T-spine Cobb's angle correctability was 47% and the L-spine was 84%, another case, the L-spine Cobb's angle correctability was 59%, the other case, the T-spine Cobb's angle correctability was 32% and the L-spine was 52%. Conclusion : These results suggest that the dynamic corrective brace was effective treatment modality on patient of idiopathic scoliosis.

X-ray 영상을 이용한 척추측만증 영상처리 알고리즘 개발 (Algorithm Development of Scoliosis Image Processing using X-ray Imaging)

  • 본 연구에서는 코브 각도 측정법을 이용하여 척추 X-ray 영상을 측정할 때에 적용이 용이하도록 추간원판을 뚜렷하게 만드는 영상처리 알고리즘과 코브 각도 측정법을 일일이 적용하지 않고도 척추 전장을 포함하는 X-ray 필름의 기립 후전방 척추 영상을 하나의 척추선 함수 그래프로 변환하여 척추측만 정도 측정에 적용 또는 응용이 용이하게 하는 영상처리 알고리즘을 제시하였다. 척추 X-ray영상의 코브 각도 $1^{\circ}$당 L값의 평균을 산출한 결과는 0.568로 나타났고, 이를 기준으로 척추측만 정도 검출이 가능하였다.

An Analysis of Design Factors for Developing Opuntia Humifusa Spines Removal Device

  • Purpose: Opuntia Humifusa has been used in the food and beauty industry after removing spines and glochids clearly. This study compared the methods used in removing spines and analyzed the design factors for developing a spine removal device. Method: This study compared the spine removal ratios in accordance with the length of brush, water spray pressure, the number of water spray, and the size of Opuntia Humifusa in a rotating brush device and a water spray device. In addition, this study compared the reversal ratios according to the inclination angle of a conveyor, the drop height of Opuntia Humifusa, and the speed of the conveyor to analyze the reversal factors. Results: The spines were not removed clearly in the rotating brush method, and the glochids were nailed deeply. The spine removal ratio was 96.9% with the water spray pressure of 20 $kgf/cm^2$ and the conveyor speed of 10 cm/s in the water spray method. The number of water spray was correlated with the spine removal ratio, and the average spine removal ratio was 95.6% with three cycles of water spray. The reversal ratio was 97% with the inclination angle of the conveyor $20^{\circ}$, the drop height of 380 mm, and the conveyor speed of 10 cm/s. Conclusions: In order to develop a Opuntia humifusa spine removing device, this study compared the rotating brush and water spray methods. As a result, each spine removal performance of the rotating brush and water spray methods was 96.9% and 95.6%, respectively. Although the performance of the rotating brush method was slightly higher than that of the water spray method, the water spray method was suitable for removing spines from stem because the epidermis of stem was damaged and the glochids were nail deeply in the rotating brush method. Further studies on appropriate arrangement of spray nozzles, maintaining the optimal water spray pressure, the speed and angle control of the feeding conveyor, and devices for inducing the stem to the center will be needed in combining the water spray device and the reversal device.

  • 헬스케어 시스템에서 인간 목(경추)은 가장 중요한 기관 중 하나이고, 사람의 머리를 지탱하는 부위가 바로 경추이다. 요즘에는 스마트 폰으로 전화를 걸거나 모니터를 보기 위해 종종 목이 과로로 피곤한 경우가 많다. 이 논문에서 우리는 경추의 가쪽 돌림과 및 회전의 이상을 조사한다. 가쪽 돌림의 정상 각도는 $20^{\circ}{\sim}45^{\circ}$이고 정상적인 회전 각도는 $50^{\circ}{\sim}90^{\circ}$이다. 이 각도가 정상보다 낮아 통증이 느껴지면 경추에 문제가 있는 것이다. 또한 목 또는 경추의 가쪽 돌림 및 회전을 측정하는 방법과 이상을 치료하는 방법에 대해 알아본다. 그리고 경추의 가쪽 돌림 및 회전이상을 예방하는 방법도 살펴보도록 한다. 목이 비정상적인지 알아보기 위해 50 대 남성과 여성으로 100 명을 대상으로 실험을 수행하였다.

Free Hand Insertion Technique of S2 Sacral Alar-Iliac Screws for Spino-Pelvic Fixation : Technical Note, Acadaveric Study

  • A rigid spino-pelvic fixation to anchor long constructs is crucial to maintain the stability of long fusion in spinal deformity surgery. Besides obtaining immediate stability and proper biomechanical strength of constructs, the S2 alar-iliac (S2AI) screws have some more advantages. Four Korean fresh-frozen human cadavers were procured. Free hand S2AI screw placement is performed using anatomic landmarks. The starting point of the S2AI screw is located at the midpoint between the S1 and S2 foramen and 2 mm medial to the lateral sacral crest. Gearshift was advanced from the desired starting point toward the sacro-iliac joint directing approximately $20^{\circ}$ angulation caudally in sagittal plane and $30^{\circ}$ angulation horizontally in the coronal plane connecting the posterior superior iliac spine (PSIS). We made a S2AI screw trajectory through the cancellous channel using the gearshift. We measured caudal angle in the sagittal plane and horizontal angle in the coronal plane. A total of eight S2AI screws were inserted in four cadavers. All screws inserted into the iliac crest were evaluated by C-arm and naked eye examination by two spine surgeons. Among 8 S2AI screws, all screws were accurately placed (100%). The average caudal angle in the sagittal plane was $17.3{\pm}5.4^{\circ}$. The average horizontal angle in the coronal plane connecting the PSIS was $32.0{\pm}1.8^{\circ}$. The placement of S2AI screws using the free hand technique without any radiographic guidance appears to an acceptable method of insertion without more radiation or time consuming.
