Objectives: The aim of this study was to report the effectiveness of TMJ balancing therapy and Traditional Korean Medicine (TKM) treatments on a thalamus cerebral infarction patient with hemiparesis and gait disturbance. Methods: The patient was treated with TMJ balancing therapy, along with korean herbal medicine, acupuncture, moxibustion, cupping and physical therapy. The motor function was evaluated through the Manual Muscle Test (MMT), Grasp power and Functional Ambulatory Category (FAC). And the daily life ability was evaluated by the Korean version of the Modified Barthel Index (K-MBI) and Korean National Institute of Health Stroke Scale (K-NIHSS). Results: After 17 days of TMJ balancing therapy and TKM treatments, the score of MMT did not improved, but the score of FAC, K-MBI, K-NIHSS improved. Conclusions: This study suggested that TMJ balancing therapy and TKM treatments could be effective for hemiparesis and gait disturbance in patients with thalamus cerebral infarction.

Background: Incorrect postures of adolescents caused by the use of smart devices have been noted as a factor causing spinal diseases. Objectives: To examine the effect of joint mobilization and stretching on Cobb's angle and respiratory function in adolescent idiopathic scoliosis (AIS). Design: Cluster-randomized controlled trial. Methods: A total of 22 subjects with AIS were enrolled. They were allocated to two groups: the joint mobilization (n=11) and the stretching (n=11). All interventions were conducted for 30 minutes, three times a week for six weeks. Outcome measures were the Cobb's angle and respiratory function. The Cobb's angle and respiratory function measured using the X-ray and Micro-Quark. Results: Joint mobilization group showed significant differences in Cobb's angle and respiratory function, but stretching group showed significant differences Cobb's angle. The differences in peak expiratory flow (PEF) between the two groups were significant. Conclusion: This study proved that joint mobilization is a more effective intervention for AIS to improve Cobb's angle and respiratory function, when compared to stretching.

Purpose: This study compared the subjective and objective sleep quality between subjects with and without thoracic hyper-kyphosis. Methods: Forty participants were divided into a hyper-kyphosis (n=17) and normal group (n=17) by thoracic spinal angle measurement. The subjective sleep quality was measured using PSQI, a self-report, and objective sleep quality was measured using an actigraphy that measures time according to sleep patterns. Results: The PSQI scores of subjects with thoracic hyper-kyphosis were significantly higher than those with normal thoracic curvature (p=0.013). The total sleep time and real sleep time were less in subjects with hyper-kyphosis than in normal subjects (p=0.006; p=0.029). The light sleep time was less in subjects with excessive spondylolisthesis than in normal subjects (p=0.010). Light sleep time was less in those with hyper-kyphosis, but deep sleep time was similar to the subjects with a normal thoracic curvature (p=0.003; p=0.140). Conclusion: Subjects with thoracic hyper-kyphosis had a decrease in subjective sleep quality, such as sleep discomfort, and objective sleep quality, such as a decrease in sleep time compared to normal subjects.
Abusive head trauma (AHT) is the most severe form of physical abuse in children. Such injury involves traumatic damage to the head and/or spine of infants and young children. The term AHT was introduced to include a wider range of injury mechanisms, such as intentional direct blow, throw, and even penetrating trauma by perpetuator(s). Currently, it is recommended to replace the former term, shaken baby syndrome, which implicates shaking as the only mechanism, with AHT to include diverse clinical and radiological manifestations. The consequences of AHT cause devastating medical, social and financial burdens on families, communities, and victims. The potential harm of AHT to the developing brain and spinal cord of the victims is tremendous. Many studies have reported that the adverse effects of AHT are various and serious, such as blindness, mental retardation, physical limitation of daily activities and even psychological problems. Therefore, appropriate vigilance for the early recognition and diagnosis of AHT is highly recommended to stop and prevent further injuries. The aim of this review is to summarize the relevant evidence concerning the early recognition and diagnosis of AHT. To recognize this severe type of child abuse early, all health care providers maintain a high index of suspicion and vigilance. Such suspicion can be initiated with careful and thorough history taking and physical examinations. Previously developed clinical prediction rules can be helpful for decision-making regarding starting an investigation when considering meaningful findings. Even the combination of biochemical markers may be useful to predict AHT. For a more confirmative evaluation, neuroradiological imaging is required to find AHT-specific findings. Moreover, timely consultation with ophthalmologists is needed to find a very specific finding, retinal hemorrhage.

Low back disorders (LBDs) have been the most common musculoskeletal problem in Korean workplaces. It affects many workers, and is associated with high costs to many companies as well as the individual, which can negatively influence even the quality of life of workers. The _evaluation of low back disorder risk associated with manual materials handling tasks can be performed using variety of ergonomic assessment tools such as National Institute for Occupational Safety and Health (NIOSH) Revised Lifting Equation (NLE), the Washington Administrative Code 296-62-0517 (WAC), the Snook Tables etc. But most of these tools provide limited information for choosing the most appropriate assessment method for a particular job and in finding out advantage and disadvantage of the methods, and few have been assessed for their predictive ability. The focus of this study was to _evaluate spinal loads in real time with lifting and pulling heavy cow leathers in variety of postures. Data for estimating mean trunk motions were collected as employees did their work at the job site, using the Lumbar Motion Monitor. Eight employees (2 males, 6 females) were selected in this study, in which the load weight and the vertical start and destination heights of the activity remained constant throughout the task. Variance components (three dimensional spaces) of mean trunk kinematic measures were estimated in a hierarchical design. They were used to compute velocity and acceleration of multiple employees performing the same task and to repetitive movements within a task. Therefore, a results of this study could be used as a quantitative, objective measure to design the workplace so that the risk of occupationally related low back disorder should be minimized.

Purpose. This study was classified into normal and demented elderly through K-MMSE. The purpose of this study was to analyze gait characteristics of normal elderly and demented peoples using GAITRite walking system. Methods. The subjects of this study were selected as elderly people receiving home visit physical therapy. An independent t-test was conducted to verify the statistical significance of the time-space variables of the elderly with dementia. Results. Step time(p=0.041), cycle time(p=0.037), distance(p=0.024), and cadence(p=0.048) were significantly shorter in the normal elderly than in the demented elderly on flat place. The mean age was significantly longer in normal elderly than in elderly persons with dementia. Step time(p=0.022), cycle time(p=0.023), distance(p=0.019), and cadence(p=0.015) were significantly shorter in the mat walking. The mean age was significantly longer in normal elderly than in elderly patients with dementia. Stretch time, cycle time, distance, and hair support time were significantly shorter in the mat walking. The mean age of the elderly was significantly longer than that of the elderly with dementia. The spinal support time, which is a spatial variable, was significantly shorter in the normal elderly than in the demented elderly. Conclusions. It compares the various gait characteristics of the normal and demented elderly people, thereby increasing the walking ability of the elderly person more effectively. This study should be utilized as basic data for preventing fall-down.

PURPOSE: This study aims to provide effects of therapeutic techniques as well as basic materials of safety by comparing and analyzing the effects of hamstring flexibility and dynamic stability of lower lumbar according to Stretching and Massage Techniques to adults with reduced the flexibility of hamstring. METHODS: This study conducted differential diagnosis through sit and reach test(SRT) and Schober test to select subjects who have shortened hamstring without any spinal problem. Selected subjects were divided into two groups randomly; HSG(Hamstring Stretching Group, n=8) and HMG(Hamstring Massage Group, n=8) and they received treatment for 2 weeks. To take statistics, SRT and dynamic view using x-ray were used. RESULTS: On SRT, HSG and HMG showed significant difference between pre and post test. A comparison of the difference value between HSG and HMG, HSG($9.73{\pm}1.78$) has more remarkable outcome than HMG($2.78{\pm}0.56$). Lower lumbar intervertebral disc length test for Intervertebral disc length(IDL)L45 and IDLL5S1 did not show significant differences between two groups and difference value. CONCLUSION: This study showed that stretching is more effective to improve hamstring flexibility than massage technique. Especially, flexibility increase of the hamstring in vertebral stabilization cannot affect improvement possibility will make a flexibility in order and the intervention and stabilization exercise of the spine.

Purpose : The purpose of this study is to find out the effects of Joint and Muscle Mobilization applied to the patients with Degenerative Disc Disease by measuring, assessing and analyzing the changes on pain and in flexibility before and after Joint and Muscle Mobilization. Method: Surveyed from June. 2008 to July. 2009 were 10 patients suffering from DDD. Joint and muscle Mobilization were applied for 15 minutes in total. After Joint Mobilization, the Oswestry Disability Index(ODI), Visual Analogue Scale (VAS) were used to measure the degree of the pain on the patients. After Joint and Muscle Mobilization, Spinal-$Mouse^{(R)}$ were used to measure the degree of the flexibility on the patients. Result: 1. There was significant decrease in the numerical values of the VAS & ODI after Joint and Muscle Mobilization (p<0.01). 2. There was significant decrease in the degree of the pain on the patients after Joint and Muscle Mobilization (p<0.01). 3. From the analysis into DDD in the degree the pain before and after Joint and Muscle Mobilization with Paired Sample T-test, It became evident that the longer the period of treatment was, the higher the pains decreases drastically, while significant difference was shown in the flexibility and the degree of the pain (p<0.01). Conclusion: Summed up, it can be generally concluded that Joint and Muscle Mobilization is an effective treatment to rid the patients with DDD of pains safely and promptly. It is, therefore, suggested to continue and expand the study on the cure of DDD and to motivate patients. Joint and Muscle Mobilization is considered as safest and most efficient pain remedy.
Purpose: This study aimed to investigate and report the current status of physical therapy (PT) performed in Korean neonatal intensive care units (NICU) to present foundational data that promotes the advances in neonatal PT in Korea. Methods: Based on the Health Insurance Review and Assessment (HIRA) data, we administered a questionnaire survey to 74 hospitals (39 tertiary and 35 general hospitals) in Korea equipped with a NICU and pediatric PT unit. We developed a 32-item questionnaire with reference to previous Korean studies. The questionnaires were distributed and retrieved via regular mail and an online system. Results: Of the 74 hospitals, 58 (78%) practiced neonatal PT and the duration of each session significantly differed according to the hospital rating. PT was given, depending on clinical symptoms, to infants who were preterm and low birth-weight (96.5%), had brain and spinal cord diseases (84.5%), had pathological tonus (94.8%), with respiratory problems (65.5%), for range of motion exercises (82.8%), for neurodevelopment approaches (72.4%), and for positioning (70.7%). Interdisciplinary meetings were held to share clinical decisionmaking in 17.2% of the hospitals surveyed and parent-participating education to ensure a family-centered approach was offered in 63.8% of the hospitals. The barriers of neonatal PT included low insurance fees, insufficient awareness of colleagues, and the severity of the patient. Conclusion: This study is the first report of the current status of neonatal PT in Korea. The findings of this study will serve as foundational data to review the current neonatal PT practice and promote further advances.

Objective : To analyze the accuracy of iliac screws using freehand technique performed by the same surgeon. We also analyzed how the breach of iliac screws was related to the clinical symptoms resulting in revision surgery. Methods : From January 2009 to November 2015, 100 patients (193 iliac screws) were analyzed using postoperative computed tomography scans. The breaches were classified based on the superior, inferior, lateral, and medial iliac wall violation by the screw. According to the length of screw extrusion, the classification grades were as follows : grade 1, screw extrusion <1 cm; grade II, 1 cm ≤ screw extrusion <2 cm; grade III, 2 cm ≤ screw extrusion <3 cm; and grade IV, 3 cm ≤ screw extrusion. We also reviewed the revision surgery associated with iliac screw misplacement. Results : Of the 193 inserted screws, 169 were correctly located and 24 were misplaced screws. There were eight grade I, six grade II, six grade III, and four grade IV screw breaches, and 11, 8, 2, and 3 screws violated the medial, lateral, superior, and inferior walls, respectively. Four revision surgeries were performed for the grade III or IV iliac screw breaches in the lateral or inferior direction with respect to its related symptoms. Conclusion : In iliac screw placement, 12.4% breaches developed. Although most breaches were not problematic, symptomatic violations (2.1%) could result in revision surgery. Notably, the surgeon should keep in mind that lateral or inferior wall breaches longer than 2 cm can be risky and should be avoided.
