The Effect of Thoracic Cage Mobilization and Breathing Exercise of Respiratory Function, Spinal Curve and Spinal Mobility in Elderly with Restrictive Lung Disease

  • The purpose of this study was to examine the effects of combined respiratory physical therapy on respiratory function, spinal curve and spinal mobility for community-dwelling elderlies with restrictive lung diseases. In total, 10 patients participated in an 8-week intervention program of thoracic cage mobilization and breathing exercise in combination. The results of the study are as follows: for respiratory function, the forced expiratory volume in 1 second (FEV1), forced vital capacity(FVC), and FEV1/FVC were significantly improved to $.30{\pm}0.31{\ell}$, $.46{\pm}.42{\ell}$, and $18.10{\pm}11.39%$, respectively (p<.05). For spinal curve, the thoracic curve and the lumbar curve were improved significantly to $-2.20{\pm}1.40^{\circ}$ and $-1.20{\pm}1.14^{\circ}$, respectively (p<.01). For spinal mobility, the thoracic flexion ($3.40{\pm}2.99^{\circ}$), thoracic extension ($3.50{\pm}1.43^{\circ}$), lumbar flexion ($4.50{\pm}4.74^{\circ}$), and lumbar extension($-1.50{\pm}1.84^{\circ}$) were all significantly improved (p<.05). These findings indicate that thoracic cage mobilization and breathing exercise in combination improve the respiratory function, spinal alignment, and spinal mobility in elderly people with restrictive lung diseases.

Effectiveness of Breathing Exercises on Spinal Posture, Mobility and Stabilization in Patients with Lumbar Instability

  • PURPOSE: This study was conducted to monitor the performance of breathing exercises by patients with lumbar instability who had altered breathing patterns. METHODS: To investigate the effects of breathing exercises on spinal posture, mobility, and stabilization in patients with lumbar instability with altered breathing patterns, 30 adult participants were enrolled on the basis of the selection criteria and randomly assigned to the breathing exercise group (BEG) or trunk stabilization exercise group (SEG). A pre-test was performed prior to the intervention exercise program. The intervention exercise program consisted of 15 sessions (three sessions per week for 5 weeks) between August and September of 2016. The post-test was performed on the 6th week of intervention. RESULTS: Pre- and post-test comparisons of BEG and SEG revealed significant improvements in all tested items in the SEG, except for spinal mobility, while significant improvements in spinal postures 1 and 2, spinal mobility, and stabilization were found in the BEG. Between-group comparisons revealed that there were no significant differences in spinal posture 1, spinal posture 2, spinal mobility, or stabilization, whereas significant differences were found in spinal posture 2 and spinal mobility, with the BEG showing greater improvements than the SEG. CONCLUSION: Based on the findings in the present study, it is believed that breathing exercises have important effects on spinal posture, mobility, and stabilization in patients with lumbar instability who have altered breathing patterns.

Immediate Effect of Flexion-Distraction Spinal Manipulation on Intervertebral Height, Pain, and Spine Mobility in Patients with Lumbar Degenerative Disc Disease

  • Objective: This study aimed to investigate the short-term effects of flexion-distraction spinal manipulation on intervertebral height, pain, spine mobility in patients with lumbar degenerative disc disease. Design: Randomized controlled trial with a pretest-posttest control group design Methods: A total of 96 participants with degenerative disc disease participated in the study and were randomly divided into two groups. Both groups received intervention for 3-5 minutes a day. The experimental group (n=48) underwent flexion-distraction spinal manipulation for 3-5 minutes, and the control group (n=48) was maintained in the same position as the experimental group for 5 minutes without any intervention. The intervertebral height was measured by computed tomography, pain was assessed using visual analog scale, and the spine in flexion mobility was measured using the finger-to-floor distance test and passive straight leg raise test. Pre-test and post-test measurements were obtained. Results: The experimental group showed significant improvement in intervertebral height, degree of pain, and spinal mobility (p<0.05). The intervertebral height increased from 6.32±1.90 to 6.93±1.85 mm (p<0.05), lower back pain decreased from 69.17±13.35 mm to 48.48±12.20 mm (p<0.05), lumbar spine mobility changed from 17.37±4.49 to 12.69±4.34 cm (p<0.05), and passive straight leg raise test range increased from 46.94±13.05° to 56.01±12.20° (p<0.05). Conclusions: This study suggests that flexion-distraction spinal manipulation could be an effective treatment for decreasing pain and improving function in patients with degenerative disc disease.

만성요통 환자의 질환명에 따른 흉요추부의 관절가동범위 비교 (Comparison of Ranges of Motion in The Thoracolumbar Region for Clinical Diagnoses of Patients with Chronic Low Back Pain)

  • Purpose : The purpose of this study was to compare differences in thoracolumbar ranges of motions by comparing ratios among 4 type diagnosis for patient with chronic low back pain. Methods : The subjects were 58 chronic low back pain patients. A motion analysis program (Global Postural System) was used after photography for posture measurement. To analyze differences in mobility percentages and ratios of thoracolumbar ranges of motion, one-way ANOVA was used. Results : Regional difference spinal posture and movement were found to diagnosis patients with chronic low back pain. Comparison of thoracolumbar ranges of motion revealed significant differences in the thoracic region during forward-bending of the trunk (p<.05). In the upper thoracic region, the herniated intervertebral lumbar disc (HILD) group was significantly larger than the spinal stenosis/herniated intervertebral lumbar disc (SS/HILD) group (p<.05). In the lower thoracic region, chronic sprains (CS) were significantly greater than in the spinal stenosis (SS) group and in the (SS/HJLD) group (p<.05). Comparative analysis of thoracic/lumbar mobility ratio showed the CS group's ratio during forward bending was largest: 1.66, while the HILD group's ratio was smallest: 84 a significant difference between the two groups (p<.01). Diagnosis was not associated with difference in thoracolumbar backward-bending range of motion (p>.05). Conclusion : Theses results indicate the clinical efficacy of diagnosing for chronic low back pain by evaluating spinal mobility.

한국형 척추교정기의 개발을 위한 인간공학적 접근 (An Ergonomics Approach for Developing Korean Style Chiropractic Table)

  • Spinal injuries are common these days and are increasing due to the increasing sports activities, auto-related accidents, and occupational incidences. Most of spinal injuries of strain and sprain are associated with trauma and resulted in loss of optimal musculoskeletal system in spinal column area. Some of these injuries requires surgical treatments. Fortunately, some people with spinal disorder are achieving outstanding results with the chiropractic care which utilizes muscle stimulation and strengthening, traction therapy, and spinal adjustments. These treatments using chiropractic table can ease pain by restoring alignment, improving mobility, and relieving pain and stiffness in neck and spinal areas. The purpose of this study was thus to gather the comprehensive information about spinal injuries, to define the specific dimensions, and to recommend functions of chiropractic table specially suited for Korean. This study was an integrated approach in applying the concepts of biomechanical correction of the musculoskeletal system. Also, this study was to utilize a knowledge of physics, ergonomics, and rehabilitation to the development of spine cure medicine.

Mobility of Intradural Extramedullary Schwannoma at Spine : Report of Three Cases with Literature Review

  • Although very rare, a few cases of intradural extramedullary (IDEM) spinal tumor migration have been reported since Tomimatsu first reported a mobile schwannoma of the cervical cord in 1974. Schwan noma is a neurogenic tumor which originates from nerve sheath that it is relatively well-marginated tumor with little attachment or adhesion to surrounding tissue. Mobility of tumor in spinal canal sometimes can result in negative exploration at the expected area. We found three interesting cases in which different tumor locations observed in repeated magnetic resonance image (MRI) findings. All tumors were intradural and extramedullary schwannoma. We reviewed the literature about moving tumor in the spine through PUBMED search.

청소년기 경직형 뇌성마비아동의 체간강화 운동이 척추분절 및 보행에 미치는 영향 (The Effects of Trunk Strengthening Program in Gait and Spinal Segment with Spastic Cerebral Palsy)

  • Purpose : This study aims to examine effects of trunk strengthening exercise in three rigid cerebral palsy adolescents aged between 11 and 13 on their spinal segments and gait. Methods : 2A trunk strengthening program for proprioceptive neuromuscular facilitation was made focusing on improvement in their school life and mental and social activities during their school years, and then trunk strengthening exercise was applied to the three subjects for 30 minutes five times per week for four weeks. With the subjects in a static state, each group's gait analysis was made and maximum anterior flexion, maximum extension, maximum left and right lateral flexion, and cross rotation of the trunk were measured using Sonosens (Germany) prior to and after the intervention. Results : Assessment of spinal segment mobility with the subjects in a static state showed that the trunk muscle strengthening exercise increased their maximum joint movement angles from the right side to the center and rotational abilities of the cervical and lumbar spines. Gait analysis indicated increased movements in the thoracic and lumbar spines and relatively decreased anterior-posterior movement of the cervical spine. Conclusion : Trunk strengthening program is effective in enhancing spinal segment mobility and balance ability of cerebral palsy adolescents and considered able to be used together with diverse treatment interventions.

Selection of polymer material in the design optimization of a new dynamic spinal implant

  • "Dynamic stabilization" systems have been developed in recent years to treat degenerative disorders of the spinal column. In contrast to arthrodesis (fusion), the aim here is to conserve intervertebral mobility to maximize comfort. When developing innovative concepts, many mechanical tests need to be carried out in order to validate the different technological solutions. The present study focuses on the B Dyn$^{(R)}$ "dynamic stabilization" device (S14$^{(R)}$ Implants, Pessac, France), the aim being to optimize the choice of polymer material used for one of the implant's components. The device allows mobility but also limit the range of movement. The stiffness of the ring remains a key design factor, which has to be optimized. Phase one consisted of static tests on the implant, as a result of which a polyurethane (PU) was selected, material no.2 of the five elastomers tested. In phase two, dynamic tests were carried out. The fatigue resistance of the B Dyn$^{(R)}$ system was tested over five million cycles with the properties of the polymer elements being measured using dynamic mechanical analysis (DMA) after every million cycles. This analysis demonstrated changes in stiffness and in the damping factor which guided the choice of elastomer for the B Dyn$^{(R)}$ implant.

척수경색 환자의 보행불가 증상에 대한 한의복합치료 1례 (A Case Study of Combined Korean Medicine Treatment of Paraplegia Diagnosed as Spinal Cord Infarction)

  • Spinal cord infarction is one of the rare strokes with no clear signs of serious nerve damage or after-effects. This study reports on the effectiveness of a combined treatment of Korean medicine and acupuncture on bilateral paraplegia, dysuria, and constipation in a patient with sudden-onset spinal cord infarction. An 84-year-old male patient was diagnosed with spinal cord infarction in August 2022. After diagnosis by whole spine MRI, he received treatment for two months at another hospital, but the improvement was insignificant. He then received Korean medicine treatment, and during this period, his lower extremity manual muscle test grade improved from 3 to 4 and his modified Rivermead mobility index score increased by 13 points, compared with hospitalization. Dysuria improved with acupuncture, and constipation improved with herbal medicine treatment. A combination of herbal medicine and acupuncture can be used to treat paraplegia, dysuria, and constipation caused by spinal cord infarction.

운동치료와 관절가동술이 하지길이 차이에 미치는 영향 (The Effects of Joint Mobilization and Therapeutic Exercise on Difference for Length of Lower Limbs)

  • Object: to evaluate the effects of two different treatments-joint mobilization and therapeutic exercise on difference for length of lower limbs. Method: The subjects were participated twenty six who has difference for length of lower limbs more 10mm. All subjects randomly assigned to Joint mobilization group(n=13) and therapeutic exercise group. Joint mobilization group received joint mobilization for 2 minutes, Therapeutic exercise group received for 15 minutes per day and 3 times a week during 4 week period. Tape measure method was used to measure the difference for length of lower limbs. Biodex was used to measure the muscle power of lower limbs(Knee flexion, extension). Finger to floor test was used to measure the mobility of spinal column. All measurement of each subjects were measured at pre-experiment, after 2weeks and post-experiment. Result: The result of this study were summarized as follows : 1. Both treatment decreased difference for length of lower limbs while joint mobilization more decreased difference for length of lower limbs than therapeutic exercise. 2. Both treatment increased mobility of spinal column while joint mobilization more increased mobility of spinal column than therapeutic exercise. 3. Joint mobilization increased muscle power while therapeutic exercise decreased muscle power. Conclusion: in a group-wise comparison joint mobilization is more effective than therapeutic exercise.
