Background and Objectives: Top-down restoration of distorted speech, tapped as phonemic restoration of speech in noise, maybe a useful tool to understand robustness of perception in adverse listening situations. However, the relationship between phonemic restoration and speech perception in noise is not empirically clear. Subjects and Methods: 20 adults (40-55 years) with normal audiometric findings were part of the study. Sentence perception in noise performance was studied with various signal-to-noise ratios (SNRs) to estimate the SNR with 50% score. Performance was also measured for sentences interrupted with silence and for those interrupted by speech noise at -10, -5, 0, and 5 dB SNRs. The performance score in the noise interruption condition was subtracted by quiet interruption condition to determine the phonemic restoration magnitude. Results: Fairly robust improvements in speech intelligibility was found when the sentences were interrupted with speech noise instead of silence. Improvement with increasing noise levels was non-monotonic and reached a maximum at -10 dB SNR. Significant correlation between speech perception in noise performance and phonemic restoration of sentences interrupted with -10 dB SNR speech noise was found. Conclusions: It is possible that perception of speech in noise is associated with top-down processing of speech, tapped as phonemic restoration of interrupted speech. More research with a larger sample size is indicated since the restoration is affected by the type of speech material and noise used, age, working memory, and linguistic proficiency, and has a large individual variability.

The aims of the present study are two-fold: 1) to explore differences on speech perception between younger and older adults according to noise conditions; and 2) to investigate which cognitive domains are correlated with speech perception. Data were acquired from 15 younger adults and 15 older adults. Sentence recognition test was conducted in four noise conditions(i.e., in-quiet, +5 dB SNR, 0 dB SNR, -5 dB SNR). All participants completed auditory and cognitive assessment. Upon controlling for hearing thresholds, the older group revealed significantly poorer performance compared to the younger adults only under the high noise condition at -5 dB SNR. For older group, performance on Seoul Verbal Learning Test(immediate recall) was significantly correlated with speech perception performance, upon controlling for hearing thresholds. In older adults, working memory and verbal short-term memory are the best predictors of speech-in-noise perception. The current study suggests that consideration of cognitive function for older adults in speech perception assessment is necessary due to its adverse effect on speech perception under background noise.

Further for this comparison, a speech perception in noise test was constructed using existing mono-syllabic and bi-syllabic word lists in Tamil. Subjects and Methods: A total of 30 participants with normal hearing in the age range of 18 to 25 years participated in the study. Speech-in-noise test in Tamil (SPIN-T) constructed using mono-syllabic and bi-syllabic words in Tamil was used as stimuli. The stimuli were presented in the background of multi-talker speech babble at two SNRs (0 dB and +10 dB SNR). Results: The effect of noise on SPIN-T varied with SNR. All the participants performed better at +10 dB SNR, the higher of the two SNRs considered. Additionally, at +10 dB SNR performance did not vary significantly for neither mono-syllabic or bi-syllabic words. However, a significant difference existed at 0 dB SNR. Conclusions: The current study indicated that higher SNR leads to better performance. In addition, bi-syllabic words were identified with minimal errors compared to mono-syllabic words. Spectral cues were the most affected in the presence of noise leading to more of place of articulation errors for both mono-syllabic and bi-syllabic words.

Although it is well-known that environmental noise can lead to hearing loss in individuals, the true extent of subway noise effects in the general population remains poorly understood. The purpose of the present study is to see changes of acoustic reflex thresholds and speech perception scores when passengers listen to music presented from their personal listening device in the subway. Forty subjects with normal hearing participated being divided into two groups, experimental and control groups. As a baseline, all subjects were measured by acoustic reflex thresholds in five test frequencies and Korean speech perception in noise (KSPIN) test at 0 and -5 dB SNR. In the experiment, the control group read newspaper or magazine in the subway noise, whereas the experimental group listened to music presented from their smartphone under the subway noise through speakers at 73.45 dBA for 60 min. After completing the experiment, two groups also conducted both acoustic reflex thresholds and KSPIN tests in the same condition as the baseline. The results showed that there was a significant difference of correct percent in speech-in-noise test between experimental and control groups and of that between two signal-to-noise ratios, which means the double noise exposure of 60 min might cause some degradation of speech perception when noise increases compared to only subway noise condition that was not statistically significant difference. We concluded that a risk of some degraded speech perception ability would be expected when passengers have a habit of listening to music in the subway noisy situation for a long duration.

Therefore, we aimed to evaluate the effect of DNR on music, in addition to speech perception, using objective and subjective measurements. The DNR-off condition in HAs may be beneficial for pitch discrimination at some frequencies.

Subjects and Methods: Twenty-seven subjects (15 normal-hearing and 12 hearing-impaired listeners) participated. The recorded Korean 0-9 digits were used to form quasirandom digit triplets; 50 target digit triplets were presented at the most comfortable level of each subject while presenting speech-shaped background noise at various levels of signal-to-noise ratios (-12.5, -10, -5, or +5 dB). Subjects were then instructed to listen to both target and noise masker unilaterally and bilaterally through a headphone. K-SPIN test was also conducted using the same procedure as the K-DIN. After calculating their percent correct responses, K-DIN and K-SPIN results were compared using a Pearson-correlation test. Results: Results showed a statistically significant correlation between K-DIN and K-SPIN in all hearing conditions (left: r=0.814, p<0.001; right: r=0.788, p<0.001; bilateral: r=0.727, p<0.001). Moreover, the K-DIN test achieved better testing efficacy, shorter average listening time (5 min vs. 30 min), and easier performance of task according to participants' qualitative reports than the K-SPIN test. Conclusions: In this study, the Korean version of digit triplet test was validated in both normal-hearing and hearing-impaired listeners. The findings suggest that the K-DIN test can be used as a simple and time-efficient hearing-in-noise test in audiology clinics in Korea.
