광범위한 대뇌병터를 보인 대뇌형 부신척수신경병증 1예 (A Case of Cerebral Adrenomyeloneuropathy with Extensive Cerebral Lesions)

  • We report a 31-year-old man with cerebral adrenomyeloneuronopathy variant, who presented as progressive gait disturbance. He had spastic paraparesis, hyperreflexia without Babinski's sign and sensory symptom. No adrenal insufficiency was noted. Brain MRI showed extensive high signal intensities in bilateral temporal lobes and posterior periventricular white matter in T2 weighed imaging without cerebrospinal fluid abnormality. His nerve conduction study showed sensorimotor demyelinating polyneuropathy and the level of saturated very-long-chain fatty acids was high in his plasma, although neuropsychological test was normal.

Cervico-Thoracic Intradural Extramedullary Lipoma

  • A 42-year-old female was admitted with an 11-month history of progressive spastic paraparesis and ataxic gait. Magnetic resonance imaging showed intraspinal space occupying lesion compressing the spinal cord posteriorly, located from C5 to T2 with iso to high signal intensity at T2-weighted images and high signal intensity at T1-weighted images. The patient underwent surgery for decompression of the affected spinal cord because of the progressive neurological deficit. At surgery, the lesion was intradural extramedullary lipoma composed with mature adipose tissue. Partial tumor removal to decompress the neural structures and laminoplasty to avoid postoperative instability and deformity were performed. Postoperatively, she demonstrated improvement in paraparesis and was able to walk without assistance. Though attempts to decrease the size of or even to totally remove a lipoma are not required to achieve satisfactory results and carry considerable risks of surgical morbidity, a careful and limited decompression of the affected spinal cord through a partial removal of the tumor and laminoplasty could result in a significant neurological improvement.

  • We present a case of chondroblastoma in the thoracic vertebra. A 40-year-old patient with upper back pain and lower extremity weakness was admitted to our clinic. On neurological examination, the patient exhibited lower extremity spastic paraparesis. Magnetic resonance imaging revealed a mass infiltrating the 7th thoracic vertebra and its adjacent structures with concomitant compression of the epidural space. After right upper lung tuberculoma was resected through the transthoracic approach, T7 total corpectomy was done with anterior stabilization using a MESH cage and T7 rib bone graft. Two weeks after the first operation, remained part of vertebra was removed and posterior stabilization was performed using a pedicle screw fixation and cross linkage bar with the assistance of the navigation system. The final pathologic diagnosis of the vertebral lesion was benign chondroblastoma.

Atypical Presentation of Spinal Tuberculosis Misadiagnosed as Metastatic Spine Tumor

  • We present a case of a 68-year-old female with an atypical form of spinal tuberculosis, which involved posterior elements with multiple foci in two noncontiguous vertebral levels. The lesions caused spastic paraparesis and urinary hesitation. There was no evidence of pulmonary or other extrapulmonary tuberculous disease. Based on clinical and radiographic findings, this atypical spinal tuberculosis was preoperatively misdiagnosed as metastatic spine tumor. The histopathologic finding confirmed tuberculosis and the patient was treated successfully with surgery and antituberculous therapy. In case of a spinal lesion of unknown origin, it is important to be aware that atypical spinal tuberculosis can mimic metastatic spine tumor and tuberculosis should always be considered.

  • The study reports the clinical case of a patient with lower extremity weakness and gait disturbance treated with Korean medicine treatments including Chuna therapy. The patient suffered lower extremity weakness and gait disturbance with diagnosis of lumbar disk herniation and spinal stenosis. As a treatment, the doctor applied Chuna therapy, herbal medicine, acupuncture, pharmacopuncture, cupping and moxa. The effect of treatment was evaluated by numeral rating scale (NRS), self-walkable distance, functional independence measure (FIM), Oswestry disability index (ODI) and manual muscle test (MMT). NRS decreased from 6 to 4 at the leg. Self-walkable distance increased from 0 m to 10 m, FIM increased 85 to 96 points while ODI decreased 64% to 54%. MMT of hip flex and knee extension improved from grade 3+, grade 3 to grade 4, respectively. Korean medicine treatment can be effective for patients who suffer lower extremity weakness and gait disturbance due to lumbar disk herniation and spinal stenosis. Further clinical studies are required to verify these findings.

두 마리 고양이에서 발생한 급성 척수 경색의 자기 공명 영상학적 진단 증례 (Magnetic Resonance Imaging Features of Suspected Acute Spinal Cord Infarction in Two Cats)

  • 척수 경색은 고양이에서 급성 척수 병증의 주요한 원인으로써 인식되고 있다. 사후 조직 병리학적 검사를 통해 확진 할 수 있지만 MR 영상학적 특징은 척수 경색의 진단에 가치있는 정보를 제공한다. 본 증례의 목적은 두 마리의 고양이에서 발생한 급성 척수 경색의 영상학적 특징을 설명하고 그 진단에 있어서 low field MRI (0.3 Tesla)의 유용성을 평가하는 것이다. 미확인 연령의 중성화 수컷, mixed breed 고양이가 급성의 사지 부전 마비를 주증으로 내원하였고 9살령의 중성화 암컷 domestic short hair 고양이는 후지 부전 마비로 내원하였으며 하루 후 후지 완전 마비가 나타났다. 이후 실시된 MR 영상에서 첫번째 고양이의 경우 두번째 경추부터 여섯번 째 경추 수준의 척수에서 병변이 분포하였으며 두번째 고양이의 경우 두번째 요추부터 다섯번째 요추 수준에서 병변이 나타났다. 두 고양이에서 공통적으로 주로 회백질에 분포한 척수 실질 내에 국소적인 병변이 확인되었으며 T2 강조 영상 및 FLAIR 영상에서 고신호를 나타내었고 DWI 영상에서 고신호, ADC map 에서는 저신호를 나타내었다. 히스토리, 임상증상 및 다른 실험실적 검사와 함께 MR 영상학적 특징을 통해 두 고양이에서 급성 척수 경색이 진단되었다.

성인형 알렉산더병의 자기공명영상 소견 및 추적 관찰상의 변화: 증례 보고 (Characteristic MR Imaging Features and Serial Changes in Adult-Onset Alexander Disease: A Case Report)

  • 성인형 알렉산더병은 운동 실조, 경련성 마비 또는 뇌간 징후를 나타내는 드문 유전 질환이다. 성인형 알렉산더병의 진단은 흔히 자기공명영상 검사 소견을 통해 내려진다. 이에 저자들은 glial fibrillary acidic protein (이하 GFAP) 유전자 변이 분석을 통해 확진된 37세와 61세 성인형 알렉산더병 환자의 특징적인 자기공명영상 소견과 추적 검사상 변화 사례에 대해 보고하고자 한다. 자기공명영상에서 환자들은 전형적인 뇌간 위축과 백질 이상 소견을 보였다. 특징적인 자기공명영상을 바탕으로 추정진단을 내렸으며 이후 GFAP 유전자 변이 분석을 통해 확진되었다. 또한, 추적 관찰 검사에서 연수와 상부 경부 척수 위축의 진행을 보였다. 이러한 성인형 알렉산더병의 특징적인 자기공명영상 소견에 대한 이해를 바탕으로, 감별진단을 위한 GFAP 유전자 검사 시행 결정에 도움이 될 수 있을 것이다.
