  • This is a case report of the rupture of the trachea and the esophagus following external blunt trauma without any associated injury. A 7 year old male patient was brought to the emergency room. Hanyang University Hospital, on 23 Jul, 1974 while he played under the pile of pingpong table A table was fall down over the patient accidentally. The patient was injured by the table and found lying in unconsciousness on the ground. There was subcutaneous emphysema on his anterior chest and neck. A contusion on the upper part of the sternum was noted. The breathing sound were diminished over the left side of the chest. Ronchi were heard over the right lung field. This patient was suffered from vomiting, dypsnea, and irritable mental state after this accident. On the chest roentgenogram in A-P view, hyperlucency at the mediastinum and parapericardiac area suggested the pneumomediastinum. On the next day, a diagnosis of the tracheal and esophageal rupture was confirmed by the esophagogram with Lipiodol swallowing. A right thoracotomy was performed and ruptured orifice of the trachea and the esophagus were closed with interrupted sutures. Postoperative course was uneventful and primary closure of the rupture of the trachea and the esophagus were succeeded.

  • In this paper, we proposed a novel method for automatic detection for snoring and heart beat using a single piezoelectric sensor. For this study multi-rate signal processing technique was applied to detect snoring and heart beat from the single source signal. The sound event duration and intensity features were used to snore detection and heart beat was found by autocorrelation. The performance of the proposed method was evaluated on clinical database, which is the nocturnal piezoelectric snoring data of 30 patients that suffered obstructive sleep apnea. The method achieved sensitivity of 88.6%, specificity of 96.1% with accuracy of 95.6% for snoring and sensitivity of 94.1% and positive predictive value of 87.6% for heart beat, respectively. These results suggest that the proposed method can be a useful tool in sleep monitoring and sleep disordered breathing diagnosis.

  • The air filter in engine intake system filters the dirt in the breathing air but also it attenuates the noise although the phenomenon has been regarded negligible. For the analysis of the acoustical performance of air filter, an acoustical model is suggested in this paper. The air filter consists of a porous filter element, which catches the particulate dirt, and a plastic filter box, which supports the filter element. Fibrous structure of the filter element is modeled as a micro-perforated panel using the flow resistivity and porosity. The pleated geometry of the filter element is modeled as two coupled ducts and a mathematical model is developed for the analysis of sound propagation. The filter box Is modeled as a rigid rectangular box. By combining two models, a 4-pole transfer matrix for the air filter is derived. The transmission loss calculated using the transfer matrix of the suggested model is compared with the measured data. Reasonably good agreement is observed. The result can be improved by considering the visco-thermal effect in modeling, in particular at a frequency range near the troughs of TL curve.

  • This paper proposes a new algorithm for sleep apnea detection based on stroke volume. It is very important to detect sleep apnea since it is a common and serious sleep-disordered breathing (SDB). In the previous studies, methods for sleep apnea detection using heart rate variability, airflow and blood oxygen saturation, tracheal sound have been proposed, but a method using stroke volume has not been studied. The proposed algorithm consists of detection of characteristic points in continuous blood pressure signal, estimation of stroke volume and detection of sleep apnea. To evaluate the performance of algorithm, the MIT-BIH Polysomnographic Database provided by Phsio- Net was used. As a result, the sensitivity of 85.99%, the specificity of 72.69%, and the accuracy of 84.34%, on the average were obtained. The proposed method showed comparable or higher performance compared with previous methods.

  • The purpose of this study was to offer the preliminary data for the development of forest therapy program. This exploratory study is about the development of forest healing program by identifying what is the forest healing program preferences of potential consumers and would be an important basis for the operation. The survey of interests and needs of 620 people on the forest therapy was analyzed. Using SPSS 21.0 program, statistical analysis, frequency analysis, T-test, One-way ANOVA, we looked at the differences in forest healing program preferences according to gender, age, occupation. Forest therapy program preference was found to vary depending on sex, age and job. First, according to gender, men preferred athletic in the forest and camping compared to women, and women showed a higher preference than men in the overall program. Second, depending on age, as compared to other age, for 20s; stress assessment & diagnosis, cooking in the forest, photo therapy in the forest, for 30s; walking in the forest, counseling, listening to lectures(stress-related or interpersonal relationships-related), communication-related lectures and vision quest, for 40s; meditation, viewing the forest, forest bathing wind bathing sun bathing, walking on barefoot in the forest, for 50s; breathing breathing exercises, yoga, mountaineering, climbing in silence, eating wild food, respectively, each of those programs were especially favored. Third, the forest healing program preference in accordance with the job is as follows. For students; stress assessment & diagnosis, cooking in the forest, etc., for teachers; walking in the forest, mountaineering, reading in the forest, viewing the forest, forest bathing wind bathing sun bathing, camping etc., for housewives; yoga, listening to the sound of water flowing, drinking herbal tea, eating wild food, for specialist researcher; breathing breathing exercises, climbing in silence, meditation, sleeping in the forest, respectively, each of those programs were especially favored. We expect the results of this study to be utilized as basic data for the development of forest therapy program targeting on adults.

  • Pure red cell aplasia is unusual cause of anemia and a selective aplastic disorder that affects the erythroid series of the bone marrow. Fifty percent of all patients with red cell aplasia will have a thymoma. Twenty-five to 30% of those who undergo thymectomy will be cured. A 57-years-old man was admitted to the medical department of Korea University hospital with complaints. Physical examination reveals a sick looking man with a pale lip, anemic conjunctiva and subicteric sclera. On auscultation, coarse breathing sound and moist rale was heard on the right lung field. Neither the liver nor spleen was palpable. A blood count showed the erythrocytes to number 2,640,000/mm3 and hemoglobin to be 7.0gm/dl. A white blood cell count was 5,000/mm3 and a platelet count was 328,000/mm3 Reticulocyte count was 0.7%. Examination of the peripheral blood smear showed the red cell, to be normocytic and normochromic. Urine sugar was three positive and GTT was positive. The anterior-posterior and lateral view of Chest X-ray was suggestive of an anterior mediastinal mass. A bone marrow biopsy reveals absence of red cell precursors and a normal myeloid series and megakaryocytes. At thoracotomy in May 1980 an encapsulated, lobulated, benign thymoma, which measured 5x7x5 cm was removed, microscopic examination showed it was of the spindle cell type. The postoperative course was uneventful, but the patient never had a return of hemoglobin to the blood. The patient was discharged on the postoperative] 3 days. At postoperative 1 month, the patient was readmitted for bone marrow study and had no return of red cells to bone marrow. At now, patient has been treated with steroid and the further follow up study will be needed.
