GENERAL ANESTHESIA AND DEEP SEDATION FOR THE SPECIAL NEED DENTAL PATIENTS (장애인의 치과치료 시 외래전신마취와 진정법)

  • This article discusses the issues of benefit and risk associated with outpatient general anesthesia and deep sedation for the special need dental patients. The purpose of administering anesthesia for patients with special needs is to provide effect dental treatment. But there are many complications such as sore throat, nausea and vomiting, airway trauma, airway obstruction and hypoxic brain damages, etc. In order to decrease incidence of complication, before general anesthesia meticulous patient evaluation is much important. But, there are a number of factors that make it difficult to accurately assess the anesthetic risk for many people with special needs. These include limited medical workups, uncooperative behavior, and difficulties in postoperative cares, etc. But Judging from several years experience of many contries, it appears that the incidence of mortalities for people with special needs in dental setting is minimal and the incidence of morbidity is limited. In the long run, the delivery of general anesthesia and deep sedation for people with special needs can be considered a very safe and successful procedure.

  • Objective: This study is performed for the purpose of observing the effect of JSD on the cough Methods: 35 patients with the cough treated with JSD herbal acupuncture at 肺兪穴(BL13) and 平喘穴, then self-conscious complain rates were studied. Results: The result as follows. 1. There are remarkable improvement in self-conscious complain rate about chronic cough, sputum, sneezing, runny nose, sore throat, shortness of breath which chronic cough patients complained. 2. There is no significance in self-conscious complain rate according to suffering and treatment period. 3. These result indicates that the effect of JSD herbal acupuncture on chronic cough si exellent.

  • Styloid process is a slender pointed bone of variable length, which project downward, forward, and slightly medialward from the posteroinferior portion of the tympanic part of the temporal bone. Embryologically, the styloid process is derived from Reichert's cartilage, a structure of second branchial arch origin. Most patients with elongated styloid process remain asymptomatic, but some patients complain pain and sensation of foreign body in the throat, dysphagia, dysphonia, referred otalgia. Recently, we have experienced a case of elongated styloid process in a fifty-nine year old man. He had been suffered from Rt. side sore throat, headache and pharyngeal discomfort during head movement. Through various X-ray examination, Xeroradiography, and palpation of tonsillar fossa, elongated styloid process on Rt. side was confirmed. Under the general anesthesia, it was successfully removed out via transoral approach technique, described by Eagle. The resected styloid process was 2.5cm in it's length and he was freed from the symptoms without further complication.

  • The detection of foreign bodies in the upper-aerodigestive tract is a fairly frequent event and can occur in various areas and for various reasons. In rare cases, foreign bodies can simulate a neoplasia. We evaluated similar cases during emergency regimen with an oral cavity mucosal lesion, causing lockjaw, sore throat, dysphagia, and swelling of the submandibular and laterocervical region. Physical examination revealed an extensive mucosal ulceration in the floor of the mouth and the lateral surface of the tongue, comparable to oral cancer. During a second, more accurate assessment, a partially deteriorated iron supplement tablet was found embedded in a mucosal pocket. After removing the tablet, gradual normalization of the tissue was observed without any sequelae. This is one of the many reasons why it is advisable and useful in cases of oral lesions to collect a detailed medical history and to perform an accurate clinical evaluation, including inspection and palpation of the lesion, before proceeding to further diagnostic assessments, especially in elderly patients taking many medications. However unlikely, it is possible that difficulty in swallowing pills or tablets could generate tumorlike lesions.

Direct Open Venous Drainage: An Alternative Choice for Flap Congestion Salvage

  • In this report, we present a scalp defect reconstruction with lateral arm free flap. We highlight the difficulty in obtaining a recipient vein and the venous drainage managed through an open end of the donor vein. A 52-year-old woman presented with a pressure sore on the left scalp. A lateral arm free flap was transferred to cover this $8{\times}6cm$ defect. The arterial anastomosis was successful, but no recipient vein could be identified within the wound bed. Instead, we used a donor venous end for the direct open venous drainage. In order to keep this exposed venous end patent, we applied heparin-soaked gauze dressing to the wound. Also, the vein end was mechanically dilated and irrigated with heparin solution at two hour intervals. Along with fluid management and blood transfusion, this management was continued for the five days after the operation. The flap survived well without any complication. Through this case, we were able to demonstrate that venous congestion can be avoided by drainage of the venous blood through an open vessel without the use of leeches.

  • Dizziness is one of the common symptoms in many patients. The disorders of the labyrinth, vestibular nerve, vestibular neclei, or their central connections are responsible for practically all vertigo. Most disorders of the central connections are the vascular diseases and tumors. This study is based on the clinical consideration of one patient who sufferd from the cerebellar infarction and the upper respiratory infection. The patient, 61 - year - old man was diagnosed as the cerebellar infarction has been troubled with dizziness, a headache, a sore throat, a cough etc. The symptoms are classed as the Sanchopungyeul.(上焦風熱) Pungdam(風痰), and we prescribed Qingyanligetang(淸咽利膈湯) for him and his symptoms took a tum for the better.

  • Sodium azide (NaN3) is a white to colorless, crystalline powder that is highly water soluble, tasteless, and odorless. It is used mainly as a preservative in aqueous laboratory reagents and biologic fluids and also as an automobile airbag gas generant. Although it has caused deaths for decades, the toxic properties and effects of sodium azide in humans remains unknown. A 31-year-old comatose female was transported to the emergency department with an empty bottle labeled sodium azide. She developed cardiac arrest 15 minutes after arrival and expired in spite of 30 minutes of resuscitative effort. Subsequently, resuscitation team members incidentally suffered from sodium azide's exposure and developed eye discomfort, skin rashes parasthesias, pruritus, sore throat, and headache.

  • RFID could be applied in the various fields such as distribution beside, circulation, traffic and environment on information communication outside. So this can speak as point of ubiquitous computing's next generation technology. However, it is discussed problem of RFID security recently, so we must prepare thoroughly about RFID security for secure information. In this paper, we proposed a method which could protect private information and ensure RFID's identification effectively storing face feature information on RFID tag. Our method which is improved linear discriminant analysis has reduced dimension of feature information which has large size of data. Therefore, we can sore face feature information in small memory field of RFID tag. Our propose d algorithm has shown 92% recognition rate in experimental results and can be applied to entrance control management system, digital identification card and others.

  • This study investigates the actual domestic and overseas cyclewear wearing conditions for male high school cyclists. The study results provide factors and degree of dissatisfaction as basic data for cyclewear development. This study was conducted on 35 male high school cyclists (freshman to seniors) by a questionnaire and interview method. Study results were: Male high school cyclists considered functionality as most important when they bought cyclewear and the brand they most often bought was a cheap domestic brand. Dissatisfaction with the crotch, thigh circumference and minimum leg circumference was higher than other parts during wearing. A cyclist is more sensitive to wear because they require more lower body motion than other parts. Cyclewear should be: less transformed even by frequent laundering, made of breathable material and use a sewing technique that minimizes air resistance and increases fit the body. Functional pads ergonomically designed with high tactile materials should be developed to prevent 'saddle sore' and groin soreness region that happens because of a lower body bending posture when cycling. A follow-up study is recommended to further develop excellence in cyclewear functionality and dimension suitability for male high school cyclists through the size system.

  • Infectious mononucleosis(IM) Is an acute self-limiting lymphoproliferative disorder associated with infection by the Epstein-Barr Virus(EBV), with the characteristic triad of fever, sore throat, and cervical or generalized lymphadenopathy. And also there are atypical lymphocytes in the peripheral blood. Cytological findings of IM lymphadenitis are characterized by a florid immunoblastic and atypical lymphoid cell proliferation. However, the small number of cases were studied by fineneedle aspiration cytology(FNAC) even though there was a complexity of lymph node pathology. It is important to recognize the reactive pattern of IM that would initiate EBV study and to avoid unnecessary biopsy We studied findings of lymph node FNAC from 4 patients with EBV infection confirmed by EBV-specific serologic studios. All of the cases were positive for viral capsid antigen(VCA) and one case was positive for anti-EBV nuclear antigen(EBNA). Cytologically, all of the cases exhibited high cellularity and atypia with great numbers of large immunoblastic lymphocytes.
