  • Image quality is improved if the high frequency domains of boundary surface are made distinct. We suggested a method of correcting indistinct defects in ultrasonic images resulting from dynamic imaging. According to the results of the experiment, boundaries in input ultrasonic images became distinct. However, we need to solve the problem that information on pixels of low signal intensity adjacent to boundaries is lost because high-frequency components in the boundaries are strong.

Correlation of Posterior Echo Patterns and Histopathologic Features in Invasive Ductal Carcinoma of Breast (유방 침윤성 관상피암의 초음파 소견 중 후방 에코 양상과 조직소견의 상호 비교)

  • Traditionally posterior shadowing is regarded as a malignant criterion in the evaluation of breast mass by sonogram. But on the basis of our clinical experiences of breast sonogram, we often met a breast mass without posterior shadowing later confirmed breast carcinoma through pathologic examination. For the focus of what character of pathologic breast tissue influence the posterior shadowing in breast sonogram, we analyzed retrospectively the sonographic findings of 26 histologically proven invasive ductal carcinomas. Even though invasive ductal carcinoma is the only one of the many breast cancers, it represents the greater part of breast malignancy. The posterior echo pattern was compared with various histologic characteristics, such as the amount of connective tissue, degree of elastosis, necrosis, gross circumscription, accompanying inflammation, histologic differentiation, and mitotic index. Nine breast masses (35%) demonstrated posterior echo shadowing, while 17 masses (65%) showed enhancement. The tumors with posterior echo shadowing had more abundant connective tissue, increased elastosis, and poorly demarcated margin (p<0.05). Other histologic characteristics are not influence in posterior shadowing with significant in stastically. On the basis of our study, the phenomenon of posterior shadowing by sonogram is difficult to accept as a specific criterion for malignancy. It is only a phenomenon influenced by the amount of connective tissue volume and elastosis.

One case report of abdominal aortic aneurysm (복부대동맥파류 1예수술치험)

  • There have been several reports of abdominal aortic aneurysm according to increase in life span and greater awareness of the diagnosis in Korea. We had a surgical experience of abdominal aortic aneurysm due to arteriosclerosis in 76 years old male. With abdominal sonogram, he was diagnosed. Aortoiliac bypass was done with double velour Dacron Y graft. Postoperative course was uneventful.

A Correlation Study Using the Sonogram Between the Muscle Strength and the Diameter of Biceps Brachii Muscle (윗팔두갈래근의 근력과 근육직경 간의 상관성에 대한 초음파 영상 연구)

  • Background: The purpose of our experimental study, we want to know a correlation study using the sonogram between the muscle strength and the diameter of Biceps brachii muscle by gender. So we set to hypothesis that Biceps brachii contracted maximally would be a diameter of the muscles increased according to the muscle strength will be increased. Methods: In this study, we measured the muscle strength of biceps brachii on contracting its maximum using the Biodex and the diameter of the muscles using Myo-Lab (25Gold). We experimented 30 subjects of twenty-aged men and women. The subjects is comprised of a persons who has not external injuries and pain of elbow joint, no mobility limitation of ROM, no experiences of surgery in past history. Result: We found that the men have a highly correlation between maximum strength and the diameter of the muscles more than women (p<.05). Conclusion: The results of our experimental study between the muscle diameter and the muscle strength to biceps brachii on relaxation and contraction is a statistically significant relation (r=.394), especially in rather men then women (p<.05). So this results would be based data for further clinical muscle efficiency application in physical therapy field.

A Renal Size Discrepancy among the Findings of Renal Sonogram in Children with Their First Episode of Pyelonephritis is One of the Useful Parameters to Predict the Presence of Cortical Defects on the Acute DMSA Renal Scan

  • Purpose: We investigated whether a renal size discrepancy on a renal sonogram (US) in children with febrile urinary tract infection (UTI) was correlated with the presence of cortical defects on their dimercaptosuccinic acid (DMSA) renal scan. Methods: We examined 911 children who were admitted consecutively to our hospital with their first episode of febrile UTI from March 2001 to September 2014. All enrolled children underwent a US and DMSA scan during admission. According to the US findings, including the renal size discrepancy, data were compared between children with positive and negative DMSA scan results. A positive DMSA scan result was defined as reduced or absent tracer localization and indistinct margins that did not deform the renal contour. Results: Mean renal lengths of the right and left kidneys were larger in children with positive DMSA scan results than in children with negative DMSA scan results ($63.2{\pm}11.3mm$ vs. $58.4{\pm}7.8mm$, P<0.001; $64.9{\pm}11.2mm$ vs. $59.9{\pm}7.9mm$, P<0.001; respectively). A significant difference was observed in both renal lengths between children with positive and negative DMSA scan results ($4.6{\pm}3.8mm$ vs. $3.3{\pm}2.6mm$, P<0.001). A multiple logistic regression analysis, revealed that a small kidney, cortical thinning, and a renal length discrepancy on US findings were significant factors for predicting the presence of cortical defects on an acute DMSA scan [P=0.028, 95% confidence interval (CI) 1.054-2.547; P= 0.004, 95% CI 1.354- 4.810; P<0.001, 95% CI 1.077-1.190, respectively]. Conclusion: In conclusion, a renal size discrepancy on US findings in children with their first episode of febrile UTI was a helpful tool for predicting the presence of cortical defects on an acute DMSA scan.

Post-natal Outcome of Fetal Hydronephrosis Detected with Prenatal Ultrasonography (산전 초음파로 진단된 태아 수신증의 생후 경과 관찰)

  • Purpose : Hydronephrosis constitutes a great portion of fetal anomalies screened by prenatal sonogram. The present authors made an attempt to access its natural courses through follow up neonatal hydronephrosis diagnosed by prenatal sonogram. Methods : The study was composed of 23 neonates(36 renal units) who were diagnosed with hydronephrosis through prenatal sonogram screening and confirmed 3-7 days after birth with sonographic evaluation at Kyung Hee University Hospital. The neonates were closely monitored for 12-24 months with renal sonogram, diuretic renogram, intravenous pyelography(IVP) and voiding cystoureterography(VCUG). Results : The underlying diseases were composed of 16 cases(44%) of functional abnormalities, 14 cases(39%) of ureteropelvic junction(UPJ) obstruction, three cases(8%) of vesicoureteral reflux (VUR) and on case each of multicystic dysplastic kidney, UPJ obstruction with ureteral stenosis and ureterovesical junction(UVJ) obstruction with VUR. The degree of hydronephrosis was divided into three classes according to its severity. In 30 renal units with UPJ obstruction and functional abnormalities, 26(87%) showed mild hydronephrosis, while four(13%) were moderate. During the follow up period, six cases(20%) showed natural resolution of hydronephrosis, 15 cases(50%) showed improvement while two cases(7%) were aggravated with improvement only after surgery of the underlying disease. The cases which showed natural resolution were all mild hydronephrosis at diagnosis and the cases which underwent surgery due to continuous aggravation were mild one case and moderate one case. Conclusion : Those with cases of mild hydronephrosis show rapid natural improvement. On the other hand, in some cases, follow up monitoring reveal aggravation of the situation, emphasizing the necessity for thorough follow up for a long period of time.

Adrenal Hemorrhage in a Neonate (신생아의 부신 출혈)

  • Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRl) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a $3.8{\times}3.0$ cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This finding was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.

Renal Rupture by Cystadenocarcinoma in a Yorkshire Terrier

  • A 13-year-old, spayed female, Yorkshire terrier was presented with a history of vomiting and diarrhea. Radiographic and ultrasonographic studies suggested rupture of right renal cyst. Left kidney had increased cortical echogenecity with indistinct corticomedullary junction on sonogram. Unilateral nephroureterectomy of right kidney was performed. Histopathologic examination of the resected renal mass confirmed a renal cystadenocarcinoma. To the best of our knowledge, it is the first case report of renal cystadenocarcinoma in a small breed dog in Korea.

  • Chyloascities is an extravasation of milky chyle into the peritoneal cavity due to tumor , inflammation or rarely postoperative lymphatic trauma.It is an unusual complication that can lead to significant immunologic and nutritional consequences.We experienced one case of chyloascites after aorto-bifemoral bypass graft in a patient with aorto-iliac occlusive disease.The patient was a 62-year-old male, who suffered from severe progressive claudication for 5 months. A 16$\times$ 8mm gelsealed Dacron-Y shaped graft was used in arterial reconstruction. A bloody-milky fluid was drained through the operative wound from 3days after operation and evaluated biochemically.Diagnosis of chyloascites was made with repeated paracentesis and examination of the fluid.After Total parenteral nutrition[T.P.N] for 3 weeks from 6days after operation, chyloascites was controlled sufficiently and maintained a good graft-patency in abdominal sonogram.

Clinical Experience of Mediastinal Immature Teratoma in a Newborn (선천성 종격동 기형종(신생아에 발생한 미성숙 기형종) 치험 1례 보고)

  • The teratomas seem to be the most common mediastinal tumor in Korea. However, the immature teratomas are the rarest type of these mediastinal teratomas. A case of immature teratoma of the mediastinum occurring in a newborn male is reported. He suffered from severe dyspnea since birth. Simple chest X-ray and chest sonogram revealed huge mass shadow in fight whole lung field. On 6th day after birth, the operation was done and the mass was completely excised. Biopsy showed elements of mature tissues derived from 3 germ cell layers including immature neuronal components. The baby is well without evidence of disease till now.
