Patients' Selection for Treatment of Caliceal Diverticular Stones with Extracorporeal Shock Wave Lithotripsy (신장게실결석 치료를 위한 체외충격파쇄석술 적용 환자의 선택)

  • Symptoms of caliceal diverticular stones are commonly associated with pain, recurrent urinary tract infection and hematuria. The aim of this study is to select the proper patient for the application of more successful extracorporeal shock wave lithotripsy(ESWL) as a treatment of caliceal diverticular stone. 16 patients with caliceal diverticular stones were treated with ESWL, and all patients had single caliceal diverticulum. The diagnosis of caliceal diverticulum with stones was made by intraveneous pyelography to all patients. On these intravenous pyelogram, we also classified diverticular type, whether the diverticular neck is connected with urinary tract patently, diverticular site and stone number and size. All patients were followed after ESWL by plain film of the kidneys, ureters and bladder and interviewed. Of all patients 44% was shown stone-free completely, also 83% was rendered symptom-free. All patients whose diverticular neck connected with urinary tract patently on the intraveneous pyelogram became stone- free. Of solitary stone 60% and multiple stones(more than 2) 17% became stone free. All patients rendered stone-free became symptom-free, and of patients with residual stones 44% became symptom-free. The patients with infection before ESWL 75% had residual stones, of these patients 33% had slightly flank pain, and 25% of patients with stones recurred become stone-free. We propose that more successful ESWL for patients with caliceal diverticular stones select satisfactory patients including that the diverticular neck is connected with urinary tract patently, solitary stone and no infection simultaneously.

Solitary Wave-like Ship Induced Waves and Its Associated Currents in a Water Channel of Narrow Width (협수로에서 생성되는 고립파 형태의 항주파와 항주파류)

  • In the narrow water channel, which has been frequently deployed in the artificial canal in the South Korea due to the lack of available land, solitary wave type ship induced waves can occur. In order to test this hypothetical view, we carried out the numerical simulation. Numerical model consists of Navier-Stokes Equations and VOF, and the verification is implemented using the data by PIANC (1987) and the analytical model derived in this study. It was shown that numerically simulated front wave height are much larger than the one by PIANC (1987), and the fluctuation of free surface near the channel bank persists much longer (around 20s). For the case of stern waves, numerically simulated wave height are somewhat smaller than the data by PIANC (1987). These results seriously deviates from the general characteristics of ship induced waves observed in the wide water channels, and leads us to conclude that ship induced waves is severely affected by the width of water channel. It was also shown that the currents from the channel banks toward a ship, and currents from the ship toward the channel banks are alternatively occurring due to reflection at the channel banks. The velocity of currents reaches its maximum at 0.90 m/s, and these values are sustained through the entire depth. which implies that severe scourings at the channel bottom can be underway.

PHOTOELASTIC STRESS ANALYSIS OF IMPLANT SUPPORTED FIXED PROSTHESES WITH DIFFERENT PLACEMENT CONFIGURATIONS IN MANDIBULAR POSTERIOR REGION (하악 구치부에서 임플랜트 배열방식에 따른 임플랜트지지 고정성 국소의치의 광탄성 응력 분석)

  • Statement of problem. More than 70% of patients who need the implant supported restoration are parially edentulous. The principles of design for implant supported fixed partial denture in mandibular posterior region are many and varied. Jurisdiction for their use is usually based on clinical evaluation. There are several areas or interest regarding the design of implant supported fixed partial denture in mandibular posterior region. 1) Straight and tripod configuration in implant placement, 2) Two restoration types such as individualized and splinted restorations. Purpose. The purpose of this study was to compare the amount and distribution of stress around the implant fixtures placed in the mandibular posterior region with two different arrangements and to evaluate the effects of splinting using the photoelastic stress analysis. Material & methods. 1) Production of study model: Mandibular partially edentulous model was waxed-up and duplicated with silicone and two models were poured in stone. 2) Fixture installation and photoelastic model construction: Using surveyor(Ney, USh), 3 fixtures(two 4.0 $\times$13 mm, one 5.0$\times$10 mm, Lifecore, USA) were insta)led in straight & tripod configurations. Silicone molds were made and poured in photoelastic resin (PL-2. Measurements group, USA). 3) Prostheses construction: Four 3-unit bridges (Type III gold alloy, Dongmyung co., Korea) were produced with nonhexed and hexed UCLA abutments and fitted with conventional methods. The abutments were tightened with 30 Ncm torque and the static loads were applied at 12 points of the occlusal surface. 4) Photoelastic stress analysis : The polarizer analyzer system with digital camera(S-2 Pro, Fujifilm, Japan) was used to take the photoelastic fringes and analysed using computer analysis program. Results. Solitary hexed UCLA restoration developed different stress patterns between two implant arrangement configurations, but there were no stress transfer to adjacent implants from the loaded implant in both configurations. However splinted restorations showed lesser amount of stresses in the loaded implants and showed stress transfer to adjacent implants in both configurations. Solitary hexed UCLA restoration with tripod configuration developed higher stresses in anterior and middle implants under loading than implants with straight configurations. Splintied 3 unit fixed partial dentures with tripod configuration showed higher stress development in posterior implant under loading but there were no obvious differences between two configurations. Conclusions. The tripod configuration of implant arrangement didn't show any advantages over the straight configuration. Splinting of 3 unit bridges with nonhexed UCLA abutments showed less stress development around the fixtures. Solitary hexed UCLA restoration developed tilting of implant fixture under offset loads.

Ultrastructural Study on Development of the Superior Cervical Ganglion of Human Fetuses (인태아 상경신경절 발육에 관한 전자현미경적 연구)

  • The development of the superior cervical ganglion was studied by electron microscopic method in human fetuses ranging from 40 mm to 260 mm of crown-rump length (10 to 30 weeks of gestational age). At 40 mm fetus, the superior cervical ganglion was composed of clusters of undifferentiated cell, primitive neuroblast, primitive supporting cell, and unmyelinated fibers. At 70mm fetus, the neuroblasts and their processes were ensheated by the bodies or processes of satellite cells. The cytoplasm of the neuroblast contained rough endoplasmic reticulum, mitochondria, Golgi complex, Nissl bodies and dense-cored vesicles. As the neuroblasts grew and differentiated dense-cored vesicles moved away from perikaryal cytoplasm into developing processes. Synaptic contacts between the cholinergic axon and dendrites of postganglionic neuron and a few axosomatic synapses were first observed at 70 mm fetus. At 90 mm fetus the superior cervical ganglion consisted of neuroblasts, satellite cells, granule-containing cells, and unmyelinated nerve fibers. The ganglion cells increased somewhat in numbers and size by 150 mm fetus. Further differentiation resulted in the formation of young ganglion cells, whose cytoplasm was densely filled with cell organelles. During next prenatal stage up to 260 mm fetus, the cytoplasm of the ganglion cells contained except for large pigment granules, all intracytoplasmic structures which were also found in mature superior cervical ganglion. A great number of synaptic contact zones between the cholinergic preganglionic axon and the dendrites of the postganglionic neuron were observed and a few axosomatic synapses were also observed. Two morphological types of the granule-containing cells in the superior cervical ganglion were first identified at 90 mm fetus. Type I granule-containing cell occurred in solitary, whereas type II tended to appeared in clusters near the blood capillaries. Synaptic contacts were first found on the solitary granule-containing cell at 150 mm fetus. Synaptic contacts between the soma of type I granule-containing cells and preganglionic axon termials were observed. In addition, synaptic junctions between the processes of the granule- containing cells and dendrites of postganglionic neuron were also observed from 150 mm fetus onward. In conclusion, superior cervical ganglion cells and granule-containing cells arise from a common undifferentiated cell precursor of neural crest . The granule-containg cells exhibit a local modulatory feedback system in the superior cervical ganglion and nay serve as interneurons between the preganglionic and postganglionic cells.

Investigation of Injury Types and Frequency of Bearded Dragons According to Stocking Density (사육밀도에 따른 턱수염 도마뱀(Bearded dragon)의 부상 형태와 빈도 조사)

  • This study was conducted to determine the injury type and frequency of bearded dragons during single and group breeding according to stocking density, for 16 weeks. A total of 14 bearded dragons compromising three groups were selected for this study. Six and four bearded dragons bred in a cage of 45 cm × 45 cm × 45 cm were designed as group A and B, respectively. Group C was made of four partitions (20 cm × 20 cm × 20 cm) of equal size in the kennel, and the dragons, four in number, were reared alone. Injury type criteria were given 1 score each in 6 stages. Most of the injury types in group A were primary tail cutting, which was 1 point, depending on the score, except for 4 scores. The injury type of group A varied, that is 1, 2, 3, and 5 scores, and the pattern lasted for 2 to 9 weeks. After 10 weeks, the dragons showed no response to injury type, except 4 scores at 11 weeks and 2 scores at 14 weeks with a frequency of one, respectively. The representative injury types of the group B were primary tail cutting, which was 1 point, and the scores of the injury type from 2 to 11 weeks were the highest; the frequency of occurrence at 1 and 2 scores was 5 times and 3 times, respectively, and the frequency of occurrence at 3 scores was twice. In addition, 5 scores were found to have the lowest frequency of occurrence. Solitary breeding showed no response to the type of injury for 16 weeks. Consequently, single breeding with an appropriate stocking density did not affect stress, and it could be expected a positive aspect for pet welfare.

Primary Thyroid Lymphoma: Multi-Slice Computed Tomography Findings

  • Background: The objective of this study was to investigate the MSCT characteristics of PTL in order to enhance the awareness of this uncommon entity among both clinicians and radiologists. Materials and Methods: The clinicopathological data and MSCT images of 27 patients with PTL were retrospectively reviewed. The MSCT appearances were classified into three types: type 1, solitary nodule surrounded by normal thyroid tissue; type 2, multiple nodules in the thyroid, and type 3, enlarged thyroid glands with a reduced attenuation with or without peripheral thin hyperattenuating thyroid tissue. Results: The patients were enrolled in the study with a mean age of 68 years (range, 51-86years) and compression symptoms or enlarged cervical lymph nodes at diagnosis. Hashimoto's thyroiditis was in 20 patients. All patients had non-Hodgkin lymphoma of B-cell in origin, including 22 cases of diffuse large B-cell lymphoma (DLBCL) and 5 of low-grade B-cell lymphoma of mucosa-associated lymphoid tissue (MALT). For MSCT appearance, type 1 pattern was observed in 2 patients, type 2 in 8, and seventeen type 3 in 17. The lesions occurred in more than one lobe with a mean maximal transverse diameter of 6.9 cm and an ill-defined margin. Most tumors showed a homogeneous attenuation equal to that of surrounding muscles before contrast and obvious enhancement after contrast. Cervical lymph node involvement and invasion of the trahea and (or) esophagus were mainly observed in patients with DLBCL. Conclusions: PTL should be clinically considered in elder patients presenting with a history of Hashimoto's thyroiditis and cervical lymphadenopathy. The MSCT characteristics of PTL includes a mass diffusely affecting more than one thyroid lobe, isointense to muscle and obvious enhancement before and after contrast. DLBCL, the most common histological subtype of PTL, is associated with a higher invasive tendency.

  • This paper describes the generation and propagation of internal solitary wave in a two-layer fluid system by numerical analysis. Characteristics of interfacial soliton such as wave type, wave height, wave celerity are investigated numerically with respect to an extent of initial disturbance, fluid thicknesses of the two fluids and etc. The difference between the internal wave propagation on sloping beach and flat bottom was also examined. Laboratory experiments were conducted in the wave flume and compared with the results of numerical computation for verification.

Leaf Spot of Cotton Rose Caused by Corynespora cassiicola in Korea

  • A leaf spot of cotton rose(Hibiscus mutabilis) occurred severely in the flower beds of cotton rose around Uiryeong-gun, Gyeongnam Province in Korea. The causal fungus was identified as Corynespora cassiicola on the basis of cultural and morphological characteristics of the fungus. The fungus grew well on potato dextrose agar and the colony color was gray to brown. Conidia were solitary or catenate, obclavate to cylindrical in shape, and pale olivaceous brown or brown in color. They had 420 pseudosepta, and measured $35.2{\sim}173.6{\times}8.8{\sim}19.9{\mu}m$. Conidia germinated as a bipolar type. Conidiophores were pale to mid brown in color, and measured $74.2{\sim}275.6{\times}3.8{\sim}10.8{\mu}m$. Optimal temperature for mycelial growth was $30^{\circ}C$. The fungal isolate grown on PDA showed strong pathogenicity to cotton rose plant. This is the first report on the corynespora leaf spot of cotton rose(Hibiscus mutabilis) caused by Corynespora cassiicola in Korea.

Numerical Wave Tank Technology for Multipurpose Simulation in Marine Environmental Engineering (해양환경공학의 다목적 시뮬레이션을 위한 수치파랑수조 기술)

  • A virtual reality technology for multipurpose numerical simulation is developed to reproduce and investigate a variety of ocean environmental problems in a 3D Numerical Wave Tank(NWT). The governing equations for solving incompressible fluid motion are Navier-Stokes equation and continuity equation. The Marker-Density function technique is adopted to implement the fully nonlinear freesurface kinematic condition. The marine environmental situations, i.e., waves, currents, etc., are reproduced by use of multi-segmented wavemakers on the basis of the so-called ″snake-principle″. In this paper, some numerical reproduction techniques for regular, and irregular waves, multi-directional waves, Bull's-eye wave. wave-current, and solitary wave are presented, and a model test in motion with large amplitude of roll angle is conducted in the developed 3D-NWT, using a overlaid grid system.

Development of Multidirectional Nonlinear Numerical Wave Tank by Naoe-FOAM-SJTU Solver

  • A three-dimensional multidirectional nonlinear numerical wave tank (NWT) based on the Navier-Stokes equations and the Finite Volume Method (FVM) is developed by using the two-phase hydrodynamic flow solver naoe-FOAM-SJTU based on the open source toolbox OpenFOAM. The free surface is capturing with the Volume Of Fluids (VOF). The directional wave including Stokes wave, solitary wave and nonlinear wave are simulated and verified. The multi-directional waves are also simulated with particular wave spectral such as JONSWAP and wave directional spreading function. The obtained numerical results show the capability of the solver to generate different type of multidirectional nonlinear waves accurately. Meanwhile, it implies that the presented NWT can easily extend to model the wave-structures interactions, which will be great help to the offshore structures design.
