영상에 의한 인체의 운동 분석에서 발생하는 오차 중 가장 큰 부분을 차지하는 것은 피부와 같은 연조직의 골격에 대한 상대운동이며 이를 STA라 한다. 본 연구의 목적은 고관절 운동 중 대퇴에서 발생하는 STA를 선형혼합모형을 이용하여 예측하는 것이다. 모형에 포함되어 있는 고정효과는 고관절 회전중심과 마커의 위치로 대퇴 골격에 의한 운동의 효과, 임의효과는 고관절 중심으로부터 각 마커의 편차로 STA에 의한 효과로 각각 가정하였다. 이를 위하여 근골격계 질환 경력이 없는 다섯 명의 남성 피험자를 선정하여 대퇴에 아홉 개의 마커를 부착하여 고관절의 기능적 운동을 수행하였다. 동시에 고속카메라를 이용하여 마커의 3차원 좌표를 얻었다. 이 3차원 위치벡터에 선형혼합모형을 적용하여 임의효과를 예측하였다. 분석결과 다섯 명의 피험자는 비슷한 패턴을 보였다. 고관절에 가까운 지점의 STA가 큰 것으로 나타났으며 작은 크기를 보인 부분은 원위 대퇴의 전방이다.
신생아에서 흔히 발생하는 연조직 종양으로는 신생아의 구개 및 치은 낭종(palatal & gingival cyst of the newborn), 선천성 치은종(congenital epulis), 혈관종(hemangioma), 기형종(teratoma), 화농성 육아종 및 자극성 섬유종(pyogenic granuloma & irritation fibroma)이 있다. 이렇게 신생아의 치조제에서 발생한 연조직 종양은 대부분 외과적으로 절제하여 치료하며, 치료하지 않는 경우 흡인(aspiration)으로 인한 기도 폐쇄 및 호흡 곤란, 구강으로 수유시 불편감, 비강으로 수유시 구토가 불가능해 흡인폐렴(aspiration pneumonia) 유발 가능성이 있다. 본 증례는 신생아의 치조제에 발생한 연조직 종양으로, 외과적으로 절제하여 치료하였으며, 조직 검사 소견상 화농성 육아종 및 자극성 섬유종과 유사하였지만 다핵형 거대세포(multinucleated giant cells)가 관찰되었고 선천적으로 발생하였다는 점에서 차이가 있었다. 이 종물의 병리 기전은 아직까지 명확하지 않으며 어떤 특정 질환으로 포함시키는 데 문제점이 있으므로 이에 보고하는 바이다.
Von Recklinghausen's disease is a systemic hereditary disorder with varied manifestations in bone, soft tissue, nervous system, and skin, the most common of which is the developement of multiple, small, cutaneous tumors with a characteristic histologic picture. Tumors develop after birth and before puberty in most cases, and they increase in number until old age. Malignant neoplasms that complicate multiple neurofibromatosis include gliomas of the optic nerve, astrocytomaas of the cerebral and cerebellar hemispheres, and sarcomas of peripheral nerves (femoral, tibial and intercostal nerves) and somatic soft tissues. Little attention has been paid to the presence of cystic lung disease in association with neurofibromatosis. Currently, most think of thoracic involvement in neurofibromatosis in terms of posterior mediastinal neuroma, pheochrocytoma, meningocele or, less commonly parenchymal pulmonary neurofibromas. Author have experienced 2 cases of Von Recklinghausen's disease. One case developed a hyge malignant Schwannoma in the parietal pleura of left 4th intercostal space and multiple benign neurofibromas (two in intercostal spacees and one in the neck) , and the other has several episodes of pneumothorax resulting from diffuse cystic lung disease which required closed thoracotomy drainage.
Background: Osteoradionecrosis is the most dreadful complication after head and neck irradiation. Orocutaneous fistula makes patients difficult to eat food. Fibular free flap is the choice of the flap for mandibular reconstruction. Osteocutaneous flap can reconstruct both hard and soft tissues simultaneously. This study was to investigate the success rate and results of the free fibular flap for osteoradionecrosis of the mandible and which side of the flap should be harvested for better reconstruction. Methods: A total of eight consecutive patients who underwent fibula reconstruction due to jaw necrosis from March 2008 to December 2015 were included in this study. Patients were classified according to stages, primary sites, radiation dose, survival, and quality of life. Results: Five male and three female patients underwent operation. The mean age of the patients was 60.1 years old. Two male patients died of recurred disease of oral squamous cell carcinoma. The mean dose of radiation was 70.5 Gy. All fibular free flaps were survived. Five patients could eat normal diet after operation; however, three patients could eat only soft diet due to loss of teeth. Five patients reported no change of speech after operation, two reported worse speech ability, and one patient reported improved speech after operation. The ipsilateral side of the fibular flap was used when intraoral soft tissue defect with proximal side of the vascular pedicle is required. The contralateral side of the fibular flap was used when extraoral skin defect with proximal side of the vascular pedicle is required. Conclusions: Osteonecrosis of the jaw is hard to treat because of poor healing process and lack of vascularity. Free fibular flap is the choice of the surgery for jaw bone reconstruction and soft tissue fistula repair. The design and selection of the right or left fibular is dependent on the available vascular pedicle and soft tissue defect sites.
Purpose: There is an increasing amount of evidence that S100B could function as a marker of brain damage. However, the cerebral specificity of S100B has been questioned, so the extracerebral sources of S100B have been paid attention. We performed this investigation to show serum S100B levels after extracranial fracture in patients without current head injury and without prior neurological disease. Methods: At the emergency department, we obtained the blood samples within 6 hours from trauma patients hospitalized with extracranial fractures. S100B levels were compared between one fracture and more than two fractures, and analyzed according to the presence of soft tissue damage. Results: Patients with one fracture and those with more than two fractures did not differ by age (mean, 54.70 vs. 47.03, p=0.130), and there was no significant difference in the male-to-female ratio(33:32 vs. 21:12, p=0.226). In patients with one fracture, the mean value of S-100B was $0.56{\mu}g/L$ (95% CI: 0.35-0.77) whereas in those with more than two fractures, the corresponding value was $1.09{\mu}g/L$ (95% CI: 0.46-1.7, p=0.048). The S100B level of patients with soft tissue damage($1.32{\pm}0.38$) was higher than that of patients without soft tissue damage($0.81{\pm}0.21$), whether one fracture or more than two fractures(p=0.049). Conclusion: We present here that S100B levels were raised in 77% of patients with extracranial fractures without cerebral injury who were hospitalized from the emergency room and that the presence of soft tissue damage contributed to the increased S100B rather than the size of the fractured bone size or the number of fracturest. Thus, this study suggests that soft tissue injury may be considered as an important extracerebral source of S100B.
Purpose: Liposarcoma is the most common soft tissue sarcoma, and usually occurs on the thigh or in the retroperitoneal space, but rarely in the oral region. This report presents a case of liposarcoma of the cheek and includes a review of the literatures. Methods: A 21-year-old woman was admitted with a palpable mass in her cheek of about two years duration, which increased in size gradually initially, but had increased rapidly over the three months. There was no particular pain or tenderness. MRI showed a well-enhanced, well-defined mass, which suspected to be hemangioma. Results: The spherical, well-encapsulated mass was surgically excised. Biopsy results revealed myxoid liposarcoma. FDG PET-CT on the seventh postoperative day, revealed a minimal to mild FDG-uptake soft tissue lesion around the mass defect area without evidence of distant metastasis. The patient is being observed and undergoing radiation therapy. Conclusion: Liposarcoma in the head and neck region is a rare disease, and can be overlooked as a benign tumor without a pathologic diagnosis. Therefore, proper treatment and follow-up are required based on an understanding of this disease.
This is an extermely rare disease and a case of tuberculous osteomyelitis of mandible in a 45-year-old woman was reported. The patient was visited for complaining slight facial swelling on left side and purulent discharge from extracted socket. Roentgenograhic examination of the mandible revealed bone destruction on left side of mandibular body. Destructive lesion was removed with saucerization and specimen from the involved soft tissue and bone were sent for microscopic examination. This case proved to be primary tuberculous osteomyelitis caused by the absence of primary focus. After 3 months follow-up check, we noticed good prognosis of bone and soft tissue healing.
The pathogenesis and mechanism of obstructive sleep apnea (OSA) has been under investigation for over 25 years, but its etiology and mechanism remains elusive. Skeletal (maxillary and/or mandibular hypoplasia or retrodisplacement, inferior displacement of hyoid) and soft tissue (increased volume of soft tissue, adenotonsillar hypertrophy, macroglossia, thickened lateral pharyngeal walls) factors, pharyngeal compliance (increased), pharyngeal muscle factors (impaired strength and endurance of pharyngeal dilators and fixators), sensory factors (impaired mechanoreceptor sensitivity, impaired pharyngeal dilator reflexes), respiratory control system factors (unstable respiratory control) and so on facilitate collapse upper airway. Therefore, OSA may be a heterogeneous disorder, rather than a single disease entity and various pathogenic factors contribute to the OSA varies person to person. As a result, patients may respond to different therapeutic approaches based on the predominant abnormality leading to the sleep-disordered breathing.

Background: Anterolateral thigh (ALT) flaps are versatile soft tissue flaps that have become the standard soft-tissue flaps used for head and neck reconstruction. They provide a long vascular pedicle, constant vessel diameter, abundant soft tissue coverage, and minimal donor site morbidity. The ALT flap was initially designed on the basis of a septocutaneous (SC) perforator. However, more recent research has shown that a substantial number of ALT flaps are now based on musculocutaneous (MC) perforators, and the ratio between MC and SC perforators varies among studies. In this study, we analyzed the perforating pattern of ALT flaps along with their clinical outcomes during head and neck reconstruction in the Korean population. Methods: From October 2016 to July 2020, 68 patients who had undergone an ALT flap procedure for head and neck reconstruction were enrolled retrospectively. The perforating pattern of the cutaneous perforator vessel (MC perforator/SC perforator/oblique branch), pedicle length, and flap size were analyzed intraoperatively. Patient demographics and flap necrosis rates were also calculated. Results: The highest number of cutaneous perforator vessels supplying the ALT flap were the MC perforators (87%). The proportion of MC perforators was significantly higher than that of the SC perforators and oblique branches. Flap necrosis occurred in seven cases (11.86%); sex, hypertension, diabetes mellitus, coronary artery disease, perforator course, and history of radiotherapy did not significantly affect flap necrosis. Conclusion: The ALT free flap procedure remains popular for reconstruction of the head and neck. In this study, we observed that the majority of cutaneous vessels supplying the flaps were MC perforators (87%). When using the MC perforator during flap elevation, careful dissection of the perforator is required to achieve successful ALT flaps because intramuscular dissection is difficult. Perforator pattern and history of radiotherapy did not affect flap necrosis.
면역글로불린 G4(immunoglobulin G4; 이하 IgG4) 관련 질환은 드문 전신성 섬유염증 상태로 IgG4 형질 세포가 풍부한 림프구 침윤과 관련된 기관 비대 또는 종창성 병변을 특징으로 한다. 우리는 48세 여자 환자에서 드문 소견인 연조직 종괴로 발현된 왼팔의 피하층을 침범한 IgG4 관련 질환 증례를 보고하고자 한다. 초음파 및 자기공명영상을 촬영하였으며 악성 또는 염증 병변이 의심되는 불규칙하며 침윤성 경계를 갖는 연조직 종괴가 관찰되었다. 우리는 IgG4 관련 질환의 진단 기준, 조직 병리학적 특징, 영상의학적 특징, 및 치료 방법에 대해 논의하고자 한다.
