Purpose: To understand the pattern of prescribing contact lenses in Korea from 2010 to 2013. Methods: Four hundred survey questionnaires were randomly posted to Korean Optometric clinics for 4 years. The questionnaire was the same form as a form used for International Contact Lens Prescribing Trend. It was also designed to collect information about the contact lenses prescribed to the first 10 patients after its receipt. Results: 1937 fits were used for the analysis among the four hundred survey questionnaires. The average patient age was $26.2{\pm}7.0$ in 2010, $26.1{\pm}7.6$ in 2011, $24.7{\pm}6.6$ in 2012 and $24.8{\pm}7.2$ in 2013. The patients were mainly women (75%). Of all the soft contact lenses, silicon hydrogel lenses were 18% of fits (17% in 2010, 10% in 2011, 20% in 2012, 24% in 2013). By degine, the percentage of toric design lenses accounted for 17% of soft lenses fits and had increased during 4 years (10% in 2010, 18% in 2011, 16% in 2012, 22% in 2013), and cosmetic contact lens made up for 29% (15% in 2010, 20% in 2011, 40% in 2012, 41% in 2013). Conclusions: The contact lenses wearers were mostly women and the average patient age becomes slightly younger. In relation to the increase in the mumber of younger female contact lens wearers,the percentage of cosmetic lenses prescription was much higher (29%) than the global average (7%), and it has gradually increased for last 4 years. This trend could be related to the growth of beauty market in Korea. By emphasizing on toric lens marketing in manufactures, the prescription rate has been gradually increased.

Purpose: In the present study, the effect of artificial tears with different ingredients on tear film stability and subjective symptoms of dry eyes in the early stage of soft contact lenses wear was investigated. Methods: The three kinds of artificial tears and saline solution were respectively applied onto 50 dry eyes which wore soft contact lenses made of etafilcon A. Then, non-invasive tear film break-up time (NIBUT) was measured at every five minutes for 30 minutes right after the instillation of artificial tears, and the changes in blink rate and subjective symptoms were estimated after 30 minutes from the instillations. Results: All three kinds of artificial tears increased NIBUT larger than saline. The effect of increasing NIBUT immediately after the instillation of artificial tears was the largest when the artificial tear containing viscosity enhancer was used. The duration time of the effect was the longest when the artificial tear having both surfactant and viscosity enhancer was applied. The blink rate was not significantly changed when both artificial tears and saline solution were instilled. Subjective symptoms were significantly improved by the instillation of both artificial tears and saline solution and dryness, irritation, tiredness, stiffness, dazzling were improved by the use of most solutions. However, the subjective symptoms were rebounded to the level before the instillation of artificial tears after 30 minute-instillation. Conclusions: It was revealed that both artificial tears and saline solution could improve the subjective symptoms of dry eye in the early stage of soft contac lenses however, the effect of these solutions on the tear film stability was different according to the components. From the results, it is suggested that the proper selection of artificial tears is necessary for the improvement of dry eye symptoms at the early stage of soft contact lenses wear in dry eye.

Purpose: The present study was conducted to compare any difference caused by pigmentation in the centrations and movement patterns on the cornea between circle and soft contact lens with identical material and parameters during lens wearing. Methods: Soft and circle contact lenses with identical material and parameters were applied to twenty eyes with normal tear volume for 3 hrs a day during a total of 5 days and then their lens centrations, the moving distances and rotations by repeat blinking were compared. Results: The lens centration beyond the fitting criteria was shown in initial wears of both soft contact lens and circle contact lens. However, the centration of soft contact lens was changed to the pupil center for being suitable to the fitting criteria with longer wearing time. On the other hand, the decentration of circle contact lens in the horizontal direction was still presented even with longer wearing time. The moving distances of soft contact lens and circle contact lens decreased with the expand of lens wearing but were not significantly different between total wearing period and daily wearing time. The rotation of circle contact lens by repeat blinking was significantly different from that of soft contact lens even when the wearing period was extended. Conclusions: We revealed that the lens centration and movement of circle contact lens on cornea were different from those of regular soft contact lens resulting in bigger difference with the expand of wearing period in the study.
Purpose: To evaluate the changes of refractive power when worn soft contact lenses were temporarily removed. Methods: 91 soft contact lens wearers (15 males and 76 females; total 182 eyes) from 17 to 39 years of age (average: $24{\pm}4.8$ years) were participated. Objective and subjective refraction, and corneal radius were measured at 0, 30, 60 and 90 min after lens removal. The changes in refractive power were evaluated between measurements over time. The other parameters such as types of lenses, fitting and wearing conditions were also assessed. Results: Objective refraction, subjective refraction and corneal radius were significantly changed according to measured time (p<0.0001). A moderate myopic shifts was observed at the beginning (30 min after lens removal) and a slight myopic shift at the late of measurement (60 min to 90 min after lens removal). There are no significant differences between lens types, fitting states, wearing time, wearing days and sleeping time in the previous day. However, there was significant interaction in changes for corneal radius between measuring time and lens type (p=0.017), fitting state (p=0.019), and sleeping time prior to the test (p=0.010). Conclusions: Time to reach refractive and corneal radius stability after contact lens removal revealed at least more than 60 min, regardless of types of lenses, fitting and wearing conditions. Therefore, refraction for correction should be performed after waiting for more than that time as possible.

Purpose: To investigate ocular higher order aberrations (HOA) and spherical aberration changes caused by an aspheric soft contact lens designed to reduce spherical aberration (SA) of the eye. Methods: Fifty subjects who have successfully experienced soft contact lenses were refitted with aspheric design (Soflens Daily Disposable: SDD, Bausch+Lomb) soft contact lens. Ocular higher order aberrations (HOA) and stand alone SA were measured and analyzed for a 4-mm pupil size using Wave-Scan Wavefront$^{TM}$ aberrometer (VISX, Santa Clara, CA, USA). High and low contrast log MAR visual acuity and contrast sensitivity function (CSF) were also measured under photopic and mesopic conditions (OPTEC 6500 Vision Tester$^{(R)}$). All measurements were conducted monocularly with an undilated pupil. Results: The RMS mean values for total HOA with SDD contact lenses were significantly lower than those at with unaided eyes (p<0.001) and a reduction for SA in the SDD was close to the baseline SA (zero ${\mu}m$) (p<0.001). For the SDD lens, there was a statistically significant correlation between the changes in the total HOA and the contact lens power (r=0.237, p=0.018) as well as between the changes in SA and the lens power (r=0.324, p=0.001). High contrast visual acuity (HCVA) and low contrast visual acuity (LCVA) with SDD lenses were $-0.063{\pm}0.062$ and $0.119{\pm}0.060$, respectively under photopic and $-0.003{\pm}0.063$ and $0.198{\pm}0.067$, respectively under mesopic condition. Contrast Sensitivity Function (CSF) with SDD lenses under both photopic and mesopic conditions was $3.095{\pm}0.068$ and $3.087{\pm}0.074$, respectively. Conclusions: The SDD contact lens designed to control SA reduced the total ocular HOA and SA of the eye, resulting in compensating for positive SA of the eyes. Thus, the optical benefits of the lens with SA control would be adopted for improving the quality of vision.
Purpose: The material properties of contact lenses were measured by varying the amounts of an initiator and a cross-linking agent that are the basis for the manufacture of contact lenses, and the drug delivery effects depending on the material properties of contact lenses were compared. Methods: Contact lens samples were manufactured using HEMA by varying the concentration of the cross-linking agent and the initiator. To investigate the changes in physical characteristics depending on the material properties, the results of the experiments on the amount of drug elution, water content, refractive index, and the amount of protein adsorption were compared. Results: For the contact lenses manufactured by varying the amount of the initiator, the water content hardly changed, and the refractive index also hardly changed. The amount of drug elution was not much affected by the changes in the initiator, but the amount of elution increased as the water content increased. The amount of protein adsorption was hardly affected by the changes in the initiator, but the amount of adsorption increased as the water content decreased. Conclusions: The changes in the properties were hardly affected by the changes in the amount of the initiator, but were significantly affected by the changes in the amount of the cross-linking agent. As the amount of the cross-linking agent increased, the water content decreased, while the refractive index increased. Also, when the water content increased, the amount of drug elution increased, while the amount of protein adsorption decreased.
Purpose: The purpose of this study was to compare the accommodative function of young adult in their 20s wearing monovision, modified monovision, and aspheric multifocal contact lenses at near task. Methods: Thirty young adults ($23.53{\pm}2.37years$) were fitted with monovision, modified monovision (the application of single vision contact lenses and center-near low addition aspheric multifocal contact lenses), and aspheric center-near multifocal contact lenses. After wearing these modalities during a week for adaption, and after watching visual display at computer for inducing accommodative pressure for 1 hour. The following assessments of accommodative function were made: contrast visual acuity (VA) at distance and near; accommodative response; near accommodative facility; and negative relative accommodation (NRA)/positive relative accommodation (PRA). All measurements were carried out binocularly. Results: In binocular distance VA with contrast of 10%, monovision was the worst among the four modalities (p=0.005). In accommodative response at 1 m (1.00 D), monovision was the lowest (p<0.05) and accommodative response at 40 cm (2.50 D) with monovision was lower than that of modified monovision and multifocal contact lens (p<0.05). We also found that there were no significant differences in accommodative facility and NRA/PRA among the four modalities. Conclusions: In young adult (20s), monovision with low add reduced the accommodative response at near task, however, modified monovision and multifocal lens with center-near type did not affect accommodative relaxation.
The study investigated the effects of base curve radius art the fit of thin, mid-water contact lenses. It was found that central corneal curvature(as measured with the keratometer) was not predictive of the best fitting base curve. Proper lens fit may be the single most important factor that ultimately determines the success of contact lens wear. Comfort, vision, and physiological response are all dependent on the fit of the lens. The percent of optimal fits was highest with the 8.4 mm base curve lens for all three ranges of keratometry values. When fit with the 8.4 mm lens. For most eyes, fitting a flatter lens led to greater decentration, decreased comfort, and no increase in lens movement. The 8.4 mm lens was found to provide on "optimal" fit in over 60% of eyes tested and a fit of "good" or "better" in nearly 90% of eyes tested. Comparisons of different manufactures' lens found that similiar lenses do not always fit in the same way due to subtle design and production differences. Therefore, different products may require different base curve radii to fit the same patient. This is even true when water content, center thickness, and diameter are approximately the same. A praditioner fitting a new patient in this lenses should begin with the 8.4 mm base curve radius.
Purpose: The study was performed to compare the differences in lens rotation, lens movement by blinking and lens centration with alignment or steep fitting of soft contact lens in normal and dry eyes. Methods: Total 40 eyes (aged 20~30 years) were classified into the normal (n=20) or dry eye group (n=20) by the diagnosis methods for dry eyes and worn soft contact lens (polymacon material) with alignment fitting or steep fitting. Lens rotation, lens movement by blinking and lens centration were separately measured immediately after lens wearing and after stabilization of tear film and compared by fitting states of soft contact lenses. Results: With steep fitting of soft contact lens in dry eyes, averaged lens rotation immediately after lens wearing was not significantly different from that of the normal eye group with alignment fitting however, lens rotation after stabilization in dry eyes was significantly larger than that in normal eyes. Any significant difference in lens movement by blinking was not shown in normal eyes. However, lens movement by blinking in dry eyes was increased with steep fitting. The range of lens centration on cornea in normal eyes with alignment fitting was more vertically distributed. On the other hand, the range of lens centration on cornea in dry eyes with alignment fitting was more horizontally distributed. Lens centration was shown to be changed by stabilization of tear film. That is, lens centrations were somewhat vertically widespread immediately after lens wearing and restrictively distributed in horizontal direction, respectively, with steep fitting in dry eyes. Conclusions: These results suggested that lens movements and centration in dry eyes were different from those of normal eyes. Especially, those differences between normal and dry eyes were much bigger with steep fitting of soft contact lenses. Thus, those differences should be considered for the comfortable and safe fitting of soft contact lens in dry eyes.
Purpose : The purpose of this study was to investigate subjective symptoms according to the wearing, purchase and fitting status of soft contact lens wearers, and consumer perception of base curve. Methods : A survey was conducted for those who visited optical shops and lens shops in Seoul from June to August 2018. A total of 98 answer sheets they submitted were used for the analysis. The chi-square test and Fisher's exact test were used to compare subjective symptoms (dryness, glare, uncomfortable fitting, and decreased vision) according to whether fitting condition is screened (case history and push-up test, ect relevant to wearing sensation), and the odds ratio (OR) was obtained by the logistic regression analysis. Results : When the contact lenses were purchased, the rate of which the visual acuity test was performed was 86.6% at optical shop and 64.7% at lens shop. When purchasing contact lens and the contact lens was not tested for fitting, they more experienced dryness (OR 4.41, 95% CI 1.25-15.62) and uncomfortable fitting (OR 2.68, 95% CI 1.08-6.64) than testing for fitting. In addition, it was investigated that 87.8% of contact lens wearers did not know about the term base curve, 92.9% did not listen to an explanation related to the base curve when purchasing contact lenses, and 96.9% did not experience with base curve test. Conclusion : It would be thought that satisfaction of the contact lens fitting of existing consumers should be improved by changing a proper base curve by confirming the fitting condition when prescribing soft contact lens.
