The Study about The Discourse on The Medications and Prescriptions on The DongyiSooseBowon Gabobon (" 동의수세보원(東醫壽世保元) 갑오본(甲午本)의 약방(藥方)"에 관한 연구(硏究))

  • 1. Background and Purpose At the year of 2000, the society of Sasang Constitutional Medicine acquired ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$ which is seen as a written copy of ${\ulcorner}$GaboBon${\lrcorner}$. According to Lee Sung-su who is the great-grandson of DongMu's older brother, ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$ was worked by Lee Jin-yoon who is the grandson of DongMu's older brother, and it was copied by Han Min-gab and now is owned by Lee Sung-su who is son of Lee Jin-yoon. 2. Methods This paper was written in order to understand of the character of the discourse on the medications and prescriptions of the ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$. 3. Result and Conclusion 1) Newly created prescriptions of ${\ulcorner}$GaboBon${\lrcorner}$ is composed of the 125 medications of Soeumin 46, Soyangin 36, Taeumin 29, Taeyangin 14. Regarding with that prescriptions, the average number of the medications per prescription is 9.3 and the average capacity of that is 42.6gram. 2) The experimental prescription is from ${\ulcorner}$GaboBon${\lrcorner}$. All the 37 prescriptions of ${\ulcorner}$Sanghanlon${\lrcorner}$ is found at the text of ${\ulcorner}$GaboBon${\lrcorner}$, and only 12 of 37 prescriptions of Traditional Medicine excepting ${\ulcorner}$Sanghanlon${\lrcorner}$ is described at the text of it. Hardly any case of treatment by Traditional Prescription can be found at the discourse on constitunal symptoms and diseases. 3) Only 13 of 69 newly created prescriptions is not prescribed at ${\ulcorner}$GaboBon${\lrcorner}$, and 7 of the 13 prescriptions don't have the symptom of charge also. 4) 6 prescriptions which are supposed to newly created prescriptions of ${\ulcorner}$The Discourse on Constitutional Symptoms and Diseases${\lrcorner}$ are not describe to the composition of medications. 5) All the newly created prescriptions of ${\ulcorner}$The General Remarks on the Soeumin${\lrcorner}$ of ${\ulcorner}$SinChucBon${\lrcorner}$ are to be regarded as GaboBon's.

A Case Report of a Tracheostomized Patient with Stroke Suffering from Sputum Secretion Treated with Herbal Prescriptions for dispelling Dam-eum (기관절개술을 시행한 중풍 환자의 객담에 대한 담음(痰飮) 처방 투여 1예(例))

  • This is a case report of a tracheostomized 80-year-old man suffering from sputum after stroke. His symptoms were watery sputum production requiring frequent sputum suction. The subject was diagnosed as having a deficiency of spleen qi and was treated with Gami-ijung-tang, Yukgunja-tang, Soeumin Bojungikgi-tang, and Ijin-tang extract in the herbal prescription known to dispell Dam-eum (phlegm-retained fluid). Frequency of ssputum suction and condition of sputum were checked everyday. Gamiijung-tang, Soeumin Bojungikgi-tang with Ijin-tang extract led to improvement by reducing sputum secretion. There were limitations on frequency of suction and condition of sputum which were thought to be derived from tracheostomy, however. Consequently, pathology and treatment of sputum will be different whether tracheostomy was performed or not.

A Clinical Study of Soeumin Doksamkwankaeijungtang (독삼관계이중탕(獨參官桂理中湯)의 소음인(小陰人) 인후통(咽喉痛) 치험례(治驗例))

  • 1. Background and Purpose I intented to be helpful to understand the consitutional symptoms of soar throat and the application of the prescription through a clinical study of Soeumin Doksamkwankaeijungtang. 2. Methods I studied 16 patients(1 man and 15 women) treated by Doksamkwankaeijungtang in Chewon Oriental Clinic. And then I investigated the characteristics of patients treated by Doksamkwankaeijungtang, remedial values and side effects. 3. Results Doksamkwankaeijungtang was used in the age groups between twenties and sixties. There were 28 kinds of chief complaints including soar throat, tonsilitis, allergic rheum, bitter taste, dry throat, dry cough, stomatitis, lower abdominal pains, climacteric syndrome, etc. The distributions of remedial values were completely recovered (56.25%), much improved (18.75%), slightly improved(6.25%), no improved(12.5%), aggravated(6.25%), side effects(18.75%). There was one aggravated patient. Using the prescription, I prescribed it without any changing and addition according to the symptoms of soar throat. The remedial value of Doksamkwankaeijungtang was effective and not influenced by the dosage of Ginseng. The onset of action was very quick by the changes of dosage without the relation of age. Side effects were upward tendency of blood presser, headache, chest pain, etc. 4. Conclusion Doksamkwankaeijungtang was specifically effective in soar throat of Soeumin.

The Current Status about Sleep Disorder in the Journal of Oriental Neuropsychiatry for Evidence Based Medicine (한방신경정신과 영역의 수면장애 관련 연구현황)

  • Objectives : Sleep Disorders are very common in clinical stages and there are many reports and papers regarding this subject. I try to understand the present state of sleep research in JON (The Journal of Oriental Neuropsychiatry) for the benefits of clinical practice. Methods : We searched and read every article on JON from 1990 to 2012 and selected ones that are related to Sleep Disorder; then analyzed the data to 5 categories, like literature study, case study, Clinical Data-Analysis study, experimental study, and clinical trial. Results : 1. We Found 41 articles relating to sleep disorder in JON form 1999 to 2012, and there were 9 literature studies, 13 case studies, 13 Clinical Data-Analysis studies, 4 experimental studies, and 2 clinical trials. 2. There were Sa-Am Acupuncture Method, Pharmacopuncture, electroacupuncture, and Auricular acupuncture to treat Sleep Disorder. 3. There were many additional treatments like Purgative Therapy, etc. 4. They reported the ordinary sleep pattern of Soeumin and Soyangin, too. 5. In the experimental study, they reported several herbs, herbal prescription on the expression of melatonin receptors, and Punsimgeimgamibang on insomnia, anxiety, emotionality in rats. 6. There were 2 clinical trials, one for Hwabyung-patient with insomnia by acupuncture treatment and another for insomnia in the elderly by ETF-I program. 7. There were 2 case of other sleep disorder (enuresis, sleepwalking) and 2 literature study of dream. Conclusions : In JON regarding sleep disorder, the majority was due to insomnia. Therefore, we also have to expand our sight into other sleep diseases. We need more clinical trials and experimental researches for the construction of EBM Sleep Disorder in Oriental Neuropsychiatry.

The study about Sasang Constitution diagnosis and research trends in the case reports (증례논문에 나타난 사상체질진단의 특징 및 연구동향에 대한 연구)

  • 1. Objectives : We analyzed the case reports published by the journal of Sasang Constitutional Medicine, to study Sasang Constitution diagnosis and characteristic of clinical practice. 2. Methods : It has been examined 138 case reports of Sasang Constitution society published from 1994 to 2008. We analyzed. patient's number, publication year, distribution of Sasang Constitution, title's characteristics and bases of constitution diagnosis. 3. Results and conclusions : 1) In the analysis of patient's number, 1 case report was almost part of the case reports, and the case reports ware many published after 2001. 2) In the analysis distribution of Sasang Constitution, Soyangin was the most Sasang Constitution, and Tae-umin was the next one and Soeumin, Taeyangin followed them. 3) In the analysis of the journal's title, Sasang Constitution, disease and symptom of Sasang Constitution were more important than prescription recently. 4) In the analysis of the journal's purpose, unique disease and response of treatment were almost part of the case reports. 5) In the analysis of diagnosis bases, subjective bases were more important than objective bases and Sasang Constitutional drug response. therefore we should do researches in objective bases and drug response in the future.

  • 1. Purpose This paper was written in order to understand the concept of ${\ulcorner}$DongMuYooGo YakSungGa${\lrcorner}$ (東武遣稿 藥性歌) which was descending as the material of Sasang pharmacology. So we are planning to make use of the fundamental material of the study on the Sasang Medications and Prescriptions. 2. Methods Through comparing each clause of ${\ulcorner}$DongMuYooGo YakSungGa${\lrcorner}$ on all the sasang -records, we understood that concept. 3 Result 1) ${\ulcorner}$DongMuYooGo YakSungGa${\lrcorner}$ of a literary work of DongMu(東武) which contain special elucidative-method of herb-nature has 18 type of YakSungGa which explain each 27 herbs for the Taeeumin, Soyangin and Soeumin. The period of authoship is supposed to the time just before ${\ulcorner}$DongyiSuseBowon Gabobon${\lrcorner}$ (東醫壽世保元 甲午本)(1894, DongMu 58 years old) or middle of fifties(53-57 years old). 2) YakSungGa of ${\ulcorner}$DongMuYooGo${\lrcorner}$ which is published by the BoGunsung(保健省) of North Korea is supposed to close to original type. The main meaning of that is the herb-nature which works on the weakst point of each Sasangin. And then the herbs were classfied by the diseases and symptoms possessing Bomyungjiju(保命之主), the herb's efficacy is explained at the each constitution. 3) The meaning of ${\ulcorner}$DongMuYooGo YakSungGa${\lrcorner}$ is exposure of the opinion of DongMu's early stage that proposed herb-nature which is applied to the principle of Jang(臟) of Sasangin, reason and purpose to enact newly created prescriptios by the constitutional diseases and symptoms, and the compositional principle of newly created prescription. 4) The selection of herbs or cogution of herb-nature of DongMu's pharmacological opinion of his early stage was changed by accumulating clinical experience. So all the newly created prescriptios of ${\ulcorner}$DongyiSuseBowon${\lrcorner}$ could not be explained by only the herb and the herd-nature on ${\ulcorner}$DongMuYooGo YakSungGa${\lrcorner}$. Therefore the purpose of DongMu's YaksungGa is not explaining the symptoms in charge of each herb or classifying the herb by constitution, but offering the principle of composition of prescription of Sasang, from the necessary herb which possess and expand Bomyungjiju to nature of herb which is applied to the constitution for treating all the diseases and sypmtoms by the heavy or light and slow or speedy.

The Origin, Changes and Compositive Principles of Palmulgunja-tang (팔물군자탕(八物君子湯)의 기원(基源), 변천과정(變遷過程) 및 구성원리(構成原理))

  • 1. Objectives This paper investigates the origin, the progressive changes and the constructive principles of Palmulgunja-tang (八物君子湯). 2. Methods Palmulgunja-tang and other related prescriptions were analyzed in terms of their pathological indications, based on previous literature including "Donguisusebowon Chobongwon (東醫壽世保元 草本卷)", "Donguisusebowon Gabobon (東醫壽世保元 甲午本)", "Donguisusebowon Sinchukbon (東醫壽世保元 辛丑本)" and "Dongmuyugo (東武遺稿)". 3. Results and Conclusions 1) The Palmulgunja-tang most likely originates from Paljin-tang introduced in "Donguisusebowon Chobongwon (東醫壽世保元 草本卷)". Paljin-tang progressively transformed into Seungyangpalmul-tang ("Donguisusebowon Gabobon (東醫壽世保元 甲午本)") and ultimately into Palmulgunja-tang ("Donguisusebowon Sinchukbon (東醫壽世保元 辛丑本)"), a prescription appropriate for usage in the Ulgwang symptomatology (鬱狂證). Also, Seungyangikgi-tang in "Donguisusebowon Sinchukbon (東醫壽世保元 辛丑本)" can be presumed to have been affected by Seungyangpalmul-tang. 2) The variational prescriptions (變方) of Palmulgunja-tang shows increasing Seungyang (升陽) effect in order of Baekhaogunja-tang, Sipjeondaebo-tang, Palmulgunja-tang, and Doksampalmul-tang. 3) Palmulgunja-tang is composed of 8 herbs. Of these, Paeoniae Radix Alba (白芍藥), Glycyrrhizae Radix(甘草), Angelicae gigantis Radix (當歸), and Cnidii Rhizoma (川芎) fortify the Soeumin Spleen Element (脾元). Ginseng Radix (人蔘) and Astragali Radix (黃芪) support the ascension of Yang, whereas Atractlodis Rhizoma White (白朮) and Citrus unshiu (陳皮) encourage the descension of Yin.

A Study on the ${\ulcorner}$DongMuSunSaSaSangYakSungSangHumGoGa${\lrcorner}$'s Origin and Meaning ("동무선사사상약성상험고가(東武先師四象藥性嘗驗古歌)"의 연원(淵源)과 의의(意義)에 대한 연구(硏究))

  • I think that the study on the ${\ulcorner}$DongMuSunSaSaSangYakSungSangHumGoGa${\lrcorner}$ of ${\ulcorner}$DongMuYooGo${\lrcorner}$ is basic material explaining the character of sasang-herb and prescription. Through the study on the ${\ulcorner}$DongMuSunSaSaSangYakSungSangHumGoGa${\lrcorner}$'s origin and meaning, I can take the result as follows. The first, that is composed of total 237 herbs of 90 spleen-herb (Soeumin-herb), 61 kidney-herb (Soyangin-herb), 67 lung-herb (Taeeumin-herb) and 19 liver-herb (Taeyangin-herb). The second, the origin of that is ${\ulcorner}$JeJungSinPyun${\lrcorner}$, ${\ulcorner}$BangYakHapPyun${\lrcorner}$ and ${\ulcorner}$EuiHakIpMum${\lrcorner}$. And DongMu classified traditional YakSungGa as spleen-herb, kidney-herb, lung-herb and liver-herb which is suitable to Sasangin, not created that. So we can know that the first understanding for DongMu' s herb is focused on the traditional YakSungGa. The third, DongMu's unique terminology of spleen-herb, kidney-herb, lung-herb and liver herb show his sight of symptom, disease and pharmacology, that is focused on small JangBu. The forth, through herb-nature gathered from traditional YakSungGa, that purposed to classfying each constitutional herb. So we can know his idea that the choice of herb is not in general efficacy but in personal constitution. As mentioned abovet, that can refer to an herbal argument of DongMu and ${\ulcorner}$DongMuYooGoYakSungGa${\lrcorner}$ that make known to be common can refer to an prescriptional argument of DongMu.

  • 1. Purpose : The correct constitutional diagnosis and the accurate prescription are very important in clinical application of Sasang Constitutional Medicine. Lee Je-ma emphasized that symptom is the best clue to diagnose constitution in "DongYi Suse Bowon". After research the characteristics of each constitution's symptoms and the backgrounds of constitutional prescriptions, this paper is to know the correct clinical application of Sasang Constitutional Medicine. 2. Method : Through the clinical applications of "DongYi Soose Bowon" and "Dongyi Sasang Sinpeun", the characteristics of constitutional symptoms and the application of prescription were researched 3. Results & Conclusions 1) The symptoms of Sasang Constitutional Medicine were came from the Hyung-Sang Medicine(形象醫學) which were important to mind-body equally and from the summarizing spirit of four Qi such as Warm-Hot-Cool-Cold(溫熱凉寒) 2) The symptoms of Soeumin and Soyangin are the Cold-Hot symptoms of ingestive food(水穀) and the treatment of symptoms is to control the ascent-descent of up and down. The symptoms of Taeumin and Taeyangin are the Warm-Cool symptoms of Qi-Yack(氣液) and the treatment of symptom is to control the unfasten-fasten of interior and exterior. 3) The symptoms of Taeyangin are 'Weak Lower part and Firm Upper part symptom'(下虛上實病證) and 'Blood and Yack Exhasted Symptom'(血液俱耗病證), the symptoms of Soeumin are 'Fall Down Symptom'(下陷病證) and Stomach Cold Symptom(胃寒病證), the symptoms of taeumin is 'Dryness Fever Symptom'(燥熱病證) and 'Interior Fever Symptom'(燥熱病證), the symptoms of Soyangin is 'Fire Fever Symptom'(火熱病證) and 'Interior Fever Symptom'(燥熱病證). 4) The characteristics of sasang constitutional symptoms are the exterior-interior symptoms classified with nature-emotion and cool-hot, the inclusive control of exterior-interior symptoms with healthy energy, and the classification of ingestive food symptoms and Qi-Yack symptoms. 5) The characteristics to treat symptoms are the classification of seriousness and obedience, the use herbs according to each constitutions, and inclusive symptoms control.

A bibliographical research of the 『Dongyi Suse Bowon Sasang ChobonGuen』 (『동의수세보원사상초본권(東醫壽世保元四象草本卷)』의 서지학적(書誌學的) 연구(硏究))

  • This research was intended to make the writer and the written times of "Dongyi Suse Bowon Sasang ChobonGuen" clear. Considering the contents of "Dongyi Suse Bowon Sasang ChobonGuen", it was the manuscripts of "Dongyi Suse Bowon". So it was written by Lee Je-ma and it was written when he was in the late of 40s and the beginning of the 50s. The chapter of "The origin of human being" was the manuscript of the "Dongyi Suse Bowon" and it had many clues to understand the view points to recoginze the human being, and the chapter of "The herbology and The prescription" had many drafts of original prescription of the "Dongyi Suse Bowon". 'Bangang-tang' is the original prescription of 'GyejiBanhaSnggang-tang' of "Dongyi Suse Bowon" and 'GumiChyunMunDong-tang' is the original prescription of 'CheongsimYeonja-tang' of "Dongyi Suse Bowon". Compared with the 'The pharmacology' of "Dongyi Suse Bowon", it showed the order when the pharmacology of each constitution was completed. The pharmacology of Soyangin was completed the early ears of his c1inical experience, and secondly that of Taeumin was completed, and that of Soeumin was completed in the end.
