고 나트륨 섭취 노인의 영양지식, 식습관 및 식품섭취패턴 (A Study on Food Frequency, Dietary Habits and Nutrition Knowledge of the Elderly Who Intake High Sodium)

  • 본 연구는 65세 이상 노인을 대상으로 고나트륨 섭취군을 분류하여 노인들의 식습관 개선 및 영양교육을 위한 사전연구로서 이들의 영양지식, 식습관, 식품섭취패턴을 조사하였다. 65세 이상 노인 135명의 조사대상자 중 42.96%인 58명이고나트륨 섭취군에 속하였다. 생활습관에서 음주, 간식횟수, 외식횟수, 수분섭취량에서 고나트륨 섭취군이 유의적으로 높게 나타났으며, 자반류, 젓갈류, 장아찌, 조림류, 국물 섭취량, 간장과 소금의 추가량에 관한 식습관 점수가 고나트륨 섭취군에서 유의적으로 높았다. 고나트륨 섭취군의 나트륨에 관한 영양지식 총점이 남녀노인 모두 유의적으로 낮았으며, 남자노인은 나트륨과 인스턴트식품, 나트륨과 인공조미료에 관한 문항에서, 여자노인은 나트륨과 질병과의 관계, 나트륨 섭취권장량에서 유의적으로 낮았다. 고나트륨 섭취군에서 Na index 14의 김치찌개, 된장찌개, 미역국, 멸치볶음에서의 섭취빈도가 유의적으로 높았다. 또한 인스턴트식품을 통한 총 나트륨 섭취량은 고나트륨 섭취군에서 남자노인 1562.49 mg, 여자노인 1227.82 mg으로 유의하게 높았으며, 특히 면류를 통한 섭취량이 70% 이상이었다. 영양지식 점수가 높을수록 Na index 14 점수(p<0.05), 인스턴트식품을 통한 나트륨 섭취량(p<0.01), 나트륨 섭취 관련 식습관 점수가 낮았다. Na index 14 점수에 대해 세끼 식사의 규칙성, 식사량의 적정성, 천천히 식사함(p<0.01) 항목에서 유의한 음의 상관관계를 나타내었으며, Na index 14 점수가 높을수록 인스턴트 국류(p<0.01), 탕류(p<0.01), 죽류(p<0.01), 스프류(p<0.01), 면류(p<0.01), 레토르트류(p<0.01)의 섭취가 높았다. 본 연구는 65세 이상 노인을 대상으로 하여 나트륨 과잉 섭취 노인의 특성 및 섭취요인을 진단하였다는 점에서 그 의의가 있다고 사료된다. 이에 고나트륨 섭취군을 대상으로 본 연구에서 드러난 문제점을 도출하여 이들의 영양지식, 식습관, 식품섭취패턴이 개선될 수 있도록 구체적인 대안을 제시하고 행동변화에 중점을 두는 영양교육이 요구되어지는 바이다.

The association between dietary sodium intake and obesity in adults by sodium intake assessment methods: a review of systematic reviews and re-meta-analysis

  • BACKGROUND/OBJECTIVES: The scientific evidence of a sodium-obesity association is limited by sodium intake assessments. Our specific aim is to synthesize the association between dietary sodium intake and obesity across the sodium intake assessments as evidenced by systematic reviews in adults. SUBJECTS/METHODS: A systematic search identified systematic reviews comparing the association of dietary sodium intakes with obesity-related outcomes such as body mass index (BMI), body weight, waist circumference, and risk of (abdominal) obesity. We searched PubMed on October 24, 2022. To assess the Risk of Bias in Systematic Reviews (ROBIS), we employed the ROBIS tool. RESULTS: This review included 3 systematic reviews, consisting of 39 unique observational studies (35 cross-sectional studies and 4 longitudinal studies) and 15 randomized controlled trials (RCTs). We found consistently positive associations between dietary sodium intake and obesity-related outcomes in cross-sectional studies. Studies that used 24-h urine collection indicated a greater BMI for those with higher sodium intake (mean difference = 2.27 kg/m2; 95% confidence interval [CI], 1.59-2.51; P < 0.001; I2 = 77%) compared to studies that used spot urine (mean difference = 1.34 kg/m2; 95% CI, 1.13-1.55; P < 0.001; I2 = 95%) and dietary methods (mean difference = 0.85 kg/m2; 95% CI, 0.1-1.51; P < 0.05; I2 = 95%). CONCLUSIONS: Quantitative synthesis of the systematic reviews has shown that cross-sectional associations between dietary sodium intake and obesity outcomes were substantially different across the sodium intake assessments. We need more high-quality prospective cohort studies and RCTs using 24-h urine collection to examine the causal effects of sodium intake on obesity.

  • The aim of this study was to investigate the effects of dietary sodium nitrite and meat on human health. Sodium nitrite in processed meat is known to be one of the main precursors of carcinogens, such as N-nitroso compounds. However, we previously found that processed meat is not the primary source of sodium nitrite; nitrate or the conversion of nitrate in vegetables are contribute to generate more than 70% Sodium nitrite or nitrate containing compounds in body. Although the heavy consumption of meat is likely to cause various diseases, meat intake is not the only cause of colorectal cancer. Our review indicates that sodium nitrite derived from foods and endogenous nitric oxide may exhibit positive effects on human health, such as preventing cardiovascular disease or improving reproductive function. Therefore, further epidemiological studies considering various factors, such as cigarette consumption, alcohol consumption, stress index, salt intake, and genetic factors, are required to reliably elucidate the effects of dietary sodium nitrite and meat on the incidence of diseases, such as colorectal cancer.

Comparison of Nutrient Intakes of Elderly Korean and American Women Using Diet Quality Index

  • Diet quality index DQI) offers a new way of comparing eating habits across populations and across countries. Nutrients and food consumption data from 100 elderly Korean women aged 65 and older were collected in Seoul or Kyunggi-do, Korea by the 24 hour recall method. Diet quality index (DQI) was computed for 1049 elderly women (65 and older) from the 1994-1996 Continuing Survey of Food Intakes by Individuals (CSFII) in the US according to US dietary guidelines, and applied to the diet of elderly Korean women for purposes of comparison. A modified 16-point DQI awarded 2 points each for moderate consumption of fat ($\leq$ 30% energy), saturated fat ($\leq$ 10%), cholesterol ($\leq$ 300 mg/day), sodium ($\leq$ 2400 mg/day), and protein ($\leq$ 100% RDA), adequate intakes of carbohydrate ($\qeq$50% energy) and calcium ($\qeq$ 100% RDA), and plenty of fruits and vegetables ($\qeq$ 5 servings). Criteria were based on US dietary guidelines. Partial scores were given if subjects were close to meeting these cutoff points. Diets with $\leq$ 300 mg cholesterol/day were reported by 97% or the Korean sample and 82% of the American sample, while 90% of the Koreans and 42% of the Americans met the goal of $\leq$ 30% of energy from fat, and 98% of the Koreans and 47% of American met the recommendation of $\leq$ 10% of energy from saturated fat. In contrast, only 8% of the Korean sample met the sodium recommendation of $\leq$ 2400 mg sodium per day, whereas 54% of the American subjects met this goal. The mean DQI scores were 10.1 for the elderly American women and 11.3 for the elderly Korean women. Overall, the elderly Korean diet was more consistent with the US dietary guidelines than the elderly American diet.

어린이 영양지수로 살펴본 지역아동센터 어린이의 식생활과 영양상태 - 경주지역 - (Assessment of Nutritional Status of Children in Community Child Center by Nutrition Quotient(NQ) - Gyeongiu -)

  • The present study was performed to evaluate food behavior and nutritional status of children in a community child center using nutrition quotient according to age and growth index of children. The number of subjects enrolled in this study was 152 including 73 boys and 79 girls. Participants consisted of children from 5 to 12 years old, who visited a community child center in the area. Growth index was assessed through body mass index by height and weight and examined according to age. Subjects were divided into underweight, normal weight, overweight and obese according to questionnaire, which consisted of five categories including balance, diversity, moderation, regularity and practice, were analyzed by SPSS statistical program. Current results show significant differences in the diversity and regularity (p<0.05, p<0.001, respectively). Additionally, four items in the form of 10 questions showed significant differences related to habits of sodium-related food intake. This study design will be a useful tool in the evaluation of food behavior and nutritional status of preschoolers or elementary school children in community child centers, together with food frequency questionnaires related with sodium food intake.

Correlation between sodium intake and obesity with related factors among Koreans: a cross-sectional study on dietary intake and eating habits

  • Purpose: Sodium is essentially required for homeostasis and physiological functions, but excessive sodium consumption increases the risk of obesity and other chronic disorders. Korean studies on the sodium-obesity relationship are limited, and thus, this study was undertaken to determine the nature of the relationship between sodium intake and obesity in Korean adults. Methods: Forty-two participants were divided into 2 groups according to body mass index (BMI, non-obese BMI < 25 kg/m2, obese BMI ≥ 25 kg/m2). Dietary intakes and eating habits were analyzed using 3-day food records and a food frequency questionnaire. Anthropometric data were obtained from bioimpedance results, and fasting glucose and lipid levels were measured. Results: Mean weight, BMI, waist and hip circumferences, and body fat mass were greater in the obese group than in the non-obese group for men and women. Skeletal muscle mass and body fat mass were higher in obese women than in non-obese women. Biochemical data were no different in these two subgroups except triglycerides (TGs), which were higher in obese women. Nutrient intakes were not significantly different in obese and non-obese groups. However, obese men consumed excessive sodium, while obese women consumed slightly more than non-obese women. Obese men preferred salty foods and tended to overeat. Positive correlations were found between sodium intake and weight in men and percent body fat mass (PBFM) in women. Correlation analysis (adjusted for energy intake) of the relation between sodium intake and obesity-related factors showed sodium intake was positively correlated with PBFM and TG in women. Conclusion: This anthropometric and biochemical data analysis emphasizes the need for awareness and interventions to mitigate the health risks of elevated sodium consumption. Our findings should aid future studies on the relationship between sodium and obesity and contribute to preventing and managing this metabolic condition.

Relationship between dietary sodium, potassium, and calcium, anthropometric indexes, and blood pressure in young and middle aged Korean adults

  • Epidemiological evidence of the effects of dietary sodium, calcium, and potassium, and anthropometric indexes on blood pressure is still inconsistent. To investigate the relationship between dietary factors or anthropometric indexes and hypertension risk, we examined the association of systolic and diastolic blood pressure (SBP and DBP) with sodium, calcium, and potassium intakes and anthropometric indexes in 19~49-year-olds using data from Korean National Health and Nutrition Examination Survey (KNHANES) III. Total of 2,761 young and middle aged adults (574 aged 19~29 years and 2,187 aged 30~49 years) were selected from KNHANES III. General information, nutritional status, and anthropometric data were compared between two age groups (19~29 years old and 30~49 years old). The relevance of blood pressure and risk factors such as age, sex, body mass index (BMI), weight, waist circumference, and the intakes of sodium, potassium, and calcium was determined by multiple regression analysis. Multiple regression models showed that waist circumference, weight, and BMI were positively associated with SBP and DBP in both age groups. Sodium and potassium intakes were not associated with either SBP or DBP. Among 30~49-year-olds, calcium was inversely associated with both SBP and DBP (P = 0.012 and 0.010, respectively). Our findings suggest that encouraging calcium consumption and weight control may play an important role in the primary prevention and management of hypertension in early adulthood.

표백방법에 따른 번색된 무수치의 색조 변화에 관한 연구 (A STUDY ON THE COLOR CHANGES OF DISCOLORED NONVITAL TEETH BY THE NONVITAL BLEACHING TECHNIQUES)

  • The purpose of this study was to compare and evaluate the bleaching efficacy of three nonvital bleaching techniques: Walking bleaching, Walking bleaching and heat application, and Walking bleaching and bleaching light application. 36 extracted anterior teeth with intact crowns were immersed in 5 % sodium hypochlorite solution for twenty-four hours to loosen extrinsic debris. Lingual access openings were prepared in all teeth and the pulps were extirpated. The teeth were stored in 5% sodium hypochlorite for twenty-four hours to open the dentinal tubules and they were stained via whole blood. Once the teeth were stained, they were evaluated with Colorimeter. The teeth in each group were ranked from lightest to darkest and divided equally into three experimental groups in which the following bleaching techniques were used. Intracoronal base was placed 2mm below cementoenamel junction. Group 1 Walking bleaching (Superoxol + Sodium perborate) Group 2 Heat application + Walking bleaching (Superoxol+Sodium perborate) Group 3 Light application+Walking bleaching (Superoxol+Sodium perborate) The bleaching agents were changed every 3 days and the teeth were bleached for a total of 14 days. The teeth were evaluated with Colorimeter before the start of any bleaching and on day 14. The results were as follows: 1. At the end of 14 days, all the sample teeth demonstrated the increase of Lightness Index at cervical 1/3 of crown (p<0.05) Lightness Index was significant difference in group 2 and 3, but there were some minor differences among groups (p>0.05). 2. In all groups, there was significant difference in red chromacity (p<0.05), but there were some minor differences among groups (p>0.05). 3. In all groups, there was no significant difference in yellow chromacity (p>0.05).

광주지역 성인의 나트륨 배설량과 비만의 관계 (Association between Sodium Excretion and Obesity of Adults in Gwangju)

  • Purpose: The aim of this study was to analyze the association between sodium excretion and obesity for healthy adults in the Gwangju area. Methods: The participants included 80 healthy adults aged 19 to 69 years in Gwangju. The dietary intake and sodium excretion were obtained using the 24-hour recall method and 24 hour urine collection. The participants were classified into two groups according to the amount of urinary sodium excretion: (${\leq}141.75mmol/dL$, > 141.75 mmol/dL). Results: After adjusting for sex, age, smoking history, and income, the high excretion of sodium group was significantly higher for weight, body mass index, body fat mass, percent body fat, visceral fat area (VFA), waist circumference, hip circumference, and WHR. The energy and nutrients intake were significant after adjusting for sex, age, smoking history, and income. The LSE group had a significantly higher fat intake and Na/K intake ratio. The HSE group had significantly higher fiber intake, and K intake. As the amount of urinary sodium excretion increased, the risk of obesity before correction was 3.57 (95% CI: 1.13-11.25) times greater, and the risk of obesity of T3 increased significantly by 3.33 times (95% CI: 1.05-10.59). After correcting for sex and age, the obesity risk of T2 increased significantly by 4.23 times (95% CI: 1.11-16.06), and after correcting for sex, age, smoking history, and income, the obesity risk of T2 increased significantly by 6.81 times (95% CI: 1.44-32.19) the risk of obesity. Conclusions: An association exists between sodium excretion and obesity in Korean adults. In this study, the high excretion of sodium group was obese and the risk of obesity was higher than the low excretion of sodium group.
