Implementation of ISO45001 Considering Strengthened Demands for OHSMS in South Korea: Based on Comparing Surveys Conducted in 2004 and 2018

  • Background: According to the previous studies, the work-related accident rate decreased in Korea after the introduction of occupational health and safety management system (OHSMS), but there were several disasters in Korea such as subway worker's death at Guui station in 2016 and the Taean thermal power plant accident in 2018, which escalated the social demand for safety. In 2018, OHSMS became an international standard, as ISO45001 was announced. Methods: A survey was conducted to research the implementation status of OHSMS and changes in people's perception, and the results were compared with those of a past survey. Results: Enhanced social demand and various stakeholders' (not only buyer) needs, and social responsibility are perceived as the motivation for the introduction of OHSMS rather than legal compliance or customer demand. In the questionnaire about problems with the implementation of OHSMS, the factors with higher response rate in 2018 than 2004 were "excessive cost" and "complicated documentation management." In the questionnaire about how to promote OHSMS in organizations, most people answered "reduction of workers' compensation insurance rate" in 2004, but most people answered "exemption from health and safety supervision" in 2018. Conclusion: For the effective implementation of ISO45001, emphasis is placed on social demand, training to recognize health and safety as a part of management, and the reduction of certification and consulting costs to promote the introduction of OHSMS. Incentives such as insurance premium cuts and exemptions from health and safety supervision are needed.

입원환자 본인부담액에 영향을 미치는 요인 (Factors Affecting Cost-Sharing Charges for Inpatients)

  • In order to strengthen assurance of National Health Insurance, co-payment should be reduced. This can happen with collaborative efforts of patients, medical institutes, and government altogether at the same time. This research applied Dutton(1986)'s medical service research model with high R-square, and analyzed 2008 Korea Health Panel Data (Beta Version 1), that was examined by Korea Institute for Health and Social Affairs and National Health Insurance, in order to figure out influential variables on co-payment. In result of Multiple Linear Regression Analysis, R-square was 46.7%, the older the age, the patients who had surgery, the longer days of hospital treatment are, the higher gross income of a household is, the more hospitalized in upper grade general hospitals, and the more upper grade rooms and selecting a doctor are used. The results have statistical significance. When conducting research applying medical service research model, there is a need to apply Dutton(1986)'s medical service research model with high R-square. In order to strengthen assurance of National Health Insurance, first conditions should be that patients are hospitalized in upper grade general hospital, and at the same time, are patients who had surgery with long stay of hospitalization. In addition, if proven that patients used upper grade rooms and selecting a doctor due to lack of regular treatment and rooms, for certain number of days of such hospitalization, it is suggested to be provided with health care insurance in upper grade rooms and selecting a doctor in calculating co-payment limit.

보험심사 근무직의 직무스트레스와 정신건강 (The Job Stress and Mental Health of the Insurance Reviewer)

  • 국내의 의료보험 제도의 순기능은 높이 평가받지만 불완전한 보장성의 문제로 인해 국민들은 민영의료보험 가입을 통해 보장성을 확보하고자 한다. 이로 인해 최근 민영의료보험의 가입률이 높아지게 되었고, 청구율 또한 높아지게 되었다. 이처럼 민영의료보험을 찾는 사람이 늘어남에 따라 민영의료보험사의 근로자는 직무 부담이 가중되었으며, 특히 보험금 지급을 담당하는 보험심사 근무직의 경우 보험금을 고객과의 소통, 회사의 수익 상출에 기여 등 다양한 이유로 직무스트레스가 나날이 증가하고 있으며 그로 인한 부작용을 경험하고 있다. 이에 본 연구는 보험심사 근무직의 직무스트레스가 정신건강에 미치는 영향에 대해 분석하여 보험심사 근무직의 직무스트레스를 감소시키고 정신건강을 증진하고자 서술적 조사 연구를 실시하였다. 분석 결과, 보험심사 근무직의 직무스트레스는 정신건강에 유의한 정(+)의 영향을 미치는 것으로 나타났다. 세부적으로는 직무스트레스가 역할수행능력·자기신뢰도, 우울, 수면장애·불안, 일반건강·생명력의 정신건강 4가지 요인 모두에 유의한 영향을 미치는 것으로 나타났다. 본 연구를 통해 보험심사 근무직의 직무스트레스가 정신건강에 유의한 정(+)의 영향을 미친다는 사실을 알 수 있었다. 직무스트레스는 근로자로 하여금 직무수행에 대한 열의를 감소시키고 이직률, 퇴사율을 높이는 등 조직적인 측면에서의 부작용을 초래하기도 하지만 개인의 신체 건강을 악화시키고 우울, 불안 등과 같은 질환을 유발하여 정신건강을 피폐하게 만들기도 한다. 따라서 이를 예방하기 위하여 대상자의 특성에 맞는 적절한 직무스트레스 예방법과 대처법을 제공하여 직무스트레스를 감소시키고 정신건강을 증진할 수 있도록 지원하여야 할 것이다.

우리나라 보건의료 발전을 위한 의료기술평가의 역할 (Roles of Health Technology Assessment for Better Health and Universal Health Coverage in Korea)

  • Health technology assessment (HTA) is defined as multidisciplinary policy analysis to look into the medical, economic, social, and ethical implications of the development, distribution, and use of health technology. Following the recent changes in the social environment, there are increasing needs to improve Korea's healthcare environment by, inter alia, assessing health technologies in an organized, timely manner in accordance with the government's strategies to ensure that citizens' medical expenses are kept at a stable level. Dedicated to HTA and research, the National Evidence-based Healthcare Collaborating Agency (NECA) analyzes and provides grounds on the clinical safety, efficacy, and economic feasibility of health technologies. HTA offers the most suitable grounds for decision making not only by healthcare professionals but also by policy makers and citizens as seen in a case in 2009 where research revealed that glucosamine lacked preventive and treatment effects for osteoarthritis and glucosamine was subsequently excluded from the National Health Insurance's benefit list to stop the insurance scheme from suffering financial losses and citizens from paying unnecessary medical expenses. For the development of HTA in Korea, the NECA will continue exerting itself to accomplish its mission of providing policy support by health technology reassessment, promoting the establishment and use of big data and HTA platforms for public interest, and developing a new value-based HTA system.

노인장기요양보험인지도에 영향을 미치는 요인에 대한 탐색적 연구 (Factors Affecting Awareness of Long Term Care Insurance: An Exploratory Study)

  • 본 논문은 노인장기요양보험 인지도에 영향을 미치는 요인을 알아보고 연령집단에 따른 차이점을 분석하고자 한 탐색적 연구이다. 이를 위해 연구대상을 45세에서 64세 이상의 중장년 집단과 65세 이상의 노인 집단으로 구분하여, 인구사회학적 특징, 조사자의 사회관계망 요인, 건강요인이 노인장기요양보험 인지도에 미치는 영향을 파악하고자 하였으며, 고령화연구패널조사의 6차 조사자료를 토대로 2차자료 분석을 하였다. 연구결과를 보면, 45-64세 중장년 집단의 경우 성별, 자원봉사활동, ADL, IADL, 우울이 인지도에 영향을 미치는 것으로 나타났다. 이와 반면에 65세이상 노인 집단에서는 나이와 우울 외에 종교모임, 친목목임, 여가활동, 동창회모임, 자원봉사활동, NGO활동과 같은 사회관계망 관련 요인들이 노인장기요양보험의 인지도에 영향을 주었다. 연구결과를 토대로 노인장기요양보험 인지도 향상방안에 대한 정책적 함의를 제시하였다.

예술인의 직업적 지원과 권리보호를 위한 관련법의 개선방안 (Improvement Plan of the Relevant Law to Protect Professional Support and Rights of Artists)

  • 예술인의 직업적 지위와 권리보호를 위한 법 제도인 노동관계법, 사회보험법, 예술인복지법의 개선을 통하여 근로자성 인정, 고용보험법 가입특례, 국민건강보험법과 국민연금법 적용특례 등이 인정되어야 한다. 이를 위해 노동관계법에 대한 법원의 해석이나 입법에 의해 근로자 범위를 확대해야 하고, 현재 제외되어 있는 예술인의 고용보험법 가입 특례와 국민건강보험법 적용 특례 등 사회보험의 지원범위도 넓혀나가야 한다. 보험료 전액 본인부담에 임의가입 방식인 예술인 산재보험제도도 보험료 지원을 통해 제도의 실효성을 가져가야 한다. 예술인복지법 역시 예술인에 대한 법적 보호를 강화하는 등의 내용으로 개정이 필요하다. 또한 예술인복지사업에 대한 재원의 확보도 중요하다. 그리고 표준계약서를 의무화하고 예술인의 경력증명시스템을 구축하여 예술 활동기준을 적용해 복지수혜가 필요한 예술인이 누락되지 않도록 해야 한다.

생애주기별 민간의료보험 가입 영향 요인 및 의료이용 행태 : 2016~2019년 한국복지패널자료를 사용하여 (Influencing Factors to Enrollment in Private Health Insurance and Medical Use by Life Cycle : Analysis of 2016-2019 Korea Welfare Panel)

  • 본 연구는 생애주기별 민간의료보험 가입 현황 및 가입 요인을 알아보고, 민간의료보험 가입에 따라 생애주기별 의료이용 행태 차이를 파악하고자 시행하였다. 한국복지패널 12차-15차(2016-2019년) 자료를 SPSS 26 프로그램을 사용하여 58,223명의 인구사회학적 및 건강 특성에 따른 민간의료보험의 가입 현황 차이는 Chi-square, 가입 영향 요인은 다항 로지스틱 분석, 의료이용 행태는 평균분석하였다. 분석결과 민간의료보험의 가장 큰 가입요인은 가구소득이었으며, 성장기의 민간의료보험 가입이 생애주기 중 가장 높았고, 가구소득에 의해 복수 가입되었다. 청장년기는 가구소득과 유배우자, 장애가 없는 경우, 중년기는 가구소득과 경제활동, 유배우자, 건강수준이 큰 영향요인으로 나타났다. 노년기의 민간의료보험 가입률이 가장 낮았고, 저소득가구, 낮은 건강수준, 장애가 있는 경우에서 낮았다. 민간의료보험 가입자들의 의료이용 증가도 성장기와 청장년기에 나타났다. 국민건강보험의 보장성 강화와 이를 보충하기 위한 민간의료보험의 역할을 생애주기에 맞춰 정립하여 상호보완적으로 작용하게 하여 의료보장의 사각지대를 없애고, 국민의 건강과 후생을 최대화하여야 할 것이다.

민간의료보험이 의료기관 종별 선택에 미치는 영향: 관절염 환자의 외래 이용을 중심으로 (The Impacts of Private Health Insurance on Medical Institution Selection: Evidence from Outpatient Service Utilization among Arthritis Patients)

  • Recently, with the increase in the number of private health insurance subscribers, interest in overuse of the medical service is increasing. This study analyzed the impacts of private health insurance (PHI) on medical institution selection in outpatient service utilization among persons with arthritis. In order to control patients' health status, we extracted outpatient episodes with the same disease (KCD6, M13) from Korea Health Panel. The unit of analysis was an outpatient visit with arthritis in 2014 (n=23,363). In the light of insurance coverage, we redefined three type of private health insurance (ex, indemnity, fixed benefit, and non-insured) as a test variable and two type of medical institution (ex, hospital and physician visit) as a dependent variable. We conducted a probit regression analysis to identify the impacts of PHI on medical institution selection controlling for heteroscedasticity. The results of this study showed that the insured with indemnity were more likely to choose hospital departments than clinics (marginal effect=0.0475, p=0.000). The impact of participation of fixed benefit PHI was not as clear as that of indemnity type (marginal effect=0.0162, p=0.047). In conclusion, this study confirmed that PHI, particularly indemnity type has a significant impact on the selection of medical institutions. Healthcare policy makers should consider that PHI not only affects the overall quantitative increase in healthcare utilization, but also influences the selection of medical institutions.

  • Taiwan has experienced rapid economic growth during the past two decades. As a result, the demand for health care in Taiwan has increased rapidly. To meet the rising demand, Taiwan implemented a National Health Insurance (NHI) program on March 1, 1995. This program now covers more than 96 percent of Taiwan's citizens. Implementation of the NHI in 1995 represents fulfillment of a primary social and health policy goals of Taiwan. The goals of the NHI program is to eliminate financial barriers of health care for the citizens, to improve the quality of care. 조산원의 건강보험수가 산출방법과 추계 (Methods and Estimates of the Reimbursement for the Nurse Midwifery Center in the National Health Insurance)

  • Purpose: The purpose of this study is to develop the optimal nursing fee for nurse-midwifery center (MC) in the national health insurance system. Methods: The three methodologies used to calculate the conversion factors for the MCs in the national health insurance include cost accounting method, sustainable growth rate (SGR) model, and index model. In this study, the macro-economic indicators and the national statistics were used to estimate the conversion factors for the MCs. Results: The optimal nursing fee for the MCs in 2011 was estimated to be an increase of 57.7% by cost accounting analysis, a decrease of 17.1% by SGR model, and a decrease of 16.1% by index model. The results from SGR model and index model could had been biased due to the upswing of medical spendings in the short-term period (2008~2009). A sensitivity analysis of pre-delivery subsidy program for OB & GYN hospitals and clinics showed that the program has substantially diminished the demand for the MC services. Conclusion: More reliable methodologies to estimate nursing fees precisely are required to prove the value of nurses' services and a government subsidy program for the MC services should be followed from a social perspective.
