환경영향평가 전문가의 직무스트레스 수준 및 관련 요인 (Job Stress Levels in Environment Impact Assessment, and Related Factors Including Expert)

  • A number of recent studies have shown that occupational stress is closely associated with increased fatigue and decreased job satisfaction. This study was designed to assess stress in workers in the field of EIA (environment impact assessment) from July 2009 through september 2009, and questionnaires were uesd to analyze the stress scores and identify the primary factors influencing stress in this occupation. The number of respondents was 272 working in the Capital (Seoul and Kyunggido), Chungcheongdo, Jejudo area. A structured questionnaire was used to evaluate the participants' socio-demographics, job-related factors, health-related behaviors, occupational stress, reaction factor (self-perceived fatigue, job satisfaction) and buffer factor (social support). Occupational stress and self-perceived fatigue were assessed using the Korean Occupational Stress Scale-Short Form (KOSS-SF) and the Multidimensional Fatigue Scale (MFS), respectively. The analysis revealed a strong correlation between occupational stress, job satisfaction, and social support, with occupational stress being associated with an increased risk of fatigue and decreased job satisfaction. In the multiple regression analysis(stepwise), the main factors influencing occupational stress were found to be job satisfaction, supervisor, fatigue, working time, no. of personnel, career. The results of this study suggest that occupational stress is a determinant predictor of self perceived fatigue and job satisfaction. Thus, a strong recommendation is made for a stress management program for reduction of occupational stress, and for the development of relevant experts on the health and quality of life of environment impact assessment workers.

Social Determinants of Health and 5-year Survival of Colorectal Cancer

  • Background: Early in the 21st century, cancers are the second cause of death worldwide. Colon cancer is third most common cancer and one of the few amenable to early diagnosis and treatment. Evaluation of factors affecting this cancer is important to increase survival time. Some of these factors affecting all diseases including cancer are social determinants of health. According to the importance of this disease and relation with these factors, this study was conducted to assess the relationship between social determinants of health and colon cancer survival. Materials and Methods: This was a cross-sectional, descriptive study for patients with colon cancer registered in the Cancer Research Center of Shahid Beheshti University of Medical Science, from April 2005 to November 2006, performed using questionnaires filled by telephone interview with patients (if patients had died, with family members). Data was analyzed with SPSS software (version 19) for descriptive analysis and STATA software for survival analysis including log rank test and three step Cox Proportional Hazard regression. Results: Five hundred fifty nine patients with ages ranging from 23 to 88 years with mean${\pm}$standard deviation of $63{\pm}11.8$ years were included in the study. The five year survival was 68.3%( 387 patients were alive and 172 patients were dead by the end of the study). The Cox proportional hazard regression showed 5-year survival was related to age (HR=0.53, p=0.042 for>50 years versus<50 years old) in first step, gender (HR=0.60, p=0.006 for female versus male) in second step, job (HR=1.7, p=0.001 for manual versus non manual jobs), region of residency (HR=3.49, p=0.018 for west versus south regions), parents in childhood (HR=2.87, p=0.012 for having both parents versus not having), anatomical cancer location (HR=2.16, p<0.033 for colon versus rectal cancer) and complete treatment (HR=5.96, p<0.001 for incomplete versus complete treatment). Conclusions: Social determinants of health such as job, city region residency and having parents during childhood have significant effects in 5-year survival of colon cancer and it may be better to consider these factors in addition to developing cancer treatment and to focus on these determinants of health in long-time planning.

비교대 중년여성 근로자의 웰빙 영향요인: 2차자료분석 (Factors Influencing the Well-being of the Middle-aged Non-shift Female Workers: Using Secondary Data)

  • Purpose: This study aimed to analyze the factors influencing the well-being of middle-aged non-shift female workers using health-determinant models. Methods: This study analyzed data from the fifth Korean Working Conditions Survey (KWCS), involving 5,449 participants. A hierarchical multiple regression analysis was employed to examine the relationships between various factors and well-being. Results: The analysis identified key factors affecting well-being in middle-aged female non-shift workers, including supervisor support, presenteeism, satisfaction with the working environment, autonomy in working hours, support from coworkers, workplace discrimination, occupation, work-life balance, sleep problems, workplace size, weekly work hours, and sickness absence. This study confirms that the well-being of middle-aged non-shift female workers is influenced by factors at the individual, social, and community levels as well as by conditions related to love. Conclusion: To enhance the well-being of middle-aged female non-shift workers, it is essential to reinforce positive factors such as support from coworkers and superiors. Additionally, addressing and mitigating negatively influencing factors such as workplace discrimination and sleep problems is crucial in promoting well-being. By implementing measures to improve these aspects, organizations and policymakers can contribute to a healthier and more supportive work environment for middle-aged, non-shift female workers.

익명의 알코올중독자(AA) 모임 참여자의 삶의 질에 영향을 미치는 요인 (Factors Influencing Quality of Life of Alcoholics Anonymous Members in Korea)

  • Purpose: The purpose of this study was to determine quality of life (QOL) related factors in Alcoholics Anonymous (AA) members based on PRECEDE Model. Methods: A cross sectional survey was conducted with participants (N=203) from AA meeting in 11 alcohol counsel centers all over South Korea. Data were collected using a specially designed questionnaire based on the PRECEDE model and including QOL, epidemiological factors (including depression and perceived health status), behavioral factors (continuous abstinence and physical health status and practice), predisposing factors (abstinence self-efficacy and self-esteem), reinforcing factors (social capital and family functioning), and enabling factors. Data were analyzed using t-test, one way ANOVA, Tukey HSD test and hierarchical multiple regression analysis with SPSS (ver. 21.0). Results: Of the educational diagnostic variables, self-esteem (${\beta}=.23$), family functioning (${\beta}=.12$), abstinence self-efficacy (${\beta}=.12$) and social capital (${\beta}=.11$) were strong influential factors in AA members' QOL. In addition, epidemiological diagnostic variables such as depression (${\beta}=-.44$) and perceived health status (${\beta}=.35$) were the main factors in QOL. Also, physical health status and practice (${\beta}=.106$), one of behavioral diagnostic variables was a beneficial factor in QOL. Hierarchical multiple regression analysis showed the determinant variables accounted for 44.0% of the variation in QOL (F=25.76, p<.001). Conclusion: The finding of the study can be used as a framework for planning interventions in order to promote the quality of life of AA members. It is necessary to develop nursing intervention strategies for strengthening educational and epidemiological diagnostic variables in order to improve AA members' QOL.

Social Support and Quality of Life in Domestic Korean and Korean American Elderly People

  • Purpose: The aim of this study was to investigate the relationship between social support and quality of life (QOL) in elderly people and to examine potential cross-cultural differences. Methods: The author surveyed 82 elderly people, over the age of 65, residing in a nursing home in a Korean rural area and 73 Korean immigrants living in Houston Texas USA who were also elderly. The degree of social support was measured with 20 questions that were answered using a 5 grade scale. The quality of life was measured in 5 fields (mobility, self-care, usual activities, pain/discomfort, and anxiety/depression, each scored using a 3 grade scale) with EUROQOL EQ-5D. Results: The mean score of social support was 65.43 for domestic Korean elderly people (range: 26 to 98); it was 73.43 for elderly Korean Americans (range: 26 to 100). The score was lower for domestic Koreans despite the traditional environment (p<0.05). Regarding QOL, 61% of domestic Korean elderly people complained of pain/discomfort and 47.5% appeared to have anxiety/depression, 30.5% complained of limitations in mobility, 20.7% reported limitations in usual activities, and 14.6% had limitations in basic self.care activity such as washing or dressing. A similar proportion of Korean American elderly had limitations in usual activities (19.2%) and self care activity (9.5%). But significantly fewer complained of anxiety/depression (16.5%, p<0.01) or pain/discomfort (38.3%, p<0.01). The major differences in psychological domains of QOL may be related to the degree of social support and the difference in the socio.cultural environment. Conclusion: Social support is a major determinant of QOL, especially for psychological domains, and these relationships are under the influence of the socio.cultural environment.

Evaluation of Head and Neck Cancer Awareness and Screening Status in Jeddah, Saudi Arabia

  • Background: Head and neck cancer (HNC) is one of the deadliest cancers worldwide. Early detection is a key determinant of HNC prognosis. Hence, raising awareness of this disease may improve survival rates. The present study evaluated the level of awareness and screening status for HNC in the general population of Jeddah, Saudi Arabia. Materials and Methods: An e-questionnaire was distributed during our HNC awareness campaign at the Red Sea Mall in Jeddah. The questions assessed HNC knowledge and screening status among participants. Results: Of the 112 respondents, 68% indicated that they had no knowledge of HNC. Social media was the major source of information (39%) for respondents. The majority (40%) believed that it was the joint responsibility of dentists, dental hygienists, and general physicians to screen for HNC; 82% had never been screened. Conclusions: Knowledge and awareness of HNC must be increased in the general population and among dental health professionals.

Extended Low Anterior Resection with a Circular Stapler in Patients with Rectal Cancer: a Single Center Experience

  • Background: to evaluate the outcome of stapled colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma. Materials and Methods: A retrospective study of fifty patients who underwent coloanal anastomoses after extended low anterior resection was conducted at Imam Hospital from September 2007 up to July 2012. Results: The distance of the tumor from anal verge was 3 to 8 cm. Anastomotic leakage developed in 6% of patients and defecation problems in 16%. One-year local recurrence was 6% while three-year local recurrence was 4%. One-year systemic recurrence was seen in 22% while three-year systemic recurrence was seen in 20%. Conclusions: Colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma can be conducted safely.

베이비부머의 라이프스타일에 따른 실버타운에 대한 인식과 선호 결정요인 (Determinant Factors and Recognition about Silver Town according to Baby Boomer's Life Style)

  • The purpose of this study was to identify the lifestyle of babyboomer, the difference of recognition about silver town and preferable determinants according to lifestyle. By the Social Survey Research with questionnaire, data from 108 people of 46 years old ~ 65 years old who inhabited in Seoul City area were collected during October, 2010. Data were analyzed by the SPSS program Win 18.0, and frequency, percentage, t-test, ANOVA, factor analysis, regression were employed. The major findings were as follows. First, by the factors analysis on lifestyle, factors were named "interest about external", "inner-oriented", "pursuit of health", "prepare for later life", "social aspects". Second, by the frequency analysis on recognition about silver town, 97.2% of total respondents replied "at least heard about silver town", and 64.8% of total respondents recognized the necessity of silver town. While, intention to live in silver town was low (by 38.0% of total respondents). Third, by the regression analysis on preference tendency about silver town according to life style, factor of 'inner-oriented' in the decision about residential area and total household number, factor of 'pursuit of health' in the decision about monthly rent, factor of 'social aspects' in the decision about cost solution method were influential factors. In conclusion, to supply the house which had the diversity of baby bommer's life style, government and corporation's efforts to increase intention to live in silver town and to change the recognition about silver town are needed.

구강건강행태 및 치과의료서비스 이용과 치주질환과의 관련성 (A study of the relationship between oral health behaviors, use of dental clinics and periodontitis)

  • Objectives: This study was designed to examine the effects of oral health behaviors and use of dental clinics on periodontal diseases in women, the purpose of this study is to provide basic information to develop program for dental health promotion in young women. Methods: We conducted a survey targeting 486 women that agreed with the survey and were going to dental clinic from October 1, 2013 to October 18, 2013. We surveyed them by self-administered questionnaire and analyzed it by SPSS Win 19.0 program. Results: 1. For differences in periodontitis according to general characteristics, if th age is high, ratio of periodontitis is high. Married woman is higher than single one in ratio of periodontitis. Also the more number of birth, the higher ratio of periodontitis is.(p<.05) 2. For differences of periodontitis by dental care behavior, ratio of periodontitis of non-smokers is significantly low. For one that brush one's teeth broadside, the ratio of periodontitis is high.(p<.05) 3. For differences of periodontitis by use of dental clinic service, ratio of periodontitis of one that have more times visit to a dental clinic, more annual average cost for dental care, and regular dental check-ups is relatively low.(p<.05). 4. For determinant factors influencing on periodontitis, ratio of periodontitis of one that is old, have more stress, have a low level of education, and have preventive dental care is high. Ratio of periodontitis of one that have less toothbrushing and brush one's teeth after having lunch or a snack and before sleeping is significantly low. Conclusions: From this study, dental health behavior as the factor influencing on periodontitis of young women is statistically meaningful. Because this dental health behavior has a lot of potential to be improved by government and local community efforts such as education and social support, we think that systematic and various educational program development is needed to strengthen self dental care ability effectively with policy support.

개인의 성별이 재난적 의료비 지출 여부에 미치는 영향: 세부집단분석을 통한 젠더적 접근 (The Effect of Gender on Catastrophic Health Expenditure in South Korea: Gender-Based Approach by Subgroup Analysis)

  • Background: Catastrophic health expenditure (CHE) occurs when medical expenditure of a household passes over a certain ratio of household income. This research studied the effect of gender on CHE based on Korea Health Panel data. Methods: This study implemented binary logistic regression model to figure out whether gender affects CHE and how different gender groups show pattern of CHE process. With gender, age, marital status, income level, economic activity, membership of private insurance, existence of chronic disease, and self-rated health were included in the model. Results: Results showed that females faced CHE 1.5 times more than males (odds ratio, 1.241). Also, main determinants of CHE in female groups were marital status, while age and economic activity status were significant in male groups. Subgroup analysis displayed that married female under 35 years old are located in intersectionality of CHE including pregnancy and delivery, multiple health risk behaviors, mental stress, and relatively vulnerable social status due to lower income. Meanwhile, both gender above 50 years old faced remarkably high chance of CHE, which seems to be caused by complex health risk behaviors and chronic diseases. Conclusion: Such results implied not only that gender is an important determinant of CHE, but also other determinants of CHE differ according to gender, which suggests a necessity of gender-based CHE support and rescue policy.
