장애인의 흡연에 영향을 미치는 요인분석 (Impacts of Sociocultural Factors on Smoking among Disabled People)

  • 본 연구의 목적은 개인의 흡연여부와 다양한 사회경제적 변수 간 영향관계가 성별과 장애여부에 따라 구분된 하위집단에서 차별적으로 나타나는지를 확인하는데 있다. 특히 본 연구는 일반인의 흡연 여부가 아니라 장애인을 대상으로 흡연의 여부가 장애인의 건강에 미치는 영향을 분석한 연구로 연구의 의의가 있다고 볼 수 있다. 본 연구에서는 장애인의 흡연율과 흡연 요인을 분석하는데 목적이 있다. WHO에 따르면 담배로 인한 사망자는 매년 600만명에 이른다. 담배는 심혈관질환, 폐질환, 암 등 비전염성질환과 조기 사망에 매우 강한 인과관계를 갖는 것으로 알려져 있다. UN이 제시한 지속가능 발전목표(Sustainable Development Goals, 이하 SDGs)는 건강한 삶의 보장과 모든 세대의 복지 증진(Ensure healthy lives and promote well-being for all at all ages)을 위해 WHO의 담배규제기본협약(Framework Convention on Tobacco Control)을 이행할 것을 요구하고 있다. 이것은 흡연의 모든 과정에서 발생하는 다양한 위협요인이 흡연자뿐만 아니라 경제, 사회, 환경 등 다양한 영역에서 다차원적으로 작용하는 위협하기 때문이다. 본 연구목적의 달성을 위해 본 연구는 2013년 국민건강영양조사를 활용하여 흡연과 선행연구에서 확인된 흡연 영향요인 간 인과관계를 확인하였다. 본 연구를 위하여 특히 성별과 장애여부에 따라 구분된 4개의 하위집단을 독립적 모형으로 하여 분석하였으며 흡연여부를 종속변수로 하는 로지스틱회귀분석을 활용하였다. 본 연구 분석결과 독립변수의 영향력 여부는 성별과 장애유무에 따라 구분된 각 집단별로 뚜렷한 차이를 나타내는 것이 확인되었다. 상대적으로 비장애집단에서 독립변수의 영향이 유의한 반면 장애집단에서는 유의성이 상실되는 경향을 보였으며 이 같은 경향은 특히 장애여성집단에서 더욱 명백하게 확인되었다. 분석결과를 기반으로 장애집단의 특성을 고려하지 않은 기존의 선행연구가 가진 설명의 한계를 확인하였으며 장애의 물리적·심리적 특수성을 충분히 고려한 연구모형을 통해 흡연과의 상관관계를 재분석해야 함을 함의로 제시하였다. 추후 장애인의 건강한 삶의 질을 개선하기 위하여 건강증진 영향들을 분석한 이후 건강증진에 기여할 수 있는 대표적인 변인들을 고려하여 적용하는 다양한 연구들이 수행되어져야 할 것으로 여겨진다.

니코틴 의존도에 따른 양생 실천 수준에 대한 조사 연구 (A Study on the Practice Level of Yangseng (Traditional Health Behavior) according to Nicotine Dependence)

  • Objectives : The purpose of this study is to investigate the practice level of Yangseng according to nicotine dependence. Methods : The subjects of this study were 137 male smokers in their thirties and forties, who visited Wonmi Public Health Center. They were evaluated with Fagerstorm's test for nicotine dependence (FTND) and Tool in Measuring Yangseng (TMY). By FTND score, they were divided into three groups; the mild nicotine dependent group, moderate group and severe group. Results : In TMY score, there were statistically significant differences in the characteristics of Diet, Activity & Rest, Sleep and total score among the three groups. The value of FTND comparing the values of Mind, Diet, Activity & Rest, Sleep and total score in this study resulted in negative correlations, which showed statistical significance. Conclusions : The above results suggest that there are significant relations between nicotine dependence and the practice level of Yangseng. These results can be used for planning a smoking cessation program and a Yangseng health promotion program based on Korean traditional medicine.

  • 최근 흡연자 본인 스스로에 대한 질병뿐만 아니라 가족들 또는 비흡연자에게 피해가 가는 것 또한 문제로 제기되고 있다. 이러한 사회적 현상으로 금연을 원하는 사업장의 근로자들이 증가하고 있는 추세에 있으며, 금연제도는 기업의 의도대로 흡연 직원의 건강은 물론 비흡연 직원들의 건강과 더 나아가 개선된 사내 분위기와 환경을 제공할 수 있다. 이로 인해 직원들의 업무효율증대등의 경영성과나 조직몰입과 같은 조직성과를 기대할 수 있다. 또한 대외적으로는 소비자들에게 우리 기업이 친환경기업, 깨끗한 기업, 글로벌 선두 기업 등의 긍정적인 이미지의 기업으로 인식될 수 있다. 본 연구는 사업주 주도의 금연프로그램을 성공적으로 운영하고 있는 금연프로그램의 사례를 살펴보고자 한다.

  • Purpose: This study was done to investigate correlations among Health Promoting Lifestyles, Health Risk Indicators, Activities of Daily Living, and Depression of the in-house stroke patients. Method: The subjects were 58 in-house stroke patients in a health center and two welfare centers. Data was collected using questionnaires and measuring health risk indicators such as blood pressure, total cholesterol, triglyceride, blood sugar, body fat rate. Data was analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients. Result: The health promoting lifestyle performance showed a significant negative correlation with health risk indicators and depression. There was a significant negative correlation between activities of daily living and depression. Conclusion: Health promoting lifestyle which focus on regular physical check-up, medication, diet management, exercise, smoking cessation, drinking abstinence, and stress management should be developed to improve health risk indicator and depression of the in-house stroke patients.

여성의 건강과 운동 (Exercise and Health in Women)

  • The purpose of this study was to examine the effects of physical activity on women's health. I conducted literature reviews for meta-analyses and randomized controlled trials with the target diseases including cardiovascular diseases, diabetes, cancers osteoporosis, and pregnancy outcomes. Women who were active had less total mortality and smaller incidence of hypertension, coronary heart diseases, and stroke. Exercise was better than drug therapies in preventing diabetes and effective in preventing colon and breast cancers. Exercise can reduce the risks of falling injury in elderly women. Walking during pregnancy was not harmful to the mothers and their infants, and desirable to prevent the complications of pregnancy or weight gain after pregnancy. Physical activity at work and leisure-time showed similar effects on women's health. Based on these results, moderate-intensity physical activity should be recommended to all women, Resistive, muscle strength, and balance-training exercise also can be recommended. Doctors' advices are effective to make women exercise, especially using handouts or motivational interviewing techniques. It is desirable to recommend to peform other health promotion measures together such as smoking cessation, weight control and diet control.

한국 제칠일 안식일 예수 재림교인과 일반인의 건강행동 비교연구 (Comparison of Health Behaviors Patterns between Korean Seventh-Day Adventists and the General Korean Population)

  • 연구목적: 이 연구는 한국의 재림교인과 일반 인구집단간의 생활습관 및 건강행동 비교 분석 연구를 통하여 특성과 관련요인들을 도출하고자 실시하였다. 연구방법: 수도권 지역 만 20세 이상 재림교인 878명과 제4기 2차년도 국민건강영양 조사자료 중에서 수도권 지역 만 20세 이상 3,000명을 대상으로 건강행태와 관련된 건강검진, 흡연, 음주, 신체활동, 주관적 스트레스, 체중관리, 구강건강 관련 항목의 비교는 카이제곱 검정을 사용하였으며, 건강행동에 기여요인을 확인하기 위해 로지스틱회귀분석을 실시하였다. 연구결과: 건강검진율은 재림교인이 일반 인구집단보다 높게 나타났으며(p<0.001), 재림교인은 연령이 낮은 20-30대에서 검진율이 낮게 나타났다(p<0.001). 평생 흡연 경험율 및 현재 흡연율에서 재림교인이 일반 인구집단보다 현저하게 낮게 나타났으며(p<0.001), 재림교인 중 과거 흡연자의 금연 성공율은 매우 높게 나타났다. 음주율에서는 재림교인이 일반 인구집단보다 현저하게 낮게 나타났으며, 재림교인은 신앙기간이 짧을수록 음주를 많이 하는 것으로 나타났으며(p<0.001), 남자가 여자보다 음주율이 높은 것으로 나타났다(p<0.001). 중등도 신체활동 및 걷기 운동에서 재림교인은 일반 인구집단보다 실천율이 높게 나타났으며, 재림교인들의 남자가 여자보다 중등도 신체활동은 높은 것으로 나타났다(p<0.001). 결론: 이상에서 나타난 재림교인들의 건강생활습관에서 흡연 및 음주 등에서는 일반 인구집단에 보다 탁월하게 건강생활을 실천하고 있어 추후 보다 심층적인 추적관찰 연구 등이 필요해 보인다.

일부 대학생들의 건강관련행위 (Health Practices of University Students)

  • This paper analyzed health practices of students in an university in Chollabukdo Province. It attempted to study how many university students had what kinds of health practices. Furthermore, it tried to find out what are the major problems in health practice among university students. To study health practices of university students, this study sampled 1,469 students randomly. Many of the students had poor practices of teeth care. In tooth-brushing, many students (35.8%) brushed their teeth before breakfast in the morning. Most of the students (95% or more) visit dental clinic only when they had problem(s) in their teeth. For weights, there exists a great gap between perception and BMI (Body Mass Index), especially among female students. Among the female students who perceived their weights were above average, 96.4% of them were normal according to BMI. Many of the students were drinking greater amount of alcohol than safe amount suggested by the National Health and Medical Research Council of Australia. The proportion of the students drinking harmful or hazardous amount of alcohol were 79.5% of the male students and 74.9% of the female students. It suggests that health promotion fund or tax need to be levied on alcohol to frustrate unhealthful drinking. More than half (59.0%) of the male students and about 6.0% of the female students were smoking cigarettes. The smoking initiation ages were 17.8 for male and 18.2 for female students. It means that many of the students started smoking in their junior high or high schools. It implies that smoking prevention and smoking cessation programs need to be emphasized in these schools. For perception on sex, 27.8% of the male students and 60.9% of the female students thought to keep their virginity before their marriage. The female students were more conservative in perception on sex. The average number of sexual intercourses per year of male students was 17.0 times with their lovers, 7.9 times with prostitutes, and 7.0 times with maids. The female students had sexual intercourses 5.0 times with their lovers and 4.0 times with waiters per year. The proportion of students who had experience of sexual intercourse with homosexual partners was 1.0%. The finding this study was that the university students were vulnerable to poor health practices. It is suggested that public health intervention program be provided for university students to keep them in good healthy lifestyle.

한국 성인에서 일반담배 또는 가열 전자담배를 이용한 흡연 형태와 폐 기능 검사 결과와의 관련성: 2014-2019년도 국민건강영양조사 자료를 이용하여 (The Association between Pulmonary Function Test Result and Combustible Cigarette Smoking or Electrical Cigarette Smoking in Korean Adults : Using the 2014-2019 Korean national health and nutrition examination survey data)

  • Purpose : Smoking is a major factor in chronic obstructive pulmonary disease (COPD), but the effect of electrical cigarette smoking on COPD development is still uncertain. This study aimed to compare the functions of airways and lungs exposed to combustible cigarettes and electrical cigarettes based on the pulmonary function test (PFT) results from the Korean National Health and Nutrition Examination Survey (NHANES). Methods : This study used data from 8,942 participants with PFT results out of 47,309 total subjects from the 6th to 8th Korean NHANES (2014-2015, 2016-2018, and 2019, respectively). Individuals with diseases such as cancer, ex-smokers, and dual tobacco users were excluded. The PFT results were analyzed according to the COPD diagnostic criteria. After adjusting for confounding variables, a complex sample generalized linear model ANOVA test was performed to investigate the association between PFT results and combustible smoker or electrical cigarette user groups. Results : In an analysis based on the obstructive ventilatory disorders (forced expiratory volume in 1 second[FEV1]/forced vital capacity[FVC]<.7), combustible cigarette smokers showed a 3.46 times higher risk of COPD compared to non-smokers, while electrical cigarette smokers exhibited no significant difference in terms of COPD-related risks compared to non-smokers. FEV1 showed a negative relation with combustible cigarette smokers as reported elsewhere (B=-.07, p<.001). FEV1/FVC was negatively related to both combustible cigarette smokers (B=-.03, p<.001) and electrical cigarette smokers (B=-.02, p<.001). Conclusion : FEV1/FVC decreases were observed in the long-term exposure to both combustible and electrical cigarettes. The lower FEV1 in the combustible cigarette group implies the worsening of the severity of COPD, suggesting more damage to the airways and lungs in the short term. Therefore, the temporary electrical cigarettes use for the transition period in order to smoking cessation potentially aids to reduce the harmful effect of combustible cigarettes in COPD development.

A Tobacco Cessation Intervention with Rural, Medically Underserved, Blue-collar Employees: A Quasiexperimental Study

  • Background: The aim of this study was to increase knowledge regarding the dangers associated with tobacco use, and decrease secondhand smoke exposure and tobacco use behaviors with an antitobacco messaging campaign among rural, medically underserved, blue-collar workers. Methods: A quasiexperimental study was conducted with employees at two worksites. One worksite received the intervention, which consisted of nine different antitobacco messages. Baseline and follow-up surveys were conducted at each worksite to assess change in knowledge and behavior; the data were compared across the two worksites. Results: Two hundred twenty-two and 243 participants completed baseline and follow-up surveys at the intervention and comparison sites, respectively. A statistically significant difference was seen over time between the worksites on knowledge of the dangers of tobacco (p < 0.0001); the mean knowledge score increased at the intervention site, but remained unchanged at the comparison site. In general, non-smokers at both worksites appeared to try to decrease exposure to secondhand smoke over the follow-up period. Repeated measures analysis indicated that there were no differences in motivation to quit (p = 0.81), interest in quitting (p = 0.40), thinking about quitting (p = 0.53), or several tobacco-use behaviors over time among smokers at the intervention and comparison worksites. There were slight increases over time in the proportion of smokers who do not allow smoking in their homes/vehicles at the intervention worksite, although not statistically significant. Conclusion: Participants at the intervention worksite increased their knowledge regarding the dangers of tobacco use and secondhand smoke exposure. Among current tobacco users, the intervention appeared to increase family rules regarding secondhand smoke exposure in their homes and vehicles.

Extending Application of the 'Hardcore' Definition to Smokeless Tobacco Use: Estimates from a Nationally Representative Population in India and its Implications

  • Background: The term 'hardcore' has been applied to use of smoking tobacco and generally referred to as the inability or unwillingness of regular smokers to quit. The component constructs of hardcore except nicotine dependence are product neutral. With the use of 'time to first chew' as a measure of nicotine dependence, hardcore definition can be extended to characterize smokeless tobacco users. Hardcore users respond less to tobacco cessation interventions, and are prone to tobacco induced diseases including cancer. Thus identifying hardcore users would help in estimate the burden of high risk population for tobacco induced diseases. Smokeless tobacco use is predominant and accounts for more than 50% of oral cancer in India. Hence, hardcore chewing information could be used for planning of tobacco and cancer control interventions. The objective of this study was to assess the prevalence and associated factors of hardcore smokeless tobacco use in India. Materials and Methods: Global Adult Tobacco Survey (GATS)-India 2010 data were analyzed to quantify hardcore smokeless tobacco use in India with following five criteria: (1) current daily smokeless tobacco use; (2) no quit attempt in the past 12 months of survey or last quit attempt of less than 24 hours duration; (3) no intention to quit in next 12 months or not interested in quitting; (4) time to first use of smokeless tobacco product within 30 minutes of waking up; and (5) knowledge of smokeless tobacco hazards. Results: The number of hardcore smokeless tobacco users among adult Indians is estimated to be 5% (39.5 million). This group comprises 23.2% of daily smokeless tobacco users. The population prevalence varied from 1.4-9.1% across different national regions of India. Logistic regression modeling indicated age, education and employment status to be the major predictors of hardcore smokeless tobacco use in India. Conclusions: The presence of a huge number (39.5 million) of hardcore smokeless tobacco users is a challenge to tobacco control and cancer prevention in India. There is an unmet need for a universal tobacco cessation programme and intensification of anti-tobacco education in communities.
