서울 시내 일부 초 . 중 . 고등학교 학생의 흡연위험요인에 관한 연구 (Risk Factors for Smoking Behavior in Elementary, Middle and High School Students)

  • Objectives: The rate of adolescent smoking is important because lead to adult smoking. And early onset of smoking is associated with chronic disease in adult life. The purpose of this study is to describe the smoking status of adolescents and identify the risk factors for smoking in each school. Methods: We selected the participant schools that are located in Yang-cheon Gu, Seoul, Korea, considering the social economic status. The subjects were included 719elementary school students,1,251 middle school students and 1,513 high school students. Self-reported questionnaires were administrated between December 16 and December 24 , 2002. Students were informed in writing that the survey was voluntary. The response rate was 98.7, 81.1 and 95.4 in elementary, middle, and high school, respectively. Results: The results of this study were as follows: 1) Students who have attempted to smoke were 8.8% in elementary school, 29.3% in middle school and 35.0% in high school. 2) The current smokers were 2.0%, 7.9% and 11.5% in elementary, middle and high school, respectively. 3) In elementary school, smoking of mends and physical health were significantly predicted the attempt to smoke. Stress, smoking of mends, alcohol, and binge drinking were associated with the attempt to smoke of middle school students. In high school, the attempt to smoke was related to gender, maternal education, depression, school performance, smoking of brothers, smoking of mends, alcohol, and binge drinking. 4) Current smoking was associated with smoking of mends and binge drinking in middle school and gender, school performance, smoking of brothers, smoking of mends, alcohol, and binge drinking in high school in multiple logistic regression. Conclusion: There is a need for health education and specific strategies to help adolescents prevent from smoking. Smoking prevention programs which include risk-taking behavior such as alcohol, method of copying with stress, and supporting environment were required.

학교금연교육프로그램 현황 및 문제점: 부산광역시 중.고등학교를 중심으로 (School-based Educational Programs for Anti-tobacco Use at Busan Middle and High Schools)

  • Purpose: The purpose of this study was to describe the current status of managing educational programs for tobacco use prevention at middle and high schools located in Busan. Methods: Using a survey method with a 29-item self-administration questionnaire, the data were collected from January 10 through February 18, 2005, from school nurses as well as teachers major in healthcare of 227 participant schools. Results: A strategic plan for school-based educational programs of tobacco use prevention was mainly developed by a school nurse at middle school and a teacher in charge of school discipline at high school. The educational programs for tobacco use prevention were implemented for approximately 40% of schools in Busan with the budget ranged from 110,000 won to 200,000 won a year. Eighty five percepts of the schools had anti-tobacco regulations and guidelines in place. Twenty two percents of the middle and high schools collected the data regarding adolescents' smoking status periodically. Over 70% of schools provided tobacco use prevention education programs as a part of regular semester curriculum. Conclusions: For making the management of educational programs for tobacco use prevention effective and consistent with government's anti-smoking policy, schools need (a) development of tobacco management guideline and regulations with participation of teachers, students, and parents, (b) clear job and role descriptions for personnel to plan and implement anti-tobacco programs, (c) compilation of the budget enough to implement the programs, (d) periodically assessment of adolescents' smoking status, and evaluation of the programs to monitor whether those are effective for adolescents' health promotion; and (e) to make an effort to less range of quality in educational programs for tobacco use.

Effects of Self-Concept Levels and Perceived Academic Achievements of Turkish Students on Smoking Perceptions

  • Background: The objective of this study was to examine the effect of self-concept levels and perceived academic achievements of sixth, seventh and eighth grade primary school students upon their perceptions about smoking. Method: The data were collected with the Socio-Demographic Data Collection Form, Pier-Herris Self-Concept scale and Children's Decision Balance Scale. The study sample consisted of 374 students receiving education in the sixth, seventh and eighth grades of three primary schools, which were selected among primary schools of Izmir Provincial Directorate for National Education representing three socio-economic groups with a simple random sampling method. The data were collected in December 2012-January 2013. Percentages and the t test were used in the evaluation of the data. Results: While students with a positive self-concept had score averages of $7.12{\pm}2.18$ regarding the lower dimension of smoking pros and $29.0{\pm}2.47$ regarding the lower dimension of smoking cons, their counterparts with a negative self-concept had score averages of $8.61{\pm}3.76$ (p=0.000) and $28.1{\pm}3.49$ (p=0.004), respectively. According to self-perception, there was statistical difference between perceptions of students regarding smoking (p<0.01). While students perceiving themselves successful had score averages of $7.81{\pm}3.13$ and $28.5{\pm}3.19$ regarding the lower dimension of smoking benefit and harm, students perceiving themselves unsuccessful had score averages of $8.27{\pm}3.39$ (p=0.333) and $29.01{\pm}2.05$ (p=0.235), with no difference determined. Conclusion: Students with a positive self-perception had a low perception of smoking pros and a high perception of smoking cons. Perception of academic achievement did not affect the pros and cons perceptions of children regarding smoking.

병원기반 금연중재가 뇌경색 환자의 퇴원 후 12개월 지속 금연율에 미치는 효과 (The effect of a hospital-based smoking cessation intervention on the continuous abstinence rate from smoking over 12 months in patients with cerebral infarction)

  • Objectives: The aim of this study was to investigate the effectiveness of a hospital-based smoking cessation intervention for increasing continuous abstinence rate from smoking in patients with cerebral infarction. Methods: One-hundred and two smokers with cerebral infarction who decided to quit smoking were enrolled in the smoking cessation intervention from December 2012 to February 2015. The smokers underwent six consecutive times of individual intervention with nurse specialist on smoking cessation including education on behavioral modification, counseling for withdrawal symptoms, and anti-smoking advice over a 12-month period. Results: Among the total participants, the continuous abstinence rate from smoking changed from 79.4% at 1 month to 60.8% at 12 months after discharge. The continuous abstinence rate from smoking after 12 months was 88.5% in participants who completed the entire program (6 times), while 51.3% in participants who did not complete the entire program (${\leq}5$ times) (P=0.001). After adjustment for general and smoking-related characteristics, complete implementation of hospital-based smoking cessation intervention was significantly associated with continuous abstinence from smoking after 12 months (odds ratio: 5.93; 95% confidence interval: 1.45-24.22). Conclusions: The hospital-based smoking cessation intervention might be effective for smoking cessation in patients with cerebral infarction, especially when the intervention was implemented thoroughly.

Psychosocial Factors and Health Behavior among Korean Adults: A Cross-sectional Study

  • Objective: This study was an attempt to identify associations between health behavior, such as smoking, alcohol consumption, healthy diet, and physical activity, and psychosocial factors. Methods: This crosssectional study was conducted among 1,500 participants aged between 30 and 69 years, selected from a population-based database in October 2009 through multiple-stratified random sampling. Information was collected about the participants' smoking and drinking habits, dietary behavior, level of physical activity, stress, coping strategies, impulsiveness, personality, social support, sense of coherence, self-efficacy, health communication, and sociodemographics. Results: Agreeableness, as a personality trait, was negatively associated with smoking and a healthy diet, while extraversion was positively associated with drinking. The tendency to consume a healthy diet decreased in individuals with perceived higher stress, whereas it increased in individuals who had access to greater social support. Self-efficacy was found to be a strong predictor of all health behaviors. Provider-patient communication and physical environment were important factors in promoting positive healthy behavior, such as consumption of a healthy diet and taking regular exercise. Conclusions: Psychosocial factors influence individuals' smoking and drinking habits, dietary intake, and exercise patterns.

Hardcore Smoking in Three South-East Asian Countries: Results from the Global Adult Tobacco Survey

  • Background: Hardcore smoking is represented by a subset of daily smokers with high nicotine dependence, inability to quit and unwillingness to quit. Estimating the related burden could help us in identifying a high risk population prone to tobacco induced diseases and improve cessation planning for them. This study assessed the prevalence and associated factors of hardcore smoking in three South-East Asian countries and discussed its implication for smoking cessation intervention in this region. Materials and Methods: Global Adult Tobacco Survey (GATS) data of India, Bangladesh and Thailand were analyzed to quantify the hardcore smoking prevalence in the region. On the basis of review, an operational definition of hardcore smoking was adopted that includes (1) current daily smoker, (2) no quit attempt in the past 12 months of survey or last quit attempt of less than 24 hours duration, (3) no intention to quit in next 12 months or not interested in quitting, (4) time to first smoke within 30 minutes of waking up, and (5) knowledge of smoking hazards. Logistic regression analysis was carried out using hardcore smoking status as response variable and gender, type of residence, occupation, education, wealth index and age-group as possible predictors. Results: There were 31.3 million hardcore smokers in the three Asian countries. The adult prevalence of hardcore smoking in these countries ranges between 3.1% in India to 6% in Thailand. These hardcore smokers constitute 18.3-29.7% of daily smokers. The logistic regression model indicated that age, gender, occupation and wealth index are the major predictors of hardcore smoking with varied influence across countries. Conclusions: Presence of a higher number of hardcore smoking populations in Asia is a major public health challenge for tobacco control and cancer prevention. There is need of intensive cessation interventions with due consideration of contextual predictors.

Insights into Smoking and its Cessation among Current Smokers in India

  • Background: Initiation, perpetuation and cessation of smoking are all multifactorial. It is essential to explore interactions among various parameters influencing smoking and its cessation for effective smoking cessation interventions. Objectives: To obtain insights into smoking and its cessation among current smokers in India. Materials and Methods: The present study was conducted among current smokers visiting the Department of Oral Medicine and Radiology, Manipal College of Dental Sciences (MCODS), Manipal University, Mangalore. Knowledge, attitudes, behavior, worksite practices towards smoking and its cessation, barriers to smoking cessation and socio-demographic variables were explored using a structured, pretested, self-administered questionnaire. Results: A total of 175 current smokers participated in the study. Mean knowledge, attitude, worksite practice and barrier scores were $15.2{\pm}5.67$ (66.1%), $57.5{\pm}7.67$ (82.1%), $4.18{\pm}2.02$ (41.8%) and $57.4{\pm}12.37$ (63.7%) respectively. Correlation analysis revealed: association of knowledge with education, occupation and religion; attitude with education and occupation; worksite practices with occupation; knowledge with attitude; and barriers negatively with worksite practices. The majority (85.7%) of respondents intended to quit smoking and this was associated with higher attitude scores, whereas actual quit attempts were associated with high knowledge, attitudes, worksite practices and low barrier scores. Conclusions: Various socio-demographic factors associated with smoking and its cessation were identified. The present study highlights the importance of identifying and targeting these interactions while framing guidelines and interventions for effective tobacco cessation in a developing country like India.

흡연예방교육에 의한 청소년들의 흡연에 대한 지식 및 태도변화와 흡연량의 감소 효과 (Effectiveness of Smoking Prevention Program based on Social Influence Model in the Middle School Students)

  • 경상북도 구미시에 소재한 2개 남자 중학교 1학년 학생 665명(교육군 10개 학급 367명, 대조군 8개 학급 298명)을 대상으로 1999년 4월부터 2000년 4월까지 교육군과 대조군으로 구분하여 시행한 지역사회에서의 준실험설계 연구를 통하여 흡연예방과 금연을 위한 교육프로그램이 학생들의 흡연에 대한 지식과 태도를 변화시키는 정도와 흡연율에 미치는 영향을 분석하고, 이들 변화와 관련되 요인을 분석하는 것에 목적을 두고 시행한 본 연구의 그 결과는 다음과 같다. 교육군과 대조군의 흡연에 대한 지식점수의 변화는 교육군에서 1.31점으로 대조군의 0.13점에 비하여 유의한 차이를 보였다(p<0.01). 교육군의 흡연에 대한 사전조사에서의 태도점수는 13점 만점에 $9.89{\pm}2.51$점이었고, 1년 후에는 $10.16{\pm}2.32$점이었고, 대조군은 사전조사에서의 $9.86{\pm}2.64$점이었고, 1년 후에는 $9.12{\pm}3.09$점이었다. 태도점수의 변화는 교육군에서 0.27점으로 대조군의 -0.74점에 비하여 유의한 차이를 보였다(p<0.05). 흡연에 때한 법적 제제에 대한 태도 중에서 담배광고 금지에 대한 교육군에서 사전조사에서 42.8%가 담배광고금지를 찬성하였으며, 주적조사에서는 54.2%가 찬성하여 유의한 변화를 보였으며(p<0.05), 교육군과 대조군의 변화를 비교하면 교육군과 대조군에서 각각 11.4%와 3.0% 포인트 변화하여 두 군간의 변화가 유의한 차이가 있었다(p<0.05). 담배자판기 설치를 반대하는 비율은 교육군에서는 4.6% 포인트 상승하였고, 대조군에서 각각 -11.1%감소하여 두 군간에 유의한 차이가 있었다(p<0.01). 성인이 되어 흡연을 할 의향이 있는 흡연 의도율은 교육군이 사전조사와 추적조사에서 각각 24.3%와 24.8%로 거의 변화가 없었고, 대조군은 26.5%와 32.2%로 5.7% 포인트 증가하였다. 교육군에서 대조군에 비하여 유의하게 낮았다(p<0.05). 친구의 흡연 권고를 거절할 수 있다는 비율은 교육군에서 사전조사와 추적조사에서 각각 90.2%와 88.3%로 1.9% 포인트 감소한 반면, 대조군에서는 사전조사와 추적조사에서 각각 86.39와 76.2%로 10.7% 포인트 감소하여 유의하게 감소하였다(p<0.01). 교육군과 대조군간에도 유의한 차이가 있었다(p<0.01). 흡연율은 교육군과 대조군에서 각각 1.6%와 1.7%에서 1년 후 2.7%와 3.0%였으며, 통계적으로 유의한 차이는 없었다. 흡연량은 교육군에서는 $3.7{\pm}8.2$개피에서 $1.4{\pm}2.5$개피로 일일 평균 흡연량이 2.3개피 감소하여 유의하게 감소하였고(p<0.05), 대조군의 경우는 $2.9{\pm}7.1$개피에서 3.1개피로 다소 증가하였다. 흡연에 대한 지식점수의 변화를 종속변수로 한 다중회귀분석에서는 지식점수의 변화에 유의한 영향을 미치는 변수는 흡연예방교육여부와 흡연에 대한 사전지식점수, 학생의 학교성적이었다. 흡연에 대한 태도 접수의 변화를 종속변수로 한 다중회귀분석에서는 흡연예방교육여부와 흡연에 대한 사전지식점수가 유의한 변수였다. 이상의 결과로 볼 때 사회영향모델에 의한 학교에서의 학생에 대한 흡연예방교육과 상담 및 교내환경과 학교주변 환경의 변화를 위한 사회영향 모형에 의한 교육 프로그램에 의하여 학생들의 흡연에 대한 지식과 태도와 흡연 의도율 등을 유의하게 변화시킬 수 있었으며, 흡연량은 유의하게 감소하였다. 향후 장기적인 추적을 통한 코호트 연구를 통하여 흡연율 감소효과를 평가하여야 할 것이며, 단순한 지식을 제공하는 일회성의 단편적 교육에서 벗어나 학생자신은 물론 학생주변 환경의 변화를 포괄하는 다양한 방법에 의한 흡연예방과 금연교육이 시행되어야 할 것이다.

유아간접흡연 예방에 대한 부모의 참여의도 관련변인 연구 (A study on related factors of parent's participation intention for early childhood secondhand smoke prevention)

  • This study aims to 1) examine the difference of parent's participation intention for early childhood secondhand smoke prevention depending on sex, education level, income level, and smoking or nonsmoking of parent, parent's knowledge and attitude of early childhood secondhand smoke 2) estimate the influence of independent factors on parent's participation intention for early childhood secondhand smoke prevention centering around factors stated previously, and 3) suggest effective strategies to ultimately encourage parent's behavior for early childhood secondhand smoke prevention. A total of 279 parents with four- and five-year-old day care children participated in this study. The following are the main results of this study. First, the higher the level of income parent had, the higher the level of participation intention for early childhood secondhand smoke prevention parent had. Second, nonsmoking parent exhibited higher level of participation intention than smoking parent. Finally, parent's knowledge and attitude of early childhood secondhand smoke had influence on parent's participation intention for early childhood secondhand smoke prevention, but parent's attitude had a high degree of influence on parent's participation intention.

건강증진사업을 위한 보건교육프로그램 개발 : 금연, 절주교육을 중심으로 (Developing Health Education Programs for Health Promotion: Development of a Training Guide on Prevention of Smoking and Excessive Drinking)

  • Background: Alcohol and tobacco use are two major behavioral risk factors implicated in increased morbidity and mortality. Since both substances are widely used in Korea, a concerted effort is currently underway to reduce the use of tobacco and alcohol in Korea. Objectives: Efforts directed toward educating health promotion planners and health educators from local health departments to organize and implement health education programs to reduce the proportion of people smoking and excessive drinking in the community. Methods: A training guide on prevention of smoking and excessive drinking has been developed. Comprehensive multi-media health promotion materials were developed based on health behavior theories and strategies for effective health behavioral interventions. To better control of behavioral risk and promote health enhancement, the materials were developed to introduce a user-oriented developmental approach by making messages more persuasive and organizing content in a user-friendly manner. Sections of the report explain theoretical background of the intervention, choosing goals and a target population, specific program activities to include in an intervention, development, organization, methods of making program content vivid and persuasive, and evaluation,. Implications for Practitioners: The process followed in developing the health intervention materials is described in detail to assist practitioners who need to develop effective programs to reduce the use of tobacco and excessive alcohol. Health educators from all local health departments in the country were trained in the use of the materials, to enable them to develop community interventions to reduce smoking and excessive drinking.
