Comparison of Risk Factors for Men and Women According to Severity Classification in Patients with Coronary Artery Disease (관상동맥질환 중증도 분류에 따른 성별 위험요인 비교)

  • The aim of this retrospective study was to compare risk factors between men and women through secondary data of nursing information and medical records according to the severity classification in patients of 340 men and 221 women with coronary artery disease(CAD) who were admitted for the first time at a general hospital and underwent first coronary artery angiography. Consequently, men presented with risk factors such as age(p=.004), total cholesterol(p=.040), triglycerides(p=.049), HbA1c(p<.001), smoking(p<.001), alcohol consumption(p=.002) and comorbidities(p=.036) that showed statistically significant differences. Among women, age(p=.002) and comorbidities (p=.018) were the only factors that showed significant differences. Significant risk factors influencing the classification of CAD severity in men were total cholesterol (OR 0.97, 95% CI 0.96-1.00, p=.014) in 1VD, alcohol consumption (OR 52.47, 95% CI 2.99-91.95, p=.007)in 2VD, and total cholesterol in the 3VD(OR 0.98, 95% CI 0.95-0.98, p=.026). A significant risk factor affecting the classification of CAD severity in women was comorbidity (OR 0.30, 95% CI 0.11-0.82, p=.020) in the 3VD. This study identified the importance of nursing care for male CAD patients, such as smoking cessation and quitting drinking, blood sugar control, cholesterol, and accompanying disease management, and provided evidence of individually tailored nursing care.

Factors Affecting Health-Related Quality of Life in Patients with Chronic Obstructive Pulmonary Disease using Health-Related Quality of Life Instrument with 8 Items (Health-Related Quality of Life Instrument with 8 Items을 사용한 만성폐쇄성폐질환 환자의 건강관련 삶의 질 영향요인)

  • This study was attempted to identify the health-related quality of life of Chronic Obstructive Pulmonary Disease (COPD) patients and factors influencing the quality of life, focusing on Health-related quality of life with 8 items (HINT-8). The subjects of this study were 451 adults aged 40 years or older who performed lung function tests and whose ratio is less than 0.7 by measuring forced respiratory volume in 1 second [FEV1] to forced vital capacity in the 2019 National Health and Nutrition Examination Survey, It was analyzed using SAS program. As a result, both the HINT-8 index and EuroQol five-dimensions 3-level version (EQ-5D-3L) index were appropriate as tools to measure the health-related quality of life in COPD patients, and the factors affecting the health-related quality of life were age, gender, income, and smoking status, comorbidities, stress, and subjective health status. Therefore, in order to improve the health-related quality of life of COPD patients, an individualized management program suitable for the characteristics of subjects such as the low-income class and the elderly, including smoking cessation education and stress management, should be developed and applied.

Health Behaviors between a Health Promotion Demonstration Health Center and a General Health Center (건강증진사업 시범여부에 따른 지역주민의 사업인식과 참여도, 건강행위 비교 연구)

  • Purpose: The purpose of the study was to compare community residents' perceptions, participation, satisfaction, and behavioral changes between a health promotion demonstration health center and general health center. Method: The design of the study was ex-post facto that compared community residents in demonstration health centers and general health centers. The sample included 2,261 community residents who were conveniently selected from demonstration (792 participants) and general health centers (1,496 participants). Result: The results of the study were as follows: 1) Perception and participation rates of exercise, nutrition, and hypertension management programs were significantly higher in the participants of demonstration health centers than those of general health centers.; 2) Satisfaction rates of all programs except the smoking cessation program were significantly higher in the participants of demonstration health centers than those of general health centers. However, only the exercise rate among risk behaviors of participants was significantly higher in demonstration health centers than general health centers. Conclusion: Systematic efforts for health promotion were effective not only in improving the community's awareness, participation, and satisfaction of the program, but also in changing health behaviors. This evidence should be used to foster and disseminate health promotion programs toother health centers to improve community residents' health status and quality of life.

Estimating the Economic Burden of Lung Cancer in Iran

  • Objectives: Lung cancer is a major public health problem and one of the most costly illnesses. The study aimed to estimate the economic burden of lung cancer in Iran in 2014. Methods: A cross-sectional study was conducted to estimate the direct and indirect costs for patients with lung cancer using a prevalence-based approach. A human capital approach was employed to estimate the indirect costs. Data were obtained from several sources such as through patient interview using structured questionnaire, medical records, the GLOBOCAN databases, the Iranian Statistical Center, the Iranian Ministry of Cooperation, Labor and Social Welfare, and the Institute for Health Metrics and Evaluation (IHME). Results: The economic burden of lung cancer in Iran in the year 2014 was 3,225,998,555,090 IR. The main components of the cost were associated with mortality (81.9 %) and hospitalization (7.6 %). The costs of direct medical care, non-medical aspects, patient time, and mortality accounted for 10.8%, 2.7%, 4.5%, and 81.5% of the total cost, respectively. Conclusion: Findings from this study indicated that the economic burden of lung cancer is substantial both to Iran's health system and to society as a whole. Early diagnosis, strengthening cancer prevention, implementing new cancer therapy and medical technology, and effective smoking-cessation interventions could offset some of the costs associated with lung cancer in Iran.

Effects of Cardiac Rehabilitation Teaching Program on Knowledge Level and Compliance of Health Behavior for Patients with Myocardial Infarction (심장재활 교육프로그램이 심근 경색증 환자의 질병관련 지식과 건강행위 이행에 미치는 효과)

  • The purpose of this study was to investigate the effects of cardiac rehabilitation teaching program on knowledge level and compliance of health behavior for the patients with myocardial infarction. Method: The subjects were 47 patients 23 were assigned to the experimental group and 24 were for the control. The cardiac rehabilitation teaching program is a individualized teaching program which was delivered to the experimental group during hospitalization period by present researcher. Data were collected through questionnaire surveys for knowledge level and compliance of health behavior from September 15, 1999 to December 31, 2000. The collected data was analyzed by using the SAS program. Results: 1. With regard to the knowledge scores 1) The total knowledge level in the experimental group was significantly higher than in the control group. 2) As to the knowledge domains, nature of disease, risk factors, diet, medication, exercise, and daily activities were significantly higher in score in the experimental group than in the control group. 2. With regard to the compliance of health behavior 1) The average compliance with good health behavior was significantly higher in the experimental group than in the control group. 2) As to the health behavior domains smoking cessation, diet, stress management, regular exercise, and other measures for lifestyle modification were significantly higher in score in the experimental group than in the control group. 3. The pre-treatment knowledge score was positively correlated to the post-treatment knowledge score and post- treatment knowledge score was positively correlated to the post-treatment compliance of health behaviors. Conclusion: The above findings indicate that the cardiac rehabilitation teaching program for the experimental group was effective in increasing level of knowledge and improvement of compliance with good health behavior of patients with myocardial infarction.

A Critical Evaluation of DNA Adducts as Biological Markers for Human Exposure to Polycyclic Aromatic Compounds

  • The causative role of polycyclic aromatic hydrocarbons (PAH) in human carcinogenesis is undisputed. Measurements of PAH-DNA adduct levels in easily accessible white blood cells therefore represent useful early endpoints in exposure intervention of chemoprevention studies. The successful applicability of DNA adducts as early endpoints depends on several criteria:i.adduct levels in easily accessible surrogate tissues should reflect adduct levels in target-tissues, ii. toxicokinetics and the temporal relevance should be properly defined.iii. sources of inter- and intra-individual variability must be known and controllable, and finally iv. adduct analyses must have advantages as compared to other markers of PAH-exposure. In general, higher DNA adduct levels or a higher proportion of subjects with detectable DNA adduct levels were found in exposed individuals as compared with non-exposed subjects, but saturation may occur at high exposures. Furthermore, DNA adduct levels varied according to changes in exposure, for example smoking cessation resulted in lower DNA adduct levels and adduct levels paralleled seasonal variations of air-pollution. Intra-individual variation during continuous exposure was low over a short period of time (weeks), but varied significantly when longer time periods (months) were investigated. Inter-individual variation is currently only partly explained by genetic polymorphisms in genes involved in PAH-metabolism and deserves further investigation. DNA adduct measurement may have three advantages over traditional exposure assessment: i. they can smooth the extreme variability in exposure which is typical for environmental toxicants and may integrate exposure over a longer period of time. Therefore, DNA adduct assessment may reduce the monitoring effort. ii. Biological monitoring of DNA adducts accounts for all exposure routes. iii. DNA adducts may account for inter-individual differences in uptake, elimination, distribution, metabolism and repair amongst exposed individuals. In conclusion, there is now a sufficiently large scientific basis to justify the application of DNA adduct measurement as biomarkers in exposure assessment and intervention studies. Their use in risk-assessment, however, requires further investigation.

Research Trends for Chronic Obstructive Pulmonary Disease in Complementary and Alternative Medicine (만성폐쇄성폐질환의 보완대체의학 임상연구 동향)

  • Objectives : This study analyzed the contents of research papers of complementary and alternative medicine (CAM) concerning chronic obstructive pulmonary disease (COPD) published in PubMed during the last 5 years. This study was conducted to help clinical studies for treating COPD with Oriental medicine. Materials and Methods : We inspected 31 theses and scrutinized their objectives, periods, participants, materials and methods, methods of assessment, results and Jadad score. Results : The treatments in the studies were supplements, physical training, acupuncture, acu-TENS, Bojungikgi-tang (Buzhongyiqi-tang), counseling, breathing training, osteopathic manipulative treatment, reflexology, and distractive auditory stimuli. The aims of treatment were improvement of exercise capacity, lung function, quality of life, oxidative status, nutrient status, systemic inflammation, and cessation of smoking. The median for treatment period of study was 8 weeks, the median number of participants was 35 and the differences between mean $FEV_1$ from groups were less than 10% in 22 studies. The methods of assessment were lung function test, exercise capacity test, muscle strength test, questionnaire of QoL, laboratory studies, and measurement of nutrient state. The mean of Jadad score was $2.4\;{\pm}\;1.03$, and 24 treatment were assessed as effective. Conclusion : Recent CAM studies of COPD have focused on various topics in alternative and complementary medicine, and it is necessary to provide objective studies for treatment of this disease with Oriental medicines.

Health Behavior and Utilization of University Health Clinics (대학생의 건강생활 실천 및 대학보건실 활용 수준에 관한 연구)

  • Objectives: This study was conducted to analyze health behavior and utilization of university health clinics by college and university students. Methods: There are 376 colleges and universities in Korea as of May 2007. Of them 243 (69.6%) run health clinics in their campuses. Twenty of them were selected and 1,754 students were surveyed by using self reporting questionnaires. The data collected were analyzed by SPSS ver. 12.0. Results: Out of 1,754 students 56.5% were in good health. Of the subjects 15.6% were cigarette smokers, 30% were non drinkers, 20.8% exercise regularly, 53.7% sleep for seven to eight hours a day, 48% had proper body weight (BMI) 20-25) and 20% had physical examination in the past two years. The scores of health practices were higher in male students than female students (P 0.05). Perception rates of university clinics were higher in female students than male students. (P 0.01). Many students have intention to use the clinics (P 0.01). On health promotion program 35.2% of the subjects used clinics for emergency service, 32.8% participated in drinking control program, 32.8% in smoking cessation program, 20.4% in immunization program and 19.2% in regular health screening program. Conclusions: Comprehensive data for health behavior and need of health promotion services by the students should be collected regularly in order to meet the needs of students and faculty members in higher education institutes. The university authorities should pay more attention on the activities of health clinics in order to promote health of the students.

In Vivo Prediction and Biopharmaceutical Evaluation of Nicotine Transdermal Patch (생체내 예측 및 흰쥐를 이용한 니코틴 패취의 약물동력학적 평가)

  • Nicotine transdermal therapeutic systems $(TTS_S)$ have been regarded as an effective mean to aid smoking cessation. However, most of nicotine $TTS_S$ in the market have some problems such as unpleasant side effects and skin irritation due to the excess amount of the drug permeated and the properties of the additives employed. In order to solve these problems, new nicotine $TTS_S$ were formulated using biocompatible additives. The optimized formula of the drug layer consisted of nicotine, propylene glycol and poloxamer 188 at the ratio of 1.2: 17.0: 2.0. The drug layer had the sickness of $1,250\;{\mu}m$, the pH of 8.12. The skin permeation rate of nicotine from optimized nicotine patch (NP) was $21.5\;{\mu}g/cm^2/h$. Transdermal administration of nicotine patch has been carried out for the determination of pharmacokinetic parameters in rats. Steady-state plasma concentration of nicotine following transdermal application of NP (area of patch = $15\;cm^2$) on the dorsal skin of rats was 143.2 ng/ml and AUC for 24 hrs was 3,022 ng h/ml. In case of $EXODUS^{\circledR}$ and Nicotinell $TTS^{\circledR}$, the steady-state plasma concentration of nicotine and ACU for 24 hrs were 428.9 ng/ml, $9,121\;ng{\cdot}hr/ml$ and 155.3 ng/ml, $3,152\;ng{\cdot}h/ml$, respectively. NP showed the experimental plasma nicotine concentration profile was very similar to the simulated one and had an appropriate skin permeation rate and a steady-state concentration of nicotine, which can show therapeutic blood levels of the drug for 24 hrs without severe side effects.

Analysis of Community-based Health Promotion Program Targeting School-aged Children and Program Using Forest: Based on National Community Health Plan (학령기아동 대상 지역사회 건강증진 프로그램 및 숲을 활용한 프로그램 분석: 전국 지역보건의료계획을 기반으로)

  • Purpose: The purpose of this study is to analyze community-based health promotion program for school-aged children and program using forest. Methods: Seventeen health promotion programs focused on school-aged children from Community Health Plan were selected to analyze after assembling 227 of the 5th National Community Health Plans. The analysis duration was from 2012 July to November. Results: Among 17 programs, the health promotion program targeting school-aged children were included in 16 programs except one program focusing on community- orientated rehabilitation program. Eight health promotion programs using forest in 7 different areas were found. The majority of the community-based health promotion programs were focused mainly on smoking cessation, obesity, physical activity, nutrition, mental health programs. Furthermore, there was a limitation of programs utilizing forest as a health promotion resource and most of the programs using forest were located in Jeollanamdo and focusing mainly on atopy prevention and treatment. Conclusion: The importance of this study is that it analyzed nation-wide community health plan systematically, and analyze community-based health promotion program targeting school-aged and the program using forest. The results of the analysis can be used as baseline data for developing physical and mental health promotion programs using forest targeting school-aged children.
