The Modeling of the Differential Measurement of Air Pressure for Non-intrusive Sleep Monitoring Sensor System

  • The respiratory and heart beat signals are the fundamental physiological signals for sleep monitoring in the home. Using the air mattress sensor system, the respiration and heart beat movements can be measured without any harness or sensor on the subject's body which makes long term measurement difficult and troublesome. The differential measurement technique between two air cells is adopted to enhance the sensitivity. The concept of the balancing tube between two air cells is suggested to increase the robustness against postural changes during the measurement period. With this balancing tube, the meaningful frequency range could be selected by the pneumatic filter method. The mathematical model for the air mattress and balancing tube was suggested and the validation experiments were performed for step and sinusoidal input. The results show that the balancing tube can eliminate the low frequency component between two cells effectively. This technique was applied to measure the respiration and heart beat on the bed, which shows the potential applications for sleep monitoring device in home. With the analysis of the waveform, respiration intervals and heart beat intervals were calculated and compared with the signal from conventional methods. The results show that the measurement from air mattress with balancing tube can be used for monitoring respiration and heart beat in various situations.

  • Purpose: This study aimed to observe change in factors over time in the first cohort of the 2018 Children and Adolescent Panel Middle School. In addition, this study attempted to examine the causal relationship and influence between variables by setting a time gap between independent and dependent variables. Methods: Frequency and descriptive statistical analyses were conducted to determine the general characteristics of the study participants. Hierarchical regression analysis was conducted to analyze the effects of smartphone dependence on health, sleep quality, and depression. After inputting the control variables (Model 2), the influence of the variables was identified based on the input model. Results: Smartphone dependence positively impacted depression in both male and female students and negatively impacted sleep and health. Conclusion: Smartphones are closely associated with teenagers' lives. Additionally, as adolescents experience various psychological anxieties owing to rapid physical changes, there are concerns that psychological dependence may increase, considering that adolescence is the most emotionally unstable period. Therefore, the results of this study consistently prove that smartphone dependence has a causal relationship with emotion-related variables, such as emotional stress, depression, and anxiety.

교대근무 간호사의 혈액과 체액 노출 경험과 수면장애, 피로, 직무스트레스와 상관성 (Association of Sleep Disturbance, Fatigue, Job Stress and Exposure to Blood and Body Fluid in Shift-work Nurses)

  • Purpose: The purpose of this study was to identify associations among the sleep disturbance, fatigue, job stress, and blood and body fluid (BBF) exposure of shift-work nurses. Methods: A total of 299 shift-work nurses from two tertiary hospitals were enrolled in this study. We used the Insomnia Severity Index (ISI), and the Fatigue Severity Scale (FSS), and the Korean Occupational Stress Scale-Short Form (KOSS-SF) to evaluate sleep disturbance, fatigue, and job stress, respectively. The data were analyzed using t-test or chi-squared test and Logistic regression analysis using the SPSS 23.0 program. Results: We found that 43.8% of participants reported BBF exposure over the past year. Splash or exposure to broken skin of BBF were most frequent (56.9%), and followed by needlestick injuries (30.4%) and sharp injuries (12.8%). Age, hospital, working period, level of stress, sleep disturbance ($ISI{\geq}15$), fatigue (FSS $score{\geq}4$), job demand and organizational climate subset in KOSS-SF were significantly associated with BBF exposure in shift-work nurses. In multivariate analysis after adjusting age and hospital, the risk factors of BBF exposure in shift-work nurses were the level of stress and fatigue (FSS $score{\geq}4$). Conclusion: Fatigue and job stress were related to BBF exposure in shift-work nurses. Our results suggest that management of sleep disturbance, fatigue, and high job stress in shift-work nurses is needed to reduce risk of BBF exposure.

The Effect of Hypnotics on Sleep Quality and Cognitive Function in Patients with Brain Tumors

  • Objective : We investigated the effect of hypnotics on sleep quality, cognitive function, and depressive mood in patients with insomnia following brain tumor resection. Methods : From patients who underwent brain tumor resection, we recruited 10 patients with insomnia who received hypnotics for more than 1 week during a 3-week follow-up period (insomnia group). We also recruited 12 control patients with brain tumors but without insomnia (control group). We evaluated sleep quality at baseline and 3 weeks later using the Insomnia Severity Index (ISI), the Pittsburgh Sleep Quality Index (PSQI), the Stanford Sleepiness Scale (SSS), and the Epworth Sleepiness Scale (ESS) and investigated cognitive function and depression using the Computerized Neuropsychological Test and the Beck Depression Inventory (BDI). Results : At baseline, SSS, ISI, PSQI, and BDI scores were significantly higher and visual continuous performance test (VCPT) and auditory continuous performance test (ACPT) scores were significantly lower in the insomnia than in the control group. Three weeks later, the patients who had received hypnotics had significantly higher ISI, PSQI, ESS, VCPT, ACPT, visual span forward and backward, and visual recognition test scores, and significantly lower BDI scores. Conclusion : Quality of sleep in patients with insomnia following brain tumor resection was initially poor but improved significantly after taking hypnotic medication. Further, the hypnotic medications appeared to contribute to the amelioration of cognitive impairments and depressive moods in patients who previously underwent brain tumor resection. We thus recommend the use of hypnotics for patients with brain tumors with insomnia.

수면무호흡증을 관리를 위한 스마트 베개 시스템의 설계 (Design of Smart Pillow System for Managing Sleep Apnea)

  • 전문의에 의해 옆으로 누워도 편안함을 주는 등 수면과학과 인체공학을 고려한 베개가 개발되었다. 이 베개는 천연라텍스를 소재로 하여 일정시간 지나면 복원력이 떨어지는 점을 개선하였다. 이 베개에 새로운 아이디어가 추가되었는데 베개는 당연이 수면을 위해 사용되는 것이나, 건강관리를 위한 부가적인 기능을 여기에 추가할 수도 있지 않을까 하는 것이었다. 여기서 건강관리는 심각한 질병과 연관된 것으로 알려진 수면무호흡증을 대상으로 한다. 본 논문은 압력센서와 음성센서를 이용해 정보를 구하고 이 정보로부터 질병에 관한 이상증상을 파악하여 전문의에 의뢰하는 종합적인 서비스를 설계함을 목적으로 한다. 그리고 이 시스템의 성공 가능성을 확인하기 위한 기초적인 설계와 구현을 다룬다. 이 설계를 바탕으로 얻어진 정보를 DB화하고 전문의와의 상담을 위한 서버 시스템을 완성하여 수면 무호흡증을 위한 보조건강기기로 역할을 담당할 수 있도록 업그레이드하는 방안에 대해 살펴본다.

향요법 발반사마사지가 복지시설노인의 스트레스, 우울 및 수면에 미치는 효과 (Effects of Aromatherapy and Foot Reflex Massage on Stress, Depression, and Sleep Pattern of the Institutionalized Elderly)

  • The purpose of this study was to investigate the effects of aroma therapy and foot reflex massage on stress, depression, and sleep pattern of institutionalized elderly. The design of this study was non-equivalent control group pre-posttest design. The data were collected from February 24 to April 9 of 2005. Fifty-one elderly were divided into two groups, 26 institutionalized elderly for the experimental group and 25 institutionalized elderly for the control group. The experimental group underwent aromatherapy and foot reflex massage two days per week(40 minutes per session} over five weeks period. Stress(subjective stress and serum cortisol level), depression, and sleep pattern were assessed in both groups before and after intervention. Five weeks after intervention were spended, the experimental group was again assessed. Five weeks without intervention were spended, the control group was again assessed. The data were analyzed by the statistical program SPSS WIN 12. The results of this study were as follows; $\cdot$ Stress of the experimental group with the intervention was significantly lower than that of the control group. $\cdot$ Depression of the experimental group was significantly lower than that of the control group. $\cdot$ Sleep pattern of the experimental group was significantly higher than that of the control group. In conclusion, the aroma therapy and foot reflex massage program had positive effect on Institutionalized elderly's on stress(subjective stress and serum cortisol level), depression, and sleep pattern.

여대생의 지각된 스트레스, 우울 및 수면장애가 월경곤란증에 미치는 영향 요인 (Factors Affecting Perceived Stress, Depression and Sleep Disturbance on the Dysmenorrhea among Female University Students)

  • 본 연구는 여대생의 지각된 스트레스, 우울, 수면장애, 월경곤란증의 정도 및 그들 변수 간의 관계와 월경곤란증에 미치는 영향 요인을 파악하여 추후 월경곤란증 완화를 위한 프로그램 개발의 기초자료로 제공하기 위한 서술적 조사연구이다. 대상자는 C도 J대학교에 재학 중인 여자대학생을 대상으로 11월 11일-12월 9일 총 4주 동안 총 160부의 자료를 수집하여 분석하였다. 연구결과, 월경곤란증과 지각된 스트레스(r=.420, p<.001), 우울(r=.459, p<.001), 수면장애(r=.429, p<.001)와는 양의 상관관계를 나타냈다. 월경곤란증에 미치는 영향 요인은 수면장애, 우울, 지각된 스트레스 순으로, 전체 설명력은 29.3%로 나타났다. 따라서, 대학생의 월경곤란증을 완화하기 위해서는 수면의 질을 높이고 우울은 감소시키며 지각된 스트레스는 낮추기 위한 중재프로그램 개발 및 방안이 모색되어야 하겠다.

조혈모세포 이식환자의 입원 후 수면의 질과 수면양상의 변화 (Sleep Quality and Sleep Patterns of Patients Treated by the Hemopoietic Stem Cell Transplantation)

  • 목적: 조혈모세포이식환자의 입원 후 수면양상의 변화 및 수면장애를 파악함으로써, 조혈모세포이식 환자의 수면을 위한 간호중재를 개발하기 위한 기초자료를 제공하기 위하여 시도하였다. 방법: 서울 C대학병원에 2002년 8월부터 2003년 8월까지 조혈모세포이식를 위해 입원한 환자 52명을 대상으로, PSQI와 수면양상 설문지를 이용하여 조사하였다. 조사된 자료는 SAS프로그램을 이용하여, 입원전과 입원 후의 PSQI 차이는 paired t-test로, 입원시와 퇴원시의 수면양상의 변화는 카이검정과 Fisher's exact test로 분석하였다. 결과: 대상자의 입원 후 한 달 동안 PSQI점수는 11.8점으로 입원 전 한달 동안의 5.3점보다 유의하게 높았다(t=11.41, P=0.0000). 7개의 항목 즉 수면의 질, 수면지연, 수면기간, 수면의 효율성, 수면장애, 수면제 복용, 일상생활의 장애 모두에서 입원 후 한달 동안의 PSQI점수는 입원 전 한달 동안에 비하여 유의하게 높았다. 대상자의 입원시와 퇴원시의 수면양상을 살펴보면 취침시각, 잠드는데 걸리는 시간, 기상시각, 총 수면시간, 낮잠 시간, 밤에 자다 깨는 횟수, 수면제 사용 횟수, 잠이 안 오면 하는 일, 수면을 취할 수 없는 이유 등에서 유의한 차이가 있었다. 결론: 조혈모세포이식환자는 입원 후 PSQI에 의한 수면의 질 점수가 입원 전에 비해 유의하게 높아, 입원기간동안 수면에 어려움이 많은 것을 알 수 있었다. 수면양상도 입원시에 비해 퇴원시 취침 시간, 잠드는데 걸리는 시간, 기상시각, 총 수면시간, 낮잠시간, 밤에 자다 깨는 횟수, 수면제사용횟수, 잠이 안 오면 하는 일, 수면을 취할 수 없는 이유의 항목에서 유의한 변화가 있음을 알 수 있었다.%), 산재형 47명(17.0%), 가정형 34명(12.3%)의 순으로 나타났다. 결론: 본 연구의 결과 입원환자 가족의 호스피스에 대한 인지 및 요구도는 전반적으로 높았으며 일반적 특성에 따라 호스피스 인지 및 요구도에 차이를 보였다. 따라서 대상자의 일반적 특성을 고려한 개별적이고 다양한 접근을 시도할 수 있는 호스피스 교육 및 중재 프로그램을 개발하여 수행해야 할 것이다.'(10면)이나, 비참석가족들은 '외로움(7명)', '경제적 문제/역할수행상의 어려움'(7명)의 빈도였다. 대처방법에서는, '영적 승화'(13명), '일상생활에의 몰두'(10명), '애도과정에의 몰입(계속 슬퍼함)'(3명)으로 응답하였다. 병원으로부터 사별가족모임의 초대장을 받았을 때의 느낌에 대해서는 '반가움과 고마움'(21명), '슬픔'(4명), '괴로움'(4명), 무응답(11명)이었다. 비 참석 가족들은 '반가움과 고마움'(4명)이었다. 사별가족 모임 참석에 대한 망설임의 유무와 이유에 대해서는, '망설이지 않았다'(34명)가 '망설였다'(6명)보다 높게 나타났다. 사별가족모임에 참석한 후의 소감, 개선사항, 아쉬운 점에 대한 질문에는, 대부분의 응답자들이 '의미 있는 시간이었다', '사별가족에 대한 배려와 관심에 대해 감사한다.'라는 긍정적인 응답이 있었고, '고인에 대한 회상을 할 수 있는 자리여서 좋았다.' '사별가족모임이 일년에 한 번이 아니라 계속적으로 있었으면 한다.', '한편은 슬프고 한편은 기쁘다.' 등의 의견이 있었다. 사별 후 느끼는 가장 큰 어려움에 대해 참석가족은, '우울감'(10명), '외로움'(7명)의 빈도가 높았고, '그리움'(1명), '경제적/역할상 어려움'(4명), 무응답(6명)으로, '우울감'이 가장 큰 어려움으로 나타났다

느린 안구 운동(SEM)과 뇌파의 스펙트럼 동시 분석을 이용한 1단계 수면탐지 (Automatic Detection of Stage 1 Sleep Utilizing Simultaneous Analyses of EEG Spectrum and Slow Eye Movement)

  • 목 적:1단계 수면은, 입면 시점과 관련하여 수면다원기록의 해석에 중요한 정보를 제공한다. 1단계 수면은 각성 상태에서 수면 상태로의 짧은 전이 기간으로, 특징적인 지표가 없어 디지털 분석을 통한 수면 단계 결정에 어려움이 있다. 본 연구에서는, 뇌파와 안전도에 대한 디지털 분석을 통하여 1단계 수면을 자동으로 탐지하는 프로그램을 개발하고자 하였다. 방 법:야간수면다원기록 중 검사 시작 시점부터 2단계 수면이 출현하기 이전의 자료를 분석하였다. 뇌파의 스펙트럼 분석을 통해 알파파와 세타파의 상대 파워를 계산하였고, 알파파의 상대 파워가 50% 이하, 세타파의 상대 파워가 23% 이상일 경우 1단계 수면 판정의 기준 변수로 하였다. 또 안구운동의 지속시간이 1.5초에서 4초 사이에 있는 경우에 느린 안구운동(SEM)으로 판정하고 1단계 수면 판정의 기준변수로 하였다. 이 들 세 기준 변수들을 고려하여 해당 판독단위에 대해 각성 혹은 1단계 수면으로 최종 판정하였다. 결 과:연구 대상자는 7명으로 모두 남성이었으며, 23세였다. 개발된 프로그램을 이용하여 169개의 판독단위를 분석하였다. 기준과의 일치도는 79.3%였으며, 카파값은 0.586이고, 통계적으로 유의하였다. 느린 안구운동은 169개의 판독단위 중 54개(32%)에서 나타났으며, 70.4%의 일치도를 보였다. 결 론:기존 연구의 디지털 분석을 통한 수면 단계 판정의 일치도는 70%이다. 본 프로그램의 일치도 79.3%는 기존 연구 결과에 비해 향상된 것이며, 본 프로그램이 1단계 수면 판정에 유용하다고 판단된다. 뇌파 외에 안전도를 고려한 다중적 접근이 일치도 향상에 기여했을 것으로 생각되며, 1단계 수면 판정에 있어 안전도의 중요성을 확인할 수 있었다.

계획된 간호 정보가 수면량에 미치는 영향에 관한 연구 -개심술 환자를 중심으로- (The Effect of Structured Information on the Sleep Amount of Patients Undergoing Open Heart Surgery)

  • The main purpose of this study was to test the effect of the structured information on the sleep amount of the patients undergoing open heart surgery. This study has specifically addressed to the Following two basic research questions: (1) Would the structed in formation influence in the reduction of sleep disturbance related to anxiety and Physical stress before and after the operation? and (2) that would be the effects of the structured information on the level of preoperative state anxiety, the hormonal change, and the degree of behavioral change in the patients undergoing an open heart surgery? A Quasi-experimental research was designed to answer these questions with one experimental group and one control group. Subjects in both groups were matched as closely as possible to avoid the effect of the differences inherent to the group characteristics, Baseline data were also. collected on both groups for 7 days prior to the experiment and found that subjects in both groups had comparable sleep patterns, trait anxiety, hormonal levels and behavioral level. A structured information as an experimental input was given to the subjects in the experimental group only. Data were collected and compared between the experimental group and the control group on the sleep amount of the consecutive pre and post operative days, on preoperative state anxiety level, and on hormonal and behavioral changes. To test the effectiveness of the structured information, two main hypotheses and three sub-hypotheses were formulated as follows; Main hypothesis 1: Experimental group which received structured information will have more sleep amount than control group without structured information in the night before the open heart surgery. Main hypothesis 2: Experimental group with structured information will have more sleep, amount than control group without structured information during the week following the open heart surgery Sub-hypothesis 1: Experimental group with structured information will be lower in the level of State anxiety than control group without structured information in the night before the open heart surgery. Sub-hypothesis 2 : Experimental group with structured information will have lower hormonal level than control group without stuctured information on the 5th day after the open heart surgery Sub-hypothesis 3: Experimental group with structured information will be lower in the behavioral change level than control group without structured information during the week after the open heart surgery. The research was conducted in a national university hospital in Seoul, Korea. The 53 Subjects who participated in the study were systematically divided into experimental group and control group which was decided by random sampling method. Among 53 subjects, 26 were placed in the experimental group and 27 in the control group. Instruments; (1) Structed information: Structured information as an independent variable was constructed by the researcher on the basis of Roy's adaptation model consisting of physiologic needs, self-concept, role function and interdependence needs as related to the sleep and of operational procedures. (2) Sleep amount measure: Sleep amount as main dependent variable was measured by trained nurses through observation on the basis of the established criteria, such as closed or open eyes, regular or irregular respiration, body movement, posture, responses to the light and question, facial expressions and self report after sleep. (3) State anxiety measure: State Anxiety as a sub-dependent variable was measured by Spi-elberger's STAI Anxiety scale, (4) Hormornal change measure: Hormone as a sub-dependent variable was measured by the cortisol level in plasma. (5) Behavior change measure: Behavior as a sub-dependent variable was measured by the Behavior and Mood Rating Scale by Wyatt. The data were collected over a period of four months, from June to October 1981, after the pretest period of two months. For the analysis of the data and test for the hypotheses, the t-test with mean differences and analysis of covariance was used. The result of the test for instruments show as follows: (1) STAI measurement for trait and state anxiety as analyzed by Cronbachs alpha coefficient analysis for item analysis and reliability showed the reliability level at r= .90 r= .91 respectively. (2) Behavior and Mood Rating Scale measurement was analyzed by means of Principal Component Analysis technique. Seven factors retained were anger, anxiety, hyperactivity, depression, bizarre behavior, suspicious behavior and emotional withdrawal. Cumulative percentage of each factor was 71.3%. The result of the test for hypotheses show as follows; (1) Main hypothesis, was not supported. The experimental group has 282 minutes of sleep as compared to the 255 minutes of sleep by the control group. Thus the sleep amount was higher in experimental group than in control group, however, the difference was not statistically significant at .05 level. (2) Main hypothesis 2 was not supported. The mean sleep amount of the experimental group and control group were 297 minutes and 278 minutes respectively Therefore, the experimental group had more sleep amount as compared to the control group, however, the difference was not statistically significant at .05 level. Thus, the main hypothesis 2 was not supported. (3) Sub-hypothesis 1 was not supported. The mean state anxiety of the experimental group and control group were 42.3, 43.9 in scores. Thus, the experimental group had slightly lower state anxiety level than control group, howe-ver, the difference was not statistically significant at .05 level. (4) Sub-hypothesis 2 was not supported. . The mean hormonal level of the experimental group and control group were 338 ㎍ and 440 ㎍ respectively. Thus, the experimental group showed decreased hormonal level than the control group, however, the difference was not statistically significant at .05 level. (5) Sub-hypothesis 3 was supported. The mean behavioral level of the experimental group and control group were 29.60 and 32.00 respectively in score. Thus, the experimental group showed lower behavioral change level than the control group. The difference was statistically significant at .05 level. In summary, the structured information did not influence the sleep amount, state anxiety or hormonal level of the subjects undergoing an open heart surgery at a statistically significant level, however, it showed a definite trends in their relationships, not least to mention its significant effect shown on behavioral change level. It can further be speculated that a great degree of individual differences in the variables such as sleep amount, state anxiety and fluctuation in hormonal level may partly be responsible for the statistical insensitivity to the experimentation.
