Pancreatic Paraganglioma: a Case Report and Literature Review

  • Paraganglioma is a rare tumor of paraganglia, derived from neural crest cells in sympathetic or parasympathetic ganglions. It can be widely distributed from the skull base to the bottom of the pelvis. The pancreas, however, is a rare location of this neoplasm, and only a limited number of cases have been reported in the English literature, especially with gadoxetic-acid-enhanced magnetic resonance imaging (MRI) and diffusion-weighted images (DWI). We herein report a case of pathologically proven paraganglioma in the pancreas head with a literature review on endoscopic ultrasonography (EUS), computed tomography (CT), gadoxetic-acid-enhanced MRI, and DWI sequence.

  • 부신경절종은, 신경외배엽 기원의 교감신경 신경내분비 세포로 이루어져 있는, 부신 이외의 부신경절에서 생긴 종양으로, 주로 갑상선, 목동맥체, 종격동, 폐, 십이지장, 대동맥 주변 부위와 후복막 부위에 잘 생긴다. 악성도는 조직학적인 진단에 의한 것이 아니라, 국소 림프절 재발과 원격 전이에 의하여 판별되며, 골 전이가 드물지만 전이가 일어나면 주로 두개골 기저, 척추에 생기며, 드물게 골반골, 대퇴골로 전이한다. 저자들은 대퇴부 피하층에 발생하여 혈관 기원성의 종양과 감별되었던 부신경절종과 조기 골 전이를 보인 후복막에 발생한 부신경 절종을 경험하여 문헌고찰과 함께 보고하고자 한다.

  • 상악암은 비교적 진행되어서야 발견되는 수가 많지만 경부 임파절 전이나 원격 전이는 다른 두경부 암에 비해 흔하지 않기 때문에 종양의 국소 억제가 치료의 성패를 좌우하게 된다. 그러나, 상악동은 안와나 두개저에 접하고 있어서 다른 악성 종양처럼 충분하게 안전한 절제연을 두고 제거하는 것이 불가능한 수가 많다. 한편 대부분의 시설에서 수술과 방사선요법을 단독 또는 병행요법으로 시행하고 있는데 그 치료성적이 보고자에 따라 다양하며 비교적 좋지 못한 형편이다. 저자들은 88년 t월부터 91년 12월까지 상악암으로 진단받고 동맥내 항암제 주입요법, 방사선 치료및 수술을 포함한 집합적 치료를 시행하였던 16례(전례 편평상피암, T2 1례, T3 6례, T4 9례, 평균 연령 57.2세)에 대하여 그 결과를 후향적으로 검토하였다. Kaplan-Meier법에 의한 5년 생존률은 51.95%였으며 안구나 구개, 안면 피부등을 일부 보존할 수 있었고 기능적으로도 만족할 만하였다.

비부비동 미분화암의 두개안면절제술 후 전완부자유피판을 이용한 뇌 척수액 유출 폐쇄 시행 1례 (A Case of Cerebrospinal Fluid Leakage Repair with Forearm Free Flap after Craniofacial Resection of Sinonasal Undifferentiated Carcinoma)

  • A 37 year-old male presented with sudden right proptosis and bilateral nasal obstruction. A huge tumor was noted in bilateral nasal cavities, ethmoid sinuses and central frontal sinus on magnetic resonance imaging. Right posterior frontal sinus wall and bilateral medial orbital wall was destructed by the tumor. Complete resection of the tumor was performed via minimal invasive craniofacial resection. Histopaghologic examination revealed sinonasal undifferentiated carcinoma (SNUC). Cerebrospinal fluid leakage was developed after 2 days of the surgery. On the 5th day, anterior skull base was reconstructed with forearm free flap (FFF). The patient got 6000cGy of radiotherapy and survived in 27 months of follow up.

Surgical Treatment for a Huge Maxillary Ameloblastoma via Le Fort I Osteotomy: A Case Report

  • Ameloblastomaa are odontogenic benign tumors with epithelial origin, which are characterized by slow, aggressive, and invasive growth. Most ameloblastomas occur in the mandible, and their prevalence in the maxilla is low. A 27-year-old male visited our clinic with a chief complaint of the left side nasal airway obstruction. Three-dimensional computed tomography showed left maxillary sinus filled with a mass. Except for the perforated maxillary left edentulous area, no invaded or destructed bone was noted. The tumor was excised via Le Fort I osteotomy. The main mass was then sent for biopsy and it revealed acanthomatous ameloblastoma. The lesion in the left maxillary sinus reached the ethmoidal sinus through the nasal cavity but did not invade the orbit and skull base. The tumor was accessed through a Le Fort I downfracture in consideration of the growth pattern and range of invasion. The operation site healed without aesthetic appearances and functional impairments. However, further long-term clinical observation is necessary in the future for the recurrence of ameloblastoma. Conservative surgical treatment could be the first choice considering fast recovery after surgery and the patient's life quality.

Optimize KNN Algorithm for Cerebrospinal Fluid Cell Diseases

  • Medical imaginings assume a important part in the analysis of tumors and cerebrospinal fluid (CSF) leak. Magnetic resonance imaging (MRI) is an image segmentation technology, which shows an angular sectional perspective of the body which provides convenience to medical specialists to examine the patients. The images generated by MRI are detailed, which enable medical specialists to identify affected areas to help them diagnose disease. MRI imaging is usually a basic part of diagnostic and treatment. In this research, we propose new techniques using the 4D-MRI image segmentation process to detect the brain tumor in the skull. We identify the issues related to the quality of cerebrum disease images or CSF leakage (discover fluid inside the brain). The aim of this research is to construct a framework that can identify cancer-damaged areas to be isolated from non-tumor. We use 4D image light field segmentation, which is followed by MATLAB modeling techniques, and measure the size of brain-damaged cells deep inside CSF. Data is usually collected from the support vector machine (SVM) tool using MATLAB's included K-Nearest Neighbor (KNN) algorithm. We propose a 4D light field tool (LFT) modulation method that can be used for the light editing field application. Depending on the input of the user, an objective evaluation of each ray is evaluated using the KNN to maintain the 4D frequency (redundancy). These light fields' approaches can help increase the efficiency of device segmentation and light field composite pipeline editing, as they minimize boundary artefacts.

Possible Role of Matrix Metalloproteinase in Osteolytic Intracranial Meningiomas

  • Objective: Abnormalities of the bone are frequently encountered in patients with meningioma, and hyperostosis and endostosis are common bone alterations in these tumors. Extensive bony destruction is very unusual in patients with meningioma. We report six cases of intracranial meningioma associated with an osteolytic lesion of the skull and discuss the underlying mechanisms that may be responsible for bone destruction in patients with meningioma. Methods: Six patients were classified into three groups, severe, moderate and mild, according to the degree of osteolytic bony destruction. The tumor was classified as intracranial or extracranial, depending on its location. We investigated the potential role of matrix metalloproteinase (MMP) in meningioma-associated osteolysis. The levels of MMP expression were determined by gelatin zymography, reverse transcription-quantitative PCR analysis (RT-PCR) and immunohistochemical analysis. Results: Complete surgical removal of the lesion was performed in each patient. Histological examination revealed benign meningioma in four cases, and two cases of atypical meningioma. Patients did not have a poor prognosis except one case of recurred atypical meningioma. Gelatin zymography and RT-PCR detected high levels of MMP-2 in almost all extracranial masses in comparison with the intracranial masses and MMP9 in two. There was no difference in the severity of bone destruction. Immunohistochemical analysis revealed MMP-2 expression in the vicinity of the bone destruction, and a few MMP-9-positive stainings were observed. Conclusion: Osteolysis of the skull in patients with meningiomas might not be indicative of malignant pathological features and poor prognosis. Invasion to the extracranial portion and osteolysis might be associated with MMP-2 expression in meningioma.

사이버나이프 6-D Skull Tracking의 유용성 평가 (The variability of 6-D Skull Tracking(6DST) in Cyberknife for Bone metastasis patients)

  • 목 적 : 사이버나이프(Accuray Inc., Sunnyvale, CA)를 이용한 제1, 2경추의 정위체부방사선치료(Stereotactic Body Radiation Therapy, SBRT)시 사용가능한 Xsight Spine Tracking(XST)과 6-Dimensional Skull Tracking(6DST)의 비교를 통해 6DST의 유용성을 평가하고자 한다. 대상 및 방법 : 2017년 1월부터 9월까지 본원에서 사이버나이프를 이용하여 제1, 2경추에 SBRT를 진행한 환자 9명을 대상으로, 전산화단층촬영장치(Lightspeed VCT 64, General Electric Co. Waukesha, WI, USA)를 이용해서 컴퓨터 단층 영상(Computed tomography) 을 획득한 후 전산화치료계획시스템(Multiplan system Version 4.6, Accuray, US)를 이용해 제 1, 2경추에 대한 XST와 6DST가 가능한 계획을 수립하였다. 1차 영상 유도를 통해 환자 자세를 보정한 후 동일 환자에 대한 XST와 6DST를 순서대로 진행하고 측정된 결과값을 비교 평가하였다. 결 과 : 제1경추를 대상으로 XST을 실시한 후 측정된 6D Couch 의 Y, Z, X 축 값과 Roll, Pitch, Yaw 값 중 최대값은 0.9 mm, 0.9 mm, 0.7 mm과 $1.0^{\circ}$, $1.0^{\circ}$, $1.2^{\circ}$로 측정되었고, 6DST의 경우 0.7 mm, 0.7 mm, 0.9 mm과 $0.5^{\circ}$, $0.9^{\circ}$, $0.8^{\circ}$로 측정되었다. 제2경추의 XST를 실시했을 때 측정된 값 중 최대값은 0.7 mm, 0.7 mm, 0.8 mm과 $0.9^{\circ}$, $1.0^{\circ}$, $1.8^{\circ}$였고, 6DST의 경우 0.9 mm, 0.7 mm, 0.9 mm와 $0.9^{\circ}$, $0.9^{\circ}$, $1.0^{\circ}$로 측정되었다. 결 론 : 이번 연구를 통해서 제 1, 2 경추에 대한 XST와 6DST의 6D Couch 측정값이 모두 본원에서 기준으로 하고 있는 범위 안에서 측정되고 있음을 확인할 수 있었다. 또한 두 방법을 진행하는 과정에서 설정값 변경을 진행해야하는 XST보다 6DST가 영상유도절차를 간소화 시킬 수 있다는 것을 확인할 수 있었다. 따라서 제 1, 2 경추에 대한 SBRT를 진행할 경우 XST와 더불어서 6DST를 사용한다면 정확한 치료와 더불어서 치료 시간을 단축시킬 수 있을 것으로 사료된다.

두개안면골 결손부 재건에 있어서 티타늄 그물판의 임상적 유용성 (Clinical Usefulness of Titanium Mesh in Reconstruction of the Craniofacial Bone Defects)

  • Purpose: Reconstruction of the craniofacial defects can be carried out with autogenous tissues, allogenic implants, or alloplastic materials. Titanium mesh systems have been used for bony reconstruction in non load-bearing areas. They offer several advantages: immediate availibility without any donor site morbidity, easy handling, stable 3-D reconstruction, and low susceptibility to infection. The aim of this study is to evaluate the usefulness and complications of titanium mesh system in the reconstruction of the craniofacial defects. Methods: From Jan. 2000, to Dec. 2004, we performed reconstruction of craniofacial bone defects in 21 patients who had benign or malignant tumor and fracture events in the cranium, orbit, nasal bone, maxilla, zygoma and the mandible. The size of the defects ranged from $1.0{\times}1.5cm$ to $12{\times}10cm$. Two different mesh systems, micro-titanium augmentation mesh and dynamic mesh was used for bony reconstruction in non load-bearing areas. The patients were evaluated from 1 to 4 yrs clinically and radiographically with a mean follow up period of 1.5 yrs. Results: There were no serious complications, including wound infection, foreign body reaction, exposures or loos of the mesh, central infection and pathologic findings of bone around mesh exception of one patient, who had expired of skull base tumor recurrence. Long-term stability of the reconstructions and the overall functional and aesthetic outcome was excellent. Conclusion: Our experiences demonstrate that the Titanium mesh system is a relatively safe and efficient method in the craniofacial reconstruction and have broadens our choices of therapeutic procedures in the craniomaxillofacial surgery.

두경부에 발생한 연골육종 4 예 (Chondrasarcoma of the Head and Neck - 4 Cases Report -)

  • Chondrosarcoma of the head and neck is very rare tumor and this incidence was reported to be about 10% of all chondrosarcomas. Maxilla is the most common site of involvement and mandible, paransal sinus, nasal cavity and base of skull in that order. In general, chondrosarcoma has been known to be radioresistant, however since the Princess Margaret Hospital reported that it was radioresponsive tumor in 1980, the role of radiotherapy has been emphasized in terms of local control, especially in head and neck regions where complete excion is often difficult to achieve. The authors experienced 4 cases of chondrosarcoma of head and neck among all 29 chondrosarcoma patients from 1971 to 1985. The clinical and pathologic feature of this disease, it's treatment and prognosis were reviewed along with the literatures.
