Analysis of Patients with Acute Industrial Toxic Exposure at an Emergency Department in an Industrial Complex (공단밀집지역에 위치한 일개 응급의료센터에 내원한 급성산업중독환자에 대한 분석)

  • Purpose: Surveys on poisoning usually involves intoxication rather than inhalation, skin contact, etc. Therefore, we examined the characteristics of patients who visited the emergency department in an industrial complex after acute industrial exposure to toxic materials. Methods: Medical records of patients exposed to toxic materials in the work places from April, 2006, to March, 2008, were analyzed retrospectively. Inhalation patients due to fire were excluded. Results: Subjects included 66 patients, with a mean age of $35.4{\pm}10.9$ years, mostly men (91%). Toxicity occurred in 51 patients (77%) by contact, 15 patients (23%) by inhalation, and none by oral ingestion. For toxic materials, 10 patients were exposed to hydrofluoric acid, 8 to hydrochloric acid, 7 to sodium hydroxide, 7 to metals, and others. The face and hands were the most frequent exposure site by contact. Most exposures were caused by accidents, with 29 cases (42%) exposed because of carelessness or not wearing protective equipment. Most complaints were pain on exposure site, but 7 of the inhalation patients complained of dyspnea. The majority of patients with contact exposure were discharged after wound care or observation. After inhalation exposure, 1 patient died and 5 patients were admitted to the intensive care unit. Conclusion: Major causes of workplace exposure were not wearing protective equipment or carelessness. Although contact exposures are usually benign, cautious observation and management are required in patients with inhalation exposure.

Protective effect of Caryophylli Flos on apoptosis caused by oxidative stress in HaCaT cells (HaCaT 세포의 산화 스트레스로 인한 세포자멸사에서 정향의 보호효과)

  • Objective : Caryophylli Flos has been used in Korean medicine to relieve vomiting and pains caused by chills that make fluid circulation difficult. This study was designed to investigate the protective effect of ethanol extract of Caryophylli Flos (CF) in hydrogen peroxide (H2O2)-induced apoptotic cell death in human keratinocyte HaCaT cells. Methods : CF was prepared by extracting 200 g of Caryophylli Flos in 2 L of ethanol for 48 h. Cell viability was measured by MTT assay, and the protein expression was monitored by Western blot analysis. Apoptosis was determined by terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay. Reactive oxygen species (ROS) was measured using fluorescent dye, and reduced glutathione (GSH) was determined with a colorimetric commercial kit. Results : CF protected HaCaT cells from cell death caused by oxidative stress after H2O2 treatment. H2O2 amplified generation of ROS and induced depletion of GSH, whereas these changes in ROS and GSH were inhibited by GF treatment. In addition, H2O2 resulted in apoptosis as assessed by TUNEL assay and the expression of apoptosis regulator proteins. However, cells treated with CF showed a decrease in TUNEL-positive cells and restored the reduced expression of procaspase-9, -3 and PARP. Conclusion : This study showed cytoprotective effects of CF by anti-apoptotic activity while exerting antioxidative activity in H2O2-treated HaCaT cells. These results suggest that CF could be beneficial in skin damage caused by oxidative stress.

Anti-oxidative Effect of Chungpyesagan-tang in LPS Induced RAW 264.7 Cells (LPS로 유도된 RAW 264.7 세포에서 청폐사간탕(淸肺瀉肝湯)의 항산화 효과)

  • Objectives : This study was conducted to confirm the anti-oxidative effect of Chungpyesagan-tang(CPSGT) extract. Methods : In this study, MTT assay was performed to confirm cell viability, and DPPH and ABTS were performed to confirm radical scavenging ability. The ROS scavenging ability and the protective effect against DNA damage were confirmed by 2,7-dichlorofluorescin diacetate(DCF-DA) and 4',6-diamidino-2-phenylindole(DAPI) staining and comet assay. mRNA expression of Heme oxygenase-1(HO-1) was measured by real-time PCR, and expression of HO-1 and Kelch-like ECH-associated protein 1(Keap1) proteins was measured by western blot. Results : CPSGT was not cytotoxic at 50-400㎍/㎖. The radical scavenging activity was increased, and the ROS scavenging activity and the protective effect against DNA damage were increased compared to the LPS-treated group. The mRNA expression and protein expression of HO-1 were increased in a concentration-dependent manner. The protein expression level of Keap1 was decreased in a concentration-dependent manner. Conclusion : This suggests that CPSGT has an antioxidant effect and can be used as a potential material for skin diseases.

Anthocyanins from Hibiscus syriacus L. Inhibit Oxidative Stress-mediated Apoptosis by Activating the Nrf2/HO-1 Signaling Pathway

  • Hibiscus syriacus L. is widely distributed throughout Eastern and Southern Asia and its root bark has been used as a traditional remedy. Recently, the extracts of H. syriacus L. exerts anti-cancerous, anti-microbial, and anti-inflammatory activities. However, the effect of anthocyanin-rich fraction of H. syriacus L. petals (PS) has not been studied under excessive oxidative stress. In this study, we evaluated the cellular protective effect of PS in HaCaT human skin keratinocytes under hydrogen peroxide ($H_2O_2$)-induced oxidative stress conditions. PS at below $400{\mu}g/ml$ did not show any cell death; however, over $800{\mu}g/ml$ of PS gradually increased cell death. PS at below $400{\mu}g/ml$ significantly inhibited $H_2O_2$-induced apoptosis in HaCaT cells concomitant with downregulation of Bax and upregulation of pro-PARP and p-Bcl-2. Additionally, PS remarkably reversed $H_2O_2$-induced excessive reactive oxygen species (ROS) production and apoptosis, and also significantly inhibited mitochondrial ROS production concomitant with suppression of $H_2O_2$-induced mitochondrial depolarization. $H_2O_2$-mediated ratio of Bax to Bcl-2, and caspase-3 activation were markedly abolished in the presence of PS. Moreover, the inhibition of HO-1 function using zinc protoporphyrin, an HO-1 inhibitor, significantly attenuated the cellular protective effects of PS against $H_2O_2$, indicating the significance of HO-1 in PS mediated cytoprotective effect, which was mediated by activating nuclear factor erythroid 2-related factor-2 (Nrf2). Taken together, our results suggest that cytoprotective effect of PS in HaCaT keratinocytes against oxidative stress-induced apoptosis is mediated by inhibiting cellular and mitochondrial ROS production, which is downregulated by activating Nrf2/HO-1 axis.

Protective Measures From Solar Ultraviolet Radiation for Beach Lifeguards in Tuscany (Italy): Shade and Clothing Strategies

  • Background: The exposure to solar ultraviolet radiation is a significant risk factor generally underestimated by outdoor workers and employers. Several studies have pointed out that occupational solar exposure increased eye and skin diseases with a considerable impact on the lives and productivity of affected workers. The main purpose of this study was to evaluate the effectiveness against ultraviolet radiation of some measures recently undertaken for the protection of lifeguards in a coastal area of Tuscany. Methods: Different shading structures (gazebos and beach umbrella) were tested during a sunny summer's day on a sandy beach by means of two radiometers; the UV protection offered by some T-shirts used by lifeguards was also tested in the laboratory with a spectrophotometer. Results: The analysed shading structures strongly reduced the ultraviolet radiation by up to 90%, however a not always negligible diffuse radiation is also present in the shade, requiring further protective measures (T-shirt, sunglasses, sunscreen, etc.); the tested T-shirts showed a very good-excellent protection according to the Australian/New Zealand standard. Conclusion: Results obtained in this study suggest how the adoption and dissemination of good practices, including those tested, could be particularly effective as a primary prevention for lifeguards who are subjected to very high levels of radiation for long periods.

  • Interest in the exterior design of buildings has increased in recent years. Demand for various architectural structures is increasing. However, domestic form liner and design are still limited. Thus, this research uses 3D printers to omit the existing mold production process. Use digital data to produce products economically using various materials. It can be hoped that extending the lifespan of the concrete structure will reduce the cost of saving costs. The purpose of this study is to utilize the 3D printers to develop the design of a suitable formative shape for the purposes of the concrete. In this study, 3D printed form enables the possibility that the actual application is practical. Consideration for the artistic design of the art line was proposed for the purpose of considering the use of concrete structures for concrete structures.

Occupational Characteristics and Health Status of Vietnamese Male Migrant Workers in the Republic of Korea

  • Background: The objective of this study is to identify the working conditions and health status of Vietnamese male migrant workers in Republic of Korea, in comparison to the Korean general population. Methods: We conducted our survey through the Migrant People Center, and we received completed questionnaires from 87 male Vietnamese migrant workers. The questionnaire employed was identical to those used in the Korean Working Conditions Survey and the 2020 Korea National Health and Nutrition Examination Survey. The collected data from the Vietnamese migrant workers was then compared with the Korean reference population using indirect age-standardization. Results: Vietnamese male workers demonstrated a higher prevalence of health problems including hearing problems (age-standardized prevalence ratio (aSPR) 13.22, 95% confidence interval [CI]: 8.07-20.4), skin problems (aSPR 13.49, 95% CI: 8.07-20.4), and low back pain (aSPR 8.40, 95% CI: 6.50-10.69). Elevated exposure to workplace hazards such as chemicals (aSPR 2.36, 95% CI: 1.51-3.51), organic solvents (aSPR 2.22, 95% CI: 1.44-3.28), handling of heavy objects (aSPR 1.67, 95% CI: 1.24-2.21), and high temperatures (aSPR 1.96, 95% CI: 1.46-2.57) was observed among them. Additionally, they faced a higher risk of no personal protective equipment (aSPR 2.53, 95% CI: 1.26-4.52) and a greater prevalence of unmet medical needs (aSPR 7.14, 95% CI: 4.74-10.32). Conclusion: Our findings highlight the elevated workplace hazards, health problems, and unmet medical needs among Vietnamese male workers compared to the Korean reference population. These findings underscores the urgency for enhanced scrutiny over working conditions and protective equipment provision, coupled with efforts to improve healthcare accessibility and worker education.

Protective Effects of Chijabaegpi-tang on Atopic Dermatitis in TNF-α/IFNγ-induced HaCaT Cells (피부각질세포에서 치자백피탕(梔子柏皮湯)의 아토피 피부염 개선효과)

  • Chijabaegpi-tang (CHG) is an oriental herbal medicine that has been used for its various pharmacological effects, which include anti-inflammatory, anti-oxidant and immunoregulation activities. In the present study, we investigated which skin inflammations are involved in the $TNF-{\alpha}/IFN{\gamma}$-induced HaCaT cells. We investigated the suppressive effect of CHG on $TNF-{\alpha}/IFN{\gamma}$-induced HaCaT cell production of the following chemokines: macrophage-derived chemokine (MDC)/CCL22; regulated on activation, normal T-cell expressed and secreted (RANTES)/CCL5; and interleukin-8 (IL-8); thymus and activation-regulated chemokine (TARC)/CCL17. The pre-treatment of HaCaT cells with CHG suppressed $TNF-{\alpha}/IFN{\gamma}$-induced nuclear transcription factor kappa-B ($NF-{\kappa}B$). In addition, CHG inhibited $TNF-{\alpha}/IFN{\gamma}$-induced phosphorylation of ERK and p38. $TNF-{\alpha}/IFN{\gamma}$ suppressed the expression of skin barrier proteins, including filaggrin (FLG), Involucrin (IVL) and loricrin (LOR). By contrast, CHG restored the expression of FLG, IVL and LOR. Taken together, our findings suggest that CHG could be a therapeutic agent for prevention of skin disease, including atopic dermatitis.

One-Stage Achilles Tendon Reconstruction Using the Free Composite Dorsalis Pedis Flap in Complex Wound (족배부 복합 피부-건 유리피판을 이용한 Achilles건의 일단계 재건술)

  • The soft tissue defects including the Achilles tendon are complex and very difficult to reconstruct. Recently, several free composite flaps including the tendon have been used to reconstruct large defects in this area in an one-stage effort. Our case presents a patient reconstructed with free composite dorsalis pedis flap along with the extensor digitorum longus and superficial peroneal nerve for extensive defects of the Achilles tendon and surrounding soft tissue. A 36-year-old-man sustained an open injury to the Achilles tendon. He was referred to our department with gross infection of the wound and complete rupture of the tendon associated with loss of skin following reduction of distal tibial bone fracture. After extensive debridement, $6{\times}8cm$ of skin loss and 8cm of tendon defect was noted. Corresponding to the size of the defect, the composite dorsalis pedis flap was raised as a neurosensory unit including the extensor digitorum longus to provide tendon repair and sensate skin for an one-stage reconstruction. One tendon slip was sutured to the soleus musculotendinous portion, the other two were sutured to the gastrocnemius musculotendinous portion with 2-0 Prolene. The superficial peroneal nerve was then coaptated to the medial sural cutaneous nerve. The anterior tibial artery and vein were anastomosed to the posterior tibial artery and accompanying vein in an end to end fashion. After 12 months of follow-up, 5 degrees of dorsiflexion due to the checkrein deformity and 58 degrees of plantar flexion was achieved. The patient was able to walk without crutches. Twopoint discrimination and moving two-point discrimination were more than 1mm at the transferred flap site. The donor site healed uneventfully. Of the various free composite flaps for the Achilles tendon reconstruction when skin coverage is also needed, we recommand the composite dorsalis pedis flap. The advantages such as to control infection, adequate restoration of ankle contour for normal foot wear, transfer of the long tendinous portion, and protective sensation makes this flap our first choice for reconstruction of soft tissue defect including the Achilles tendon.

Collagen Increasing and Hair Growth Effect of Supramolecular Cosmetic Materials Containing Phytochemicals (식물성 유용물질을 함유하는 초분자체 화장제재의 콜라겐 증가 및 육모효과)

  • Medicinal herbs have been shown to have protective functions for skin and hair. We investigated the effects of complex of soluble ${\beta}$-cyclodextrin and phytochemicals on the functions of skin and hair. In previous report, we evaluated the safety of supramolecules and found their anti-microbial effects and anti-fungal effect against Gram (+) and Malassezia furfur which is known to cause dandruff. Here we present that functional supramolecules-containing cream promotes the biological skin activity by inducing the collagen formation. And treatment of supramolecules-containing hair tonic increased the rate of hair growth of mouse. Taken together, supramolecular cosmetic compounds containing water insoluble phytochemicals and water soluble ${\beta}$-cyclodextrin exhibit the potential ability for hair growth promotion and delaying the aging of skin.
