Chondrosarcoma of Proximal Tibia Trated by Allograft - A Case Report - (동종골 이식술로 치료한 경골 근위부에 발생한 연골 육종 - 증례 보고 -)

  • We reported a case of chondrosarcoma in proximal tibia in a 44-year-old man. MR images demonstrated a $3.5{\times}20$ cm sized bone tumor. In reconstruction of resected proximal tibia, we used the allograft bone and soft tissue defects were covered by medial gastrocnemius rotation flap and skin graft. There were no local recurrence and distant metastasis and any complication such as secondary infection, nonunion, metal failure at the time of the last follow-up. There was no limitation of knee motion through the appropriate rehabilitaion programs.

  • Encephalitis is known as a rare complication of varicella zoster virus (VZV) reactivation. It is usually regarded as a complication of a cutaneous infection in patients with impaired cellular immunity. The reported incidence of herpetic motor involvement range between 0.5 and 31%, but is possibly more frequent as the weakness is readily obscured by pain. A 53-years-old woman, who presented with severe shoulder pain, fever, headache and seizure, which developed the day after skin eruptions, also developed motor paresis 7 days after the seizure. Her cerebrospinal fluid (CSF) was VZV-Polymerase chain reaction (PCR) negative, but VZV specific IgG antibody positive, and her brain MRI was found to be normal. With the early diagnosis and proper treatment, such as intravenous administration of acyclovir, stellate ganglion block and Yamamoto New Scalp Stimulation (YNSS), the patient completely recovered, without psychoneurological sequelae. Herein, we present this case, with a discussion of the relevant literature on the incidence, pathophysiology, diagnosis and management of central nervous system VZV involvement.

Post-Traumatic Big Toe Reconstruction using Anterolateral Thigh Free Flap: A Case Report (전외측 대퇴 유리피판을 이용한 외상 후 족무지 재건: 증례 보고)

  • Purpose: The great toe presents a reconstructive challenge for plastic surgeons. The big toe has much importance not only for the gait and the stability, but also for appearance aspects. Few reports have documented the reconstruction of big toe defects and we report a unique case of free flap reconstruction technique. Methods: A 41-year-old man with right great toe open fracture with soft tissue necrosis was referred from the orthopedic surgery department. On physical examination, there was the skin necrosis circumferentially and the fratured bone was severely fragmented. The metatarsophalangeal (MTP) joint of the big toe was intact. After the wound debridement, we harvested the innervated anterolateral thigh flap and transposed to the defected area. After five months, we finally retouched the flap by the dorsal defatting and distal debulking. Results: He had an uneventful postoperative course without infection, dehiscence and flap necrosis. He has remained asymptomatic for eight months without any recurrence such as the osteomyelitis. Finally, he kept the normal gait and posture with weight bearing on the reconstructed great toe and foot. Conclusion: The anterolateral thigh flap is described for the reconstruction of the great toe. It involves two stage procedure with the minimal donor site morbidity and provides a good cosmetic result.

The Effect of Antibacterial Agent for Candida albicans Inhibition of Diaper Rash (항균제에 의한 기저귀 발진을 일으키는 Candida albicans 억제 효과)

  • Diaper rash is found on the skin inside of babys diaper area. Most diaper rashes are caused by prolonged contact with the moisture, germs, and ammonia of the stool and urine. Some diaper rashes are caused by fungi infection such as candida albicans and Preteus vulgaris. In this study, Candida albicans was used as a test microorganism and experiment was carried out to inhibit diaper rash. Handsheets were treated with chitosan oligosaccharide, bamboo extractives, as well as mixture of monolaurin as an emulsifier and bamboo extractives. Both shake flask method and halo test were applied in order to examine the antimicrobial activity of each sample. It was found from both results of the halo test and shake flask method that handsheets treated with chitisan oligosaccharide or bamboo extractives showed antimicrobial property. In the treatment of bamboo extractives without emulsifier, no distinct difference in antimicrobial effect was found between bamboo branch and bamboo leaves extractives. In case of mixture of bamboo extractives and emulsifier, the result also suggested that bamboo branch showed better syngergistic effect than bamboo leaves. Both chitosan oligosaccharide and bamboo extractive can be used as antifungal agents against Candida albicans for making diaper.

Surgical Correction of Pectus Excavatum (누두흉의 외과적 교정)

  • Pectus excavatum. the most common congenital chest wall deformity, is manifested by deformity of the costal cartilages resulting in a depressed and often rotated sternum. Surgical correction of this disease is frequently indicated for aesthetic improvement. The most popular current repair involves resection of abnormal costal cartilages, sternal osteotomy and mobilization, followed by fixation of the sternum in the corrected position.12 patients have been operated upon for pectus excavatum by the Adkins` method for 8 years in our hospital and the results were as follows: 1. All the patients were male. Age distribution was 2-26 years[average 10.8 years] and 69.2% were below 7 years. 2. The deformity was found at neonate or infant in the most of the patients [92.3 %] 3. The subjective symptoms were frequent URI[35.7%], dyspena on exertion[21.4%], chest dis comfort[7.1%], asthma [7.1%] and most of the patients didn`t like their appearance. 4. Combined diseases were urticaria in 2 cases, bilateral cryptomhism and pulmonary tuberculosis in 1 case each other. 5. Welch index ranged from 1.7 to 5.0 with the average of 3.47. 6. Postoperative complications were pneumothorax [45.5%], wound infection [36.4%] and skin necrosis [18.2%]. Although the period of follow-up was short in some cases there was no relapse.

  • The Aeromonas strains were isolated from diseased cultured catfsh (Silurus asotus) in the ponds near by Gochang of Chunbuk. The present isolates were identified as A. hydrophila based on their biological and biochemica1 characteristics. The isolates of A. hydrophila were named GC-1, GC-2, GC-3, GC-4 and GC-5, Five strains were grew optimally at temperatures $35^{\circ}C,$ pH 7.5 and in 0.5 to l% NaCl. This bacterium(GC-1) was injected into health catfish in order to prove the causative agent of ascites and haemorrhagic ulcers. The symptoms in cat fish infected by this challenge method were observed to be very similar to the symptoms of a natural infection, but controls did not show any abnormal symptoms during the experimental period. At 24 post-injection, the red spot developed around the injection site and the haemorrhagic ulcers was extended near the gill and ventral fin. Five strains were tested for drug sensitivity by plate method. All strains were sensitivity to gentamicin and resistant to penicillin, streptomycin and tetracycline.

  • Medical disputes are increasing year by year in korean medical institutions as the frequency of patient's visiting korean medical institutions goes high and the reliability relations between korean medical doctors and patients are getting worse than the past. Frequently occurring accidents in korean medical acts fall into some categories such as medical treatment of acupuncture${\cdot}$moxibustion${\cdot}$cupping, korean herbal medication, korean medical physical therapy, wrong diagnosis. Frequently occurring accidents include these cases. Infection due to improper treatment process of acupunctrue or wet-cupping ; skin-burns caused by moxibustion, infrared therapy or haet therapy ;toxic hepatitis or kidney dysfunction induced by herbal medication ; progression of disease or missing of opportunity of timely transfer because of wrong diagnosis. It will prevent medical accidents to a great extent for korean medical doctors to have scientific diagnostic techniques and lab. test results at hand in korean medical institutions.

First report of Aeromonas veronii infection in farmed Israeli carp Cyprinus carpio in Korea

  • In May 2007, mass mortality of Israeli carp, Cyprinus carpio L., was occured on a pond farm located in Jeollabukdo Province, Korea. The mortality rates reached up to 2% of the total fish in the farm per day. Typical clinical signs were abdominal distension, reddish foci on the skin, enteritis, liver congestion and enlarged spleen and kidney. On the basis of biochemical characteristics and 16S rDNA sequence, the causative bacteria isolated from affected carp were identified as Aeromonas veronii. Histologically, degeneration of hepatocytes and congestion in sinusoids were observed in the liver. Spleen showed hemorrhage and the destruction of the sheathed tissues. In kidney, necrotized renal tubules and glomerular destructions were observed. Intestinal tissues revealed necrotized and severe hemorrhage. Mass hemorrhage was observed in muscles. This is the first report that A. veronii caused mortality in cultured Israeli carp in Korea.

Susceptibility of Koi, Koi×Red Common Carp, and Red Common Carp×Koi to Koi Herpesvirus (KHV)

  • The disease-causing koi herpes virus (KHV), also known as cyprinid herpesvirus-3 (CyHV3), causes mass mortality of koi and carp. Koi (Cyprinus carpio) is a host for KHV, one of 12 virus species in the Alloherpesviridae family. We examined the effects of KHV disease koi (KK), and on koi×red common carp (KR) and red common carp×koi (RK) cross, using a virus challenge test. The infected fish had clinical signs that included gill necrosis and skin lesions. The RK and KR were highly more resistant (cumulative mortality: RK; 6% and KR; 8%) to KHV infection than KK fish (cumulative mortality: 28%). KHV DNA was confirmed in the tissues of all dead fish in groups by use of polymerase chain reaction (PCR), and the presence of the KHV protein in kidney was confirmed by immunohistochemistry. Histological analysis showed severe gill lesions and fusion of the lamellae in KK fish, but less severe damage in RK fish. In immunohistochemistry analysis, the KHV protein localized in the cytoplasm of infected kidney cells of KK, but the cross groups had lower levels of KHV antigen. Our data indicate that the cross groups had increased resistance to KHV disease.

A Suspected Case of Hyperkeratotic Head and Neck Malassezia Dermatosis Presenting with Acanthosis Nigricans

  • A 19-year-old woman presented with an asymptomatic cutaneous discoloration on her nape. Dermatological examination revealed localized dark brown to dark pigmented velvety macules and patches. She denied a past medical history, except for a 7-kg weight gain over a 6-months period. Histopathologically, the specimen showed hyperkeratosis, epidermal hyperplasia, mild acanthosis, and papillomatosis. In addition, keratin material and fungal spore were noted between the papilloma ridges. Periodic acid-Schiff special stain showed a positive result for the fungal spore in the horny layer. She was treated with a daily oral dose of terbinafine and topical flutrimazole. After 12 weeks of treatment, she is currently under observation without pigmentation and adverse effect. We considered the initial diagnosis as acanthosis nigricans because of weight gain, similar clinical features, and histological findings. However, the final diagnosis was hyperkeratotic head and neck Malassezia dermatosis considering the presence of yeast and the dramatic improvement after antifungal treatment. To the best of our knowledge, hyperkeratotic head and neck Malassezia dermatosis is not well characterized in the literature. It is an unusual variant of pityriasis versicolor. The skin lesion appears brown to dark, scaly, and hyperkeratotic macules and patches located on the face and neck. Here, we reported an unusual case of hyperkeratotic head and neck Malassezia dermatosis.
