악성 흑색종 치료를 위한 피판술의 임상적 결과 (The Clinical Outcome of Flap Coverage for the Treatment of Malignant Melanoma)

  • In surgical treatment of the malignant melanoma, radical resection and a sentinel lymph node biopsy are essential procedures to eradicate the tumor and to minimize the risk of local recurrence. For the reconstruction of skin defect after tumor resection, a skin graft has been generally performed procedure. However, if tendon or bone is exposed after tumor resection, simple skin graft is not enough for the coverage of the defect and additional procedure is mandatory. In this study, we reviewed the clinical results of 16 patients, who had been diagnosed with malignant melanoma of the limb and underwent surgical resection and reconstruction of the defect with various methods. The sentinel lymph node dissection was performed in all patients combined with radical excision of the tumor. In 11 cases of positive sentinel lymph nodes, further elective lymph node dissections were performed. The mean tumor invasion depth was 4.54 mm (2~10 mm), and AJCC stage II was most common (9 cases). The pedicled flaps were performed in 10 cases for the reconstruction of defects. All flaps were successfully survived without significant complications until the last follow up. In conclusion, a pedicled flap coverage is very useful reconstruction strategy for the soft tissue defects after radical resection of malignant melanoma.

Outcomes of grafted skin on the dorsum of the foot after car-tire friction injuries

  • Background A car-tire friction injury on the dorsum of a child's foot often results in hypertrophic scarring of the wound margins. This study describes the clinical appearance of the injured areas and surgical complications that occurred during the follow-up period in a series of children with car-tire friction injuries who were treated with split-thickness skin grafts (STSGs). We describe the clinical features that we believe need to be highlighted when initially treating car-tire injuries in children. Methods From May 2003 to June 2016, our retrospective study included 15 patients with car-tire injuries on the dorsum of the foot who were treated with surgical excision and STSG to cover the wound. Results A total of 15 patients with car-tire injuries were treated. The average age was 6.26 years old. The average injury grade was 3.26. Two patients were treated using delayed repair, and 13 patients received STSG for initial management. Four patients experienced no complications, while 11 patients had hypertrophic scars and/or scar contracture after surgery. Conclusions A car-tire friction injury on the dorsum of a child's foot often results in hypertrophic scar formation or scar contracture even if proper management is undertaken. Since the occurrence of these complications in childhood can lead to a secondary deformity, it is important to properly treat car-tire friction wounds, inform patients and caregivers about potential complications, and ensure regular follow-up evaluations over a 12-month period following the initial surgery.

  • Stevens-Johnson syndrome (SJS) is a rare disease in which extensive toxic epidermolysis occurs after medication. Skin and mucous membranes are involved in about 90% of SJS cases, and webbing of mouth corners (microstomia) may occur when they are affected. Few reports have been issued on microstomia in SJS, and no consensus has been reached regarding treatment methods, timings, or results. We encountered a case of microstomia following SJS after ofloxacin medication in a 22-year-old woman treated by commissuroplasty using a lozenge-shaped excision. We present an appropriate correction method and surgical timing for microstomia following SJS.

두피에 발생한 혈관육종의 치험례 (A Case of Angiosarcoma on the Scalp)

  • Angiosarcoma is a rare and high aggressive malignant tumor originated from endothelial cell. Angiosarcoma has four clinical types that are in the scalp and face of old age, in chronic lymphedema, in previously irradiated sites, and malignant angioendothlioma. We report one case of angiosarcoma of the scalp in a 74-year-old male patient. The patient had $4.0{\times}4.5cm,\;4.0{\times}3.5cm$ sized, localized, asymmetric several dome-shaped nodule and plaque with crust and ulcer on the parieto-frontal area of the scalp. We performed wide excision and subsequently covered with split-thickness skin graft. Postoperatively the patient underwent radiotherapy for prevention of recurrence and he had been followed up for 1 year without evidence of recurrence.

이개에 발생한 모낭암종 1예 (A Case of Trichilemmal Carcinoma in Auricle)

  • Trichilemmal carcinoma is a rare malignant neoplasm of the hair follicle from the outer root of the hair follicle sheath. This tumor can be misleading, and a false diagnosis of a squamous cell carcinoma. We report a case of trichilemmal carcinoma with a review of literature. The patient presented with an exophytic well circumscribed nodular mass on the left auricle, which was detected 6 months ago. Histopathologically, the tumor consisted of atypical clear cells which contained abundant glycogen. The tumor cells shows lobular growth pattern with necrosis, foci of trichilemmal keratinization and peripheral pallisading. Total excision and repair with full-thickness skin graft was done with minimal surgical morbidity. The patient has been free of recurrence or metastasis for 8 months.

  • Lateral malleolar bursitis of the ankle is a commonly encountered disease in the department of orthopedic surgery. Although most cases of lateral malleolar bursitis are managed by conservative treatments, operative treatment is considered in cases of infected bursitis or complication after surgery. There are several potential complications associated with operative treatment, including wound healing problem, skin necrosis, nerve injury, or recurrence. However, synovial fistula of the ankle as a complication after surgery for lateral malleolar bursitis has not been previously reported. The author experienced the complication of synovial fistula after surgery for lateral malleolar bursitis and obtained a satisfactory result in revision surgery for the complication, which was coverage of the fistular formation with a periosteal flap from the distal fibula.

  • Excision of the metastatic pulmonary sarcoma or carcinoma in the selected cases is now the accepted and the most effective method of treatment. From January 1978 to July 1987, 14 patients with metastatic lung cancer were treated surgically at the Department of Thoracic and Cardiovascular Surgery, Catholic University Medical College. Ages ranged from 26 years to 68 years [mean=38.0 years]. Primary sites were uterus in 6 patients, colorectum in 2, sarcoma of extremities in 2, and 1 case of pancreas, kidney, skin and melanoma of hand. Fourteen posterolateral thoracotomies were done and resected the 36 metastatic masses in 14 patients. Surgical mortality was none. The performed procedures of resection were as follows; lobectomy with wedge resection in 2, lobectomy in 3, segmentectomy in 1 and wedge resection in 8. Eleven patients were received radiotherapy or chemotherapy, or both postoperatively. Among the 14 patients, there were 5 survivors. They are 6, 7, 21, 35, and 63 months postoperatively. Basal cell carcinoma and squamous cell carcinoma in a single tumor in the anterior auricular area

  • The concurrence of basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) in a single tumor is rarely encountered. We report a case of BCC and SCC in a single tumor in the anterior auricular area. A 70-year-old woman had been diagnosed with BCC by a punch biopsy performed at a dermatology clinic. We performed wide excision of the tumor with an ulcer in the anterior auricular area. Analysis of the biopsy specimen revealed the presence of both BCC and SCC in the tumor. This case illustrates that it is necessary to establish a precise diagnosis and formulate appropriate surgical and treatment plans considering the possibility that two carcinomas may coexist, although the possibility is low in patients with skin cancer.

  • Rhinophyma is a painless benign tumor of the skin of the nose. It is the most severe form of rosacea. Rhinophyma is particularly rare among Asians. It can be treated with surgical resection using various methods, including scalpel excision, dermabrasion, cryosurgery, argon laser, carbon dioxide laser, and electrocautery. However, the gold standard treatment of rhinophyma remains unknown. In the present case, we debulked a giant rhinophyma with a scalpel and contoured it using a bur. Along with a relevant literature review, we present a case of rhinophyma in an Asian patient who was treated at no additional cost using a bur widely used in general plastic surgery.

두피 및 경부에서 발생한 원발성 피부 아스페르길루스증의 1례 (A Case of Primary Cutaneous Aspergillosis on the Scalp and the Neck)

  • The cutaneous aspergillosis is one of the most common dermatologic manifestations of disseminated infections associated with the Aspergillus organisms, but the isolated primary cutaneous disease itself can rarely occur in an immunocompetent host. We report a case of the primary cutaneous aspergillosis on the scalp and the neck in a 39-year-old immunocompetent male patient. There was a single purulent ulcer surrounded by the erythematous indurated plaque on the scalp with multiple satellite papules and a multi-lobulated granulomatous plaque with a crust on the neck. Skin biopsy demonstrated a fungus, the Aspergillus, in the deep dermis as the etiologic agent. No evidence of involvement in other organs was found. The patient exhibited no other apparent systemic diseases nor immunologic defects. An elliptical excision and a primary closure was performed, and the adjuvant antifungal treatment, oral itraconazole, was applied to prevent the recurrence by the satellite lesions.
