A Modified, Direct Neck Lift Technique: The Cervical Wave-Plasty

  • Background Major problems with cervicoplasty by direct skin excision include the subjective nature of skin markings preoperatively and the confusing array of procedures offered. This technique incorporates curved incisions, resulting in a wave-like scar, which is why the procedure is called a "wave-plasty". Methods This prospective study includes 37 patients who underwent wave-plasty procedures from 2004 to 2015. Skin pinching technique was used to mark the anterior neck preoperatively in a reproducible fashion. Intra-operatively, redundant skin was excised, along with excess fat when necessary, and closed to form a wave-shaped scar. Patients were asked to follow up at 1 week, 6 weeks, and 6 months after surgery. Results The mean operation time was 70.8 minutes. The majority (81.3%) was satisfied with their progress. On a scale of 1 to 10 (1 being the worst, and 10 being the best), the scars were objectively graded on average 5.5 when viewed from the front and 7.3 when seen from the side 6 months after surgery. Complications consisted of one partial wound dehiscence (2.3%), one incidence of hypertrophic scarring (2.3%), and two cases of under-resection requiring revision (5.4%). Conclusions In select patients, surgical rejuvenation of the neck may be obtained through wave-like incisions to remove redundant cervical skin when other options are not available. The technique is reproducible, easily teachable and carries low morbidity and high patient satisfaction in carefully chosen patients.

Trichilemmal Carcinoma from Proliferating Trichilemmal Cyst on the Posterior Neck

  • Trichilemmal cysts are common fluid-filled growths that arise from the isthmus of the hair follicle. They can form rapidly multiplying trichilemmal tumors-, also called proliferating trichilemmal cysts, which are typically benign. Rarely, proliferating trichilemmal cysts can become cancerous. Here we report the case of a patient who experienced this series of changes. The 27-year-old male patient had been observed to have a $1{\times}1cm$ cyst 7 years ago. Eight months prior to presentation at our institution, incision and drainage was performed at his local clinic. However, the size of the mass had gradually increased. At our clinic, he presented with a $5{\times}4cm$ hard mass that had recurred on the posterior side of his neck. The tumor was removed without safety margin, and the skin defect was covered with a split-thickness skin graft. The pathologic diagnosis was a benign proliferating trichilemmal cyst. The mass recurred after 4months, at which point, a wide excision (1.3-cm safety margin) and split-thickness skin graft were performed. The biopsy revealed a trichilemmal carcinoma arising from a proliferating trichilemmal cyst. This clinical experience suggests that clinicians should consider the possibility of malignant changes when diagnosing and treating trichilemmal cysts.

Full-Thickness Skin Grafting with De-Epithelization of the Wound Margin for Finger Defects with Bone or Tendon Exposure

  • Background Full-thickness skin grafts (FTSGs) are generally considered unreliable for coverage of full-thickness finger defects with bone or tendon exposure, and there are few clinical reports of its use in this context. However, animal studies have shown that an FTSG can survive over an avascular area ranging up to 12 mm in diameter. In our experience, the width of the exposed bones or tendons in full-thickness finger defects is <7 mm. Therefore, we covered the bone- or tendon-exposed defects of 16 fingers of 10 patients with FTSGs. Methods The surgical objectives were healthy granulation tissue formation in the wound bed, marginal de-epithelization of the normal skin surrounding the defect, preservation of the subdermal plexus of the central graft, and partial excision of the dermis along the graft margin. The donor site was the mastoid for small defects and the groin for large defects. Results Most of the grafts (15 of 16 fingers) survived without significant surgical complications and achieved satisfactory functional and aesthetic results. Minor complications included partial graft loss in one patient, a minimal extension deformity in two patients, a depression deformity in one patient, and mild hyperpigmentation in four patients. Conclusions We observed excellent graft survival with this method with no additional surgical injury of the normal finger, satisfactory functional and aesthetic outcomes, and no need for secondary debulking procedures. Potential disadvantages include an insufficient volume of soft tissue and graft hyperpigmentation. Therefore, FTSGs may be an option for treatment of full-thickness finger defects with bone or tendon exposure.

손목 결절종제거술 시 경험한 요골동맥 기형의 치험례 (Anomaly of the Radial Artery Encountered During the Excision of Wrist Ganglion: A Case Report)

  • Purpose: An extensive knowledge of the arterial anatomy of the upper extremity and its variations is indispensable to the hand surgeon. We report a patient with anomalous radial artery, superficial course of two radial arteries, encountered during the excision of volar wrist ganglion. Methods: The patient was a 53-year-old man who had a painful mass on the left volar wrist for 1 year. Under general anesthesia, a curved incision was made around the mass. With the skin flaps retracted, the dome of the cyst was identified. Particular care was taken to identify and protect the radial artery, which was intimately attached to the wall of the ganglion. Two radial arteries completely encircled the ganglion. The pedicle was traced to the volar joint capsule, radiocarpal ligament. The joint was open and the capsular attachments were excised. Results: The patient made an uneventful recovery. There were two arterial pulsations at the volar side of the wrist joint. Compressing this site revealed that the major arterial contributor to blood supply in the hand was the ulnar artery. At angioCT, an anomaly of the radial artery was found with a duplication. The pathway of this aberrant artery was superficial to the original radial artery. It changed its course subcutaneously at the level of the tendon of the brachioradialis muscle, and crossing the wrist lateral to the original radial artery and ending in the deep palmar arch. Conclusion: Authors experienced a case of bifurcating radial artery encountered during the excision of ganglion on the volar of the wrist. Because these duplicated radial arteries make strong contributions to the thumb and index finger as well as to the deep palmar arch, when they are present there may be probably less blood supply to the hand from the ulnar artery. If the radial artery is palpated superficially on the brachioradialis muscle, it is important to remember the kind of anomaly.

양전자방출단층촬영술(PET/CT)을 이용한 메르켈 세포암(Merkel cell carcinoma)의 전이 평가 (Evaluation of Merkel Cell Carcinoma Metastasis Using Positron Emission Topography/CT)

  • Merkel cell carcinoma is rare skin malignancy originated from epidermal mechanoreceptor of neural origin. The tumor usually affects older individuals at sun exposed area such as head, neck and extremity. Subclinical involvement of regional lymph node is reported frequently at the time of initial treatment. Thus even asymptomatic patients who present with clinically localized tumor should undergo evaluation with computed tomography and lymphangiography. Positron emission tomography(PET) scans can imaging the metabolic difference of malignant tumors. Increased glucose uptake of malignant tumor cells are detected by PET scanner. PET scans can provide qualitative and quantitative informations about systemic metastasis of tumors. Although there are no data that define the efficacy of PET scans in the initial diagnostic evaluation of head and neck cancer, they could be considered. Current standards of treatment of Merkel cell carcinoma is wide surgical excision and regional lymphadenectomy if there are suspicious lymph nodes. The author reported a patient with Merkel cell carcinoma of cheek. Wide surgical excision and postoperative PET/CT was done for evaluation of regional lymph node and distant metastasis. There were two hot-uptakes in patient's neck, so they were considered as metastatic node, but finally they were proved to be tuberculosis lymphadenitis after excision.

Mucormycosis Management in COVID-19 Era: Is Immediate Surgical Debridement and Reconstruction the Answer?

  • Background Excessive use of corticosteroids therapy along with gross immunocompromised conditions in the novel coronavirus disease 2019 (COVID-19) pandemic has raised the risks of contracting opportunistic fungal infections. Here, we describe our experience with the implementation of a surgical protocol to treat and reconstruct rhino-orbital-cerebral mucormycosis. Methods A retrospective review of our prospectively maintained database was conducted on consecutive patients diagnosed with mucormycosis undergoing immediate reconstruction utilizing our "Mucormycosis Management Protocol." All patients included in this study underwent reconstruction after recovering from COVID-19. Wide local excision was performed in all cases removing all suspected and edematous tissue. Reconstruction was done primarily after clear margins were achieved on clinical assessment under a cover of injectable liposomal amphotericin B. Results Fourteen patients were included. The average age was 43.6 years and follow-up was 24.3 days. Thirteen patients had been admitted for inpatient care of COVID-19. Steroid therapy was implemented for 2 weeks in 11 patients and for 3 weeks in 3 patients. Eight patients (57.1%) had a maxillectomy and mucosal lining resection with/without skin excision, and six patients (42.8%) underwent maxillectomy and wide tissue excision (maxillectomy and partial zygomatic resection, orbital exenteration, orbital floor resection, nose debridement, or skull base debridement). Anterolateral thigh (ALT) flaps were used to cover defects in all patients. All flaps survived. No major or minor complications occurred. No recurrence of mucormycosis was noted. Conclusion The approach presented in this study indicates that immediate reconstruction is safe and reliable in cases when appropriate tissue resection is accomplished. Further studies are required to verify the external validity of these findings.

Pilomatricoma 환아에 대한 증례 보고 (CASE REPORT OF PILOMAIRICOMA)

  • Pilomatricoma(Calcifying epithelioma of Malherbe)는 모낭 세포 기원의 양성 연조직 종양으로, 보통 머리와 목 부위에 나타나며, 20세 이전의 여성들에게서 호발하고, 악성으로의 전이가 보고되기도 하나, 이는 극히 드물다. 임상적으로, 무통성, 표재성, 고형성이며 종종 적청색을 나타내고, 크기는 보통 $0.5{\sim}3.0cm$ 정도로 다양하게 나타난다. 조직학적으로, 세포기질 내에 상피조직이 존재하는 형태이며 상피조직에서는 기저세포(basaloid cell)와 유령세포(ghost cell)가 특징적으로 나타나고, 세포내와 기질내 석회화가 종종 관찰되기도 한다. 진단은, 초진시 고형 물질의 촉진으로 병소를 의심해볼 수 있고, 조직병리학적 검사로 확진할 수 있다. 감별진단으로는 아가미낭(branchial cyst), 혈관종(hemangioma), 횡문근육종(rhabdomyosarcoma), 표피포함낭(epidermal inclusion cyst), 모낭(pilar cyst), 이하선 종양(parotid gland tumor), 피부의 골종(osteomas of the skin), 림프절의 석회화(calcified lymph nodes), 죽종(atheromas) 등이 있다. 치료로는 외과적 절제술이 추천되며 재발은 드물다. 본 증례는 우측 하악각 부위에 결절이 촉진되는 6세 여자 환아에게서 임상 검사와 방사선 검사 후 병소의 외과적 절제술을 시행하였으며, 조직병리학적 검사를 통해 pilomatricoma로 확진되었고, 양호한 치료 결과를 얻었기에 이에 보고하는 바이다.

가토 피부상처에서의 개방드레싱과 밀봉드레싱의 효과에 대한 형태학적 비교 (Morphological Comparision of the Effect of Open and Occlusive Dressing on Rabbit Skin Wound)

  • 본 연구는 체중 3.0kg의 가토 수컷 10마리를 이용하여 피부의 외상을 입힌 후 개방드레싱과 밀봉드레싱이 상처의 치유과정에서의 효과와 형태학적 차이를 비교하기 위해 1cm의 절개창과 $1.0{\times}0.2cm$의 피부절제창을 만든 후 개방드레싱군은 1일 2회 betadine으로 소독한 후 상처부위를 노출시켜 5, 10 및 15일째에 관찰하였으며, 밀봉드레싱군은 매일 betadine으로 소독하고 1% silver sulfadiazine 크림을 도포한 후 거즈를 덮어 5, 10 및 15일째에 광학현미경으로 관찰하여 다음과 같은 결론을 얻었다. 피부를 절개한 군과 절제한 군을 개방드레싱 및 밀봉드레싱을 각각 시행한 5일 후에는 딱지가 형성되었고 표피의 재생은 상처의 가장자리에서부터 시작하였다. 진피의 표층에는 호중구, 단구 및 림프구의 침윤이 일어났고 부종과 섬유아세포의 증식이 경하게 일어났다. 수상 후 개방드레싱 및 밀봉드레싱을 시행한 10일 후에는 딱지는 탈락되었고 표피는 재생되어 연속적이었다. 모세혈관의 수와 염증세포의 침윤은 감소하였다. 부종과 섬유아세포의 증식은 더욱 증가하였다. 수상 후 개방 및 밀봉드레싱을 시행한 후 15일에는 표피의 재생은 완성되어 각질이 형성되었다. 진피에는 염증세포의 침윤, 부종 및 혈관의 증식은 소실되었고 섬유아세포 및 교원섬유의 양은 증가하였다. 피부를 절개한 군은 절제 군에 비해 반흔의 양은 적었고 드레싱방법에따른 치유과정의 차이는 없었다. 이상의 성적으로 보아 상처의 치료는 드레싱의 방법보다 상처의 소독이 중요한 역할을 하는 것으로 사료된다.

악성 한선 극세포종 증례보고 (Malignant Hidroacanthoma Simplex: A Case Report)

  • Purpose: Hidroacanthoma simplex (HAS) is a rare benign neoplasm of the skin and known as intraepidermal poroma. Malignant transformation of HAS has been reported in the literature, but it is very rare. We experienced a case of malignant hidroacanthoma simplex (MHA) and performed the surgery of the neoplasm. Methods: A 73-year-old patient visited our department for evaluation and treatment of the skin lesion ($3.2{\times}3.0\;cm^2$) that occurred two years before on the right ankle area. The lesion was characterized by dark-brown, hyperkeratotic and protruding patch. The patient had no symptom, and any palpable lymph node was not found on physical examination. The result of laboratory was also nonspecific. The lesion was excised completely with a resection margin of 5 mm including subcutaneous tissue partially, followed by full-thickness skin graft for reconstruction. Results: The result of biopsy was reported as malignant hidroacanthoma simplex. Histologically, the epidermis showed discrete aggregates of tumor cells which had round, hyperchromatic nuclei and pale or clear abundant cytoplasm. Cystic spaces were found within the tumor and mitosis were seen, frequently. No recurrence was found one month after surgery. Conclusion: The case of malignant hidroacanthoma simplex was treated with full-thickness skin graft following wide excision of the tumor.

Surgical Treatment of Dermatomal Capillary Malformations in the Adult Face

  • Background : Facial capillary malformations (CMs) rarely recede; they often become darker and raised in proportion to their growth. These malformations may hypertrophy in adulthood, resulting in increased disfigurement and dysfunction. Laser treatment is considered a first-line therapy for focal CMs, but thick wide lesions, which are accompanied by hypertrophy and have a well-circumscribed nodularity, may be treated with surgical excision and reconstruction. Methods : We retrospectively reviewed the records of 25 consecutive patients who had undergone complete or partial excisions of facial capillary malformations in our unit. After the excisions, the defects that encompassed their facial aesthetic units were subsequently covered by various methods, including primary closures, local flaps, expanded flaps, split-thickness skin grafts, and full thickness skin grafts. Results : The data demonstrated satisfactory results and reliability. Our patients were treated without significant complications, and all of the patients were moderately or fully satisfied with the outcome of their surgeries. Conclusions : Among the many reconstructive options for adult patients with facial capillary malformations, thick split-thickness skin grafts can be a good choice for the coverage of widely excised wounds.
