The Treatment of Gynecomastia using Ultrasound-Assisted Liposuction with Pull-Out Method or Excision through Periareolar Incision (다양한 술식을 이용한 여성형 유방증의 치료: 초음파 지방흡입술과 풀아웃법 또는 유륜절개를 통한 절제술)

  • Purpose: Gynecomastia is an abnormal increase in the volume of the male breast. Patients affected by gynecomastia with significant glandular enlargement may respond to suction alone and/or sharp dissection and excision. The purpose of this report is to introduce the indications and results of authors' two techniques. Methods: The diameter of parenchyme was determined by a pinch test after liposuction. For the parenchymal diameter less than 4 cm, ultrasound-assisted liposuction was performed, in conjunction with the "pull-out technique" to effectively remove the fibrofatty tissue of the male breast through a single 5-7 mm incision. For the parenchymal diameter more than 4 cm, ultrasound-assisted liposuction and excision were applied through 2.5 cm periareolar approach. Results: A total of 94 patients (185 breasts) underwent the operation from October 2000 to October 2003 and mean follow-up period was 12 months. The volume of aspirates ranged from 50 to 450 cc per breast. There were no major complications such as skin flap necrosis. Five reoperations were performed for 1 hypertrophic scar, 2 under-resected and 2 hematoma cases. The patient's satisfaction was high and most of them were pleased with the shape of the breasts and scars. Conclusion: These procedures can minimize scars and reduce the incidence of contour problem such as saucer deformity, and provides consistent results. Patients can return to full activities in 48 hours. It can be offered as an option for the treatment of gynecomastia.

  • All the pharmacological studies of LB17522 described here were carried out with high doses (fifteen to sixty times of the therapeutic dose) to determine an indication of potential side effects in clinical use in terms of the acute clinical signs, cardiovascular and central nervous system. LB10522 does not produce any observable clinical signs except for the symptoms such as moist eye, skin rash, slight salivation, vomitting, and slightly reduced activity. The effects of LB10522 on the hemodynamics and cardiac function of anesthetized beagle dogs are as follows; heart rates and mean arterial blood pressure had a tendency to increase mildly, which is a normal finding in anesthetized dogs. All the animals except for one showed relatively stable respiratory rates throughout the observation period. Each animal treated with LB10522 showed slight increase in the left cardiac work and left ventricular stroke work which are mainly related to corresponding increases in cardiac output. Femoral blood flow were shown to be increased in some animals treated with LB10522. The epileptogenic activities of various cephalosporins were assessed by a direct intracerebral injection of appropriate concentration of test articles. The CD$_{50}$ values (nmol) obtained from the analysis of the dose-response data are as follows; 78.2, 175.3, 156.3, and 53.5 for cefazolin, cephaloridine, ceftazidime, and LB 10522, respectively. LB10522 seems to be equipotent with cefazolin or to be three times more potent than cephaloridine and ceftazidime in causing adverse CNS stimulation. Taken into consideration all the information obtained, LB10522 is not supposed to induce much changes in the functions examined in these studies in man at therapeutic doses.s.

  • Background: Because of the lack of accurate values for applied forces in manual therapy, manual therapists relies on the magnitude of the individual's perception during applying the force. However, excessive loading maneuvers carry risks for patients. Objective: To establish the relationship between the maximal force applied to swine skin with the specific region, sex, and baseline parameters of the subject. Design: Ex-vivo Study and laboratory Experimental research Methods: 3.5 kg of Korean pork sirloin that is a piece of swine was handled and it was set 3 dimensions; #A; #B; #C. Forty-seven participants who has no experience in physical therapy randomly carried out the experiment, indicated to push each place of the pressure spots with same posture and process under supervision from the instructor who has over 15 years of manual therapy, and we measured the pressure force in each time. Results: The biggest pressure force was recorded in spot #A, and #B was represented after #C. Pressure on #A showed certain statistic relation with height (r=.317, p<.05) and weight (r=.434, p<.01); pressure on #B showed certain relation which has statistical meaning with only height (r=.401, p<.01); pressure on #C emerged to have statistic relationship with height (r=.308, p<.05)and weight (r=.428, p<.01). The age aspect revealed relation with pressure on #A, #B and #C, but that was not statistically significant. Conclusions: It can be inferred that there is the most loss of pressure in the area where cartilage is like an island in the middle.

Effect of Eucommia ulmoides Extracts on Allergic Contact Dermatitis and Oxidative Damage Induced by Repeat Elicitation of DNCB (두충 추출물이 DNCB로 유발된 알레르기성 접촉피부염과 산화적 손상에 미치는 영향)

  • Inhibitory effects of allergic contact dermatitis of hot water extract of Duchung (Eucommia ulmoides Oliver) leaf, bark and stem growing at Sancheong-gun were investigated for female BALB/c mouse induced by repeat elicitation of DNCB (2,4-dinitro-chlorobenzene). Skin reactions, consisting of increased ear thickness and the presence of ear inflammation, were observed in mice treated with DNCB and Duchung. Weight of lymph node, spleen and thymus in mice treated with Duchung extracts were lower than that of mouse treated with DNCB. Ear weight of mouse treated with Duchung extracts was decreased by increasing the concentration of sample as compared to control group and dropped as low as control level at 1,000 mg/kg. Ear thickness became thinner as test time on Duchung extract progressed. MDA (malondialdehyde) contents in liver tissue were not different in sample group with DNCB group, but were different in ear tissue. NO (nitric oxide) contents was decreased in Duchung extract groups at serum and ear tissues as compared to 1% DNCB group. In the present study, the results suggested that Duchung extract inhibits inflammatory response and oxidative damage induced DNCB allergen.

A New Gymnocalycium mihanovichii 'Gohong' with Bright Red Color (밝은 진적색 비모란 선인장 '고홍' 육성)

  • A new grafted cactus cultivar, G. 'Gohong', was developed by crossing 'DR' line with dark red color and G. 'Seolhong' with dark red color in 2004. It grew for 6 months after grafting in vitro and culturing at 100 mL test tube. After grafting and planting in greenhouse in 2005, various characteristics were evaluated three times until 2008. It has flat-round shape and dark red skin. In addition, it has 8 to 10 ribs and gray and straight spines on the body. The diameter of it was 44.1 mm after 10 months. It grew bigger and faster comparing with G 'Seolhong' cultivar. The ability of propagation in G. 'Gohong' is similar to that of G. 'Seolhong' producing approximately 12.1 daughter globes for 10 months after planting.

  • A new grafted cactus cultivar, G. mihanovichii 'Ihong', for exporting abroad, was developed by crossing G. mihanovichii 'DR' line with dark red color, and G. mihanovichii 'Seolhong' cultivar with dark red in 2005. It grew for 6 months after grafting in vitro and culturing at 100 mL test tube. After grafting and planting in greenhouse in 2006, various characteristics were evaluated three times until 2009. It has flat round shape and dark red skin. In addition, it has 8 to 10 ribs and brown straight spines on the body. Its diameter was 44.0 mm after 10 months from planting, and bigger comparing with that of 'Seolhong' cultivar. The ability of propagation in 'Ihong' was better than that of 'Seolhong' and 'Ihong' approximately produced 15.8 offsets for 10 months.

  • A new grafted cactus variety, 'Dahong', Gymnocalycium mihanovichii, is hybrid crossed between '9508021' line with red and black color, and 'Jinhong', with dark red color in 2002. It was cultivated for 6 months after grafted in vitro and planted at 100 mL test tube. After grafting in vivo in 2003, the various characteristics of this was evaluated three times until 2006. It has the flat-round shape and dark red skin, and produces the daughter globes at areoles, which are arranged well at the low part of globe. In addition, it has 8~10 ribs and the gray and slant spines in body. The ability of propagation is better than 'Jinhong' to produce approximately 13.6 daughter globes.

It has flat-round shape and bright yellow skin. In addition, it has 7 to 9 ribs and brown straight spines on the body. The diameter of it was 40.9 mm after 10 months after planting, and small comparing with G. mihanovichii 'Hukwang' cultivar. The ability of propagation in G. mihanovichii 'Hwangun' is prior to that of G. mihanovichii 'Hukwang' producing approximately 22.3 daughter globes for 10 months.

Effects of Some Medicinal Plants on 48-hour Homologous Passive Cutaneous Anaphylaxis and Chemical Mediators (수종 생약의 48시간 동종 수동 피부 아나필락시와 화학적 전달물질에 대한 작용)

  • Actions for 48-hour homolgous passive cutaneous anaphylaxis (48-hr PCA) and chemical mediators were investigated in mice and rats. The hyaluronidase activity, which was used in the in vitro screening test of the antiallergic action, was significantly inhibited by Magnoiliae Flos, Achyranthis Radix, Forsythiae Fructus, Alpiniae Fructus, Anemarrhenae Rhizoma and Ponciri Fructus among twelve medicinal plants and tranilast as a comparative drug of the antiallergic action. In the mouse ear, 48-hr PCA was significantly inhibited by intraperitoneal (i.p.) pretreatment with Magnoliae Flos, Achyranthis Radix, Alpiniae Fructus, Anemarrhenae Rhizoma, Ponciri Fructus, Ledebouriellae Radix and tranilast. And also, the increment of vascular permeability induced by histamine or serotoin was inhibited significantly by i.p. pretreatment with Magnoliae Flos, Achyranthis Radix, Alpiniae Fructus, Anemarrheuae rhizoma, Zizyphi Fructus and tranilast. In the rat dorsal skin, the increment of vascular permeability induced by histamine or serotonin was significantly inhibited by i.p. pretreatment with Magnoliae Flos, Acyranthis Radix, Alpiniae Fructus, Anemarrhenae Rhizoma and tranilast. And also, the increment of vascular permeability induced by compound 48/80 or calcium ionophore A 23187 was significantly inhibited by i.p. pretreatment with Magnoliae Flos, Achyranthis Radix, Alpiniae Fructus, Amemarrhenae Rhizoma, Zizyphi Fructus, Ledebouriellae Radix, Lithospermi Radix and tranilast. These results suggest that each water extracts of Magnoliae Flos, Achyranthis Radix, Alpiniae Fructus and Anemarrhenae Rhizoma have especially antiallergic activities.

Can prosthetic limbs made too quickly cause kidney damage?: a pilot study

  • Objective: The use of chemicals for building prosthetic sockets present the possibility of being hazardous and unsafe due to off-gassing. The purpose of the present study was to investigate if freshly made materials used in prosthetic sockets causes off-gassing that would penetrate the skin and cause damage to the kidneys or blood. Design: Cross-sectional study. Methods: In this research, the off-gassing effects during the initial curing process of styrene monomer, vinyl ester resin, epoxy methacrylate resin, benzene-1, 3-dimethaneamine, trimethylhexanedlamine, and paratertiarybutylphenol were analyzed. Acid detection strips were placed inside newly fabricated mock-prosthetic sockets and left overnight in a closed environment to find out if acid was present in the invisible fumes. The plastic was worn by 9 subjects and urinalysis was made after 48 hours to test for any kidney or blood toxicity of the resins. Results: After wearing the plastic cuff for 48 hours, the ratio of protein to creatinine in the urine was raised to an abnormal level in five out of nine subjects. Four out of the nine subjects showed normal protein to creatinine ratios after wearing the device. The results showed that damage to the kidney occurred from wearing the resins after curing in half of the subjects. Conclusions: It is very important to conduct patient intakes which includes the assessment of renal function. Off-gassing in vented chambers may be needed to protect both prosthetists and patients.
