Comparison of Dental Age and Skeletal Maturity in Korean Children with Skeletal Malocclusion (한국 어린이의 골격적 부정교합에 따른 골 성숙도와 치아의 성숙도 비교)

  • The purpose of this study was to compare skeletal maturity index and dental developmental stages based on skeletal malocclusion. A total of 192 patients (89 male and 103 female) between 6 to 14 years old were selected for this study and underwent cephalograms, panorama radiographs, and hand-wrist radiographs. Any syndromic cases were excluded. Selected clinical parameters were dichotomised for statistical analysis. Chi-square, logistic regression analysis, and independent t-tests were used for the statistical evaluation. Canine, first molar, and second molar calcification were significantly associated with skeletal maturity in the logistic regression model (p < 0.05). In addition, patients who had higher skeletal maturity index were 11.43 times more likely to be female than those who had lower skeletal maturity index (p < 0.001). The patients with skeletal class II malocclusion displayed significantly higher dental developmental stage in canines, first premolars, first molars, and second molars than the patients with class III malocclusion (p < 0.05). The dental developmental stage of the patients was significantly associated with skeletal maturity. In addition, there was a significant difference between class II and class III malocclusion with some types of tooth calcification.

  • This study was performed to investigate the influence of orthognathic surgery on the temporomandibular dysfunction in skeletal class III malocclusion. The temporomandibular joint status in 22 patients(mean age: 23.7 years) who received orthognathic surgery such as mandibular BSSRO(14 patients), maxillary Le Fort I osteotomy with mandibular BSSRO(8 patients) was evaluated by craniomandibular index. All these patients received orthognathic surgery at least 6 months ago. The mean score and standard deviation was obtained and compared with that of 22 normal individuals(mean age: 24.8years) by Student's t-test. In mandibular movement, the score of orthognathic surgery group was higher than that of the normal group. All the items except mandibular movement did not show any differences between the two groups.

Crown-root angulations of the maxillary anterior teeth according to malocclusions: A cone-beam computed tomography study in Korean population

  • Objective: To compare crown-root angulations of the permanent maxillary anterior teeth in skeletal Class I, Class II, and Class III Korean malocclusion patients using cone-bean computed tomography (CBCT) images. Methods: Sixty CBCT images were collected from orthodontic patients archive based on skeletal Class I (0˚< A point-nasion-B point angle [ANB] < 4˚), Class II (ANB ≥ 4˚), and Class III (ANB ≤ 0˚) to have 20 samples in each group. Mesiodistal crown-root angulation (MDCRA) and labiolingual crown-root angulation (LLCRA) were evaluated after orientation of images. Crown-root angulations were compared among Class I, Class II, and Class III groups and among the maxillary anterior teeth in each group. Results: LLCRAs of the maxillary central incisor and the lateral incisor were significantly lower in Class III group than those in Class I group. However, those of the canine showed no significant differences among groups. MDCRAs of the maxillary anterior teeth did not significantly differ among groups either. Conclusions: Our results suggest that skeletal Class III malocclusion might affect LLCRA of the maxillary incisors, especially the central incisor.

Maxillomandibular arch width differences at estimated centers of resistance: Comparison between normal occlusion and skeletal Class III malocclusion

  • Objective: To evaluate the differences in maxillomandibular transverse measurements at either the crown or the estimated center of resistance (CR), and to compare values between normal occlusion and Class III malocclusion groups. Methods: Dental casts and computed tomography (CT) data from 30 individuals with normal occlusion and 30 with skeletal Class III malocclusions were evaluated. Using the casts, dental arch widths (DAWs) were measured from the cusp tips, and basal arch widths (BAWs-cast) were measured as the distance between the points at the mucogingival junction adjacent to the respective cusp tips. The BAWs determined from CT (BAWs-CT) images were measured from the estimated CRs of the teeth. Results: None of the DAW measurements or maxillomandibular DAW differences showed statistically significant intergroup differences. In contrast, the maxillary BAWs-CT and BAWs-cast were lesser in the Class III malocclusion group than in the normal occlusion group. The mandibular BAWs-CT were significantly greater in the Class III malocclusion group than in the normal occlusion group. Moreover, the maxillomandibular BAW differences on both CT and cast showed significant intergroup differences in all transverse measurements. Conclusions: The maxillomandibular DAW differences showed no significant intergroup differences. In contrast, the maxillomandibular BAW differences on both CT and cast showed significant intergroup differences in all transverse measurements. The maxillomandibular BAW differences at the estimated CRs, measured using CT or casts, can reveal underlying transverse maxillary basal arch deficiencies in patients with skeletal Class III malocclusions.

Evaluation of hyoid bone position and airway size in Class III malocclusion (골격성 III급 부정교합자의 설골 위치와 기도에 대한 평가)

  • This study was performed to evaluate the hyoid bone position and airway in skeletal class III malocclusion and to prove the correlation between airway, hyoid bone position and mandibular position. The sample, considered of 47 class III malocclusion patients for experimmtal group and 52 class I malocclusion students for control group. Twenty three linear and angular measurements about hyoid bone position, airway size, mandibular position were taken from the lateral cephalograms. The differences between skeletal class III malocclusion group and normal occlusion group were compared and the correlation were evaluated statistically. The results obtained were as follows, 1. There were significant difference in S-APH, A-APH, N-APH, LAH-PBR, AA-PNS, PNS-ad between class I and class III malocclusion groups. 2. The hyoid bone was more anteriorly positioned in class III malocclusion group than class I malocclusion group and skeletal airway size in class III malocclusion group was smaller than class I malocclusion group. 3. There were significant difference in several measurements especially vertical and angular measurements of hyoid bone position and airway size between male and female. Usually the measurements in male were larger than female. 4. There were no significant correlation between hyoid bone position and airway size also airway size, and didn't showe significant correlation with mandibular position, 5. S-APH showed negative correlation with Wits appraisal and A-APH, N-APH showed positive correlation with Wits appraisal. On the contrary vertical measurements of hyoid bone position showed positive correlation with lower facial height.

ANALYSIS OF FAMILIAL TENDENCY IN SKELETAL CLASS III MALOCCLUSION (골격성 하악전돌증 환자의 유전적 경향에 대한 분석)

  • The purpose of this study was to examine the familial tendency of the patients with mandibular prognathism in three generations and to define the relationship between the familial tendency and the skeletal class III morphology. The probands of this study were 103 orthognathic surgery patients with skeletal Class III malocclusions who had undergone (48 men, 55 women) mandibular set-back surgery. A questionnaire was given to patients who sought surgical treatment for excessive mandibular length, and all answers were confirmed in interviews. Lateral cephalograms were analyzed in cranial base parameters, mandibular positional parameters and mandibular skeletal parameters. In the examined families, 58.3% had at least one member other than the proband who had mandibular prognathism. The affected ratio of total relatives was 4.5%, and the value was higher in first-degree (13.4%) than second-degree (5.9%) and third-degree relatives (1.7%). The affected ratio was 51.9% in the offsprings who had at least one affected father or mother. The comparison of the groups according to the familial tendency showed no significant craniofacial skeletal measurments. In conclusion, skeletal class III malocclusion showed high familial tendency, suggesting a significant genetic influence in the etiology. However, the patient's familial tendency did not show the special craniofacial patterns compare to the subjects without familial tendency.

  • The purpose of this study was to evaluate the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion. For this study, 74 students at the dental college of Chosun University volunteered as a normal occlusion group. They had well-balanced faces and good occlusions with acceptable Class I molar relationship. They had not received orthodontic treatment and had no signs or symptoms of temporomandibular joint dysfunction. 45 malocclusion patients enrolled for orthognathic surgical treatment with skeletal Class III malocclusion at the Department of Orthodontics, College of Dentistry, Chosun University. On the basis of this study. the results of this study were as follows: 1. Skeletal Class III malocclusion was largely due to the overgrowth of mandible in man and the undergrowth of maxilla in woman. 2. The mandible was antero-inferiorly overgrown by large MP-HP angle and large genial angle in orthognathic surgical cases with skeletal Class III malocclusion. And also, upper incisors were severely labioversioned, but on the other hand lower incisors were linguoversioned. 3. In female, lower-third facial height was characteristically shortened in comparison with middle-third facial height and also, lower facial throat angle was small in male.

Maxillary protraction using skeletal anchorage and intermaxillary elastics in Skeletal Class III patients

  • The aim of this case report is to describe the treatment of a patient with skeletal Class III malocclusion with maxillary retrognathia using skeletal anchorage devices and intermaxillary elastics. Miniplates were inserted between the mandibular lateral incisor and canine teeth on both sides in a male patient aged 14 years 5 months. Self-drilling mini-implants (1.6 mm diameter, 10 mm length) were installed between the maxillary second premolar and molar teeth, and Class III elastics were used between the miniplates and miniscrews. On treatment completion, an increase in the projection of the maxilla relative to the cranial base (2.7 mm) and significant improvement of the facial profile were observed. Slight maxillary counterclockwise ($1^{\circ}$) and mandibular clockwise ($3.3^{\circ}$) rotations were also observed. Maxillary protraction with skeletal anchorage and intermaxillary elastics was effective in correcting a case of Skeletal Class III malocclusion without dentoalveolar side effects.

  • Recently, the presurgical orthodontic duration tends to be shortened by virtue of the advancement of surgical and orthodontic techniques in class III orthognathic surgery cases. But the predictability of the surgical results should be secured by removing several uncertain factors in presurgical orthodontic treatment. The purpose of this study is to investigate the influence of immediate postsurgical occlusal stability on postsurgical mandibular change. The study includes 40 patients who underwent orthognathic surgery to correct skeletal class III malocclusion. The patients were divided into two groups based on the numbers of occlusal contact in surgical setup occlusion: group 1 (stable surgical occlusion, n=24) and group 2(unstable surgical occlusion, n=16). Changes of horizontal and vertical mandibular measurements during postsurgical follow up period(from 1 week postsurgery to 12month after debonding) were compared to examine the differences between two groups. The stability of surgical occlusion is one of the factors influencing postsurgical mandibular changes in class III malocclusion. The various class III malocclusion cases have specific prerequisites for the orthognathic surgery according to the skeletal patterns. The prerequisites should be obtained by minimum presurgical orthodontics to increase the predictability of the surgical results.

Lateral Cephalometric Measurement of Skeletal Class III malocclusion Patients with Uncertainty (불확도를 고려한 골격성 3급 부정교합 환자의 측방 두부방사선영상 계측값의 측정)

  • The purpose of this study was to calculate uncertainty of orthodontic measurement in skeletal class III malocclusion children using lateral cephalometry analysis software which obtained traceability in previous study. Using this data, standard reference of measurement value for skeletal class III malocclusion was obtained. Lateral cephalometric data was collected from 144 children who visited Pediatric Dentistry from 2017 to 2020 for orthodontic treatment. Orthodontic measurement was analyzed with software which obtained traceability. Type A evaluation of uncertainty and type B evaluation of uncertainty was calculated to obtain combined standard uncertainty and expanded uncertainty. Standard reference of skeletal class III children was compared to standard reference of skeletal class I children. Distribution range for skeletal class III malocclusion children aged 6 to 10 with 95% confidence interval was provided using calculated uncertainty of orthodontic measurement value.
