  • This study of small & Medium-Sized construction sites construction disaster prevention technology conduction-site visits from the map results report by the inspector on-site advice and technical guidance for the analysis of deficiencies and potential construction of disaster revealed the potential factors causing an accident as follows. As a results, Should not be a once a month visits. Therefore should be changed at least twice a month to help prevent accidents of this system is to be judged.

  • Construction projects have shown a dramatic increase in size and complexity during the recent years that precise management has been recognized as one of key factors leading the project to success. It has been investigated that management in construction sites is generally done based on the experience of site engineers. Such experience based decision making lacks reliability that could bring disastrous results such as delay and cost overruns. The purpose of this study is to suggest a process progress pattern analysis method that is expected to be used as an objective reference for site managers to aid the decision making done regarding process progress management.

  • Malignant peripheral nerve sheath tumors (MPNSTs) are very rare sarcomas derived from various cells in the peripheral nerve sheath. Malignant peripheral nerve sheath tumors have a known association with neurofibromatosis type 1. Diagnosis of MPNSTs is difficult in patients with chronic pain, when MPNST occurs at an overlapping area of chronic pain. Therefore, the diagnosis can be missed unless clinicians pay attention to the possibility of this disease. Here in, we report a case of concurrent malignant peripheral nerve sheath tumor with complex regional pain syndrome type 1. A 44-year female patient, who was diagnosed with complex regional pain syndrome (CRPS) type 1 in her left ankle, visited our clinic because of aggravated pain. The cause of the aggravated pain was revealed as concurrent MPNST in the left common peroneal nerve territory, which overlapped the site of pain from CRPS.

  • Peri-apical implant lesion, also known as 'retrograde peri-implantitis' can occur with multifactorial etiological factors. The purpose of this case report is to demonstrate resolution of periapical implant lesion by removal of causative factors and saving implant by regenerative therapy. A 54-year old male patient with mild dull pain around implant on the right mandibular second premolar area due to persistent peri-apical infection of the adjacent first premolar was treated. Extraction of tooth with symptomatic apical periodontitis and regenerative therapy on the buccal fenestration area of the implant and extraction site were performed. After 6-month reentry, notable regenerated bone tissue around implant was found, and implant placement on the previous extraction site was performed. After 14-month follow-up from the regenerative therapy, neither biological nor mechanical complication could be found around the implant, evidenced by high implant stability, normal clinical probing depth, and absence of discomfort spontaneously and during masticatory function. In conclusion, surgical intervention including regenerative therapy using bone graft and barrier membrane on periapical implant lesion can be suggested as one of the treatment options considering the extent of periapical lesion.

Nontuberculous Mycobacterial Infection after Body Contouring Procedure, Case Report (체형 교정 시술 후 발생한 비정형 마이코박테리아 감염, 증례 보고)

  • Purpose: NTM (non tuberculous mycobacteria) is rare cause of surgical site infection after plastic surgery in immunocompetent patients. There are some reports about NTM infection after body contouring procedure from Latin America. But, there is no report in Korea. The purpose of this article is to report 2 patients with soft tissue infection caused by NTM after body contouring procedure. Methods: Two young female patients exhibited signs of inflammation and abscess after body contouring procedure. One patient underwent liposuction. The other underwent HPL (hypotonic pharmacologic lipo-dissolution) injection. Results: The result of tissue cultures were positive for NTM. All patients responded to the combined therapeutic approach. Conclusion: The goal of this article is to raise awareness among plastic surgeons who may encounter such patients in their practice. NTM should be included in the differential diagnosis of surgical site infection after body contouring surgery.

  • Expansion in the scope and technique of head and neck tumor resection during the past two decades has paralleled precise tumor localization with advanced radiographic imaging and the availability of microvascular free tissue transfer. Especially, the defect reconstruction utilizing free flap results in improvement of patient survival due to decrease of local recurrence by wide resection of cancer. The rectus abdominis free flap has been used widely in reconstruction of the breast and extremities. However, the report of cases on its applications in the head and neck, based on the deep inferior epigastric artery and vein, is rare. This flap is one of the most versatile soft-tissue flaps. The deep inferior epigastric artery and vein are long and large-diameter vessels that are ideal for microvascular anastomosis. The skin area that can be transferred is probably the largest of all flaps presently in use. The versatility of the donor site is due to the ability to transfer large areas of skin with various thickness and amounts of underlying muscle. This article is to report reconstruction of midface defects utilizing the rectus abdominis free flap in 2 patients with maxillary squamous cell carcinoma and discuss briefly considerations in flap design and orbital exenteration, and healing of irradiated recipient site by hyperbaric oxygen therapy with literature review.

  • A cyst is a pathologic lesion characterized by a cavity filled with fluid, celluar products, air, or a combination of these. Dentigerous cysts were formed around the crown of unerupted teeth. The reduced enamel epithelium persists around the crown after it has formed. Proliferation of the epithelium in a fluid-filled sac may be induced by osmotic pressure. In the first decade the most frequent location is the premolar site. In each subsequent decade the largest number of cysts are in the mandibular third molar site, with the second most frequent site being the maxillary canine. The treatment of odontogenic cyst can be mostly classified into three types of cyst enucleation, marsupialization and decompression. We should consider age of patient, anatomic structure, location and size for choosing a treatment method. Advantage of cyst enucleation is fast healing, but a injury of a surrounding structure is highly. Marsupialization is conservative treatment that can reduce the damage of a adjacent structure, but it is only limited at superficial lesion. Decompression also is conservative treatment, but it has the difficulty of the oral hygiene and the troublesome of the lavage. We present the possibility that reduces the defect of decompression and cures the lesion efficiently. We report a male patient with the dentigerous cyst developed at left mandibular third molar in this study. We used the decompression for a treatment and created special appliance to treat the lesion efficiently. We report a case of the cyst treatment that is association with efficiency of decompression appliance.

A Case Report on Pressure Ulcer Patient Treated with Fractional Laser Therapy and Korean Medicine (프락셔널 레이저 치료와 한의치료를 병행한 욕창치료 1례)

  • Objectives : The purpose of this report to represent the efficacy of pressure ulcer treatment with fractional laser therapy and Korean medicine. Methods : We treated Parkinsonism patients with pressure ulcer using herbal medicine, fractional mode laser therapy, acupuncture and simple dressing for 3 weeks. Laser was set on fractional mode (on time $100{\mu}s$, dot pitch 0.6 mm, shape RECT) and irradiated around pressure ulcer site and center of pressure ulcer site once a day. The treatment effect was evaluated by The National Pressure Ulcer Advisory Panel(NPUAP) stage and the The Pressure Ulcer Scale for Healing(PUSH) tool(3.0). Results & Conclusions : Pressure ulcer site have improved markedly. NPUAP stage was improved from 2 to 1 and total score of the PUSH tool decreased from 12 to 8. Even though the wound was not cured completely, this shows that fractional laser therapy and Korean medicine could be effective treatment for improving pressure ulcers.

A Study on the Brick-constructed Platform of the West hall in Hwangnyongsa temple site (황룡사 서금당 전축기단 연구)

  • This paper aimed to identify architectural historical meaning of the brick-constructed platform of the west hall in Hwangnyoungsa buddhist temple site based on studying published the excavation survey report and comparing with other platforms of Silla remains. Mass and height of the building has a hierarchical structure within a buddhist temple site, this is applicable in Hwangnyongsa. So in case of a rank equal to or lower buildings than the west hall, those of platforms were built of brick-constructed. As stone relics, jidaeseoks are very narrow, I think that wooden pagoda and main hall's platform were built of brick in first construction period. West hall's platform was built at the late 6th~the mid of 7th century, these brick-remains were considered as a quite earlier construction period relics. Because in west hall there is no jidaeseok under bricks, and all bricks relics have a rectangular shape.

A study on the Main-path Remains in the Hwangnyongsa Temple Site (황룡사 답도 연구)

  • This paper aimed to identify a main-path remains in the Hwangnyongsa temple site based on studying published the excavation survey report and researching relics related to the path in the ancient temple site. Hwngnyongsa temple, there were three type's paved footway, straight path to the central axis line of the layout, outside path around the main buildings and the front square of the lecture hall. These remains were expected that installed for some purposes, such as marches, touring, sort of a Buddhist ceremony been performed at the time of Silla. Straight path shows there were two rows of the main access inside the roofed corridor. A row consisted of the 2~3 pieces processed stone(Jangdaeseok) altogether with 92cm width and combined with square stone and stepping stone on the end of the road. It is regarded as very characteristic form and one of the architectural elements of Hwangnyongsa temple in Unified Silla period.
