Navigation guided small craniectomy and direct cannulation of pure isolated sigmoid sinus for treatment of dural arteriovenous fistula

  • Dural arteriovenous fistula (DAVF) is a rare condition affecting approximately 1.5% of 1,000,000 individuals annually. It frequently occurs in the transsigmoid and cavernous sinuses. An isolated sigmoid sinus is extremely rare and is treated by performing transfemoral transvenous embolization along the opposite transverse sinus. A 69-year-old woman presented with asymptomatic Borden type III/Cognard type III DAVF involving an isolated sigmoid sinus. She underwent a staged operation in which a navigation system was used to expose the sigmoid sinus in the operating room before transferring the patient to the angio suite for transvenous embolization. Various modalities have been used to treat DAVF, including surgical disconnection, transarterial embolization, transvenous embolization, and stereotactic radiosurgery. However, treating DAVF cases where the affected sinus is isolated can be challenging because an easily accessible surgical route may not be available. In this case, direct sinus cannulation and transvenous embolization were the most effective treatments.

  • Acute suppurative thyroiditis is rare, infectious thyroid disorder because the thyroid gland is resistant to infection. We report a 26-year-old man with acute suppurative thyroiditis due to a pyriform sinus fistula. He presented with anterior neck swelling and tenderness for 2 weeks. Antibiotic treatment failed to improve his symptoms and signs. Diagnosis was made by bariums studies, computed tomography scan and endoscopic examination. The pyriform siuns fistula was successfully treated by chemical cauterization, partial thyroidectomy and ligation of fistula tract.

이상와 누공 1례 (A Case of Piriform Sinus Fistula)

  • It is commonly believed that the branchial anomalies are remnants of the branchial apparatus. Third and fourth branchial pouch anomaly is an extremely rare disease involving the branchial apparatus. Virtually mostly reported cases have been on the left side and has been identified in recurrent suppurative thyroiditis, retropharyngeal abscess and repeated episodes of cellulitis, neck swelling, lateral neck fistula. Recently the authors have experienced a case of left piriform sinus fistula accompanied with left suppurative thyroiditis, presumably of fourth branchial origin in a 50-years old male patients, which was successfully treated with coagulation, and so report this case with summary of branchial anomaly by the some review of the literatures.

  • Development of de novo dural arteriovenous fistula (DAVF) at a different site after resolution of an initial DAVF, is rare. Here we report two cases, which we encountered in our hospital. A 68-year-old woman presented with pulsatile tinnitus on the left side. Cerebral angiography demonstrated a left anterior condylar confluence (ACC) DVAF and she underwent transvenous embolization. Four years after this treatment, she presented with tinnitus on the left side, and cerebral angiography revealed a right DAVF around the sinus of the lesser sphenoid wing. Another 69-year-old woman presented with left-sided orbital bruits, chemosis, and conjunctival hyperemia. Cerebral angiography showed left cavernous sinus (CS) DAVF, for which she underwent transvenous embolization for CS DAVF. One year later, she developed a left ACC and transverse-sigmoid sinus (TSS) DAVF.

  • Superior petrosal sinus (SPS) dural arteriovenous fistula (DAVF) is one of tentorial DAVFs with significant morbidity, which usually drains into the petrosal vein and its tributaries. Unless there is a connection with venous sinus, surgical obliteration is required. We present two cases of SPS DAVF which were successfully treated with the presigmold retrolabyrinthine approach.

  • The authors present a case of isolated dural arteriovenous fistula (DAVF) in the transverse sinus, which developed six years after microvascular decompression caused by hemifacial spasm via suboccipital craniectomy. The lesion was successfully treated by transarterial embolization using Onyx. We reviewed the related radiologic and therapeutic features of DAVF involving an isolated sinus and described the feasibility of the use of Onyx.

  • We experienced 6 patients with the sinus Valsalva aneurysm rupture during last 12 years [Jan. 1977-Sep. 1989]. Of them. 5 cases were reviewed. They consist of 3 males and 2 females, and the age ranged from 12 years to 40 years with the mean age of 25 years. 4 patients showed congestive heart failure symptoms. The diagnosis was made by 2D-Echo and cine-angiogram. In 4 patients. sinus Valsalva aneurysm ruptured from the Rt. coronary sinus to the Rt. ventricle, and in one from non-coronary sinus to the Rt. atrium. In 2 cases, resection of the aneurysm and simple stitch closure was made. Resection of the aneurysm k patch closure and AVR in one, closure of the fistula, AVR and patch closure of the associated VSD in one, and closure of the fistula, AVR k TVR in one were made in another 3 cases. There was no postoperative mortality case.

Transcranial Direct Middle Meningeal Artery Puncture for the Onyx Embolization of Dural Arteriovenous Fistula Involving the Superior Sagittal Sinus

  • A 66-year-old woman presented with intermittent paraparesis and generalized tonic-clonic seizure. Cerebral angiography demonstrated dural arteriovenous fistula (AVF) involving superior sagittal sinus (SSS), which was associated with SSS occlusion on the posterior one third. The dural AVF was fed by bilateral middle meningeal arteries (MMAs), superficial temporal arteries (STAs) and occipital arteries with marked retrograde cortical venous reflux. Transfemoral arterial Onyx embolization was performed through right MMA and STA, but it was not successful, which resulted in partial obliteration of dural AVF because of tortuous MMA preventing the microcatheter from reaching the fistula closely enough. Second procedure was performed through left MMA accessed by direct MMA puncture following small decortications of cranium overlying the MMA using diamond drill one week later. Microcatheter could be located far distally to the fistula through 5 F sheath placed into the MMA and complete obliteration of dural AVF was achieved using 3.9 cc of Onyx.

치성 피부 누공 환자의 진단과 치료 : 후향적 연구 (Diagnosis and Treatment of odontogenic cutaneous sinus tract : a retrospective study)

  • The odontogenic cutaneous fistula in facial area is uncommon but, well defined disease. It is difficult to diagnose from the dental origin of cutaneous sinus tract. Most patients may visit to the dermatologists or general hospital without cause of disease. They usually be treated by repeated surgical excisions, biopsies, and antibiotic medications, but suffered from recurrences. We studied odontogenic cutaneous fistula through retrospective study in Jeju Special Self-Governing Province between 1 January 2009 and 12 December 2015. There were 3 males, 5 females from 14 to 78 years old with an average age of 50.4 years old. Only 2 patients felt the toothache, others didn't detect it. They suffered from recurrences and repeated treatments for 3 to 11 months with an average period of 7.9 months. They visited average 2.8 hospitals before a precise diagnosis in a dental clinic. All cases were fully healed endodontic treatment or extraction of origin teeth without recurrences. In conclusion, the cause of cutaneous fistula in facial area can be odontogenic. If dentists or doctors diagnose a patient with cutaneous fistula on face, they should check dental problems or take x-ray views for precise diagnosis. It could be helpful for differential diagnosis.

  • Sinus lifting was introduced for prosthetic reconstruction in 1960 decades by Boyne and is specifically used for implant surgery in maxillary posterior region with insufficient bony height and has been indicated for reconstruction of oro- antral and palato-alveolar cleft. We report that subtotal maxillectomy using sinus lifting and skin graft in mild maxillary cancer cases results in good prevention of oro-antral and oro-nasal fistula by preserving intact maxillary sinus mucosa and improves better functional outcome after maxillectomy.
