Diphyllobothrium latum infection in a child with recurrent abdominal pain

  • Diphyllobothrium latum infection in humans is not common in Republic of Korea. We report a case of fish tapeworm infection in a 10-year-old boy after ingestion of raw perch about 8 months ago. The patient complained of recurrent abdominal pain and watery diarrhea. A tapeworm, 85 cm in length, without scolex and neck, was spontaneously discharged in the feces of the patient. The patient was treated with 15-mg/kg single dose praziquantel, and follow-up stool examination was negative after one month. There was no evidence of relapse during the next six months.

  • Between 1981 and 1992, 22 children from 1 to 15 years of age have undergone cardiac valve replacements at National medical center. The patients were composed of 14 males and 8 females and 7 patients had congenital heart disease and 15 patients had rheumatic heart disease. Three of these 15 patients have had second valve replacement due to prosthetic valve failure[PVF]. Single valve replacement were 20[Aortic 3 cases, Mitral 17 cases] and double valve replacments were 2. The overall mortality was 22.7 %. Actuarial survival rate was 77.28 $\pm$ 8.92 % and Complication free rate was 67.68$\pm$15.56%. The pediatric valve replacements can now be performed at a low operative risk although various problems are still remained and the choice of valve is prosthetic valve mainly due to its durability at the present time.

  • Although intraperitoneal teratomas are rare, teratomas of the greater omentuma are seldom reported. A single case of omental teratoma with torsion in a 26-month-old girl is reported. The patient presented with abdominal pain and a palpable mass. Ultrasonography and CT of the abdomen showed a large cystic mass with a non homgenious solid component composed of fat and calcification. The lesion was surgically excised. Pathology revealed a mature omental teratoma. The hospital course was uneventful.

Sleep Patterns and Early Adjustment in 1- to 3-year-old Children in Daycare (영유아의 수면양상과 어린이집 초기 적응)

  • The present study investigated the sleep patterns 1-to 3-year-old children and examined whether their sleep patterns would predict their early adjustment to daycare centers. The participants were 239 young children attending daycare centers in Seoul. The data on children's sleep patterns were collected by parent report. Children's adjustment to daycare was assessed by daycare providers' reports. The data were analyzed by means of Chi-square test, t-test, Pearson's correlation, ANOVA and discriminant analysis. The results were as follows: Children from dual-earner families had shorter nocturnal sleep duration than those from single-earner families. There was no significant relation between daytime sleep duration and sleep-onset time at night. It was found that children who go to bed earlier were more likely to get sufficient sleep without nocturnal wakefulness. Age and total sleep duration were seen to be significant variables when it came to discriminating between the adjustment versus maladjustment groups.

  • Objectives There are constant increase numbers of cases mumps in children. The purpose of this study is to analyze traditional Chinese medicine treatments of epidemic mumps in children through traditional Chinese medical journal research. Methods 20 studies were selected based on title keyword "Epidemic mumps" "Child" published in CAJ(China Academic Journal). Study researched about clinical effect, herbs, attachment, moxa, venesection treatments in epidemic mumps in children Results & Conclusions There are so many treatments about epidemic mumps in children through analyzing studies in CAJ. Single and combination traditional Chinese medicine treatment is more effective in epidemic mumps in children. Therefore, we need to study more about epidemic mumps in children and should make more clinical case about it.

Trend Analysis of Research Using Evaluation Tools of Languages Abilities for Young Children: Based on Early Children Education Journals registered with the Korea Research Foundation (유아 언어능력 평가연구의 동향 분석 -한국학술진흥재단 등재 학회지를 중심으로)

  • This study has a goal to read a trend of language research by analysing evaluation tools and methods that researchers have used for assessing young children's language abilities. Thus the study has chosen 237 language ability evaluation methods out of 121 young child's language ability evaluation researches. The treatises were selected from 4 types of early childhood education journals registered on the Korea Research Foundation. The data analysis was employed for processing the frequency and percentage of the collected data. The results were as follows: First, of single age groups the subject group most selected was five-year-olders and of mixed-age groups the subject group most selected was from three to five, and the number of subjects in researches were mostly below fifty children. The researches were sorted into an 'experimental/ investigational researching' type that has been frequently re-utilized by others, an 'interview type' using a data collection method, and a 'difference verification' type using a data analysis method which has been used in majority of studies. Second, the number of treaties that required data analysis has increased since 1996. Concludingly, the analysis of young child's language ability evaluation tools shows that the purposes of many researches were concentrated on studying children's knowledge about language, children's language functions such as speaking, reading, writing and listening, while evaluation contents were focused on speaking and writing.

Association of Norepinephrine Transporter Gene and Side Effects of Osmotic-Release Oral System Methylphenidate in Attention-Deficit Hyperactivity Disorder (주의력결핍 과잉행동장애에서 노르에피네프린 수송체 유전자와 오로스 메칠페니데이트 부작용의 연관성)

  • Objectives : The aim of our study was to investigate association of norepinephrine transporter gene (SLC6A2) polymorphism and side effects of osmotic-release oral system methylphenidate (OROS MPH) in children with attention-deficit hyperactivity disorder (ADHD). Methods : We recruited drug naive children with ADHD (N=97). We administered OROS MPH by tolerable dosage. At week 8 of treatment, parents completed the Barkley's side effect rating scale. We analyzed two SLC6A2 single nucleotide polymorphisms (SNPs), rs192303 and rs3785143, with blood of subjects. We compared the frequency and severity of each side effect among SLC6A2 genotypes of 2 SNPs. Results : In the analysis of frequency of each side effect, irritability differed according to rs192303 and rs3785143 genotype. In comparisons of severity, talking less and disinterest differed according to rs192303 genotype. In the case of rs3785143, severities of disinterest and irritability were involved with genotype. Conclusion : Side effects of OROS MPH showed an association with SLC6A2 genotype.

The Effects of Infant's Temperament, Development, Mother's Parenting Stress and Social Support on Infant Mother's Self-efficacy (영아의 기질, 발달수준, 어머니의 양육스트레스 및 사회적 지원이 영아 어머니의 자기효능감에 미치는 영향)

  • The purposes of this study was to investigate the relationship and interaction between infant temperament, development, mother's parenting stress and social support on mother's self-efficacy. Participants in this study included 1610 infants (825 boys, 785 girls) and their mothers. The major findings of this study were as follows: First, infant temperament demonstrated a direct relationship to mother's self-efficacy. Lower levels of infant temperament indicated lower levels of self-efficacy, and higher levels of infant temperament indicated higher levels of self-efficacy. Second, infant development demonstrated a direct relationship to mother's self-efficacy. Greater communicative and social interaction between mother and child demonstrated a higher level of maternal self-efficacy. Third, mothers' parenting stress demonstrated a direct relationship to mother's self-efficacy. Higher levels of parenting stress demonstrated lower levels of maternal self-efficacy. Forth, Social support demonstrated a direct relationship to mother's self-efficacy. Greater levels of social support demonstrated lower level of maternal self-efficacy. Fifth, the greatest single relationship effecting mother's self-efficacy was mother's parenting stress. This research suggests the need for development of diverse social policies and programs to help mothers reduce maternal parenting stress and support the development of positive parenting skills with the goal of boosting mother's self-efficacy.

Survival-Related Factors of Spinal Metastasis with Hepatocellular Carcinoma in Current Surgical Treatment Modalities : A Single Institute Experience

  • Objective : Recently, the survival of patients with hepatocellular carcinoma (HCC) has been prolonged with improvements in various diagnostic tools and medical treatment modalities. Consequently, spine metastases from HCC are being diagnosed more frequently. The accurate prediction of prognosis plays a critical role in determining a patient's treatment plan, including surgery for patients with spinal metastases of HCC. We investigated the clinical features, surgical outcomes, and prognostic factors of HCC presenting with spine metastases, in patients who underwent surgery. Methods : A retrospective review was conducted on 33 HCC patients who underwent 36 operations (three patients underwent surgical treatment twice) from February 2006 to December 2013. The median age of the patients was 56 years old (range, 28 to 71; male : female=30 : 3). Results : Overall survival was not correlated with age, sex, level of metastases, preoperative Child-Pugh classification, preoperative ambulatory function, preoperative radiotherapy, type of operation, administration of Sorafenib, or the Tokuhashi scoring system. Only the Tomita scoring system was shown to be an independent prognostic factor for overall survival. Comparing the Child-Pugh classification and ambulatory ability, there were no statistically differences between patients pre- and post-operatively. Conclusion : The Tomita scoring system represents a practicable and highly predictive prognostic tool. Even though surgical intervention may not restore ambulatory function, it should be considered to prevent deterioration of the patient's overall condition. Additionally, aggressive management may be needed if there is any ambulatory ability remaining.

Genome-Wide Analysis Reveals Four Novel Loci for Attention-Deficit Hyperactivity Disorder in Korean Youths

  • Objectives: The molecular mechanisms underlying attention-deficit hyperactivity disorder (ADHD) remain unclear. Therefore, this study aimed to identify the genetic susceptibility loci for ADHD in Korean children with ADHD. We performed a case-control and a family-based genome-wide association study (GWAS), as well as genome-wide quantitative trait locus (QTL) analyses, for two symptom traits. Methods: A total of 135 subjects (71 cases and 64 controls), for the case-control analysis, and 54 subjects (27 probands and 27 unaffected siblings), for the family-based analysis, were included. Results: The genome-wide QTL analysis identified four single nucleotide polymorphisms (SNPs) (rs7684645 near APELA, rs12538843 near YAE1D1 and POU6F2, rs11074258 near MCTP2, and rs34396552 near CIDEA) that were significantly associated with the number of inattention symptoms in ADHD. These SNPs showed possible association with ADHD in the family-based GWAS, and with hyperactivity-impulsivity in genome-wide QTL analyses. Moreover, association signals in the family-based QTL analysis for the number of inattention symptoms were clustered near genes IL10, IL19, SCL5A9, and SKINTL. Conclusion: We have identified four QTLs with genome-wide significance and several promising candidates that could potentially be associated with ADHD (CXCR4, UPF1, SETD5, NALCN-AS1, ERC1, SOX2-OT, FGFR2, ANO4, and TBL1XR1). Further replication studies with larger sample sizes are needed.
