PEKK(Polyetherketoneketone) Surface Treatment Effects on Shear Bond Strength to Dental Veneering Resin (PEKK(Polyetherketoneketone) 표면처리가 치과용 베니어 레진의 전단결합강도에 미치는 영향)

  • The purpose of this study was to investigate the bond strength between PEKK(Polyetherketoneketone) and Sinfony(3M ESPE, Seefeld, Germany) the dental composite resin by proposing the three representative surface treatment methods and evaluate to see if they affect the bond strength between two materials. A total of 30 PEKK($Pekkton^{(R)}$ Ivory, $Cendres+M{\acute{e}}taux$, Bienne, Switzerland) specimens were prepared, embedded in acrylic resin, polished(P 1200 grid) to surface, and each group was divided into 10 specimens. After then, by the surface treatment method, it classified into three groups(n=10) such as Air abrasion group(PN), applying Single Bond Universal(3M ESPE) after Air abrasion(PB), applying OPAQUE(3M ESPE) after Air abrasion(PO). Then, veneering was performed by using Sinfony(3M ESPE, Seefeld, Germany). All completed specimens were allowed to rest in a $37^{\circ}C$ water bath for 24 hours. Shear bond strength of each group was measured and fracture patterns were classified. Statistic analysis was performed with One-way ANOVA followed by post hoc Scheffe tast (p<.05). Statistical analysis was performed using the SPSSWIN 21.0 program. As a result of one-way ANOVA, the average value of PB group was $27.67{\pm}4,18MPa$ and it was shown as the highest bond strength, PN and PO were $20.43{\pm}1.70$ and $19.8{\pm}4.77MPa$ each, and these were relatively low(F=18.4, P<.001), and as the post-test the Scheffe test was conducted and verified (p<.05). After examining the scheffe test, it was showed significant differences as PB>PO, PB>PN(p<.001). Through this study, in order to enhance the bonding force between PEKK and the composite resin, perform the Air abrasion and surface treatment by using Single Bond Universal(3M ESPE) is recommended, and as coMPared with other studies. And it is assumed that the increase of the application time of the Air abrasion affects the increase of the shear bond strength. Thus, further research is required.

The effect of low-dose intravenous bisphosphonate treatment on osteoporosis in children with quadriplegic cerebral palsy

  • Purpose: Quadriplegic children with cerebral palsy are more susceptible to osteoporosis because of various risk factors that interfere with bone metabolism. Pamidronate is effective for pediatric osteoporosis, but there are no guidelines for optimal dosage or duration of treatment in quadriplegic children with osteoporosis. We aimed to evaluate the efficacy of low-dose pamidronate treatment in these patients. Methods: Ten quadriplegic patients on antiepileptic drugs (6 male, 4 female patients; mean age, $10.9{\pm}5.76years$), with osteoporosis and gross motor function classification system level V, were treated with pamidronate (0.5-1.0 mg/kg/day, 2 consecutive days) every 3-4 months in a single institution. The patients received oral supplements of calcium and vitamin D before and during treatment. The lumbar spine bone mineral density (BMD) z score and biochemical markers of bone metabolism were measured regularly during treatment. Results: The main underlying disorder was perinatal hypoxic brain damage (40%, 4 of 10). The mean cumulative dose of pamidronate was $4.49{\pm}2.22mg/kg/yr$, and the mean treatment period was $10.8{\pm}3.32months$. The BMD z score of the lumbar spine showed a significant increase from $-4.22{\pm}1.24$ before treatment to $-2.61{\pm}1.69$ during treatment (P=0.008). Alkaline phosphatase decreased during treatment (P=0.037). Significant adverse drug reactions and new fractures were not reported. Conclusion: Low-dose pamidronate treatment for quadriplegic children with cerebral palsy increased lumbar BMD and reduced the incidence of fracture.

  • Fracture detection has always been very attractive to the log, because it is important in many of our prospecting activities, e.g. in understanding the underground rock formation and also the fluid flow as a high permeability path. This paper demonstrates the use of high resolution borehole acoustic scanner for the detection of fractures. The tool, known as Televiewer, is the first acoustic borehole imaging system to use a focussed beam. The acoustic beams generated by a single transducer are sent toward the borehole wall, scanning the wall in a tight helix as the tool moves along the borehole. The amplitudes and travel times of the reflected signals are then measured, which produces the corresponding images. The highly resolved amplitude image allows to recognize various size of fractures and in addition to derive the rock strength from the image. Meanwhile, the travel time image itself can be directly converted to a precise caliper image, providing detailed information of deviations of the borehole shape. It also allows correction of and explanations for amplitude variations. Field measurements were carried Out at the Cheongyang study sites in Korea to illustrate the efficiency of the televiewer log.

Identification of a Copy Number Variation on Chromosome 20q13.12 Associated with Osteoporotic Fractures in the Korean Population

  • Osteoporotic fractures (OFs) are critical hard outcomes of osteoporosis and are characterized by decreased bone strength induced by low bone density and microarchitectural deterioration in bone tissue. Most OFs cause acute pain, hospitalization, immobilization, and slow recovery in patients and are associated with increased mortality. A variety of genetic studies have suggested associations of genetic variants with the risk of OF. Genome-wide association studies have reported various single-nucleotide polymorphisms and copy number variations (CNVs) in European and Asian populations. To identify CNV regions associated with OF risk, we conducted a genome-wide CNV study in a Korean population. We performed logistic regression analyses in 1,537 Korean subjects (299 OF cases and 1,238 healthy controls) and identified a total of 8 CNV regions significantly associated with OF (p < 0.05). Then, one CNV region located on chromosome 20q13.12 was selected for experimental validation. The selected CNV region was experimentally validated by quantitative polymerase chain reaction. The CNV region of chromosome 20q13.12 is positioned upstream of a family of long non-coding RNAs, LINC01260. Our findings could provide new information on the genetic factors associated with the risk of OF.

Free Vibration Characteristics of a Composite Beam with Multiple Transverse Open Cracks (다중 크랙이 있는 복합재료 보의 자유진동 특성)

  • Free vibration characteristics of a cantilevered laminated composite beam with multiple non-propagating transverse open cracks are investigated. In the present analysis a special ply-angle distribution referred to as asymmetric stiffness configuration inducing the elastic coupling between chord-wise bending and extension is considered. The multiple open cracks are modelled as equivalent rotational springs whose spring constants are calculated based on the fracture mechanics of composite material structures. Governing equations of a composite beam with open cracks are derived via Hamilton's Principle and Timoshenko beam theory encompassing transverse shear and rotary inertia effect is adopted. The effects of various parameters such as the ply angle, fiber volume fraction, crack numbers, crack positions and crack depthes on the free vibration characteristics of the beam with multiple cracks are highlighted. The numerical results show that the existence of the multiple cracks in an anisotropic composite beam affects the free vibration characteristics in a more complex fashion compared with the beam with a single crack.

  • Under excessive plasticity, the fracture toughness of a material depends on its size and geometry. Under fully yielded conditions, the stresses in a material near its crack tip are not unique but rather depend on the geometry. Therefore, the single-parameter J-approach is limited to a high-constraint crack geometry. The JQ theory has been proposed for establishing the crack geometry constraints. This approach assumes that the crack-tip fields have two degrees of freedom. In this study, the crack-tip stress field of a fully circumferential surface-cracked pipe under combined loads is investigated on the basis of the JQ theory by using finite element analysis. The combined loads are a tensile axial force and the thermal gradient in the radial direction. Q-stresses of the crack geometry and its loading state are used to determine the constraint effects. The constraint effects of secondary loading are found to be greater than those of primary loading. Therefore, thermal shock is believed to be the most severe loading condition of constraint effects.

  • Ultrasonic has been proposed as an attractive means of detecting bone loss. There have been several commercial ultrasound devices developed for measuring the heel to predict fracture at other bones. However, these devices select only single point of heel bone as measurement site. It causes poor assessment of bone quality due to the error of transducer positioning. In an effort to improve current ultrasound systems, we evaluated the linear scanning method which provides better prediction of bone quality and an accurate image of bone shape. The system used in this study biaxially scans a heel bone using automated linear scanning technique. The results demonstrated that the values of ultrasound parameters varied with different positions within bone specimen. It has been also found that the linear scanning method could better pre야ct bone quality, eliminating the error of transducer positioning.

Comparison between Intramedullary Nailing and Percutaneous K-Wire Fixation for Fractures in the Distal Third of the Metacarpal Bone

  • Background To compare clinical and radiographic outcomes between intramedullary nail fixation and percutaneous K-wire fixation for fractures in the distal third portion of the metacarpal bone. Methods A single-institutional retrospective review identified 41 consecutive cases of metacarpal fractures between September 2009 and August 2013. Each of the cases met the inclusion criteria for closed, extra-articular fractures of the distal third of the metacarpal bone. The patients were divided by the method of fixation (intramedullary nailing or K-wire). Outcomes were compared for mean and median total active motion of the digit, radiographic parameters, and period until return to work. Complications and symptoms were determined by a questionnaire. Results During the period under review, 41 patients met the inclusion criteria, and the fractures were managed with either intramedullary nailing (n=19) or percutaneous K-wire fixation (n=22). The mean and median total active range of motion and radiographic healing showed no statistically significant difference between the two groups. No union failures were observed in either group. The mean operation time was shorter by an average of 14 minutes for the percutaneous K-wire fixation group. However, the intramedullary nailing group returned to work earlier by an average of 2.3 weeks. Complications were reported only in the K-wire fixation group. Conclusions Intramedullary nailing fixation is advisable for fractures in the distal third of the metacarpal bone. It provides early recovery of the range of motion, an earlier return to work, and lower complication rates, despite potentially requiring a wire removal procedure at the patient's request.

Clinical Experiences of MIDCAB - Developmental Stage and Early Short-term Results - (최소침습적 관상동맥우회술의 발전단계와 경험에 대한 고찰)

  • Background: Minimally invasive direct coronary artery bypass surgery(MIDCAB) has been increasing in interest along with the new techniques in myocardial immobilization for easier and safer procedures. Until the opening of the era of new techniques, adequate accuracy and good patency of grafts were debatable. Our experiences of MIDCAB were studied according to the stages of technical developments. Material and Methods: Since March 1996, 55 patients have undergone MIDCAB procedures. The patients of off-pump CABG(no cardiopulmonary bypass under full sternotomy) were excluded from the study. In the early experience(Stage I), a left anterior small thoracotomy through the left parasternal incision was performed(n=6); then an approach through the lower partial sternotomy was used(Stage II, n=33); and recently, a chest wall elevator for harvesting the internal thoracic artery and the foot plate for myocardial immobilization have been used(USSC, Norwalk, CT)(Stage III, n=16). Result: The surgical procedures of four patients in the Stage II group have been converted to conventional bypass because of the deeply seated left anterior descending coronary artery in two patients, fracture of the calcific lesion in the right coronary artery in one patient, and a cardiogenic shock during hypothermia in the other patient with ventricular dysfunction. Two patients in stage II experienced symptomatic recurrences after surgery and restenosis was verified on angiocardiography. They were managed by interventional procedures. All the other patients were doing well without symptoms, except one patients in Stage II who underwent PTCA procedure for a lesion in the circumflex artery during the follow up period. Conclusion: The new and specialized devices are essential to the development of MIDCAB surgery. MIDCAB and the hybrid procedures in multi-vessel disease are on the way to further development. So far, our experience is limited only to a single device among the many new devices for the purpose.

Ultimate Strength Estimation of Ferritic Stainless Steel Single Shear Bolted Connections Fastened with Four Bolts (페라이트계 스테인리스강 2행 2열 일면전단 볼트접합부의 최대내력평가)

  • The purpose of this study is to investigate the ultimate strength and curling influence using finite element analysis based on the previous test results of ferritic stainless steel bolted connections. Results obtained from finite element analysis (FEA) were compared with those of test results and the validity of numerical modeling was verified. The conditions of curing occurrence for edge/end distance were investigated and ultimate strength reduction ratio caused by curling was estimated quantitatively. Moreover, the ultimate behaviors such as fracture mode and ultimate strength by FEA were compared with those predicted by current design specifications such as AISC, KBC2009, AIJ and AISI.
