Influence of CBCT parameters on image quality and the diagnosis of vertical root fractures in teeth with metallic posts: an ex vivo study

  • Objectives: The aim of this study was to evaluate the influence of peak kilovoltage (kVp) and a metal artifact reduction (MAR) tool on image quality and the diagnosis of vertical root fracture (VRF) in cone-beam computed tomography (CBCT). Materials and Methods: Twenty single-rooted human teeth filled with an intracanal metal post were divided into 2 groups: control (n = 10) and VRF (n = 10). Each tooth was placed into the socket of a dry mandible, and CBCT scans were acquired using a Picasso Trio varying the kVp (70, 80, 90, or 99), and the use of MAR (with or without). The examinations were assessed by 5 examiners for the diagnosis of VRF using a 5-point scale. A subjective evaluation of the expression of artifacts was done by comparing random axial images of the studied protocols. The results of the diagnoses were analyzed using 2-way analysis of variance and the Tukey post hoc test, the subjective evaluations were compared using the Friedman test, and intra-examiner reproducibility was evaluated using the weighted kappa test (α = 5%). Results: The kVp and MAR did not influence the diagnosis of VRF (p > 0.05). According to the subjective classification, the 99 kVp protocol with MAR demonstrated the least expression of artifacts, while the 70 kVp protocol without MAR led to the most artifacts. Conclusions: Protocols with higher kVp combined with MAR improved the image quality of CBCT examinations. However, those factors did not lead to an improvement in the diagnosis of VRF.

Diagnosis of split fractures of the mandible in adults

  • Background: Mandibular split fractures, in which the fracture occurs exclusively in the posterior wall, are uncommon. This study aimed to enhance clinicians' understanding of mandibular split fractures and offer insights for future research. Methods: This study included six patients who visited our hospital between January 2020 and June 2023 and were diagnosed with mandibular split fractures. We retrospectively collected data from patients' medical records on their age, sex, symptoms, mechanism, impact site, associated injuries, and treatment method, as well as the location, pattern, and number of fractures observed on computed tomography (CT) and panoramic images. The frequency of split fractures among all mandibular fractures was calculated. Results: The six patients included three men (50%) and three women (50%), ranging in age from 20 to 71 years (mean age, 49.8 years). The split fractures were located in the symphysis in one patient (16.7%), symphysis to parasymphysis in two patients (33.3%), parasymphysis in one patient (16.7%), and parasymphysis to the body in two patients (33.3%). Four patients (66.7%) had condylar head fractures, while two patients (33.3%) had single split fractures. The mechanism of trauma was a slip-down incident in four cases (66.7%), while two cases (33.3%) were caused by motorcycle traffic accidents. Four patients (67%) underwent intermaxillary fixation, while two patients (33%) improved with conservative treatment. Split fractures were diagnosed in all six patients on CT, whereas the fracture line was not clearly visible on panoramic images. Mandibular split fractures accounted for 5.6% of all mandibular fractures. Conclusion: This study provides insights into the clinical characteristics of rare mandibular split fractures and the diagnostic imaging findings. Furthermore, CT scans and three-dimensional image synthesis-instead of panoramic images-may be essential for accurately diagnosing mandibular fractures, including mandibular split fractures, in the future.

  • We investigated the effect of multiple tempering on the microstructure and mechanical properties of AISI 4340 steel. The austenitized and quenched AISI 4340 steels were tempered at 550, 600, and 650℃ for 1, 2, and 4 h by single-tempering (ST). The multiple tempering was conducted for 4 h by double-tempering (DT, 2 h + 2 h), and quadruple-tempering (QT, 1 h + 1 h + 1 h + 1 h). As tempering temperature increases, yield strength and ultimate tensile strength decrease and elongation increases due to recovery and recrystallization of martensite and coarsening of carbides. At 550℃, as the number of tempering cycles increases, the yield strength and tensile strength decrease at the expense of fracture elongation. At 600 and 650℃, the yield strength and tensile strength increase with increasing the number of tempering cycles while fracture elongation maintains similar values. The multiple tempering at the same tempering time of 4 h improves the modulus of toughness at all tempering temperatures, which is presumed to be due to the change in carbide precipitation behavior by multiple tempering.

Surgical Rib Fracture Fixation: Early Operative Intervention Improves Outcomes

  • Background: This study aimed to assess the outcomes of patients with complex rib fractures undergoing operative or nonoperative management at our major trauma center. Methods: A retrospective review of all patients who were considered for surgical stabilization of rib fractures (SSRF) at a single major trauma center from May 2016 to September 2022 was performed. Results: In total, 352 patients with complex rib fractures were identified. Thirty-seven patients (11%) fulfilled the criteria for surgical management and underwent SSRF. The SSRF group had a significantly higher proportion of patients with flail chest (32 [86%] vs. 94 [27%], p<0.001) or Injury Severity Score (ISS) >15 (37 [100%] vs. 129 [41%], p<0.001). No significant differences were seen between groups for 1-year mortality. Patients who underwent SSRF within 72 hours were 6 times less likely to develop pneumonia than those in whom SSRF was delayed for over 72 hours (2 [18%] vs. 15 [58%]; odds ratio, 0.163; 95% confidence interval, 0.029-0.909; p=0.036). Prompt SSRF showed non-significant associations with shorter intensive care unit length of stay (6 days vs. 10 days, p=0.140) and duration of mechanical ventilation (5 days vs. 8 days, p=0.177). SSRF was associated with a longer hospital length of stay compared to nonoperative patients with flail chest and/or ISS >15 (19 days vs. 13 days, p=0.012), whilst SSRF within 72 hours was not. Conclusion: Surgical fixation of complex rib fractures improves outcomes in selected patient groups. Delayed surgical fixation was associated with increased rates of pneumonia and a longer hospital length of stay.

Research on basic mechanical properties and damage mechanism analysis of BFUFARC

  • In order to study the mechanical properties of basalt fiber reinforced ultra-fine fly ash concrete (BFUFARC), the effects of ultra-fine fly ash (UFA) content, basalt fiber content, basalt fiber length and water reducing agent content on the compressive strength, splitting tensile strength and flexural strength of the composite material were studied through experimental and theoretical analysis. Also, a scanning electron microscope (SEM) was employed to analyze the mesoscopic structure in the fracture surface of composite material specimens at magnifications of 500 and 3500. Besides, the energy release rate (Gc) and surface free energy (γs) of crack tip cracking on BFUFARC in different basalt fiber content were studied from the perspective of fracture mechanics. Further, the cracking resistance, reinforcement, and toughening mechanisms of basalt fibers on concrete substrate were revealed by surface free energy of BFUFARC. The experimental results indicated that basalt fiber content is the main influence factor on the splitting tensile strength of BFUFARC. In case that fiber content increased from 0 to 0.3%, the concrete surface free energy at the tip of single-sided crack showed a trend of increased at first and then decreased. The surface free energy reached at maximum, about 3.59 × 10-5 MN/m. During the process of increasing fiber content from 0 to 0.1%, GC-2γS showed a gradually decreasing trend. As a result, an appropriate amount of basalt fiber can play a preventing cracking role by increasing the concrete surface free energy, further effectively improve the concrete splitting tensile performance.

  • Purpose: This study examined the influence of a metal artifact reduction (MAR) tool, sharpening filters, and their combination on the diagnosis of vertical root fracture (VRF) in teeth with metallic posts using cone-beam computed tomography (CBCT). Materials and Methods: Twenty single-rooted human premolars - 9 with VRF and 11 without - were individually placed in a human mandible. A metallic post composed of a cobalt-chromium alloy was inserted into the root canal of each tooth. CBCT scans were then acquired under the following parameters: 8 mA, a 5×5 cm field of view, a voxel size of 0.085 mm, 90 kVp, and with MAR either enabled or disabled. Five oral and maxillofacial radiologists independently evaluated the CBCT exams under each MAR mode and across 3 sharpening filter conditions: no filter, Sharpen 1×, and Sharpen 2×. The diagnostic performance was quantified by the area under the receiver operating characteristic curve (AUC), sensitivity, and specificity. These metrics were compared using 2-way analysis of variance with a significance level of α=5%. Intra- and inter-examiner agreement were assessed using the weighted kappa test. Results: Neither MAR nor the application of sharpening filters significantly impacted AUC or specificity (P>0.05). However, sensitivity increased when MAR was combined with Sharpen 1× and Sharpen 2× (P=0.015). The intra-examiner agreement ranged from fair to substantial (0.34-0.66), while the inter-examiner agreement ranged from fair to moderate (0.27-0.41). Conclusion: MAR in conjunction with sharpening filters improved VRF detection; therefore, their combined use is recommended in cases of suspected VRF.

수지 첨부 절단창의 재접합술 시 원위지 관절 고정과 운동 범위의 관계 (A Comparative Study of Range of Motion With or Without Distal Interphalangeal Joint Fixation in Replantation of the Amputated Fingertips)

  • Purpose: In the process of replantation of the amputated fingertips, the primary concern was given to survival of the amputees, while the functional aspect of digits after the surgery has been easily neglected. Although an internal fixation with a K-wire is often a part of replantation of the amputated fingertips, little consideration had been given to the study of relationship between distal interphalangeal joint fixation and post operative range of motion. A comparative study in relation to post operative range of motion was done on two different groups, one group with K-wire insertion and the other group without a K-wire insertion at the distal interphalangeal joint. Materials and Methods: The study was done on the cases of a single digit amputation conducted at our institute (the age in the range of 10 to 60) in about four-year of time span from March of 2005 to March of 2009. The cases with a thumb replantation, osteomyelitis or articular surface injury have been excluded from this study. The cases of both head and shaft fracture, except the insertion site of tendon, of distal phalanx of internal fixation with a single K-wire were reviewed for this study. A group of 24 cases without distal interphalangeal joint fixation in comparison to a group of 22 cases with distal interphalangeal joint fixation was reviewed to assess the postoperative range of motion at distal interphalangeal joint on the 6th week after the surgery. And, on the 30th month after the surgery, a group of 10 cases without distal interphalangeal joint fixation in comparison to a group of 10 cases with joint fixation was reviewed. A K-wire was removed in about 5 weeks after the fracture was reunited under the radiographic image, immediately followed by a physical therapy. Result: The active range of motion for a group without interphalangeal joint fixation was measured $49.0^{\circ}$ on average, while $28.6^{\circ}$ was measured for a group with interphalengeal fixation on the 6th week after the surgery. On the 30th month after the surgery, the active range of motion was measured $52.0^{\circ}$ and $55.0^{\circ}$ on average for a group without and with interphalangeal fixation respectively. Conclusion: In the process of replantation of the amputated fingertips, short-term(on the 6th week) improvement of postoperative active motion of range can be expected in the cases without distal interphalangeal fixation in comparison to the cases of interphalangeal joint fixation with a K-wire. However, there seems to be no difference on motion of range in a long-term (on the 30th month) follow up period.

아스팔트 혼합물의 균열 저항성 평가 연구 (Laboratory Test and Evaluation to Characterize the Cracking Resistance of Asphalt Mixtures)

  • 아스팔트 혼합물의 균열 저항성은 일반적으로 인장 강도, 스티프니스와 같은 단일 물성치를 측정함으로써 평가된다. 그러나, 아스팔트 혼합물의 균열 성능을 평가함에 있어서 단일 물성치의 이용은 의문시되어 왔다. 따라서 본 연구에서는 아스팔트 혼합물의 균열 저항성과 관련이 있는 주요 특성치를 좀 더 심도 있게 규명하고자 하였다. 이를 위해 다양한 하중 조건 하에서 파괴 시험 크리프 시험, 강도 시험이 일반 아스팔트 혼합물과 개질 아스팔트 혼합물에 대해서 수행되었다. 시험 결과, 혼합물의 균열 저항성은 주로 미세 손상 축적 속도에 영향을 받음을 알 수 있었으며, 이 값은 파괴 에너지 한계에 영향을 주지 않으면서 m값에 반영됨을 알 수 있었다. 또한, 짧은 하중 재하 시간 (탄성거동) 동안 얻어지는 스티프니스는 혼합물의 균열 저항성의 차이를 규명하는데 한계가 있음을 알 수 있었다. 따라서 아스팔트 혼합물의 균열 저항성을 보다 명확히 평가하기 위해서는 혼합물의 크리프 거동과 파괴 한계점을 동시에 고려하는 것이 필수적임을 알 수 있었다. 또한 수퍼페이브 간접 인장 강도 시험으로부터 구한 잔여 소멸 에너지는 비교적 손쉬운 실험을 통해 아스팔트 혼합물의 균열 저항성의 상대적인 차이를 보여줄 수 있는 유용한 물성치임을 알 수 있었으며, 장기 크리프 시험에서 얻어지는 파괴 변형률은 아스팔트 혼합물의 크리프 거동과 파괴 한계점을 동시에 고려함으로써 균열 저항성을 평가할 수 있는 유용한 물성치 임을 알 수 있었다.

대천-원산도 해저터널 부지조사를 위한 단일채널 탄성파자료의 정밀 처리 및 분석 (Detailed Processing and Analysis on the Single-channel Seismic Data for Site Survey of Daecheon-Wonsando Subsea Tunnel)

  • 한-중 해저터널 후보지로 선정된 대천-원산도 지역의 부지조사를 위해 버블펄서(Bubble pulser) 음원을 이용한 단일채널 탄성파 탐사와 시추조사가 2008년 및 2009년에 수행되었다. 본 연구는 이 단일채널 탐사자료를 정밀 처리하고 시추자료와 연계하여 후보지역의 해저터널 안정성 평가와 시공설계에 필요한 기반암 심도 및 특성을 분석하는데 목적이 있다. 이를 위해 일반적으로 다중채널 탄성파자료에 적용되던 자료처리 기법 중 주파수-파수영역 필터링(F-K filtering)을 적용하여 교류전원잡음 등을 제거하고, 중합후 심도구조보정(post-stack depth migration) 기법을 적용하여 심도단면을 제작하였다. 수행 결과 단일채널 탄성파자료로부터 향상된 심도영역단면을 제작할 수 있음을 확인하였고, 시추자료와 통합 해석을 통하여 조사지역의 개략적인 기반암 분포와 지질특성을 파악할 수 있었다. 단층, 파쇄대 등의 세밀한 구조 파악은 버블펄서 음원과 단일채널 자료의 특성상 어려웠으나, 좀 더 해상도가 높은 탄성파자료가 있을 시 가능할 것으로 보인다. 이에 향후 해저터널 탐사 시 고해상자료를 얻기 위한 추천사항을 수록하였다.

3-차원 보강 복합재 체결부의 피로강도 특성 연구 (A Study on the Fatigue Strength of the 3-D Reinforced Composite Joints)

  • 복합재 체결부는 뛰어난 물성과 가벼운 구조의 수요로 널리 사용되고 있다. 하지만 두께 방향의 취약한 물성으로 인해 체결부 파손이 쉽게 발생한다. 이를 극복하기 위하여 체결부 끝단의 집중되는 응력을 완화시켜주는 Z-피닝, 스티칭 등 다양한 공법들이 적용되고 있다. Z-피닝 공법은 프리프레그의 두께 방향으로 금속 핀이나 카본 핀을 적용하여 보강하는 공법이고, 스티칭 공법은 프리폼에 상부 및 하부 섬유를 교차시켜 두께방향으로 기계적 강도를 향상시키는 방법이다. I-fiber 스티칭 공법은 Z-pinning 공법과 Stitching 공법을 보완한 유망한 공법이다. 본 논문에서는 I-fiber 스티칭 공법으로 보강된 Single-lap joint 시편을 오토클레이브 진공백 성형법으로 제작하여, 모재의 두께와 스티칭 각도에 따른 인장강도 및 피로강도 특성을 평가하여, I-fiber 보강 복합재 체결부 구조물의 보강효과를 검증하였다. 실험결과, 복합재 체결부의 두께가 얇을수록 I-fiber 보강효과가 더 높게 나타났으며 I-fiber로 보강된 복합재 체결부는 파손강도에서 약 52%, 피로강도에서 약 118% 우수한 특성을 나타냄을 확인하였다.
