The prognosis of splinted restoration of the most-distal implants in the posterior region

  • PURPOSE. The aim of this study was to compare the efficacies of two-implant splinting (2-IS) and single-implant restoration (1-IR) in the first and second molar regions over a mean functional loading period (FLP) of 40 months, and to propose the appropriate clinical considerations for the splinting technique. MATERIALS AND METHODS. The following clinical factors were examined in the 1-IR and 2-IS groups based on the total hospital records of the patients: sex, mean age, implant location, FLP, bone grafting, clinical crown-implant ratio, crown height space, and horizontal distance. The mechanical complications [i.e., screw loosening (SL), screw fracture, crown fracture, and repeated SL] and biological complications [i.e., peri-implant mucositis (PM) and peri-implantitis (PI)] were also evaluated for each patient. In analysis of two groups, the chi-square test and Student's t-test were used to identify the relationship between clinical factors and complication rates. The optimal cutoff value for the FLP based on complications was evaluated using receiver operating characteristics analysis. RESULTS. In total, 234 patients with 408 implants that had been placed during 2005 - 2014 were investigated. The incident rates of SL (P<.001), PM (P=.002), and PI (P=.046) differed significantly between the 1-IR and 2-IS groups. The FLP was the only meaningful clinical factor for mechanical and biological complication rates in 2-IS. CONCLUSION. The mechanical complication rates were lower for 2-IS than for 1-IR, while the biological complication rates were higher for 2-IS. FLP of 39.80 and 46.57 months were the reference follow-up periods for preventing biological and mechanical complications, respectively.

  • Tetragonal zirconia polycrystals (TZPs) doped with $Y_{2}O_{3}$ and $Nb_{2}O_{5}$ were prepared by the conventional sintering to enhance fracture toughness and phase stability of TZPs without controlling the grain size. TZP composites were obtained by adding the transformable TZP to the non-transformable TZP in wt%. The monolithic tetragonal $ZrO_2$, same as the composite composition containing 15 wt% transformable TZP, sintered at $1550^{\circ}C$ for 10h in air, exhibited the fracture toughness of 9$MPam^{1/2}$ and no low-and high-temperature degradation at temperatures in the range of 220 to $1000^{\circ}C$ for 100h in air. The corresponding single composition was 90.24 mol% $ZrO_2$-5.31 mol% $Y_{2}O_{3}$-4.75 mol% $Nb_{2}O_{5}$. The microstructure observation revealed that the t-$ZrO_2$ grains grew grandually with sintering time and no microcraking and twinning were observed.

Microstructures and Tensile Properties in Arc Brazed Joints of Ferritic Stainless Steel using Cu-8.6%Al Insert Metal (Cu-8.6wt%Al 삽입금속을 사용한 페라이트계 스테인리스강의 아크 브레이징 접합부의 미세조직과 인장성질)

  • Microstructures and tensile properties in arc brazed joints of ferritic stainless steel, 429EM using Cu-8.6%Al insert metal was investigated as function of brazing current. The brazing speed was fixed at 800mm/min and brazing current varied in the range of 80A to 120A. The initial phase of filler metal was Cu single phase. However, the insert metal structures of brazed joints was composed of Cu matrix and intermetallic compound such as ${\gamma}_1(Al_4Cu_9)$, and flower-shape Fe-Cr. The fraction of ${\gamma}_1(Al_4Cu_9)$ phase was similar with 80A and 100A brazing currents while that of brazed with 120A was decreased. On the other hand, the fraction of Fe-Cr phase increased with increasing of the brazing current. A reaction layer at the base metal/insert metal interface was observed and this reaction layer was thickened with increasing of the brazing current. In the brazed joints with the current lower than 100A, crack was grew up along the interface which was perpendicular to the tensile stress, and then, passed through the insert metal in the final stage of fracture. As the brazing current increased to 120A, fracture occurred at the base metal.

Crack effect on the elastic buckling behavior of axially and eccentrically loaded columns

  • A close form solution of the maximum deflection for cracked columns with rectangular cross-sections was developed and thus the elastic buckling behavior and ultimate bearing capacity were studied analytically. First, taking into account the effect of the crack in the potential energy of elastic systems, a trigonometric series solution for the elastic deflection equation of an arbitrary crack position was derived by use of the Rayleigh-Ritz energy method and an analytical expression of the maximum deflection was obtained. By comparison with the rotational spring model (Okamura et al. 1969) and the equivalent stiffness method (Sinha et al. 2002), the advantages of the present solution are that there are few assumed conditions and the effect of axial compression on crack closure was considered. Second, based on the above solutions, the equilibrium paths of the elastic buckling were analytically described for cracked columns subjected to both axial and eccentric compressive load. Finally, as examples, the influence of crack depth, load eccentricity and column slenderness on the elastic buckling behavior was investigated in the case of a rectangular column with a single-edge crack. The relationship of the load capacity of the column with respect to crack depth and eccentricity or slenderness was also illustrated. The analytical and numerical results from the examples show that there are three kinds of collapse mechanisms for the various states of cracking, eccentricity and slenderness. These are the bifurcation for axial compression, the limit point instability for the condition of the deeper crack and lighter eccentricity and the fracture for higher eccentricity. As a result, the conception of critical transition eccentricity $(e/h)_c$, from limit-point buckling to fracture failure, was proposed and the critical values of $(e/h)_c$ were numerically determined for various eccentricities, crack depths and slenderness.

  • Ease of manipulation, adequate mechanical properties, long years of experience and economical cost are the factors which have established amalgam as the most widely used material for dental restorations. But amalgam restoration may require replacement because of secondary caries, fracture, "fall-out", dimensional change, tarnish or corrosion etc.. These failures of amalgam restorations seem to arise from failures during operations rather than from the inherent shortcomings of the material itself or of the patient's mismanagement. It is anticipated that notonly number of analgam restoration, but failures will be increase after more extensive utilization of the medical insurance which began in 1977. Then authors think that it would be helpful for the development of better treatment in daily dental practice, to know the duration of amalgam restorations and the reasons for their replacement. The data for this survey was compiled from 2, 856 out-patients of the Department of Dentistry, Ewha Woman's University Hospital from January 1975 to December 1977. 260 cases among 1,718 fillings were studied, of which 205 cases both had a single reason for replacement and recognized the date of the previous filling. The results obtained were as follows; 1. Amalgam fillings were 58. 5 percent of all dental restorative materials. Of these, 15. 13 percent of the amalgam restorations had to be replaced. 2. The first reason for replacement of amalgam restorations was secondary caries (56.10%), the second was fracture (23.80%) and the third was "fall-out" (8.78%). 3. Among those amalgms requiring replacement, 52.2 percent had been in place less than 3 years, 70.7 percent within 5 years and 89.8 percent had been in place less than 10 years. Only 10.2 percent had been in place more than 10 years.

Comparison between Two Kirschner Wire Fixation and Three Wire Fixation, in Treating of Metacarpal Neck Fracture Using Multiple Retrograde Kirschner Wire Fixation (다발성 역행성 K 강선을 이용한 중수골 경부 골절의 치료에서 2개의 강선과 3개의 강선 사용군의 비교)

  • Purpose: To compare clinical and radiographic outcomes of between two and three Kirschner wire(K-wire) intramedullary fixation for fractures in the neck of the metacarpal bone. Methods: A single institutional retrospective review identified 28 cases of metacarpal fractures between March 2010 and August 2014. Each of the cases met the inclusion criteria for closed, extra-articular fractures of the neck of the metacarpal bone. The patient groups were divided by the number of K-wire. Outcomes were compared for range of motion of the metacarpophalangeal joint, radiographic parameters, and period until union. Results: The fractures were treated with either 2 Kirschner wire fixation (n=10) or 3 Kirschner wire fixation (n=18). The active range of motion of metacarpophalangeal joint and radiographic result showed no statistically significant difference between the two groups. The mean union period was 5.9 weeks. However, four cases suffered distal head perforation in 2 K-wire fixation group and one case in 3 K-wire fixation group. Conclusion: Multiple retrograde intramedullary Kirschner wire fixation is a good treatment of choice for fractures in the neck of the metacarpal bone. To prevent metacarpal head perforation, it is preferred to use three K-wires than two K-wires.

Retroperitoneal Extrapleural Approach for Corpectomy of the First Lumbar Vertebra : Technique and Outcome

  • Objective : Corpectomy of the first lumbar vertebra (L1) for the management of different L1 pathologies can be performed using either an anterior or posterior approach. The aim of this study was to evaluate the usefulness of a retroperitoneal extrapleural approach through the twelfth rib for performing L1 corpectomy. Methods : Thirty consecutive patients underwent L1 corpectomy between 2010 and 2016. The retroperitoneal extrapleural approach through the 12th rib was used in all cases to perform single-stage anterior L1 corpectomy, reconstruction and anterior instrumentation, except for in two recurrent cases in which posterior fixation was added. Visual analogue scale (VAS) was used for pain intensity measurement and ASIA impairment scale for neurological assessment. The mean follow-up period was 14.5 months. Results : The sample included 18 males and 12 females, and the mean age was 40.3 years. Twenty patients (67%) had sensory or motor deficits before the surgery. The pathologies encountered included traumatic fracture in 12 cases, osteoporotic fracture in four cases, tumor in eight cases and spinal infection in the remaining six cases. The surgeries were performed from the left side, except in two cases. There was significant improvement of back pain and radicular pain as recorded by VAS. One patient exhibited postoperative neurological deterioration due to bone graft dislodgement. All patients with deficits at least partially improved after the surgery. During the follow-up, no hardware failures or losses of correction were detected. Conclusion : The retroperitoneal extrapleural approach through the 12th rib is a feasible approach for L1 corpectomy that can combine adequate decompression of the dural sac with effective biomechanical restoration of the compromised anterior loadbearing column. It is associated with less pulmonary complication, no need for chest tube, no abdominal distention and rapid recovery compared with other approaches.

Epidemiology of Facial Bone Fractures During the Coronavirus Disease 2019 Pandemic: A Single Korean Level I Trauma Center Study

  • Purpose: The medical community has been heavily impacted by the coronavirus disease 2019 pandemic. The management of facial trauma patients has been affected by the patient capacity of emergency rooms. In this study, we share our experiences of facial trauma management during the social lockdown period and investigate the epidemiological changes in facial bone fractures. Methods: A total of 997 patients who presented to Ajou University Hospital Emergency Center and were evaluated by plastic or maxillofacial surgeons for facial trauma were included in this retrospective study. Our study design was a comparative study of two groups: the 2019 group (control) and the 2020 group (the experimental group that experienced social lockdown). Results: The total number of emergency room inpatients reflected the national pandemic trends with three peaks in patient numbers. According to these trends, facial bone fractures had two different low points in August 2020 and December 2020. A comparison of the 2019 and 2020 facial bone fractures did not show a statistically significant difference in the total number of patients. An analysis of the causes of trauma showed that domestic accidents increased in 2020 (30.92%; p<0.001). Among the anatomical sites of facial injury in surgical patients, the frontozygomatic complex fracture increased the most in 2020 (p=0.018). Facial injuries with two separate sites of injury or with three or more involved sites also showed a significant increase in 2020 (p<0.001). Conclusions: We demonstrated that the incidence of facial trauma patients correlated with the incidence of patients presenting to the emergency department and that facial trauma is inextricably related to multi-trauma cases. Domestic accidents and facial trauma with multiple anatomically involved sites are increasing trends that need more attention.

Structural and Electrical Properties of (La,Nd,Sr)MnO3 Ceramics for NTC Thermistor Devices

  • (La0.5Nd0.2Sr0.3)MnO3 specimens were prepared by a solid-state reaction. In all specimens, X-ray diffraction patterns of an orthorhombic structure were shown. The fracture surfaces of (La0.5Nd0.2Sr0.3)MnO3 specimens showed a transgranular fracture pattern be possibly due to La ions (0.122 nm) as a perovskite A-site dopant substituting for Nd ions (0.115 nm) having a small ionic radius. The full-width at half maximum (FWHM) of the Mn 2p XPS spectra showed a value greater than that [8] of the single valence state, which is believed to be due to the overlapping of Mn2+, Mn3+, and Mn4+ ions. The dependence of Mn 2p spectra on the Mn3+/Mn4+ ratio according to sintering time was not observed. Electrical resistivity resulted in the minimum value of 100.7 Ω-cm for the specimen sintered for 9 hours. All specimens show a typical negative temperature coefficient of resistance (NTCR) characteristics. In the 9-hour sintered specimen, TCR, activation energy, and B25/65-value were -1.24%/℃, 0.19 eV, and 2,445 K, respectively.

Perioperative complications of the modified Stoppa approach for the treatment of pelvic bone fractures: a single-institution review of 48 cases

  • Purpose: The current study aimed to report the perioperative complications of the modified Stoppa approach for the treatment of pelvic bone fractures. Methods: We analyzed 48 consecutive operations in 45 patients who were treated with internal fixation using the modified Stoppa approach between March 2016 and July 2018. This included three revision operations. The mean age of the patients was 54.5 years, and the patients included 35 male patients and 10 female patients. All fractures occurred as a consequence of high-energy trauma and 70.3% had associated injuries at the time of the fracture. The mean Injury Severity Score was 9.03±5.60. The perioperative complications found during and immediately after surgery were recorded and were classified into three categories: vascular injuries, nerve injuries, and other complications. Results: Overall, 14 perioperative complications (29.2%) in 14 cases were identified. The most common complications were nerve injuries, which occurred in seven cases, all involving the obturator nerve. Uncontrollable vascular injuries occurred in six cases, which required additional incisions and support of vascular surgeons or postoperative interventions. Additionally, one case of peritoneal tearing occurred, which required help from an abdominal surgeon. Conclusions: While the modified Stoppa approach seems to be a viable method to treat pelvic fractures, significant perioperative complications may occur, suggesting that surgeons should pay careful attention to minimize the damage to other structures and that appropriate support from other surgical departments is paramount.
