A comparative study on the shaping ability of ProFile and Pro Taper, K-Flexofile in simulated canals with different angles of curvature

  • I. Objectives This study was done to evaluate the shaping ability of ProFile and ProTaper, K-Flexofile during instrumentation in simulated resin canals with different angles of curvature. II. Materials and Methods Ninety simulated root canals with a curvature of 15, 30 and 45 degree (following Schneider's method) were made of epoxy resin ($EPOXICURE^{TM}$, BUEHLER, USA) for this study. The canal length of simulated root canals was adjusted to 18 mm. The beginning point of curvature was positioned at 10mm from the canal orifice.(omitted)

  • The aim of this study was to determine the shaping ability of stainless-steel K file (S-S K file), nickel-titanium K file (Ni-Ti K file) and engine driven nickel-titanium file (Quantec file) in resin simulated root canal. Computed tomography was used to evaluate the change of the root canal morphology. Thirty nine resin simulated root canal were divided into four groups (A:12, B:12, C:12, D:3). Resin simulated canals were scanned by computed tomography before instrumentation (1st C-T scan). Canals were instrumented using step back preparation technique with S-S K file in group A and Ni-Ti K file in group B. Group C was prepared with engine driven Ni-Ti file. Group D was uninstrumented to compare the 1st C-T scan images with 2nd C-T scan images of root canal. Instrumented canals were again scanned using computed tomography (2nd C-T scan), and reformated images of the uninstrumented canals were compared with images of the instrumented canals. In the sections of 2mm and 6mm from the apex, Quantec file caused significantly less canal transportation than S-S K file and Ni-Ti K file (p<0.05). Quantec file produced more centered than S-S K file and Ni-Ti K file in the sections of 2mm and 4mm from the apex (p<0.05). There was no significant difference in the removed volume of canals among the each groups (p>0.05). However the removed canal volume from the apex to 5mm were significantly higher than them from 5mm to 1mm (p<0.05) in each groups. Under the conditions of this study, preparation with Quantec file was more effective and produce more appropriate canal shapes than S-S K file and Ni-Ti K file.

  • The purpose of this study was to compare the root canal shaping ability of four nickel-titanium rotary instruments. Forty eight simulated curved root canals were instrumented in plastic with the Crown-down technique using the $ProTaper^{TM}$ , the ProFile, the $GT^{TM}$, and the Quantec. Canals were instrumented until apical canal were up to size 30 by one operator. Each instrument was used only once and instrumentation time was measured.(omitted)

Micro-computed tomographic evaluation of a new system for root canal filling using calcium silicate-based root canal sealers

  • Objectives: This study evaluated by using micro-computed tomography (micro-CT) the filling ability and sealer apical extrusion promoted by a new Sealer Injection System (SIS; Angelus) with side openings needle, in comparison with the conventional injection system, associated with a new ready-to-use calcium silicate-based sealer (Bio-C Sealer). Materials and Methods: Acrylic resin models containing a main curved artificial canal and 3 simulated lateral canals in apical, middle and cervical thirds were used. The main root canals were prepared using a rotary system up to size 35.05. The canals were filled with Bio-C sealer by using a single cone technique and the conventional delivery system or SIS. Samples were scanned in micro-CT. The percentage of voids throughout the entire extension of the main root canal and in each third of the lateral canals, besides the apical extrusion of the sealer was calculated. Data were submitted to t-test (p < 0.05). Results: There was no difference between both systems in the main root canals filling. Although the volume percentage of voids was similar in the apical and middle thirds of lateral canals, SIS had the greatest filling ability of the cervical third lateral canal. Moreover, the conventional system showed the highest apical extrusion of the sealer. Conclusions: The conventional and SIS obturation systems had an appropriate filling ability of the main root canal. SIS had the best filling of the cervical third of the lateral canals, besides lower sealer apical extrusion, suggesting its clinical indication.

A micro-computed tomographic evaluation of root canal filling with a single gutta-percha cone and calcium silicate sealer

  • Objectives: The purpose of this study was to evaluate the void of root canal filling over time when a calcium silicate sealer was used in the single gutta-percha cone technique. Materials and Methods: Twenty-four J-shaped simulated root canals and twenty-four palatal root canals from extracted human maxillary molars were instrumented with ProFile Ni-Ti rotary instruments up to size 35/0.06 or size 40/0.06, respectively. Half of the canals were filled with Endoseal MTA and the other half were with AH Plus Jet using the single gutta-percha cone technique. Immediately after and 4 weeks after the root canal filling, the samples were scanned using micro-computed tomography at a resolution of 12.8 ㎛. The scanned images were reconstructed using the NRecon software and the void percentages were calculated using the CTan software, and statistically analyzed by 1-way analysis of variance, paired t-test and Tukey post hoc test. Results: After 4 weeks, there were no significant changes in the void percentages at all levels in both material groups (p > 0.05), except at the apical level of the AH Plus Jet group (p < 0.05) in the simulated root canal showing more void percentage compared to other groups. Immediately after filling the extracted human root canals, the Endoseal MTA group showed significantly less void percentage compared to the AH Plus Jet group (p < 0.05). Conclusions: Under the limitations of this study, the Endoseal MTA does not seem to reduce the voids over time.

  • ;A new root canal instrument and instrumentation technique: a preliminary report. Cleaning and shaping the root canal system has been and continues to be a challenge for even the most experienced endodontist. Curved. narrow canals. in particular. cause difficulties for the beginner as well as the specialist. A new instrument designed to incorporate new concepts was developed to ameliorate the problems in cleaning and shaping root canal systems. This new instrument has been given the name of SW (Senia and Wildey). The new SW instrument uses controlled right and left rotational forces. This motion was used to clean and shape simulated root canals in plastic blocks and root canals in extracted teeth. Instrumentation appeared to be easier. faster. and more precise than with conventional instruments. especially in curved canals where there was remarkable reduction of canal transportation. A mechanical version of the SW instrument was also developed. It was used to flare the coronal portion of the root canal system. Wildey WL. Various factors must be considered in an analysis of instrumentation of the root canal system: the dentin that is cut: the technique used to cut it: the design of the instruments: the material and manufacturing process used to make the instruments: the irrigant used during the procedure: and the anatomic configuration of the root canal system. An analysis of these factors clearly indicates that existing root canal instruments and techniques are less than ideal and. in fact. do not accomplish what is expected of them. Root canals must be properly. but. at the same time. destructive and unnecessary removal of dentin should be kept to a minimum. The Flex-R and Canal Master instruments were developed to address some of the shortcomings of existing instruments and techniques. 모조 레진블락 근관에서 LightSpeed, ProTaper 및 Hybrid technique의 성형 효율 비교 (Comparison of shaping ability using LightSpeed, ProTaper and Hybrid technique in simulated root canals)

  • The purpose of this study was to compare the shaping abilities of LightSpeed, ProTaper-Universal, and hybrid technique using S-series of ProTaper-Universal and LigthSpeed. The 72 simulated root canals of J-shape were used and classified as flowing 3 groups according to the instrumentation methods; Group P of 24 canal blocks were prepared with ProTaper-Universal, Group L was prepared with LightSpeed, and Group H was prepared with hybrid technique (initial shaping with ProTaper-Universal SI and S2 and apical shaping with LightSpeed from #25 to #50). A second-year resident of Endodontic department prepared the resin block canals to apical size #50 (F5 in Group P). The time lapses for instrumentation and the reduction of root canal curvature after shaping were measured. The pre- and post-instrumented root canals were scanned and superimposed to evaluate and calculate the increased canal width and apical centering ratio. The results were as followings: Group Land H showed significant less instrumentation time than Group P (p < 0.05). The ProTaper system showed greater reduction of root canal curvature and working length diminishment than other methods (p < 0.05). LightSpeed system showed best canal curvature preserving characteristics. The Group P had greater instrumented widths at all levels examined (p < 0.05). Group L and Group H showed lower centering ratio (ability to preserve the canal center; the lower ratio means the better canal center preservation) than Group P (p < 0.05). Group H had the lowest centering ratio at the 1 mm level.

Comparison of the shaping ability of novel thermally treated reciprocating instruments

  • Objectives: The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals. Materials and Methods: Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold. Results: The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments. Conclusion: Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.

만곡된 레진 모형 근관에서 4종의 엔진 구동형 니켈-티타늄 기구의 근관 중심율 유지 능력 (The instrument-centering ability of four Nickel-Titanium instruments in simulated curved root canals)

  • 본 연구의 목적은 새로 시판된 NRT 기구의 근관 형태 유지 능력을 기존의 3종의 기구와 비교하는 것이다. 만곡된 레진 모형 근관을 ProFile, K3, ProTaper, NRT 기구를 이용하여 형성하였고 형성 전, 후의 상을 스캔한 후 중첩시켜 이미지 분석 프로그램을 이용하여 근관 만곡도의 변화와 근관 중심축에 대한 근관의 변이 정도를 근관 중심율의 변화로 계산하여 분석하였다. 근관 변이의 유의성 검정을 위해 one-way ANOVA분석을 시행하였으며 각 기구간의 유의성 검정은 Duncan's test로 사후 분석하였다. 근첨에서 1, 2 mm 상방 위치에서의 근관 중심율은 ProFile 군이 다른 군에 비해 우수한 것으로 나타났다. (p < 0.05). NRT기구의 근관 중심율 변화를 측정한 결과 근첨으로부터 5 mm 상방에서는 Profile에 비해서만 큰 것으로 나타났고 6, 7 mm 상방에서는 다른 모든 군들에 비해 큰 것으로 나타났다 (p < 0.05). 비록 NRT기구가 근첨 부위에서는 ProFile을 제외한 다른 기구에 비해 유의차 없는 중심률 유지 능력을 보였으나 근관 입구 및 중간부위의 형성에 사용되는 스테인레스-스틸 파일로 인해 coronal flaring시 다른 기구들에 비해 더 많은 중심률의 변화를 보이는 것으로 사료된다.

A comparison of the shaping ability of reciprocating NiTi instruments in simulated curved canals

  • Objectives: The study was to compare the shaping ability of Reciproc (VDW) and WaveOne (Dentsply Maillefer) instruments compared with ProTaper, Profile and hand instrument during the preparation of simulated root canals. Materials and Methods: Five groups (n = 5) were established. Reciproc, WaveOne, ProTaper, Profile and K file (K-flexo file) were used to prepare the resin simulated canals. A series of preoperative and postoperative images were taken by a microscope and superimposed in 2 different layers. The amount of resin removed from both the inner and the outer sides of the canal was measured to the level of 10 mm from the apical tip, with a 1 mm increment. Results: The mean of resin removal from the inner canal wall was not different from the outer canal wall for Reciproc and WaveOne groups at apical third (1 - 3 mm level). There was no difference in the change of working length and maintenance of canal curvature. NiTi instruments are superior to stainless-steel K file in their shaping ability. Conclusions: Within the limitation of this present study, Reciproc and WaveOne instruments maintained the original canal curvature in curved canals better than ProTaper and Profile, which tend to transport towards the outer canal wall of the curve in the apical part of the canal.
