Diffuse-Type Histology Is Prognostic for All Siewert Types of Gastroesophageal Adenocarcinoma

  • Purpose: The optimal treatment for gastroesophageal junction adenocarcinoma (GEJA) remains controversial. We evaluated the treatment patterns and outcomes of patients with locally advanced GEJA according to the histological type. Materials and Methods: We conducted a single-institution retrospective cohort study of patients with locally advanced GEJA who underwent curative-intent surgical resection between 2010 and 2020. Perioperative therapies as well as clinicopathologic, surgical, and survival data were collected. The results of endoscopy and histopathological examinations were assessed for Siewert and Lauren classifications. Results: Among the 58 patients included in this study, 44 (76%) were clinical stage III, and all received neoadjuvant therapy (72% chemoradiation, 41% chemotherapy, 14% both chemoradiation and chemotherapy). Tumor locations were evenly distributed by Siewert Classification (33% Siewert-I, 40% Siewert-II, and 28% Siewert-III). Esophagogastrectomy (EG) was performed for 47 (81%) patients and total gastrectomy (TG) for 11 (19%) patients. All TG patients received D2 lymphadenectomy compared to 10 (21%) EG patients. Histopathological examination showed the presence of 64% intestinal-type and 36% diffuse-type histology. The frequencies of diffuse-type histology were similar among Siewert groups (37% Siewert-I, 36% Siewert-II, and 33% Siewert-III). Regardless of Siewert type and compared to intestinal-type, diffuse histology was associated with increased intraabdominal recurrence rates (P=0.03) and decreased overall survival (hazard ratio, 2.33; P=0.02). With a median follow-up of 31.2 months, 29 (50%) patients had a recurrence, and the median overall survival was 50.5 months. Conclusions: Present in equal proportions among Siewert types of esophageal and gastric cancer, a diffuse-type histology was associated with high intraabdominal recurrence rates and poor survival. Histopathological evaluation should be considered in addition to anatomic location in the determination of multimodal GEJA treatment strategies.

  • Although the incidence of gastroesophageal junction (GEJ) adenocarcinoma has been increasing worldwide, no standardized surgical strategy for its treatment has been established. This study aimed to provide an update on the surgical treatment of GEJ adenocarcinoma by reviewing previous reports and propose recommended surgical approaches. The Siewert classification is widely used for determining which surgical procedure is used, because previous studies have shown that the pattern of lymph node (LN) metastasis depends on tumor location. In terms of surgical approaches for GEJ adenocarcinoma, a consensus was reached based on two randomized controlled trials. Siewert types I and III are treated as esophageal cancer and gastric cancer, respectively. Although no consensus has been reached regarding the treatment of Siewert type II, several retrospective studies suggested that the optimal treatment strategy includes paraaortic LN dissection. Against this background, a Japanese nationwide prospective trial is being conducted to determine the proportion of LN metastasis in GEJ cancers and to identify the optimal extent of LN dissection in each type.

위식도접합부선암의 임상병리학적 특성 (The Clinicopathological Characteristics of Adenocarcinoma of the Gastro-esophageal Junction)

  • 목적: 위식도접합부선암의 임상병리학적 특성은 서구와 아시아 간의 많은 차이를 보인다고 알려져 있다. 그러나, 한국의 경우 위식도접합부선암의 유병율이 매우 낮고 이에 대한 연구 또한 부족하다. 따라서 저자들은 우식도접합부선암의 Siewert 분류에 따른 임상병리학적 특성을 조사하여 서구와 다른 국내 위식도접합부선암의 특성을 연구하고자 하였다. 대상 및 방법: 2004년 5월부터 2008년 2월까지 본원에서 위암으로 수술을 시행 받은 환자들을 대상으로 수술소견 및 병리 사진을 토대로 Siewert 분류법에 따라 위식도접합부선암을 분류하고, 의무기록을 바탕으로 환자들의 임상병리학적 특성들을 조사하였다. 결과: 위식도접합부선암은 전체 1,778예 중 70예(3.9%)였다. Siewert 1형은 3예(4.3%)로 매우 드물었으며, 2형과 3형이 각각 30예(42.8%) 37예(52.8%)였다. 2형이 3형에 비하여 평균연령이 높았으나(64세 vs 59세, P=0.049), 남녀비에는 차이가 없었다. 2형과 3형간에 Barrett 식도의 발현 정도나 Lauren 분류의 차이는 없었으나, 저분화암의 빈도는 3형에서 유의하게 높았다(P=0.045). 68예(97.1%)에서 근치적 절제가 시행되었으며, 각 아형간에 TNM 병기의 차이는 없었다. 결론: 국내의 위식도접합부선암은 매우 드물며 특히 1형의 유병율이 매우 낮다. 본 연구에서 서구의 보고와 부분적으로 일치하지 않는 병리학적 특성을 보였다. 향후 보다 많은 환자들을 대상으로 한국인의 위식도접합부선암의 특성을 연구할 필요가 있다.

Siewert 분류에 의한 협의의 분문부 위암(type II)과 분문하 위암(type III)의 검토 (Analysis of Clinocopathologic Difference between Type II and Type III Cancers in Siewert Classification for Adenocarcinomas of the Cardia)

  • Purpose: To determine the clinical value of the Siewert classification for gastic-cancer patients in Korea, we evaluated and compared the clinicopathologic factors of type II and type III cancer. Materials and Methods: The medical records of 89 consecutive patients who had undergone surgery for an adenocarcinoma of the gastroesophageal junction (GEJ) at the Department of Surgery, Hanyang University Hospital, between Jun. 1992 and Dec. 2003 were reviewed retrospectively. Results: There were one patient with type I, 12 pateints with type II and 77 patients with type III. During the same period, 1,341 patients underwent surgery for a gastric carcinoma, so proportion of GEJ cancer being $6.6\%$. The median followup duration was 31 months (range: $2\∼135$ months), and the follow-up rate was $100\%$. Between type II and type III cancers, there were no significant differences in the clinicopathologic variables including age, sex, gross appearance, histologic type, depth of invasion, and pathologic stage. The longest diameter of the tumor was larger in type III ($6.1\pm2.1$ cm) than in type II ($3.9\pm1.1$ cm)(P=0.001). A total gastrectomy with Roux-en-Y esophagojejunostomy was done most frequently, while jejunal interposition was done in 3 cases of type II and 2 cases of type III. More than a D2 lymphadenectomy was done all cases. The numbers of dissected lymph nodes and metastatic lymph nodes in type II were 43.8 and 5.8 respectively, while they were 49.8 and 8.1 in type III, but the difference between the two groups were not statistically significant. The mean length of the proximal resection margin was $15\pm5$ mm in type II and $21\pm13$ mm in type III, but this difference was not statistically significanct. The time to recurrence after operation was 19.3 months in type II and 16.9 months in type III. The five-year survival rates of type II and III were $68.8\%\;and\;52.7\%$ respectively, but difference was not significant. Conclusion: There were no significant differences in the clinicopathologic variables, including survival rate, between type II and type III cancers in Korean patients According to these findings, it appears to be reasonable to classify type III cancer as a cardia cancer in a broad sense.

History of Esophagogastric Junction Cancer Treatment and Current Surgical Management in Western Countries

  • The incidence of esophagogastric junction (EGJ) cancer has been significantly increasing in Western countries. Appropriate planning for surgical therapy requires a reliable classification of EGJ cancers with respect to their exact location. Clinically, the most accepted classification of EGJ cancers is "adenocarcinoma of the EGJ" (AEG or "Siewert"), which divides tumor center localization into AEG type I (distal esophagus), AEG type II ("true junction"), and AEG type III (subcardial stomach). Treatment strategies in western countries routinely employ perioperative chemotherapy or neoadjuvant chemoradiation for cases of locally advanced cancers. The standard surgical treatment strategies are esophagectomy for AEG type I and gastrectomy for AEG type III cancers. For "true junctional cancers," i.e., AEG type II, whether the extension of resection in the oral or aboral direction represents the most effective surgical therapy remains debatable. This article reviews the history of surgical EGJ cancer treatment and current surgical strategies from a Western perspective.

국소 진행된 Type II 분문부 선암의 절제연 확보를 위한 수술 방법: 좌측 대장 간치술 (Surgical Option for Sufficient Safety Margine in Locally Advanced Type II Cardia Cancer - Left Colon Interposition)

  • 목적: 분문부 위선암의 치료는 아직까지는 근치적 수술이 원칙이고 특히 식도 침윤을 갖는 type II 분문부 위선암은 충분한 절제연을 확보하기 위한 노력이 필요하다. 식도 침윤정도에 따라서 개흉과 개복을 통해 식도-위 절제를 하고 식도 대체술을 하는데, 그 중 저자들은 대장간치의 적응과 치료성적 등을 함께 알아 보고자 하였다. 대상 및 방법: 1994년 1월부터 2006년 12월까지 연세대학교 의과대학 외과학 교실에서 저자에게 위암으로 수술을 받은 1,087명의 환자 중 type II 분문부 위선암으로 좌 또는 우 흉복부 개복과 경복부 절개(경열공)를 이용해 근치적 식도-위절제술을 하고 식도 대체술로 좌측 대장을 간치한 환자 10예를 대상으로 같은 type으로 Roux-en-Y를 받은 환자들과 후향적으로 비교 분석 하였다. 결과: 남녀가 각각 9명, 1명 이었고 중간 연령은 52.5세(16~72세), 수술시간은 $449.00{\pm}87.39$ (SD)분이었고 평균 재원일수는 $20.60{\pm}6.73$ (SD)일이었다. 식도-위 동시성암은 1예가 있었고, 종양으로부터 근위부 절제연까지의 거리는 $6.56{\pm}3.65cm$였으며 종양의 크기는 $9.90{\pm}3.97cm$였는데 Roux-en-Y군의 값들과 차이를 보였다(sex and age matched analysis, P=0.019, P=0.046). 수술 사망은 없었고, 술 후 합병증은 2예가 있었는데 각각 문합부 협착과 폐기흉이었다. 사망은 총 4예가 있었고 수술 후 환자 대부분(83%)은 체중이 늘어 체중증가가 최대 16 kg까지 있었다. 결론: 식도 침윤을 하는 type II 분문부 위암에서 크기가 매우 큰 경우는 근치적 식도-위 절제술이 필요하다. 식도 대체 장기로는 좌측 대장을 이용 할 수 있는데 수술 시간이 길지만, 근치적 수술에 유리하고 식도-공장 문합술에 비해 수술 합병증이 많지는 않아, 재건술로 좌측 대장을 이용하는 방법도 가치가 있다고 생각한다.
