Impact of Adjuvant Chemoradiotherapy for Rectal Cancer on the Long-Term Quality of Life and Late Side Effects: A Multicentric Clinical Evaluation by the Turkish Oncology Group

  • Aim: Although preoperative chemoradiatherapy (CRT) has proven its benefits in terms of decreased toxicity, there is still a considerable amount of cases that do not receive postoperative CRT. Oncologists at different geographic locations still need to know the long-term effects of this treatment in order to manage patients successfully. The current paper reports on long-term quality of life (QOL) and late side effects after adjuvant CRT in rectal cancer patients from 5 centers in Anatolia. Methods: Rectal cancer patients treated with postoperative CRT with minimum 1-year follow-up and were in complete remission, were evaluated according to RTOG and LENT-SOMA scales. They were also asked to complete Turkish version of EORTC QLQ-C30 questionnaire and the CR-38 module. Each center participated with the required clinical data. Results: Two hundred and thirty patients with median age of 55 years participated and completed the study. Median follow-up time was 5 years. All patients received RT concomitant with chemotherapy. Common parameters that both increased functional health scales and yielded better symptom scores were long term interval after treatment and sphincter-saving surgery. In addition, surgery type and follow-up time were determined to be predictors of QOL scores and late toxicity grade. Conclusion: Postoperative CRT was found to have a great impact on the long term QOL and side effects in rectal cancer survivors. The factors that adversely affect these are abdominoperineal resection and shorter interval. The findings may encourage life-long follow-up and cooperation with patients, which should be mentioned during the initial counseling.

Clinical Study on Safety and Efficacy of Qinin® (Cantharidin Sodium) Injection Combined with Chemotherapy in Treating Patients with Gastric Cancer

  • Objectives: To assess the efficacy, side effects, and the impact on quality of life with $Qinin^{(R)}$ (Cantharidin sodium) injection combined with chemotherapy for gastric cancer patients. Method: A consecutive cohort of 70 patients were divided into two groups: experimental group with cantharidin sodium injection combined with chemotherapy, while the control group received chemotherapy alone. After more than two courses of treatment, efficacy, quality of life and side effects were evaluated. Results: The response rate of experimental group was not significantly different from that of the control group (P>0.05), but differences were significant in clinical benefit response and KPS score. In addition, gastrointestinal reactions and the incidence of leukopenia were lower than in the control group (P<0.05). Conclusions: $Qinin^{(R)}$ (Cantharidin sodium) injection combined with chemotherapy enhances clinical benefit response, improving quality of life of gastric cancer patients and reducing side effects of chemotherapy. Thus $Qinin^{(R)}$ (Cantharidin sodium) injection deserves to be further investigated in randomized control clinical trails.

Study on the Crack Control Effect of Moist Curing Equipment in Side Wall of Building (습윤양생 장치를 이용한 아파트 측벽 균열제어에 관한 연구)

  • In this study, moist curing equipment was used in the exist gang-form system. By achieving sufficient spray curing, the quality of the concrete was improved and the cracking occurred in building's side wall was decreased. The following results could be made as the conclusion. For the compressive strength, all zones showed the similar results. Comparing with the zone without using moist curing equipment, the zone used moist curing equipment showed higher rebound hardness results. For the cracking, the zone utilized moist curing equipment showed the cracking averaged as 6.6 m and the zone without using moist curing equipment showed the cracking averaged as 10.3m. The effectof reducing cracking by utilizing moist curing equipment is about 36 %. Using moist curing equipment is considered as a good solution to reduce the cracking in the structure. Considering all the factors analysed, using moist curing equipment improved the quality of the concrete and decreased the cracking. When this equipment was used in the construction site, it is expected that the construction periodcan be shrunk and the ratio of defect caused by drying shrinkage can be decreased. In this research conditions, The 0.3mm sized moist curing equipment provided the most desirable results on concrete quality and preventing cracking.

Convolutional auto-encoder based multiple description coding network

  • When data is transmitted over an unreliable channel, the error of the data packet may result in serious degradation. The multiple description coding (MDC) can solve this problem and save transmission costs. In this paper, we propose a deep multiple description coding network (MDCN) to realize efficient image compression. Firstly, our network framework is based on convolutional auto-encoder (CAE), which include multiple description encoder network (MDEN) and multiple description decoder network (MDDN). Secondly, in order to obtain high-quality reconstructed images at low bit rates, the encoding network and decoding network are integrated into an end-to-end compression framework. Thirdly, the multiple description decoder network includes side decoder network and central decoder network. When the decoder receives only one of the two multiple description code streams, side decoder network is used to obtain side reconstructed image of acceptable quality. When two descriptions are received, the high quality reconstructed image is obtained. In addition, instead of quantization with additive uniform noise, and SSIM loss and distance loss combine to train multiple description encoder networks to ensure that they can share structural information. Experimental results show that the proposed framework performs better than traditional multiple description coding methods.

Zero-Watermarking based on Chaotic Side Match Vector Quantization (무질저한 SMVQ 기반의 제로-워터마킹)

  • Digital watermarking is a technology for preventing illegal copying, for protecting intellectual property rights and copyrights, and for suggesting grounds of the ownership by inserting watermarks into digital contents. Generally speaking, watermarking techniques cannot escape from data distortion and quality degradation due to the watermark insertion. In order to overcome the shortcoming, zero-watermarking techniques which do not change the original data have been proposed recently. This paper proposes CSMVQ(Chaotic SMVQ), a zero-watermarking system for SMVQ(Side Match Vector Quantization) which shows better compression ratio and quality and less blocking effect than VQ(Vector Quantization). In SMVQ, compression progresses from left top to right bottom in order to use the information of the two neighbor blocks, so it is impossible to insert watermarks chaotically. In the process of encoding, CSMVQ dynamically considers the information of the (1 to 4) neighbor blocks already encoded. Therefore, watermark can be inserted into digital contents in chaotic way. Experimental results show that the image quality compressed by CSMVQ is better than that of SMVQ and the inserted watermark is robust against some common attacks.

Clinical Safety and Efficacy of Kanglaite® (Coix Seed Oil) Injection Combined with Chemotherapy in Treating Patients with Gastric Cancer

  • Objective: To observe efficacy and side effects, as well as the impact on quality of life, of Kanglaite$^{(R)}$ (Coix Seed Oil) injections combined with chemotherapy in the treatment of advanced gastric cancer patients. Method: A consecutive cohort of 60 patients were divided into two groups: the experimental group receiving Kanglaite$^{(R)}$ Injection combined with chemotherapy and the control group with chemotherapy alone. After more than two courses of treatment, efficacy, quality of life and side effects were evaluated. Results: The response rate and KPS score of experimental group were significantly improved as compared with those of the control group (P<0.05). In addition, gastrointestinal reactions and bone marrow suppression were significantly lower than in the control group (P<0.05). Conclusions: Kanglaite$^{(R)}$ Injection enhanced efficacy and reduced the side effects of chemotherapy, improving quality of life of gastric cancer patients; use of Kanglaite$^{(R)}$ injections deserves to be further investigated in randomized control clinical trails.

  • Rehabilitation dog-walking can be used to improve physical activity and restore QOL in stroke patients. This study investigated the effect of rehabilitation dog-walking on the QOL and physical activity of stroke patients. Twenty-four stroke patients were recruited for this study. Twenty-four stroke patients were randomized 1:1 into two groups. The evaluator measured stroke specific-quality of life (SS-QOL), six minute walk test, and maximal grip strength on the affected and non-affected side. The experimental group received gait training with a rehabilitation dog for 30 minutes once a week for 8 weeks. As a result of the study, there was a significant increase in SS-QOL, six minute walk test, and maximal grip strength on the affected side in the experimental group after the intervention. In addition, the experimental group showed significant improvement in SS-QOL, six minute walk test, and maximal grip strength on the affected side than the control group. Rehabilitation dog-walking may be an intervention method that can improve QOL and physical activity after stroke.

  • In terms of company management for market segmentation and creating new market opportunity, Blog service has much important meanings. This study was designed to investigate the casual relationship among service quality, service value, customer satisfaction, and customer loyalty in the given blog service site "Cyworld". Through the empirical results, specific factors of blog service quality were discovered to be amusement, interaction, customization, reliability and convenience. Also rte confirmed that service value intervening between blog service quality and customer satisfaction plays an important role. This paper presents much implications both theoretical and practical side.

Survey on the Status of Utilization of mediate Moxibustion by Korean Medicine Doctor (한의사의 간접구 이용실태에 대한 조사 연구)

  • Objectives : We examined the use of the moxibustion through survey on Korean Medicine Doctors (KMDs) on preparation of universal moxa product standard by fully reflecting opinions of practitioners and the industry. Methods : The questionnaire is composed of 25 questions including use of indirect moxa, side effects, complaints and improvements, general characteristics and so on, and were responded by 1,588 KMDs. For statistics, frequency analysis, chi-square analysis, T-test and ANOVA were conducted using SPSS 12.0 for windows, and the significance level was 0.05. Results : In this research, 91% of the responded KMDs was using moxibustion, and the proportion of using mediate moxibustion was shown as 63.2%. They used mediate moxibustion for side effects of direct moxa method such as risk of burn, ease of use, control of stimulation and so on. And it was mostly being conducted to women and 40 ages. Most respondents gave moxibustion treatments for the purpose of thermal stimulation on acupoint to musculoskeletal diseases. Most respondents were treating 1~2 pieces of moxibustion to patients by moxibustion points to those of 1~2 regions every time within 10 minutes. Regarding the frequency of side effects of mediate moxibustion, within 3% of respondents had experienced side effects, and the types were burn, flare and itchiness, odor and smoke. The dissatisfaction of mediate moxibustion products was moderate(2.00 out of 5 point scale), and went up public health doctors, the younger ages and the shorter clinical experience. Complaints on mediate moxibustion products were serious odor, poor attachment, risk of burn etc. Regarding ways to improve mediate moxibustion, the expansion of moxibustion's insurance cost carne out highest, followed by preparation of manual on efficacy, strengthening of training, acquiring evidence on efficacy, diversification of specifications, safety improvement, quality improvement of mediate moxibustion products. Conclusions: Many KMDs use for ease of use and give moxibustion treatments for thermal stimulation but they complain about safety, efficacy and quality of mediate moxibustion. After this it is considered that we have to elevate therapeutic effect through the improvement of mediate moxibustion products.

A Survey on Changes in Quality of Life of Patients who Received Radiation Therapy after Surgery for Breast Cancer (유방암 수술후 방사선 치료를 받는 환자의 삶의 질 변화에 관한 연구)

  • This study was done to investigate the changes in the quality of life(QOL) of patients who received radiation therapy after surgery for breast cancer. For this purpose, 35 patients were interviewed using a questionnaire. They were followed over 8 month period from the beginning of radiation therapy. Changes in the QOL, side effect of the RT(radiation therapy), depression were measured. The results can be summarized as follows : 1. The mean score of the side effect of RT increased significantly over the period of RT. When the RT was completed, the QOL score was the low-est, and the depression score was the highest, but the changes of these scores were not statistically significant. 2. When the 6-week RT was finished, the QOL was significantly correlated with QOL of the beginning of RT, depression at RT completion and age. At three months after the completion of RT, the QOL was significantly correlated with QOL of the beginning of RT, depression and side effect of RT at three months after the completion of RT. At six months after the completion of RT, the QOL was significantly negatively correlated with age. 3. In the stepwise multiple regression analysis, the major influencing variables on the QOL were various according to the time lapse after the RT. The QOL at the completion of RT was predicted by the QOL at the beginning of RT, depression at the completion of RT and age. The QOL at three months after the completion of RT was predicted by the QOL at the beginning of RT and side effect of RT and depression at three months after the completion of RT. The QOL at six months after the completion of RT was predicted by age. This study suggests that continuous nursing care be required for improving the QOL of breast cancer patients not only during the RT period but also for a long term after the completion of RT.
