정적기립자세와 치료적 클라이밍 기립자세 시 체간근 근활성도의 비교 (Comparison of Trunk Muscle Activity during Static Standing Position and Standing Position on Therapeutic Climbing Wall)

  • Purpose: The purpose of the study was to comparison of trunk muscle activity during static standing position and standing position on therapeutic climbing wall of adult. Methods: Study subject is arbitrarily classified into 10 of experimental group and 10 control group among 20 of adult. Trunk activity measured as rectus abdominalis, external oblique, internal oblique, erector spinae. Control group maintains that center of gravity of trunk pass the front of shoulder, pelvis, knee and ankle on stable surface with putting legs apart more than shoulder width. Experimental group had static exercise on 4 by 3 meter, 90 degree of Therapeutic climbing wall. Starting position is that putting arms and legs apart more than shoulder width. In order to compare the effect of it between the groups, independent t-test was used. Results: According to the test result, significant difference between among rectus abdominalis, erector spinae the experimental groups. And external oblique, internal oblique muscle atvity is no significant difference experimental groups between among the control groups was observed. Conclusion: Trunk muscle activation is activated to standing position on the Therapeutic Climbing Wall more than static standing position.

The Effect of Keyboard Height on the Muscle Activity of the Upper Trapezius Muscle

  • This study is designed to reduce worker fatigue, improve efficiency and provide a functional working environment based on previous studies that pain occurs in the shoulder area, especially the upper trapezius muscle, when the keyboard height is not appropriate. In this study, the height of the keyboard is four, the height of the elbow and desk is the same height, the height of the desk is 3cm lower than the elbow, the height of the desk is 6cm high, and the height is 9cm high. When working on the keyboard, the wrist and forerunner were organized into four groups of 10 people so that the height was different for each group. When the height of the keyboard is given in various ways compared to the height of the elbow of the subject, it is verified whether there is a difference in the RMS (Root Mean Square) of the upper trapezius muscle. The results of this study showed that the muscle activity of the upper trapezius muscle cap was significant only in the left and right keyboard height -4cm, 0cm, +4cm, +8cm group, but the difference in muscle activity was not significant in the rest group. The first study will require a study of the control of the factors affecting the tension of the subjects, the measurement of muscle activity against various muscles, and whether the length of the shoulder and fingertips of the subject affect muscle activity according to the keyboard type.

작은가슴근의 단축이 등세모근과 큰가슴근의 근 활성도에 미치는 영향 (Effects of Shortening of Pectoralis Minor Muscle on Muscle Activity of Trapezius and Pectoralis Major Muscles)

  • PURPOSE : The purpose of this study was to determine the effects of the length of the pectoralis minor on muscle activity of trapezius and pectoralis major in subjects in subjects with shortened pectoralis minor muscle. METHOD : The subjects was participated in 36 with shortened pectoralis minor muscle. All subjects was examined the length test of pectoralis minor muscle. we divided by 3 groups. group I(n=12) was for 4~5cm of length of pectoralis minor muscle, group II(n=12) was for 5~6cm, group III(n=12) was for above 6cm. The EMG activity of upper trapezius, middle trapezius, lower trapezius and pectoralis major muscle activity was measured by surface EMG while elevationg the right arm in sitting postion with head to the neutral, shoulder elevation $135^{\circ}$ with scaption. Data were analyed using one-way ANOVA with a Tukey post hoc test. RESULT : The EMG activity differed significantly among the three groups(p<.05). The group III had significantly greater EMG activity of upper trapezius and pectoralis major muscles than group I and II(p<.05). Also, The group III had significantly smaller EMG activity of lower trapezius muscle than group I and II(p<.05). But, these was no significant difference in the EMG activity of the middle trapezius muscle among the groups (p.05). CONCLUSION : Therefore, the result of this study should be suggested that the shortened pectoralis minor muscle was affected the EMG activity of the upper trapezius, lower trapezius and pectoralis major. Ultimately the length of the pectoralis minor muscle leads to the muscle imbalance in shoulder girdle.

근전도 생체되먹임 훈련이 봉우리밑 충돌증후군 환자의 등세모근 근활성도에 미치는 즉각적인 효과 (The Immediate Effects of Electromyographic Biodfeedback Training on Muscle Activity of Trapezius in Patients with Subacromial Impingement Syndrome)

  • Purpose: To investigate the effects of electromyography (EMG) biofeedback on the muscle activity of the trapezius, shoulder pain, function, and range of motion (ROM) in patients with subacromial impingement syndrome (SAIS). Methods: Sixteen patients (9 males and 7 females) with SAIS participated voluntarily. The main outcome measures were muscle activity of the trapezius, shoulder pain (VAS), ROM, and the shoulder pain and disability index (SPADI). Exercises with EMG biofeedback consisted of shoulder flexion in a standing position, shoulder external rotation in a side-lying position, and shoulder horizontal abduction in a prone position. Post measurements were taken immediately after EMG biofeedback training. Results: Middle and lower trapezius activity, as well as ROM, was significantly increased by exercise with EMG biofeedback (p<0.05). In addition, VAS and SPADI scores significantly decreased post-EMG biofeedback training (p<0.05). Conclusion: EMG biofeedback intervention is an effective exercise for SAIS patients to restore activity of the middle and lower trapezius and to improve pain, shoulder function, and ROM.

Comparison of Muscle Activity Ratio of Upper Trapezius to Serratus Anterior During Shoulder Elevation Between Subjects With and Without Pain Experienced in Upper Trapezius

  • The aim of this study was to compare the activity of the upper trapezius (UT) and serratus anterior (SA) and ratio of UT to SA during shoulder elevations. Ten subjects with UT pain (UTP) and 13 subjects without UTP participated in this study. Subjects with a UTP of over five in a pain intensity visual analogue scale (0-10 ㎝) for more than 2 months and latent myofascial trigger points (MTrPs) in the UT muscle were included in the UTP group. Electromyography (EMG) data of UT and SA at 1st and 10th elevations were analyzed. Two-way repeated analyses of variance were used to compare the EMG activity of UT and SA and the ratio of UT to SA during shoulder elevations between groups with and without UTP. There was a significant increase in UT/SA ratio in the group with UTP compared to the group without UTP (p=.01). The activity of UT and SA measured at the 10th elevation was significantly greater than that in the first elevation (p<.05). The activity of SA was significantly greater in the group without UTP than the group with UTP (p=.03). However, there was no significant difference between groups with and without UTP in terms of UT activity (p=.28). These results indicate that UTP may have relevance to the increased muscle activity ratio of UT to SA during shoulder elevations.

다양한 자세의 푸시업 플러스 융합 운동에 따른 어깨 안정근의 근활성도 비교 (Comparison of Shoulder Stabilizer Muscle Activity on Push-up plus in Convergence in Various Posture)

  • 본 연구의 목적은 4가지 자세의 푸시업 플러스(PUP) 운동 시 5번째와 7번째의 앞톱니근(SA), 위등세모근(UT), 중간등세모근(MT), 아래등세모근(LT), 큰가슴근(PM)의 근활성도를 알아보는 것이다. 25명의 대상자가 다양한 자세의 PUP 융합 운동 (일반자세, $90^{\circ}$, $120^{\circ}$, BOSU)을 실시하였다. 각 근육의 근활성도를 비교하기 위하여 반복 측정된 분산분석을 적용하였다. 다양한 PUP 융합 운동동안 SA7, PM, UT, MT에서 유의한 차이가 있었다(p <.05). SA7은 PUP 와 $90^{\circ}PUP$, BOSUPUP에서 각각 유의한 차이가 나타났다(p <.05). PM은 $90^{\circ}PUP$와 PUP, BOSUPUP에서 각각 유의한 차이가 나타났다(p <.05). UT은 $90^{\circ}PUP$와 PUP, BOSUPUP에서 각각 유의한 차이가 나타났다 (p <.05). MT는 $90^{\circ}PUP$와 PUP, $120^{\circ}PUP$, 그리고 PUP와 $120^{\circ}PUP$에서 유의한 차이가 있었다(p <.05). 본 연구 결과, 어깨의 기능부전이 없는 대상자가 어깨 안정근을 훈련할 때 일반적인 PUP가 효과적일 것이다.

Comparison of Serratus Anterior Muscle Activity between Serratus Anterior Strengthening Exercises and Scapular Upward Rotation Exercise

  • Background: The serratus anterior (SA) muscle is one of the important muscles in the upward rotation of the scapula when the arm is raised. Insufficient muscle activity of the SA can cause deformation of the shoulder rhythm resulting in shoulder pathology. Objects: This study intends to compare SA and upper trapezius (UT) activity during the conventional wall-slide and push-up plus exercises for SA muscle strengthening and the scapular upward rotation (SUR) exercise. Methods: A total of 30 subjects participated in this study, and we measured the muscle activity of the SA and UT muscles during the wall-slide, push-up plus and SUR exercises. The one-way repeated ANOVA was used to compare SA and UT muscle activities during the 3 exercises. Results: During the SUR exercise, SA muscle activity was 79.88% maximum voluntary isometric contraction (MVIC), which was significantly higher than its activity during the other 2 exercises. The UT muscle activity was 47.53 %MVIC during the SUR exercise, indicating a significantly higher UT muscle activity than during the other 2 exercises. Conclusion: These findings suggest that the SUR exercise can maximize SA muscle activity to strengthen the SA while keeping UT muscle activity at an appropriate level.

파악력의 강도가 두 가지 팔 자세에서 어깨근육 근활성도에 미치는 영향 (The Effects of Hand Grip Force on Shoulder Muscle Activity in Two Arm Posture)

  • 본 연구는 파악력의 강도가 두 가지 팔 자세에서 어깨근육 근활성도에 어떠한 영향을 미치는지 알아보기 위하여 실시되었다. 건강한 성인 22명(남 11명, 여 11명)을 대상으로 실시하였고, 4가지 파악력(최대 파악력의 0%, 30%, 50%, 70%)을 팔의 중립 자세와 팔 $90^{\circ}$ 거상 자세에서 적용 했을 때 상승모근, 하승모근, 극하근, 전삼각근, 후삼각근의 근활성도를 표면 근전도를 이용하여 측정하였다. 팔 중립 자세에서는 파악력의 강도에 따라 극하근, 전삼각근, 후삼각근이 유의한 차이를 보였고(p<0.05), 팔 $90^{\circ}$ 거상 자세에서는 전삼각근, 후삼각근이 유의한 차이를 보였다(p<0.05). 본 연구의 결과, 2가지 팔의 자세에서 각각 파악력의 강도에 따라 어깨근육 근활성도가 변화됨을 확인하였다. 손 쥐기 과제를 중립자세에서 실시할 경우 어깨관절 안정근인 극하근이 활성화되고, 팔 $90^{\circ}$ 거상 자세에서는 삼각근을 억제된다는 점을 고려하여, 손을 사용하는 작업 시 인간공학적 접근으로 어깨 손상의 예방과 손상 후 재활 훈련 시 효율적인 전략으로 사용할 수 있을 것으로 여겨진다.

A Comparison of Shoulder Muscle Activities on Sitting Posture and Shoulder Angle

  • Background: Sitting posture influences movements of scapulothoracic and glenohumeral joints and changes the shoulder muscle activities. The development and maintenance of correct sitting posture is important for the fundamental treatment of shoulder pain during rehabilitation. Objects: The purpose of this study was to investigate the effects of the sitting postures and the shoulder movements on shoulder muscle activities for both male and female. Methods: Twenty-eight subjects without shoulder-related diseases participated in this experiment. The subjects had randomly adopted three different sitting postures (upright posture, preferred posture, maximum slouched posture) and shoulder flexion angles in scapular plane ($30^{\circ}$, $90^{\circ}$, $120^{\circ}$). Surface electrodes were collected from upper trapezius (UT), anterior deltoid (AD), and posterior deltoid (PD) and the active shoulder range of motion was measured in each sitting posture and shoulder flexion angle. Results: The active range of motions of the shoulder external rotation and the flexion in the scapular plane decreased from the upright posture to the maximum slouched posture (p<.05, mixed-effect linear regression with random intercept, Tukey post-hoc analysis). All muscles showed the highest EMG activities at $120^{\circ}$ shoulder flexion with the maximum slouched posture and did not show the gender differences. Conclusion: Increased shoulder muscle activities may become the potential risk factor for the shoulder impairment and pain if people continuously maintain the maximum slouched posture. Therefore, an upright position is necessary during shoulder exercises, as well as in activities of daily living, including motions involving lifting the arms.

The Effects of Therapeutic Climbing on Shoulder Muscle Activity according to the Inclination of the Climbing Wall

  • Purpose: Therapeutic climbing has become very popular today, with it being reported as a new method for preventing and treating orthopedic trauma to the shoulder joint. However, objective studies on its effects on the musculoskeletal system are still lacking. The objective of the present study was to investigate the effects of wall inclination during therapeutic climbing on the muscle activity around the shoulder joint. Methods: In this study, the participants performed movements at three different inclination angles of $0^{\circ}$, $+15^{\circ}$, and $-15^{\circ}$. sEMG was performed to measure the activities of five different muscles around the shoulder joint (biceps brachii, serratus anterior, upper trapezius, middle trapezius, and lower trapezius muscles). Results: Biceps brachii muscle showed a significant increase at $-15^{\circ}$, as compared to $0^{\circ}$ (p<0.01), and the serratus anterior also showed a significant increase at $-15^{\circ}$, as compared to $0^{\circ}$ (p<0.05). Moreover, the middle and lower trapezius muscles also showed a significant increase at $-15^{\circ}$, as compared to $0^{\circ}$ (p<0.001). Compared to $0^{\circ}$, all muscles showed decreased values at $15^{\circ}$, but the differences were not statistically significant (p>0.05). Conclusion: Therapeutic climbing may be a new therapeutic approach that can increase muscle strength and coordination in the sensory nervous system, since it can be used as a tool that promotes active movement by altering wall inclination and causing the user to generate movements according to the existing situation.
