Purpose: The purpose of this study was to investigate whether ipsilateral shoulder pain affects the sensorimotor function of the same side shoulder in patients with stroke. Methods: Thirty stroke patients, who were divided into the ipsilateral shoulder pain group (n=15) and the ipsilateral shoulder non-pain group (n=15). Subjects were evaluated on performance of a tracking task, joint reposition test and 9-Hole pegboard test for sensorimotor functions, and Fugl-Meyer test and Motricity Index for functional ability of the contralateral side. Results: In comparison of the two groups, significant differences in performance on functional ability, including the Fugl-Meyer test(both upper and lower limb) and Motricity Index(only lower limb) were observed (p<0.05). With regard to sensorimotor functions, the ipsilateral shoulder pain group were observed significantly poor scores on the Accuracy Index, joint reposition score and 9-Hole pegboard test, when compared with the ipsilateral shoulder non-pain group (p<0.05). Conclusion: We found that ipsilateral shoulder pain could impede accurate performance of a movement and result in deteriorated proprioception of the ipsilateral shoulder. Therefore, careful evaluation and appropriate therapeutic intervention are essential for stroke patients who suffer from ipsilateral shoulder pain.
The purpose of this study was to evaluate the relationship between shoulder pain and hand edema in stroke patient with shoulder hand syndrome. In this study, 26 hemiplegic patients with the clinical symptom and sign of shoulder hand syndrome were evaluated. Hand volume was measured by hand volumeter, and hand edema was calculated by volume difference of both hands. Shoulder pain was evaluated using VAS (visual analog scale). and spasticity of shoulder was graded by modified Ashworth scale. The relationship among three factors such as shoulder pain. hand edema and shoulder spasticity was evaluated using correlation analysis. Results through correlation analysis among three factors are as follows : 1. Correlation between shoulder pain and hand edema was not significant$(\gamma=-.028)$. 2. Correlation between shoulder spasticity and hand edema was not significant $(\gamma=-.027)$. 3. Correlation between shoulder spasticity and shoulder pain was not significant $(\gamma=-.093)$. As the result of correlation analysis. this study shows that there was no statistically significant difference between shoulder pain and hand edema.
Objectives : This study was to examine the effect of Intradermal Acupuncture therapy at Kyeonjeong, Cheonryo and Kokji on the shoulder pain. The research was conducted with non equivalent control group non-synchronized design. Methods : IThe subjects were the women who was suffering for shoulder pain, who are employed at two hospitals in Busan, from April to June, 2006. The 24 women were experimental group who were applied to Intradermal Acupuncture Therapy at Kyeonjeong, Cheonryo and Kokji, while the 20 women were control group who weren't applied to any treatments. The severity of shoulder pain and the extent of the subjective symptoms of shoulder pain were pre-measured. The Intradermal Acupuncture therapy was done a total of 4 times, 2 times a week over 2 weeks and from 11 a.m. to 2 p.m. as an experimental treatment. As a post survey, the severity of shoulder pain, the frequency of shoulder pain and the extent of the subjective symptoms of shoulder pain were measured after one week and two weeks of experimental treatment. Research tools are Visual Analogue Scale by Cline et al. for measuring the severity of shoulder pain and the tool developed by Japan's industrial fatigue research committee of the industrial hygienics society and modified. Results : There was a significant difference(F=4.132, p=0.019) for the severity of shoulder pain on the interaction between time and group. There was a significant difference(F=11.193, p=0.000) for the extent of the subjective symptoms of shoulder pain on the interaction between time and. group. Conclusion : Based on the above results, it can be determined that Intradermal Acupuncture Therapy at Kyeonjeong, Cheonryo and Kokji can be used as the effective treatments for reducing of shoulder pam.
The shoulder is a complex area which makes it vulnerable to painful pathologic processes. Chronic shoulder pain has become more common recently due to increased use of computers and a ,generally more sedentary life style among most people . Trigger point injection and neural blockade are useful for the management of chronic shoulder pain which has not improved with conservative treatment. Published articles concerning trigger point injection or neural blockade for chronic shoulder pain were reviewed to evaluate promising methods. If we are careful to remain aware of the details and complications in addition to adhering to effective treatments, these should be good armamentarium for doctors enthusiastic about the management of chronic shoulder pain.

Background: Patients with rotator cuff tears are usually afflicted with shoulder pain and disability. However, it is unclear which factors are related to shoulder pain in patients with rotator cuff tears. This study was therefore undertaken to determine the factors correlated with shoulder pain in patients with painful rotator cuff tears, but without any history of trauma. Methods: We evaluated a cohort of 745 patients with painful rotator cuff tears having no trauma history, and analyzed the relationship between pain and multiple factors including demographic data, tear characteristics, and passive range of motion. Pain was analyzed with a questionnaire concerning the visual analogue scale (VAS) for pain. Tear characteristics were determined by evaluating tear size, muscle atrophy, number of torn tendons, and presence of arthritis. Multivariate linear regression analysis and chi-squared test were applied to evaluate the relationship between the VAS for pain and variable factors. Results: Shoulder pain was associated with young age (p=0.01), male sex (p=0.01) and the presence of diabetes mellitus (p<0.001). Measurements of rotator cuff tear characteristics including tear size (p=0.53), muscle atrophy (p=0.16) and the number of torn tendons (p=0.34) did not correlate with shoulder pain. Symptom duration (p=0.60) and range of motion (p>0.05) also showed no correlation with VAS for pain. Conclusions: Young age, male sex and the presence of diabetes mellitus correlated positively with preoperative shoulder pain in patients with painful rotator cuff tears without a trauma history. Combined treatment of pain management and risk factor correction could be helpful to control preoperative shoulder pain.
Objectives : The purpose of this study is to investigate the clinical effects of low level laser therapy on shoulder pain. Methods : From October 1, 2009 to November 14, 2009, we applied 15 participants who consented to an assignment had a shoulder pain to low level laser therapy(LLLT). We treated LLLT 8 times on both upper trapezius, levator scapular, pectoralis, suboccipital, rhomboid, rotator cuff tender point for 2 weeks. To evaluate the efficiency of low level laser therapy, we used questionnaire of shoulder pain symptom, questionnaire of a day living discomfort scale, shoulder pain and disability index(SPADI) and pressure pain threshold(PPT) at before treatment and after treatment 4 times and 8 times. Results : There was a significant difference in questionnaire of shoulder pain symptom, questionnaire of a day living discomfort scale, SPADI and PPT after 8th treatment. Conclusions : Low level laser therapy for shoulder pain is effective to improve patients' symptoms measured with questionnaire of pain and a day living discomfort score, SPADI and PPT significantly.
Background: This study was conducted to examine the correlation of VDT, posture and shoulder function among each group divided by according to the neck pain disorder index (NDI) scores for female patients with neck pain. Design: Cross-sectional study. Methods: Fifty adult women with neck pain voluntarily participated in this study and the neck pain disorder index questionnaire, VDT syndrome assessment tool questionnaire, craniovertebral angle, thoracic kyphosis angle, round shoulder posture, pectoralis minor length, shoulder joint hypermobility, and serratus anterior strength tests were conducted respectively. Subjects were divided into two groups where 21 subjects were allocated to the mild pain group whom have rated below 14 points on the NDI scores, and 29 subjects were in the severe pain group, whom have rated above 15 on the NDI score. Results: The study found that in the mean difference between variables in each group, VDT syndrome showed a higher mean score in the severe pain group than the mild pain group (p<0.05). In the group correlation and regression analysis, the mild pain group showed a significant negative correlation between the craniovertebral angle and round shoulder posture (r=-0.467, p<0.05), and the round shoulder posture for craniovertebral angle was shown to have significant positive influence (B=10.162, p<0.05). The severe pain group showed that the NDI and the VDT syndrome had a significant amount of correlation (r=0.520, p<0.01), the VDT syndrome showed significant positive influence (B=0.330, p<0.05), and the craniovertebral angle showed significant negative influence (B=-0.809, p<0.05). It was also shown that shoulder joint hypermobility had a significant negative correlation with the serratus anterior strength (r=-0.437, p<0.01), and that serratus anterior strength had a significant negative influence on shoulder joint hypermobility (B=-4.175, p<0.05). Conclusion: This study is of clinical significance in that it presented variables that should be considered depending on the degree of neck pain in treating patients with neck pain and that it presented patients with not only posture but also the function of the shoulder joint as factors to consider.
Objective : This report is to introduce the clinical application of Whidam's Su-Gi therapy to shoulder pain. Methods : Whidam's Su-Gi therapy for shoulder pain first identifies for red flags in order to select a person to be treated in korean medicine. After performing shoulder joint mobility evaluation and Su-Gi treatment, determine the disadvantage, and determine the mobility evaluation and Su-Gi treatment route of the shoulder, the shoulder blade, and cervical vertebrae. The advantage and pain are evaluated by a manual exercise test. After performing Su-Gi therapy according to the evaluation at each stage, pain and dysfunction are reevaluated to check the negative and proceed to the next step. Conclusions : The clinical application of Whidam's Su-Gi therapy to shoulder pain was organized around the correlation between the shoulder joint, the shoulder, the shoulder blade, and cervical vertebrae, and a medical flow chart was constructed away from the listed description method.

The purpose of this study was to examine the influences of chronic shoulder pain on the muscle tone in trunk muscles. The study's subjects were 40 men and women in their 30 to 50s, which were divided into two groups. A chronic shoulder pain group consisted of 20 subjects who had been diagnosed with chronic shoulder pain by doctors, and a painless group consisted of 20 subjects who had experienced no such pain. An analysis was performed using electromyography on the muscle tone in the rectus abdominalis, external oblique, internal oblique, and erector spinae muscles under the same conditions between the two groups. The analysis results were as follows. The chronic shoulder pain group exhibited an overall high level of trunk muscle tone than the painless group, along with a statistically significant difference in the rectus abdominalis(p<.05). Moreover, the chronic shoulder pain group showed differences in the trunk muscle tone depending on the affected side. The chronic left shoulder pain group yielded higher levels of muscle tone in the right-side trunk muscles. In particular, the group revealed statistically significant differences in the rectus abdominalis and internal oblique(p<.05). The chronic right shoulder pain group exhibited higher levels of muscle tone in the left-side trunk muscles with a statistically significant difference in the internal oblique(p<.05). The above results suggested that chronic shoulder pain influences increases in the muscle tone in the trunk muscles on the opposite side to the affected shoulder.
Purpose: The purpose of this study was to examine the effects of self stretching on shoulder pain and should flexibility of nurses. Methods: Forty two nurses who work in a university hospital participated in the study; an intervention (n=22) and a control group (n=20). The nurses of intervention group carried out a 4-week self stretching program. All participants completed 1 item NRS scale for pain intensity and 1 item 4-Likert scale for pain intensity. Both shoulder flexibilities of the nurses were measured by back and reach test. All measurements were done at baseline, 2 week, 4 week during the program and 2 week following the program. The data was analyzed by frequency, percentage, ${\chi}^2$-test, t test, Repeated measures ANOVA using SPSS 12.0. Results: There were significant differences in the pain intensity (p<.001) and frequencies(p<.001) between two groups. Also, Left and right shoulder flexibilities of the intervention group significantly increased than those of the control group (p<.001 and p=.002 respectively). Conclusion: The results showed that self stretching reduce the intensity and frequency of shoulder pain and increase both shoulder flexibilities. Thus, self stretching is recommended as an intervention to improve shoulder function of hospital nurses.
