Result of the Early Exercise and Rehabilitation after Limited Posterior Operative Treatment of the Calcaneal Fractures (종골 골절의 제한적 후방 접근법 수술적 치료 후 조기 운동 및 재활 치료의 결과)

  • Purpose: To evaluate the result of the early exercise and rehabilitation after limited posterior operative treatment of the calcaneal fractures. Materials and Methods: Between May 2005 and December 2007, 43 cases with intraarticular calcaneal fractures were treated by open reduction with K-wires and Steinmann pins. Mean age and mean follow-up period were 42.5 years old and 15 months. All patients were treated with the same postoperative protocol, the early exercise of subtalar joint without short leg splint and cast was began after postoperative 1 day, non-weight bearing crutch ambulation did after 2 weeks, partial-weight bearing crutch ambulation did after 6 weeks, without crutch ambulation did after 10 weeks. The fractures was classified by Essex-Lopresti and Sanders classification using radiographs and the functional outcomes were evaluated by circle draw exercise, AOFAS score. Results: By Essex-Lopresti classification, the tongue type was 6 cases (14%), the joint depression type was 15 cases (35%), the combined type was 22 cases (51%). By Sanders classification, type II were 10 cases (23%), type III were 15 cases (58%), and type IV were 8 cases (19%). By AOFAS functional evaluation, the mean preoperative scores were 70.3 and 70.5 respectively, mean postoperative scores were 83.7 and 86.9 respectively. There were satisfactory results in 40 cases but 3 cases were unsatisfactory including on case of lateral impingement and 2 cases of traumatic arthritis of subtalar joint. Conclusion: We obtained satisfactory result of the early exercise of subtalar joint without short leg splint and cast and rehabilitation after limited posterior operative treatment of the calcaneal fractures.

A Study of the Effects of Casting on Lower Limbs -Comparison of Casted and Noncasted Limb- (하지석고붕대 적용에 의한 활동저하가 석고붕대 적용하지와 비적용하지의 둘레, 피부두겹두께 및 근력에 미치는 효과)

  • The purpose of this study was to compare the circumference, skinfold thickness and strength of the normal and casted lower limb prior to casting and following removal of the cast. The subjects for the study were nine orthopedic patients who had had long and short leg casts due to a tibial, fibula, or calcaneus fracture, or to a lateral collateral ligament rupture. Circumference, skinfold thickness, and strength of the normal and casted lower limb following removal of the cast were compared with those prior to the application of the cast. Measurements were made before cast application and after removal of the cast. Skinfold thickness was measured by fat caliper, circumference was measured by tape and strength was determined by measuring the length of time the leg was held elevated at 45$^{\circ}$. The results can be summarized as follows 1. There was no change in the normal limb in the circumference of the midthigh and midcalf after casting as compared to before cast application. 2. In the casted limb the circumference of the midthigh decreased by 3.23% and that of the midcalf decreased significantly by 7.49% during the period of casting. 3. In the normal limb skinfold thickness of the quadriceps decreased and that of gastrocnemius increased by 20.63% during the period of cast application. 4. In the casted limb skinfold thickness of the quadriceps decreased significantly by 12.37% and that of gastrocnemius decreased by 10% during the period of cast application. 5. Strength of the normal lower extremity decreased significantly by 48.37% and that of casted lower extremity decreased remarkably by 73.07% during the period of cast application. 6. Circumference of the midthigh and the midcalf decreased significantly by 7.6% and 9.4% respectively on the casted side as compared to the normal side. Skinfold thickness of the quadriceps and the gastrocnemius on the casted side decreased by 6.12% and 18.55% respectively as compared to the normal side and strength in the lower extremity on the casted side decreased significantly by 44.32% as compared to the nor-mal side. From these results, it may be concluded that muscle atrophy occurs in the casted lower limb and muscle strength of the normal lower limb are also reduced during the period of application of a leg cast.

The Study on Effectiveness of Oriental Medicine Treatment for Lumbar Disc Herniation Inpatients on 208 Cases (요추 추간판 탈출증 입원환자 208례를 통한 한방치료의 효과 연구)

  • This study was introduced the short-term effectiveness of Oriental medicine treatment for lumbar disc herniation inpatients. We selected 208 inpatients with a magnetic resonance imaging(MRI)-confirmed lumbar disc herniation since June 2012 until August 2012. Their demographics characterisitics, numeric rating scale(NRS) and Oswestry back-related disability index(ODI) for low or leg pain, lumbar flexion and extension angle were assessed at baseline and when discharged from the hospital. When it measured the straight leg raisingscale(SLR), the low side of the angle was examined. Treatment every patient has received as follow; herbal medicine, bee venom, acupuncture and physical examination, spinal manipulation. NRS of low back pain and leg pain and ODI is $5.6{\pm}2.3$, $4.9{\pm}2.8$ and $45.5{\pm}20.0$, respectively at baseline and $2.8{\pm}1.8$, $2.7{\pm}1.9$ and $27.2{\pm}14.1$, respectively when discharged from the hospital. range of motion(ROM) of lumbar flexion and extension angle and SLR is $70.0{\pm}27.1$, $15.4{\pm}7.2$ and $61.3{\pm}23.0$ respectively at baseline and $80.5{\pm}16.9$, $18.25{\pm}4.1$ and $73.2{\pm}14.0$, respectively when discharged from the hospital. It shows that statistically significant improved(P<0.001). Furthermore, regarding patient satisfaction with the treatment, excellent(33.7%), good(55.4%), normal(10.4%), poor(0.5%), it suggests that most patients(89%) satisfied with the treatment.

  • Purpose: Total hip arthroplasty was performed using a direct anterior approach (DAA) on an ordinary operation table and a short femoral stem. The clinical radiographic results were evaluated by a comparison with those performed using the modified hardinge (anterolateral approach, ALA) method. Materials and Methods: From January 2013 to November 2015, 102 patients who underwent total hip arthroplasty using DAA (DAA group) and the same number of patients using ALA (ALA group), both performed by a single surgeon, were compared and analyzed retrospectively. The operation time and amounts of bleeding were compared, and the improvement in post-operative pain, ambulatory capacity and functional recovery of the hip joint were checked. The location of insertion of the acetabular cup and femoral stem were evaluated radiologically, and the complications that occurred in the two groups were investigated. Results: The amount of bleeding was significantly smaller in the DAA group (p=0.018). Up to 3 weeks postoperatively, recovery of hip muscle strength was significantly higher in the DAA group (flexion/extension strength p=0.023, abduction strength p=0.031). The Harris hip score was significantly better in the DAA group for up to 3 months (p<0.001) and the Koval score showed significantly better results in the DAA group up to 6 weeks (p≤0.001). The visual analogue scale score improvement was significantly higher in the DAA group by day 7 (p=0.035). The inclination angle (p<0.001) and anteversion angle (p<0.001) of the acetabular cup were located in the safe zone of the DAA group more than in the ALA group, and there was no statistically significant difference in the position of the femur stem and leg length difference. During surgery, two cases of greater trochanter fracture occurred in the DAA group (p=0.155). Conclusion: The DAA performed in the ordinary operation table using a short femoral stem showed post-operative early functional recovery. Because a simple to use fluoroscope was used during surgery with an anatomical position familiar to the surgeon, it is considered to be useful for the insertion of implants into the desired position and for an approach that is useful for the prevention of leg length differences.

  • In this study, the anatomic characteristics and the contents of nutritional ingredients and minerals of silky fowl and yeonsan ogolgye was investigated. Silky fowl is covered with white silky feather. There is a pinch of vertical white tassel on peak of the head, especially a silky fowl cock. The crown of a silky fowl has a nickname of phoenix crown, the crown of a cock is mostly like a rose, while that of a hen like strawberry or mulberry. The ear of silky fowl is mostly peacock green(Light blue turquoise), bronze for a small part. Peacock green is most common for Silky Fowl with a age of $60{\sim}150$ days, over the age of 150 days, the peacock green will be gradually replaced by purplish red. The contents of iron (Fe) and zinc (Zn) is higher in leg muscle than in breast muscle for the silky fowl and yeonsan ogolgye. Leg muscle of silky fowl contains a lot of iron (Fe), about 4 times as much as that of leg muscle of yeonsan ogolgye. For the silky fowl and the yeonsan ogolgye, leg muscles contains a lot of zinc (Zn), about 5 times, 4 times respectively as much as that of breast muscle.

  • The purpose of this study is to investigate if there is a higher rate of free flap failure in cases of vein grafts compared to non-vein graft, and to analyze the clinical usefulness and necessity in elective free flap surgery. We have used 24 vein grafts in 208 free flaps from May, 1986 until August, 1995. Vein grafts were from 2cm to 50cm in length between the recipient and donor vessels. Reconstructed sites were 10 lower extremities, 8 head and neck, 4 hand, and 2 trunk. Vein grafts were used 14 for arterial, 6 venous, 4 in both arterial and venous anastomoses. We intentionally used long vein grafts longer than 5cm for improved blood flow in cases of peripheral vascular disease, radionecrotic wounds, lower leg trauma. Short vein grafts of about 3cm in length were used to overcome the shortage of vascular length in cases of unexpected vessel anomaly, short donor vessel, and difficult access to recipient vessels after radical neck dissection. All veins were carefully handled with ligation of very small branches and were transferred to the recipient site without irrigation. 8 flaps were failed out of a total of 208 free flaps, however there was no failure among the 24 cases that needed vein grafts between the donor and recipient vessels. Success rates between non-vein grafts free flaps and vein graft free flaps were 95.6%(8/184) and 100%(0/24). Even though the vein grafts increase the operation time and the number of anastomoses, they do not result in any increased failure rate of free flap surgery(4.4% versus 0%). In addition to the reducing tension between recipient and donor vessels, the surgeon can select better recipient vessels with excellent blood flow so that vein grfats in microsurgery can provide a high success rate of free flaps.

Lower Somatotype Analysis of Middle and Older Aged Women - Focused on Women in 40s to 60s - (중·노년 여성의 하반신 체형 분석 - 40대~60대 여성을 중심으로 -)

  • The purpose of this study is to provide basic data reflecting body characteristics for production of briefs by classifying lower body types of women in their 40s, 50s and 60s. The results are as follows: First, measurements for height-related items decreased while the measurements for width and depth items increased with age. However, 'hip width' and 'hip depth' items showed no significant differences among the age groups. In addition, no visible differences appeared in the length items among the age groups but 'waist to hip' and 'outside leg' lengths showed decreasing values according to age. Second, four factors were established from the factor analysis: 'obesity level around waist and abdomen', 'vertical length of the lower body', 'thickness and obesity level of lower limbs', and, 'size and shape of buttocks'. Lastly, cluster analysis resulted in the classification of the four factors mentioned above. Type 2 appeared the most, representing women in their 40s~60s. Type 1(n=257) was referred as 'slender with a short lower body', Type 2(n=443) as 'plump with short lower body', Type 3(n=224) as 'slender with long lower body', and Type 4(n=199) as 'obese around abdomen and lower limbs'.

  • This study intends to analyze the fit of directly-imported American casual pants for Korean female college students in terms of appearance and comfortableness. The authors carried out a wearing test with sample pants from 3 brands (DKNY: A, Gap: B, Polo: C) that had the most market share and that were mostly preferred by Korean female college students. Results showed that pants A had insufficient ease in the crotch, as the depth of the crotch was short. The hip girth received a low assessment in terms of appearance. Moreover, the hem widths were wide while the pants length was long. Pants B were expected to be most appropriate for Korean women's bodies, as they had the longest crotch depth among the three brands. However, the pants appeared to pull downward due to the narrow thigh and the knee girth and hem width. Moreover, the crotch girth and hip girth received a poor appearance assessment. Pants C were rated as satisfactory for their thigh girth, knee girth, and hem width. However, the crotch was uncomfortable, as it had a short crotch depth. The hip girth received a low appearance assessment. This study suggests that a pattern adjustment is required for directly-imported American pants to provide a better fit for the bodies of Korean women. The crotch depths should be approximately 17.5 cm. The hem widths should be approximately 20 cm. We also suggest a leg length of 75 cm, a waist girth of 86 cm, and a hip girth of 97 cm.

Lower Body Type Classification of Korean Men in Their 30's for the Development of Slim-Fit Pants Pattern (슬림-핏 팬츠 패턴 개발을 위한 30대 한국인 남성 하반신 체형 분류)

  • This study analyzed the lower body type of 30's Korean men to develop a slim-fit pants pattern. As the analysis data, direct measurement data of anthropometric measured value in the 6th Size Korea(KATS, 2010) was used as basic data with 481 men in 30's as analysis objects. The result are as follows. First, the result of analyzing factors for the lower body type classification indicated five factors. Second, the result of executing group analysis (with the independent variable of 5 clusters extracted from the factor analysis)classified the following three types. Type 1(36.8%) displayed a medium height of lower body type, small waist and hip, slim and fit body type with a slim shape between the knee and ankle. The shape between the waist and hip had characteristics of a slight curve and short length. Type 2(35.6%) displayed lowest height of a lower body type that was large and thick between the waist and the hip. The drop value of the waist and the hip was small; therefore, the body type was flat with a minimal curve. The underpart type (below the knee) was the thickest and the length was short. Type 3(27.7%) displayed the highest lower body type, a medium level waist size, flat and narrow waist and belly. This body type had a curve with big drop value of the waist and the hip, lower part from the hip to the ankle (including the knee) and a thick calf with along leg.
