Breast Reconstruction make use of Contralateral Breast Tissue after Mastectomy (유방절제술 후 반대편 유방조직을 이용하여 유방 재건한 증례)

  • Purpose: Unilateral breast reconstruction after mastectomy confront the challenges of recreating a natural appearing breast mound and achieving symmetricalness of the breasts. If the patient's remaining breast is large compared to reconstructed breast, the most common procedure is reduction mammoplasty of the large breast side. The authors experienced a new method of breast reconstruction using the excess breast tissue from the contralateral breast after breast reduction. Methods: The excess tissue from the contralateral breast after vertical reduction mammoplasty with superior pedicle and remaining lower breast tissue were transferred to the mastectomy site breast through the subcutaneous tunnel on the chest wall. The main blood supplies of the flap are perforator branches of the 4th, 5th and 6th anterior intercostal artery. After elevating and detaching the flap on the lower lateral area of the breast, the turn overed flap is fixed on the upper portion of the chest wall of the mastectomy site. Results: On two cases of the breast reconstructions, remaining excess breast tissue from reduction mammoplasty was transferred to the contralateral breast side as pedicles. Both patient and operator were satisfied with the outcome of the reconstruction as the breasts were symmetrical and natural shape. Conclusion: We have performed unilateral breast reconstruction using the excess breast tissue after reduction mammoplasty of the contralateral breast. As Oriental women's breasts are relatively smaller than that of Caucasian women, delayed breast reconstruction cases of Oriental women with large breasts(macromastia) seem to be ideal for this procedure.

  • 기존의 스테레오 정합 알고리즘은 크게 명암기반기법과 특징기반기법의 두 가지로 나눌 수 있다. 그리고, 각 기법은 그들 나름대로의 장단점을 갖는다. 본 논문은 이 두 기법을 결합하는 새로운 알고리즘을 제안한다. 본 논문에서는 물체모델링을 목적으로 하기 때문에 배경을 제거하여 정합하는 방법을 사용한다. 이를 위해, 정합요소들과 정합유사함수가 정의되고, 정합유사함수는 두 기법사이의 장단점을 하나의 인수에 의해 조절한다. 그 외에도 거리차 지도의 오류를 제거하는 coarse-to-fine기법, 폐색문제를 해결하는 다중윈도우 기법을 사용하였고, 물체의 표면형태를 알아내기 위해 morphological closing 연산자를 이용하여 물체와 배경을 분리하는 방법을 제안하였다. 이러한 기법들을 기반으로 하여 여러가지 영상에 대해 실험을 수행하였으며, 그 결과들은 본 논문이 제안하는 기법의 효율성을 보여준다. 정합의 결과로 만들어지는 거리차 지도는 3차원 모델링을 통해 가상공간상에서 보여지도록 하였다.Abstract Classical stereo matching algorithms can be classified into two major areas; intensity-based and feature-based stereo matching. Each technique has advantages and disadvantages. This paper proposes a new algorithm which merges two main matching techniques. Since the goal of our stereo algorithm is in object modeling, we use images for which background is removed. Primitives and a similarity function are defined. The matching similarity function selectively controls the advantages and disadvantages of intensity-based and feature-based matching by a parameter.As an additional matching strategy, a coarse-to-fine method is used to remove a errorneous data on the disparity map. To handle occlusions, multiple windowing method is used. For finding the surface shape of an object, we propose a method that separates an object and the background by a morphological closing operator. All processes have been implemented and tested with various image pairs. The matching results showed the effectiveness of our method. From the disparity map computed by the matching process, 3D modeling is possible. 3D modeling is manipulated by VRML(Virtual Reality Manipulation Language). The results are summarized in a virtual reality space.

A Dual Modeling Method for a Real-Time Palpation Simulator

  • This paper presents a dual modeling method that simulates the graphic and haptic behavior of a volumetric deformable object and conveys the behavior to a human operator. Although conventional modeling methods (a mass-spring model and a finite element method) are suitable for the real-time computation of an object's deformation, it is not easy to compute the haptic behavior of a volumetric deformable object with the conventional modeling method in real-time (within a 1kHz) due to a computational burden. Previously, we proposed a fast volume haptic rendering method based on the S-chain model that can compute the deformation of a volumetric non-rigid object and its haptic feedback in real-time. When the S-chain model represents the object, the haptic feeling is realistic, whereas the graphical results of the deformed shape look linear. In order to improve the graphic and haptic behavior at the same time, we propose a dual modeling framework in which a volumetric haptic model and a surface graphical model coexist. In order to inspect the graphic and haptic behavior of objects represented by the proposed dual model, experiments are conducted with volumetric objects consisting of about 20,000 nodes at a haptic update rate of 1000Hz and a graphic update rate of 30Hz. We also conduct human factor studies to show that the haptic and graphic behavior from our model is realistic. Our experiments verify that our model provides a realistic haptic and graphic feeling to users in real-time.

  • It is becoming a common trend that many designers work on a very complex assembly together in a collaborative environment. In this environment, every designer should be able to see the whole assembly in a full detail or in a rough shape at least. Even though the hardware technology is being improved very rapidly, it is very difficult to display a very complex assembly at a speed to allow smooth interactions for designers. This problem could be solved if a designer could manipulate his portion of the assembly in a full resolution while the remaining portion of the assembly is displayed in a rough resolution. It is also desired that the remaining portion is converted to the full resolution when needed. To realize this environment, the capabilities to simplify the portions of an assembly and to reset to the original resolution should be added to the current CAD systems. Thus operators realizing multi-resolution on B-rep are proposed in this paper. They are: wrap-around, smooth-out, and thinning operator. Through appropriately applying these operators sequentially, an assembly model of any desired resolution can be easily generated. Of course, the assembly can go back to the finer resolution. In this paper, the data structures and the processes to realize these operators are described and a prototype modeling system with these operators is also demonstrated.

  • At present, the design of extrusion dies and operation in extrusion companies are primarily based on trial and error. The experience of the die designer, the press operator and the die corrector determine the performance of the extrusion die and the efficiency of the process. In order to produce defect-free products of desirable quality in terms of strength, surface quality and geometrical dimensions, it is important to obtain more knowledge of the processes that occur during extrusion. Recently, to reduce the costs of designing and manufacturing of extrusion dies, and to ensure the quality of the extruded products, numerical simulation for extrusion processes such as FEM (finite element method) is applied increasingly and becomes a very important tool for the design and development of new products. However, most of the studies about FE simulation have been accomplished for simple geometry and low extrusion ratio in the filed of steady metal flow conditions. The extruded products of AI alloy in industrial practice involve complicated sectional geometry. This study was designed to reduce the time of die design and manufacturing in the extrusion process using FEM simulation. FEM simulations of extrusion process were performed in non-steady states conditions by changing weld plate included in extrusion die set. Product which was employed in this study is heat sink that has been used in the parts of heat exchanger of electric circuits. It is generally applied for aluminum or its alloys due to heat efficiency and easy production of complicated shapes, and manufactured by extrusion process. The simulated results showed that weld plate shape in extrusion dies influences meta] flow and dimensional accuracy of products.

A Remote Control of 6 d.o.f. Robot Arm Based on 2D Vision Sensor (2D 영상센서 기반 6축 로봇 팔 원격제어)

  • In this paper, the algorithm was developed to recognize hand 3D position through 2D image sensor and implemented a system to remotely control the 6 d.o.f. robot arm by using it. The system consists of a camera that acquires hand position in 2D, a computer that controls robot arm that performs movement by hand position recognition. The image sensor recognizes the specific color of the glove putting on operator's hand and outputs the recognized range and position by including the color area of the glove as a shape of rectangle. We recognize the velocity vector of end effector and control the robot arm by the output data of the position and size of the detected rectangle. Through the several experiments using developed 6 axis robot, it was confirmed that the 6 d.o.f. robot arm remote control was successfully performed.

  • When oral restoration is performed with a full mouth implant-supported fixed restoration in an edentulous patient, it is very important to determine the shape and position of the definitive prosthesis in consideration of the anatomical state and the relationship with the antagonist, and the process of placing multiple implants in the planned direction and angle is very important. In this case, implants were ideally planned based on an upper prosthesis through a computer-guided surgical procedure for an edentulous maxillary patient who visited due to discomfort in the existing denture. Through this, we would like to report this because we obtained satisfactory functional and esthetic results for both the patient and the operator with the treatment of oral restoration by manufacturing a fixed prosthesis for maxillary and full jaw implants.

  • Contour detection is important for many computer vision applications, such as shape discrimination and object recognition. In many cases, local luminance changes turn out to be stronger in textured areas than on object contours. Therefore, local edge features, which only look at a small neighborhood of each pixel, cannot be reliable indicators of the presence of a contour, and some global analysis is needed. The novelty of this operator is that dilation is limited to Deluanary triangular. An efficient implementation is presented. The grouping algorithm is then embedded in a multi-threshold contour detector. At each threshold level, small groups of edges are removed, and contours are completed by means of a generalized reconstruction from markers. Both qualitative and quantitative comparison with existing approaches prove the superiority of the proposed contour detector in terms of larger amount of suppressed texture and more effective detection of low-contrast contour.

The Influence of Home Position(H.P.) and Random Position(R.P.) on the Shape of Prepared Tooth Surfaces -Upper Left 1st Molar for Full Cast Crown- (시술자세가 치관 삭제면 형태에 미치는 영향 -상악 좌측 제 1 대구치의 전부 주조관-)

  • To properly prepare teeth, dentists require a direct view of the working area and are often placed in a difficult position, moreover, a high proportion of dentists suffer from headache and back pain. Dentists who make use of the dental mirror and position their patients carefully to gain a proper view report less headache, pain and discomfort in the shoulders. It is recommended that dentists learn the "Home Position(H.P.)" which among the various "Random Position(R.P.)" methods, enables dentists to approach their patients in a stable posture. The purpose of this study was to compare tooth preparation in the H.P. and the R.P., and evaluate the clinical efficacy of the H.P.. Tooth preparation for a full cast crown was performed on the maxillary left 1st molar using the H.P. and the R.P., and the shapes of the prepared tooth surfaces at the two different operating positions were compared. The amount of occlusal reduction, marginal width, height, and axial taper were measured and analyzed. A T-test was performed separately to compare the results of the H.P. and the R.P. with respect to the accuracy of the corresponding tooth reduction. The results were as follows. ; 1. The amount of occlusal reduction was excessive on the mesiobuccal cusp(P<0.05), and deficient on the lingual cusp in the H.P.(P<0.01). In the R.P., this was excessive on the buccal cusp and deficient on the fossa and distolingual cusp(P<0.01). 2. The amounts of marginal width were excessive in all areas except on the lingual and mesial surfaces in the H.P. and lingual surface in the R.P.(P<0.01). 3. The marginal heights were achieved more accurately in the R.P. than the H.P.(P<0.01). 4. Axial surface taper was excessive in all areas in the H.P.(P<0.01). But the axial taper of measured areas was even, and tooth is more like to retain its original axis after reduction. In the R.P., axial surface taper was excessive on the mesial and buccal area, and deficient on the distolingual area(P<0.01), and therefore, the axis of the prepared teeth was tipped in the distolingual direction. 5. The times needed for preparation were 12 minutes and 49 seconds in the H.P., and 11 minutes and 35 seconds in the R.P., and the R.P. was statistically faster(P<0.01). The tooth preparation in the H.P. achieved its goal, in that it enabled the operator to make even tooth reduction. In conclusion, the H.P. system offers an improved method that can be used in clinic after specific training.

  • After limb salvage operation becomes popular, reconstruction method of epi-metaphyseal osteosarcoma is relatively standardized though it may be operator's matter of preference. Most limb salvage techniques presently used are prosthesis reconstruction and osteoarticular allograft. In the case of prosthesis, it is very stable and shows good postoperative functional score, however, finally, it has a weak point of requirement for removal procedure due to failure. In the long run, allograft reconstruction has a good merit. But, it has a weak point of difficulties in securing the demanded size and shape. Allograft reconstruction should be performed only in the case of safe margin and established indication. In limb salvage operation, intercalary reconstruction is useful method in diaphyseal osteosarcoma, however, in metaphyseal osteosarcoma, the indication is not confirmed. We treated 17 year old osteosarcoma patient with localized in the tibial medial condyle with proximal medial condylectomy and pasteurized bone reconstruction in the conviction of safe surgical margin. We report this case because we guess if this procedure may be applied to premature patients, despite failure hemiarthroplasty can be tried successfully.
