고도비만이 동반된 폐쇄성수면무호흡증 환자에서 시행된 비만대사수술 1례 (A Case of Bariatric Surgery for an OSAS Patient with Severe Obesity)

  • Obstructive sleep apnea syndrome (OSAS) has negative effects on health, including increased mortality, risk of cardiovascular disease, and neurocognitive difficulties. OSAS is common in obese patients and obesity is an important risk factor of OSAS. A 41-year-old female OSAS patient with severe obesity (body mass index [BMI] ${\geq}35$) who failed dietary weight loss underwent bariatric surgery. After surgery, there were improvements in BMI (from 36.9 to $31.7kg/m^2$) and polysomnographic data, including the apnea-hypopnea index (from 25.1 to 11.2 events/hr) and minimum SaO2 (from 69 to 82%). This case demonstrates that bariatric surgery may be an effective therapeutic option to reduce sleep-disordered breathing in severely obese patients with moderate OSAS. Bariatric surgery as a treatment option for OSAS should be considered in OSAS patients with severe obesity who failed dietary weight loss.

코골이 및 폐쇄성 수면무호흡증의 구강내 장치 치료시 부작용 및 관리 (Management of complications of oral appliance therapy for snoring and obstructive sleep apnea)

  • Oral appliances therapy is becoming increasingly recognized as a successful treatment for snoring and obstructive sleep apnea(OSA). Compared with continuous positive airway pressure(CPAP), the gold standard therapy for OSA, oral appliance therapy are less efficacious for severe OSA but are more acceptable and tolerable for patients, which in turn, may lead to a comparable level of therapeutic effectiveness. Nevertheless, the various side effects of oral appliance therapy, such as, increased salivation or dryness, pain or discomfort in the teeth or gums, occlusal discomfort in the morning, temporomandibular disorders, dental and occlusal changes may cause discontinuation of treatment or changes in treatment plan. Therefore, oral appliance therapy should be provided by a qualified dentist who can evaluate oral tissues, occlusion, and temporomandibular joints, and prevent and manage the possible side effects.

폐쇄성수면무호흡증후군 환자의 무호흡-저호흡지수와 임상양상간의 상관성 (Correlation between Clinical Characteristics and Apnea-Hypopnea Index with Obstructive Sleep Apnea Syndrome)

  • Obstructive sleep apena syndrome (OSAS) is accompanied by the following symptoms: apnea caused by upper respiratory tract obstruction while sleeping, repetitive lowering of $SpO_2$, severely affected excessive daytime sleepiness (EDS), suffocation/frequent awakeness while sleeping, daytime lethargy, and lack of concentration. OSAS was investigated with sex, age, body weight, body mass index (BMI), neck circumference and snoring sound as clinical characteristics and the anticipating factors of OSAS were studied in relation with the apnea-hypopnea index (AHI). The subjects were 42 people (male 34/female 8) who visited the clinic due to snoring and had polysomnography evaluation. AHI was differenciated into normal (less than 0~5/hr), mild (5~15/hr), moderate (15~30/hr) and severe (more than 30/hr). As the apnea-hypopnea index (AHI) gets higher, the snoring sound was louder (p<0.01), neck circumference was thicker (p<0.05) and also there were relative correlations with body weight (p<0.01), body mass index (p<0.05), snoring sound (p<0.01) and neck circumference (p<0.01). Since the snoring sound and neck circumference explained 32.8% of the AHI distribution, if the patient was severely snoring or had a thick neck circumference due to obesity, the apnea-hypopnea index showed a predisposition to the obstructive sleep apnea syndrome.

체위성 폐쇄성수면무호흡 환자와 비체위성 폐쇄성수면무호흡환자의 임상적 특징 고찰 (The Clinical Characteristics Between the Positional Obstructive Sleep Apnea Patients with the Non-positional Obstructive Sleep Apnea Patients)

  • 목 적 : 체위성 수면무호흡은 폐쇄성수면무호흡증으로 진단된 환자에서 앙와위 수면시 apnea-hypopnea index(AHI)가 비앙와위 수면시 AHI에 비해 2배 이상인 경우로 정의하며, 일반적으로 전체 폐쇄성수면무호흡 환자의 56%로 알려져 있다. 본 연구는 수면 클리닉을 방문하여 폐쇄성수면무호흡을 진단 받은 환자 중 체위성 수면무호흡의 발생빈도를 알아보고, 체위 의존성에 따른 수면무호흡의 임상 소견 및 수면다원검사를 비교 분석하였다. 방 법 : 코골이 및 목격되는 무호흡으로 내원하여 AHI가 5 이상이면서 앙와위 및 비앙와위 수면 시간이 15분 이상이고, 앙와위 시 AHI가 비앙와위시 AHI에 비하여 2배 이상인 경우를 체위성 수면무호흡으로, 그렇지 않은 경우를 비체위성 수면무호흡으로 분류하였다. 체위 의존성 여부에 따라 인구학적 특성과 수면다원검사에서 나타난 수면 변인 및 호흡 변인을 분석하였다. 결 과 : 폐쇄성수면무호흡으로 진단된 101명 중에서 체위성 수면무호흡 환자는 76명(75.2%), 비체위성 수면무호흡 환자는 25명(24.8%)이었다. 폐쇄성수면무호흡의 중증도별로 체위성 수면무호흡의 유무에 따라 발생빈도가 통계적으로 유의하게 차이가 있었으며, 특히 비체위성 수면무호흡 환자군에서는 중증 환자의 비율이 높았다. 비체위성 수면무호흡환자군이 체위성 환자군에 비하여 체중, 허리둘레, 엉덩이 둘레, 허리-엉덩이 둘레비, 체질량 지수 및 목둘레가 유의하게 더 높았다. 수면다원검사상에서는 체위성 수면무호흡군에서 비교적 수면구조가 잘 유지되었으며 주관적인 졸림도 덜하였다. 비체위성 수면무호흡군에서 체위성 수면무호흡군에 비해 평균산소포화도 및 최저산소포화도가 유의하게 낮았으며 AHI는 통계적으로 유의하게 더 높았다. 결 론 : 폐쇄성수면무호흡 환자중 체위성 수면무호흡 환자의 비율은 75% 정도였으며, 비체위성 수면무호흡 환자에 비해 체위성 수면무호흡 환자는 비만도가 덜 하고 경증 및 중등도의 폐쇄성수면무호흡의 양상을 보이는 것으로 관찰되었다.

  • 구강내 장치 요법은 가벼운 혹은 중간 정도의 폐쇄성 수면 무호흡증의 일차 치료법이며 심한 폐쇄성 수면 무호흡증의 경우에도 CPAP 또는 수술요법에서 효과가 없을 때 좋은 대안적 치료법이 될 수 있다. 하지만 다양한 구강내 장치들마다 치료 효과가 다르며 또한 모든 환자들에서 효과를 기대하기 어려운 것도 사실이다. 본 연구는 목젖입천장인두절제술(uvulopalatopharyngoplasty, UPPP) 을 받았으나 증상의 개선이 없는 심한 폐쇄성 수면 무호흡증 (obstructive sleep apnea, OSA) 환자에서 단국대 치대 구강내과에서 개발한 구강내 조절성 하악전방이동형 코골이 방지장치 (Adjustable Anterior Positioner, AAP)를 사용하여 치료한 증례를 보고하고 그에 대한 고찰을 한다.

폐쇄성 수면 무호흡증의 심각도와 심박동 변이율 : 탈경향변동분석 (Severity of Obstructive Sleep Apnea and Heart Rate Variability : Detrended Fluctuation Analysis)

  • Objectives : The detrended fluctuation analysis is one of the nonlinear methods for the investigation of biological time series. It quantifies the fractal scaling properties and is known to be useful in the evaluation of long-range correlations in time series. The heart rate variability(HRV) of obstructive sleep apnea syndrome (OSAS) patients during nighttime was analyzed by detrended fluctuation analysis to assess its relationship with the severity of the symptoms. Methods : Fifty nine untreated male OSAS patients with moderate to severe symptoms(mean age=45.4${\pm}$11.7 years, apnea-hypopnea index, AHI${\geq}$15) underwent nocturnal polysomnography. Moderate(AHI=15-30, N=22) and severe(AHI>30, N=37) OSAS patients were compared for the indices derived from detrended fluctuation analysis and frequency domain analysis of HRV. Results : In the detrended fluctuation analysis, the alpha values were 0.75${\pm}$0.11 and 0.82${\pm}$0.07 for the severe and the moderate OSAS groups respectively. The difference was significant(p<.01). The alpha value had negative correlation with AHI(r=-.425, p=.001). Negative correlation coefficients were also found in the relationships between the alpha values and very low frequency(VLF)(r=-.425, p=.001), low frequency(LF)(r=-.633, p= <.001) and the LF/HF ratio(r=-.305, p=.019) respectively. LF/HF ratio(p=.005) was higher in the severe OSAS group compared to that of the moderate OSAS group. Conclusion : In this study, the detrended fluctuation analysis showed the significant difference between the two OSAS groups classified according to their severity of symptoms. The scaling exponent showed the negative correlation with AHI and indicies of frequency domain analysis. This result suggests that detrended fluctuation analysis can be helpful to estimate the severity of OSAS.

항공업무 종사자의 폐쇄성 수면무호흡증후군 평가 및 치료 전략 (Strategy for the Diagnosis and Treatment of Obstructive Sleep Apnea of the Aviation Workers)

  • Obstructive sleep apnea syndrome (OSAS) can lead to severe complications if left untreated, and therefore should be adequately diagnosed and treated, especially in airline workers. The aviation medical examiner (AME) should induce the patient to cooperate for the diagnosis and treatment of OSAS, by notifying the subject that it does not cause disqualification of the air service and that it is a disease that can be sufficiently controlled and treated by various treatment options. AME should also warn about medical complications and the possibility of serious air accident risks when untreated, and encourage the subject to receive appropriate diagnosis and steady treatment such as continuous positive airway pressure and/or surgical treatment.

7세 연골 무형성증 남아에서 진단된 중증 폐쇄성 수면 무호흡증 1례 (Severe Obstructive Sleep Apnea in a 7-Year-Boy with Achondroplasia : A Case Report)

  • 수면 무호흡증은 상기도가 좁아지거나 막히게 되면서 반복적인 수면 중 각성과 동맥내 산소포화도의 저하를 유발하는 수면관련 호흡장애이다. 이 질환은 연골무형성증과 같이 유전 질환을 가진 어린아이들에게서 상대적으로 그렇지 않은 아이들에 비해 더 취약하다. 연골무형성증은 안면골과 두개저의 형성 저하와 함께 선천적으로 대후공의 협착이 동반되는 것을 특징으로 하는 유전 질환으로 높은 빈도의 폐쇄 수면무호흡증을 나타낸다. 저자들은 연골 무형성증 환아에서 편도절제술 이후에도 지속되는 증상에 대해 수면 다원검사를 시행하여 중증 폐쇄성 수면 무호흡증 진단 및 지속기도양압 치료를 하였던 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

수면호흡장애와 코막힘 (Sleep Disordered Breathing and Nasal Obstruction)

  • Nasal obstruction may cause or aggravate sleep disordered breathing but exact pathogenesis is not clear. The possible mechanism could be combination of alteration in upper airway aerodynaimcs, loss of nasal reflex or sensation, effect of mouth opening, and a genetic predisposition. Anatomical narrowing of nasal airway cause more rapid airflow and induce more negative inspiratory air pressure. So, it increases collapsibility of pharyngeal airway. Loss of nasal sensation to airflow block nasal reflex. Mouth opening decreases the activity of pharyngeal airway dilator muscles and narrowing the pharyngeal airway may occur. The treatment of nasal obstruction should be done according to the cause. The causes of nasal obstruction are various from problems of external nasal opening to nasopharynx. Relief of nasal obstruction may not cure sleep disordered breathing always. In some mild obstructive sleep apnea patients, treatment of nasal obstruction only may cure sleep disordered breathing. In some severe sleep apnea patients, treatment of nasal obstruction may increase compliance of continous nasal positive airway pressure.

Rapid Eye Movement-Related Obstructive Sleep Apnea: A Study on the Pathogenesis through Clinical and Polysomnographic Features

  • Purpose: The aims of this study were to evaluate the differences of clinical and polysomnographic features between rapid eye movement (REM)-related obstructive sleep apnea (OSA) and not-REM-related OSA, and to suggest the pathogenesis according to the REM dependency of OSA. Methods: One hundred ninety consecutive patients diagnosed with OSA were evaluated clinical features and performed full night polysomnography. The patients were divided into REM-related (REM apnea-hypopnea index [AHI] at least two times higher than their non-REM AHI) and not-REM-related (a REM AHI less than two times higher than their non-REM AHI) OSA groups and evaluated the differences in age, body mass index (BMI), neck circumference, Ep-worth Sleepiness Scale score, and parameters of polysomnography. Results: REM-related patients were younger and showed higher sleep efficacy, low percentage of light sleep stage (stage 1 sleep), and low rate of positional OSA. Age was significantly associated with REM dependency of OSA and REM AHI were significant correlated with BMI, neck circumference, percentage of sleep in supine position, and percentage time of snoring. Conclusions: Our results showed that REM-related OSA patients showed less severe polysomnographic parameters than not-REM-related patients. However, significant risk factors were differed depending on the REM dependency and OSA severity, and the clinical features correlated with REM AHI and non-REM AHI were also showed differently. We suggest that the occurrence of OSA according to the REM dependency can be based on different mechanisms.
