A Case of Partial DiGeorge Syndrome in Prematurity (미숙아에서 발견된 부분형 DiGeorge 증후군 1례)

  • We experienced a case of partial DiGeorge syndrome in a $35^{+5}$ week premature female infant presented with micrognathia, fish-shaped mouth, beaked nose, nasal regurgitation, obstructive sleep apnea, velopharyngeal insufficiency and late onset hypocalcemic seizures. The chromosome 22q11 microdeletion was found by the FISH method. The lab findings showed serum calcium level of 4.4 mg/dL, ionized calcium level of 0.49 mg/dL, phosphorous level of 7.5 mg/dL, magnesium level of 1.3 mg/dL and PTH-RIA level of <1 pq/mL. Initial treatment was done with 10% calcium gluconate infusion and magnesium sulfate followed by oral calcium gluconate and low phosphorousformula milk feeding. The serum calcium level was normalized in 6 days. Nasal regurgitation, desaturation with obstructive sleep apnea continued. T-cell functions & numbers(CD 3, CD 4, CD 8)were decreased but Ig G/A/M levels were normal. No visible signs of thymus shadow were seen in either chest X-ray & chest MRI. Electrocardiography and echocardiography showed normal heart. Kidney ultrasonographby showed right side mild hydronephrosis. Neurosonography was normal but EEG showed electrical partial seizure. Hearing assessment by BERA showed mild to moderate hearing impairment. Velopharyngoplasty is scheduled for further treatment. A brief review of literature was made.

Changes of Hematological Constituents in the Mullet, Mugil cephalus Exposed to Chromium (크롬 노출에 따른 숭어, Mugil cephalus의 혈액성분 변화)

  • The study was carried out to investigate the changes of hematological parameters induced by waterborne exposure of chromium (Cr) in the mullet, Mugil cephalus. The mullet was exposed to sub-chronic concentrations of chromium (0, 25, 50, 100, $200{\mu}g/L$ Cr) for 4 weeks. The major hematological findings were significant decreases in the red blood cell (RBC) count and hematocrit value in mullet exposed to ${\geq}50{\mu}g/L$ Cr. Although serum calcium concentration was significantly reduced at ${\geq}50{\mu}g/L$ Cr, magnesium concentration was found to be significantly increased at ${\geq}100{\mu}g/L$ Cr. The serum glucose and total protein concentrations were significantly increased at $200{\mu}g/L$ Cr. However, serum triglyceride concentration did not show any noticeable changes in the range of $25{\sim}200{\mu}g/L$ Cr of chromium compared to control group during the experimental period. A significant increment of GOT (glutamic oxalate transminase) and GPT (glutamic pyruvate transminase) activities was noticed at ${\geq}100{\mu}g/L$ Cr. These results indicate that hematological and serum biochemical changes in the mullet by waterborne exposure to chromium are affected at more than $50{\mu}g/L$ Cr.

The Survey of Urinary Calculi Following Long Feeding Period in Korean Native Steers (거세한우 장기비육에 따른 비뇨기결석 발생조사)

  • We surveyed the incidence of urinary calculi for feeding period, urinary lesion, composition of calculi and changes of serum chemistry with Korean native castrated steers, which are growing until 26 through 31 months. Without showing any clinical signs such as urethral obstruction, hydronephrosis and strangury for all group, $37.8\%$ (34 out of 90) of Korean native steers carried urinary calculi in carcass. The incidence of urine calculi far different feeding stages from 26 to 31 months were observed as $13.3-60.0\%$er, no significant differences were detected. The distribution of calculi were kidney $(24.4\%)$, kidney and bladder $(10\%)$ and kidney, bladder and urine $(3.3\%)$. Congestion of urinary bladder was shown in $26.6\%$ of Korean native steers. Korean native steers with urinary calculi $(41.2\%)$ had more possibilities (P<0.05) to bring outbreaks of congestion of urinary bladder than those of without calculi $(17.8\%)$. The main composition of calculi were struvite (magnesium ammonium phosphate crystals), calcium phosphate, calcium carbonate, calcium oxalate, non-oxalate calcium, ammonia and phosphate. The distribution of urine pH was $71.7\%$(above pH 8.0), $12.2\%$ (between pH 7.0 and pH 8.0) and $16.6\%$ (under pH 7.0). Like shown in herbivores, most of the urine pH was distributed as alkaline urine. Serum creatinine was shown significant difference (P<0.05) according to incidence of calculi but other serum chemistry didn't show any difference in serum chemistry. These results suggest that feeding until 26 to 31 months in Korean native steers castrated at 6 months of the age elicits urinary calculi and congestion of urinary bladder, but not clinical abnormality. And adjustment of the dietary Can ratio to a level of 2:1 or greater during feeding period may reduce the possibility of incidence of urinary calculi in Korean native steers.

  • A 45-year-old man underwent heart transplantation due to dilated cardiomyopathy. Cyclosporine, 2 mg/kg per day, was intravenously givell postoperatively. As central neurotoxicity signs that were included pin-point pupil, no light reflex, coma, were presented at 8 postoperative hours, cyclosporine was decreased to 1 mg/kg er day. At that time the cyclosporine level was 345 $\mu\textrm{g}$/L, the serum creatinine level was 1.8mg/dl and the serum magnesium level was within normal limit. He awaked at 31 postoperative hours and all sign of cyclosporine-induced central neurotoxicity was resolved after postoperative days. He was discharged without sequale at postoperative day 28.

Neurocognitive Functions in Infants with Malnutrition; Relation with Long-chain Polyunsaturated Fatty Acids, Micronutrients Levels and Magnetic Resonance Spectroscopy

  • Purpose: Malnutrition may influence neurocognitive development in children by directly affecting the brain structural development, or indirectly by affecting the children's cognition experience. Malnutrition alters the cell numbers, cell migration, synaptogenesis, and neurotransmission due to inadequate availability of necessary micronutrients to support cell growth. We aimed to analyze neurocognitive development in infants with malnutrition and its association with long chain polyunsaturated fatty acids (LC-PUFA), micronutrients levels and magnetic resonance spectroscopy (MRS) findings. Methods: The study included two groups; group 1, infants with malnutrition (n=24), group 2; healthy infants (n=21). Peripheral blood was obtained from the participants for studying micronutrients and LC-PUFA levels. The neurocognitive development was analyzed by the use of an Ankara Developmental Screening Inventory test. MRS were performed on all infants. Results: All parameters of neurocognitive development and serum calcium ($9.6{\pm}0.9mg/dL$ vs. $10.4{\pm}0.3mg/dL$, p<0.05) and magnesium ($2.02{\pm}0.27mg/dL$ vs. $2.2{\pm}0.14mg/dL$, p<0.05) levels were noted as being low in infants with marked malnutrition. No difference was found in LC-PUFA levels between healthy and malnourished infants. Thalamic choline/creatine levels were significantly high in infants with malnutrition ($1.33{\pm}0.22$ vs. $1.18{\pm}0.22$, p<0.05). Total neurocognitive development in infants was positively correlated with serum calcium levels (p<0.05, r=0.381). Conclusion: Calcium supplementation may improve neurocognitive development in malnourished infants.

Screening Moringa species focused on development of locally available sustainable nutritional supplements

  • BACKGROUND/OBJECTIVES: The leaves of Moringa oleifera (MO) and Moringa stenopetala (MS) commonly grown in Ethiopia possess potential nutritional and medicinal value. The aim of this study was to evaluate the nutritional and functional characteristics of the dried leaf powder from two Moringa species to develop sustainable nutritional supplements for Ethiopians from locally grown plant sources. MATERIALS/METHODS: Freshly harvested and air-dried MO and MS leaves were authenticated and the nutritional contents, such as protein, ash, lipids, and selected vitamins and minerals, were analyzed using standard analytical procedures. Amino acid compositions were also determined by an amino acid analyzer. Nine-week-old mice were randomly divided into four groups to investigate the anti-obesity effects of Moringa. The first group was fed a basal diet, the second group a high-fat diet, and the others were fed a high-fat diet containing 0.1% Moringa leaf powder from each species. After seven weeks, serum indices related to lipid profiles from each mouse were analyzed. RESULTS: The present study revealed high protein (28-29%) and ash (7-11%) contents. Glutamic acid, aspartic acid, proline, and leucine were the most abundantly found amino acids in both species. The predominant minerals in the leaf powder were calcium (826-1,530 mg/100 g), potassium (794-904 mg/100 g), and magnesium (286-431 mg/100 g). Pyridoxine (475.06 mg/100 g) and vitamin E (34.2 mg/100 g) were found only in MS. Niacin was found only in MO at 32.21 mg/100 g, whereas ascorbic acid was found in both species (3.89 and 6.19 mg/100 g dry weight for MO and MS, respectively). The results of the animal study showed that mice on a high-fat diet containing 0.1% MO leaf powder alleviated the elevation of cholesterol, triglycerides, and low-density lipoprotein cholesterol induced by the high fat diet. MO was more effective than MS in preventing hypercholesterolemia and fat deposition. CONCLUSION: The findings in this work confirmed that Moringa leaves of both MO and MS possessed high nutritional value but MO was better at preventing the harmful effects of the high-fat diet than MS.

Studies on the Changes in Serum Testosterone Levels and Metabolites in Growing Pig (성장중인 돼지에 있어서 혈청중 Testosterone 및 대사물질의 변화에 관한 연구)

  • This experiment was carried out to study on changing phases of the concentrations of serum testosterone and metabolites in the various grwoing stages of male pigs. The eight males were used to obtain serial blood samples at a, pp.oximately 20kg body weight intervals from birth to 130kg body weight. The blood samples were taken from the jugular veins and serum was stored at -20$^{\circ}C$ until assay. Testosterone concentrations in the serum were analyzed by radioimmunoassay. The result obtained are as follows: 1. Serum testosterone concentrations were elevated at birth and were reached a maximum level between 50 and 70kg body weight, which was when sexual maturity was reached. 2. Calcium values did not vary a, pp.eciably with body weight, and ranged from 9.6${\pm}$0.6 to 11.9${\pm}$0.8mg/100$m\ell$. Potassium and sodium concentrations ranged from 38.5${\pm}$2.9 and 233.9${\pm}$2.1mg/100$m\ell$ to 64.2${\pm}$6.5 and 269.1${\pm}$9.5mg/100$m\ell$, respectively. Magnesium values dro, pp.d at birth and then rose to peak at 15kg of body weight. Iron concentrations was 0.12${\pm}$0.02mg/100$m\ell$ at birth, rose to 0.20${\pm}$0.04mg/100$m\ell$ at 15kg of body weight and then gradually increased to 0.29${\pm}$0.04mg/100$m\ell$ at 30kg of body weight. Serum zinc concentrations rose from a low of 56${\pm}$3.3mg/100$m\ell$ at birth to a high of 83.3${\pm}$3.4mcg/100$m\ell$ at 15kg of body weight. Co, pp.r values rose from a low of 25${\pm}$2.5mcg/100$m\ell$ at birth to a high of 183${\pm}$4.3mcg/100$m\ell$ at 15kg of body weight. 3. Serum cholesterol concenrtration did not vary a, pp.eciably with body weight, and ranged from 90.5${\pm}$6.0mg/100$m\ell$ to 95.0${\pm}$6.3mg/100$m\ell$. Glucose concentrations ranged from 80.5${\pm}$1.2mg/100$m\ell$ to 108.7${\pm}$8.4mg/100$m\ell$. Serum total protein rose from alow of 2.7${\pm}$0.8mg/100$m\ell$ at birth to a rapidly high of 4.3${\pm}$0.1mg/100$m\ell$ at 15kg of body weight and then gradually increased to 7.3${\pm}$0.4mg/100$m\ell$ at 130kg of body weight. Serum albumin values ranged from 0.5${\pm}$0.1$m\ell$ to 3.0${\pm}$0.3mg/100$m\ell$. 4. The total concentrations of essential/nonessential amino acid were 944.7mg/100$m\ell$ and 934.4mg/100$m\ell$ at birth, respectively. The values of essential/nonessential amino acid gradually rose from a low level at birth to a high level at 130kg of body weight. The total concentrations of essential/non-essential amino acid ratios remained from birth to 130kg of body weight.

Two Cases of Chemical Burns by Hydrofluoric Acid (불화수소산에 의한 화학화상 2례)

  • Hydrofluoric acid is a colorless gas or fuming liquid with a strong, irritating odor. Hydrofluoric acid burns are uncommon; however, if severe burn occur, they caused death from systemic effects, such as fatal cardiac arrthmia. We experienced two cases of hydrofluoric acid chemical burns on digits. These patients had typical clinical features of hydrofluoric acid chemical burns, such as pulsating pain at the burn site, as well as ischemic and necrotic skin changes. The hydrofluoric acid chemical burn was confirmed by a history of exposure. Subsequently, we made a calcium gluconate gel by mixing 20% calcium gluconate, an antagonist against hydrofluoric acid, with lubricant, and we injected 10% calcium gluconate subcutaneously when they complained of pain rated at higher than 5 on the pain scale. Simultaneously, we monitored the patients' electrocardiographs and checked their serum total calcium, ionized calcium, and magnesium levels serially. Clinical presentations and the emergency management of hydrofluoric acid chemical burns were reviewed along with the current literature. These patients were discharged without any significant complications.

Hematological and blood chemical findings in hypophysectomized rats (뇌하수체 제거술이 수행된 랫트의 혈액학 및 혈액화학적 소견)

  • The present study was carried out to produce the hematological and blood chemical findings after hypophysectomy in rats. Hypophysectomy was performed by the parapharyngeal method and the sham surgery was performed for the control group. Two weeks after the operation, the body weight of the hypophysectomized and control rats was measured daily for 5 days. We deleted the rats the weight gain of which is less than 5 g during 5 days from the hypophysectomy group. The successful operation rate was approximately 40%. In the hypophysectomized and control rats, their blood samples were collected from posterior vena cava after celiotomy under generally anesthesia with ether. Hematological parameters such as erythrocyte count, leukocyte count, hemoglobin concentration, hematocrit level, and platelet count were determined by Animal Blood Counter. The erythrocyte count, hemoglobin concentration, and hematocrit level were lower significantly (p<0.01), and the leukocyte count was lower significantly (p<0.05) in hypophysectomy group compared with control group. But the plate count did not show significant difference (p>0.05) between hypophysectomy group and control group. Also, blood chemical parameters such as glucose, blood urea nitrogen (BUN), aspartate animotransferase, albumin, total protein, cholesterol, calcium, and magnesium in serum were determined. Except BUN concentration, all parameters were not affected by hypophysectomy. But the BUN concentration was higher significantly (p<0.01) in hypophysectomy group compared with control group.

Furosemide induced medullary nephrocalcinosis mimicking Bartter syndrome (바터씨 증후군으로 오인된 furosemide로 인한 신수질 석회화)

  • Clinical presentation of Bartter syndrome is similar to surrepitious vomiting or use of diuretics. Therefore, precise differential diagnosis of Bartter syndrome is crucial. We report a case of medullary nephrocalcinosis (MNC) induced by furosemide mimicking Bartter syndrome. A 55-year-old female patient visited our hospital with renal dysfunction on basis of hypokalemia and metabolic alkalosis. She had no history of hypertension or drug use except allopurinol and atorvastatin. She did not complain of nausea or vomiting on presentation and the serum magnesium level was normal. We performed ultrasonography, that showed MNC. For these reasons, we suspected Bartter syndrome and corrected the electrolyte imbalance. During outpatient follow up, we found that the patient had been taking 400 mg of furosemide daily for 30 years. We could diagnose furosemide induced MNC, and recommended to her to reduce the amount of furosemide.
