Nutritional Status and Indicators of Immune Function of Elderly Residing in Low Income Urban Area (도시생활보호노인들의 영양상태와 면역지표에 관한 연구)

  • This study was conducted to assess the nutritional status of the elderly from low income areas and its relationship with the indices of immunity. Ninety nine(male : 28, female : 71) healthy, subjects aged over 65 years staying in homes in Seoul were assessed with regard to their dietary intake and biochemical measurements. Their mean daily energy intakes were 1256kcal and 109kcal for males and females respectively. Mean serum IgG were 2356mg/dl in men and 2161mg/dl for women, higher than the normal range. Mean serum $C_3$ were 120.9mg/dl and 130.6mg/dl for males and females respectively and mean $C_4$ were 30.6mg/dl and 31.9mg/dl. The mean $C_3$ and $C_4$ were within the normal range and the proportion of subjects whose $C_3$ and $C_4$ were at a higher level than the upper limit were 15.4$\%$ and 16.3$\%$ respectively. Complement $C_3$ showed significant correlation with serum albumin(p〈0.01), serum cholesterol(p〈0.00l, serum TG(p〈0.05), LDL-C(p$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$$\ll$q0.001), VLDL-C(p$\ll$0.05), serum copper(p$\ll$0.05) and serum Ca(p$\ll$0.01). The Correlatin of Dietary Cr, Cu and Zn Levels with Serum Lipid Healthy College Women Living in Choongnam Area

  • The purpose of this study was to examine the intake of Cr, Cu, and Zn, which play important roles in lipid metabolism, and the relationship of these microminerals with serum lipids of healthy college living in the Choongnam area. The nutritional status of the subjects(35 women) was evaluated based on anthropometric measurements, 24-hr dietary recall for 3 days. Three-day meals and fasting blood were collected to analyze Cr, Cu, and Zn. The mean age, height, weight and BMI were 20 years, 158 cm, 55kg and 22.42kg/$m^2$ respectively. The mean daily energy intake was 85.9% of RDA for Koreans. The ratio of energy from carbohydrate, protein and fat was 60 : 24 : 16. The mean daily intake of Cr, Cu, and Zn was 60.07 $\mu\textrm{g}$/day, 2.64 mg/day, and 11.35 mg/day, respectively. The mean serum levels of Cr, Cu, and Zn were 143$\mu$g/dl, 81.34$\mu$g/dl, adn 101.54$\mu$g/dl, respectively. The mean serum levels of total cholesterol, triglycerides, HDL-cholesterol, LDL-cholesterol were 158.56mg/dl, 29.27 mg/dl, 56.00mg/dl, 6.12mg/dl, respectively. In conclusion, the subjects of the present study were healthy and consumed normal levls of dietary Cr, Cu and Zn, which play roles in lipid metabolism. Therefore, serum lipids of the subjects were all in the normal range. There was no significant correlation between dietary microminerals and serum lipids.

Mutation Analysis of Wilson Disease Gene: Arg778Leu Mutation in Korean Children (윌슨 유전자의 돌연변이 분석: 한국 윌슨병 환자에서의 Arg778Leu 돌연변이)

  • Background: Wilson disease (WD) is an autosomal recessive disorder of copper transport and characterized by degenerative changes in the brain, liver dysfunction, and Kayser-Fleischer rings due to toxic accumulation of copper. Since the identification of Wilson disease gene (ATP7B), more than 80 mutations have been detected among the different ethnic groups. Methods: Twenty three children with Wilson disease were included in this study. They were all diagnosed by low serum ceruloplasmin and increased 24 hour urinary copper excretion with characteristic clinical findings. We analysed WD gene mutation by assessing the nucleotide sequence of exon 7, 8, 9 and 10 including intron-exon boundaries of ATP7B gene from genomic DNA. Results: Arg778Leu mutation was identified in 16 WD patients; three were homozygous and 13 were heterozygous for this mutation. Of the 46 alleles, 19 alleles had a Arg778Leu mutation (19/46=41%). Homozygote patients had neurologic forms of WD. Arg778Leu mutation was not found among 50 normal healthy persons. Conclusion: Arg778Leu mutation is a common mutation in Korean WD gene. Arg778Leu mutation screening might be used as a useful supplementary diagnostic test in some patients to confirm Wilson disease in Korea.

  • The superoxide dismutase (SOD) mimetic activity of copper complex of 3,5-disopropylsalicylic acid (Cu(II)-DIPS) was tested and compared to those of human recombinant SOD (hrSOD) and its conjugate form with polyethyleneglycol (PEG) using fer- ricytochrome c reduction assay. Stability constant of Cu(II)-DIPS was measured po- tentiometrically using SCOGS2 program. In the presence of 10 g/L albumin, Cu(II)-DIPS lost most of its SOD mimetic activity. HrSOD was modified with polyethylene glycol (PEG) of M.W. 5000. These conjugates have markedly prolonged plasma half-lives of enzymatic activity (15.5 hr) compared to native hrSOD (5 min). In summary, efficient SOD mimetics should be stable enough not to dissociate in blood by serum protein. HrSOD could have longer half-life by conjugation with inert PEG for sustained SOD effect.

  • The coexistence of Wilson disease with Alport syndrome has not previously been reported. The diagnosis of Wilson disease and its ongoing monitoring is challenging when associated with an underlying renal disease such as Alport syndrome. Proteinuria can lead to low ceruloplasmin since it is among serum proteins inappropriately filtered by the damaged glomerulus, and can also lead to increased urinary loss of heavy metals such as zinc and copper. Elevated transaminases may be attributed to dyslipidemia or drug induced hepatotoxicity. The accurate diagnosis of Wilson disease is essential for targeted therapy and improved prognosis. We describe a patient with a diagnosis of Alport syndrome who has had chronic elevation of transaminases eventually diagnosed with Wilson disease based on liver histology and genetics.

A Study on Wound Healing Factors in Diabetic Foot Ulcer Patients (당뇨발 궤양을 가진 환자에서의 창상치유인자에 대한 조사)

  • Purpose: Diabetic foot ulcer is a representative disease of chronic would with multiple defects of wound healing factors. Many nutrition factors have been known to be essential for wound healing, but objective data are lacking. The purpose of this study is to determine those factors essential for wound healing, and to find out which of those factors are lacking in diabetic foot ulcer patients through this pilot study. Methods: We studied 100 patients who visited our clinic from March 2005 to February 2006 for treatment of diabetic foot ulcers with a duration of more than 6 weeks. We checked serum levels of protein, albumin, vitamin A, C, E, iron, magnesium, zinc, copper and hemoglobin by drawing 23cc blood after 8 hours of fasting. Protein, albumin, iron, magnesium levels were measured by colorimetry; hemoglobin levels were measured by auto analysis. Vitamin levels were measured by high performance liquid chromatography (HPLC), copper and zinc levels were measured by Inductively coupled plasma (ICP). They were compared with normal values. The patients were divided by transcutaneous oxygen pressure levels, age and sex to study the effects of these parameters. Results: 76% and 61% of patients had within-normal range serum protein and albumin levels, respectively. Among vitamins, only the level of vitamin C was low in 55% of the patients. Levels of vitamin A, E were normal or high in 93% and 100% of patients. As for trace elements, levels of iron and zinc were low in 63% and 60% of patients, but levels of magnesium and copper were usually normal or high. Levels of vitamin C, iron and zinc were lower in the low-transcutaneous oxygen pressure group. There were no definite differences according to age and sex. Conclusion: Serum levels of Hb, vitamin C, iron, zinc were low in most diabetic foot ulcer patients. The deficit was very severe in the low-transcutaneous oxygen pressure group.

Effects of Dietary Zinc and Ethanol on the Mineral Contents of Serum and the Changes of the Liver Tissue in Rat (식이성 아연과 에탄올이 흰쥐의 혈청중 무기질 함량과 간장조직에 미치는 영향)

  • This experiment was conducted to study the effects of dietary zinc and ethanol on the mineral contents of serum and changes of the liver tissue. Eighty male rats of Sprague-Dawley strain with average weight of 80$\pm$5g were divided into five groups according to zinc levels(5ppm and 100ppm), ethanol and isocaloric sucrose feedings. The rats were sacrificed after 4 and 7 weeks of feeding periods. The results are summarized as follows. 1. There was a decrease of weight gain in the ZD group compared with the C group and also decreased in the CE and ZDE groups fed the ethanol solution. The feed intake was decreased in the ZD and ZDE groups, and the feed efficiency was also greatly reduced in the ZDE group. 2. The liver weight of the ZD and ZDE groups was increased, however, the weight of testis was decreased in the same groups. And the weight of spleen was decreased in the ZD group. 3. The AST and ALT activities of serum were significantly increased in the ZDE group. 4. The content of serum zinc was influenced by the dietary zinc level, and the amount was significantly decreased in the ZD group. But the content of serum copper was remarkably increased in the ZD grou. The calcium and magnesium contents of serum were decreased in the ZD and ZDE groups. 5. The morphological changes of the liver tissue were not different among the C, CE and PF groups, but the abllooning denaturation of the liver cell together with the infiltration of the local inflammation cell was found in ZD and ZDE groups.

Ecological Studies of Maternal-Infant Nutrition and Feeding in Urban Low Income Areas (도시 저속득층 지역의 모자 영양 및 섭식에 관한 생태학적 연구 -I. 임산부의 인체 계측, 식이 섭취 및 혈청 지질/지방산 조성 -)

  • This study was aimed at evaluating the lipid status of 122 pregnant women, who were attending peripheral community clinics in low-income areas and were vulnerable in terms of public health, by anthropometric measurements, estimating dietary intakes and analyzing serum lipids content and fatty acids composition. Regarding the skinfold thickness, it has increased more significantly in suprailliac than in triceps, biceps, subscapular, which means body fat has been accumulated in central parts more than in distal parts of the pregnants. It is found that intakes of energy, protein, lipid and carbohydrate are desirable while calcium, zinc and copper are low and sodium is excessive. Regarding dietary fat intake, the energy $\%$ of fat was within the same range as RDA(20%) for adults, but energy $\%$ intake from each fatty acid was usually low, especially energy % of monoun-saturated fatty acid intake was below RDA for adult women. And intake ratio of $\omega$6$\omega$3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Serum lipids concentrations were increased with the length of pregnancy and were higher as compared with non-pregnant women. Polyunsaturated fatty acids, especially $\omega$6 fatty acids, have gradually decreased with the length of pregnancy. Serum lipids content and fatty acids composition were not greatly influenced by dietary fat but serum free fatty acid level was negatively related to energy, carbohydrate, fatty acids and cholesterol intake. On the basis of the above results, it can be predicted that lipid metabolism for mother and fetus is changed by pregnant status and multilateral research on maternal nutrition in terms of areas and income levels will have to be made. The Relationship of Dietary Heavy Metal Intake with Serum Trace Elements in College Women Living in Choong-Nam Area

  • The purpose of this study was to study the intake of heavy metals such af arsenic, lead and cobalt and the relationship of dietary heavy metals with serum iron, topper, and zinc, which play important roles in hematopoiesis, in healthy college women living in Choongnam Korea, where we have detected heavy metals (As, Pb, Co) in some marine products in previous studies. The nutritional status of the subjects (35 women) was evaluated by anthropometric measurements, 24-hr dietary recall for 3 days. And 3-day diets (by weighing method) and blood were collected to analyze As, Pb, Co, Fe, Cu, Zn, Hb, Hct, and MCHC. The mean age, height, weight, and BMI were 20 years, 158 cm, 55 kg and 22.42 kg/$m^2$, respectively. The mean daily energy intake was 85.85% of RDA for Koreans. The ratio of energy from carbohydrate, protein, and fat was 60 : 24 : 16. The mean daily intake of heavy metals (As, Pb, Co) was 1.77 mg/day, 75.21 $\mu$g/day and 21.12 $\mu$g/day. And the mean daily intake of iron, copper, and Zinc concentrations were 97, 68, and 92% of normal values. The mean serum heavy metals (As, Pb, Co) were 16.14 $\mu$g/dl, 4.32 $\mu\textrm{g}$/dl and 0.02 $\mu$g/dr, respectively Mean blood levels of Fe, Cu, Zn, Hb, Hct, and MCHC were at normal levels. Dietary heavy metals except Co were not significantly different from serum Fe, Cu, Zn and Hb, Hct, and MCHC. However, there was a tendency toward lower serum concentration of Fe, Hb, Hct, and MCHC in the subjects with higher heavy metals (As) intake. Among heavy metals, only dietary Co showed a significant negative correlation with Hb (p< 0.001) and Hct (p < 0.001).

Effects of Feeding Different Chelated Copper and Zinc Sources on Growth Performance and Fecal Excretions of Weanling Pigs

  • Two feeding trials were conducted to study the effects of different chelated copper and zinc compounds on the performance and fecal excretions of weanling pigs. In Exp. 1, 150 weanling pigs ($L{\times}Y{\times}D$, $12.30{\pm}2.07kg$) were randomly assigned to five dietary treatments: 170 ppm Cu from $CuSO_4$, 85 ppm Cu from Cu amino-chelate (CAC), 170 ppm Cu from CAC, 85 ppm Cu from Cu-Lysine (CL), and 170 ppm Cu from CL. In Exp. 2, 150 weanling pigs ($L{\times}Y{\times}D$, $12.52{\pm}1.80kg$) were randomly assigned to five dietary treatments: 120 ppm Zn from $ZnSO_4$, 60 ppm Zn from Zn-amino-chelate (ZAC), 120 ppm Zn from ZAC, 60 ppm Zn from Zn-Methionine (ZM), and 120 ppm Zn from ZM. In both experiments, pigs were randomly distributed to the treatments following a randomized complete block design on the basis of body weight as the blocking variable. Each experiment was conducted for 28 days. Blood and fecal samples were collected to determine mineral contents as affected by the dietary treatments. There was no difference (p>0.05) in ADG and ADFI among treatments, but F/G was improved (p<0.05) in pigs fed diet with 170 ppm CAC than 85 ppm CL but not different (p>0.05) to the control (170 ppm $CuSO_4$). Regardless of copper source, concentration of Cu in serum and feces were higher in pigs fed diet with 170 ppm Cu than pigs fed diet with 85 ppm Cu (Exp 1). In Exp 2 the ADG was higher (p<0.05) in pigs fed diet with 120 ppm ZM than in pigs fed diets with 120 ppm $ZnSO_4$ and 60 ppm ZAC and ZM. The serum zinc concentration was generally higher (p<0.05) in pigs fed diet with organic source than the control group ($ZnSO_4$). Also, there was a trend towards a decrease in fecal excretions of zinc when dietary zinc level was low. The efficacy of the two chelated copper and zinc sources is similar in terms of growth performance. The fecal excretions for Cu and Zn could be reduced in pigs fed low level of these minerals using organic sources.
