Comparison of the Walz Nomogram and Presence of Secondary Circulating Prostate Cells for Predicting Early Biochemical Failure after Radical Prostatectomy for Prostate Cancer in Chilean Men

  • Purpose: To determine the utility of secondary circulating prostate cells for predicting early biochemical failure after radical prostatectomy for prostate cancer and compare the results with the Walz nomagram. Materials and Methods: A single centre, prospective study of men with prostate cancer treated with radical prostatectomy between 2004 and 2014 was conducted, with registration of clinical-pathological details, total serum PSA pre-surgery, Gleason score, extracapsular extension, positive surgical margins, infiltration of lymph nodes, seminal vesicles and pathological stage. Secondary circulating prostate cells were obtained using differential gel centrifugation and assessed using standard immunocytochemistry with anti-PSA. Biochemical failure was defined as a PSA >0.2ng/ml, predictive values werecalculated using the Walz nomagram and CPC detection. Results: A total of 326 men participated, with a median follow up of 5 years; 64 had biochemical failure within two years. Extracapsular extension, positive surgical margins, pathological stage, Gleason score ${\geq}8$, infiltration of seminal vesicles and lymph nodes were all associated with higher risk of biochemical failure. The discriminative value for the nomogram and circulating prostate cells was high (AUC >0.80), predictive values were higher for circulating prostate cell detection, with a negative predictive value of 99%, sensitivity of 96% and specificity of 75%. Conclusions: The nomagram had good predictive power to identify men with a high risk of biochemical failure within two years. The presence of circulating prostate cells had the same predictive power, with a higher sensitivity and negative predictive value. The presence of secondary circulating prostate cells identifies a group of men with a high risk of early biochemical failure. Those negative for secondary CPCs have a very low risk of early biochemical failure.

Evaluation of Variation Method to Improve the Sensitivity of Immunoradiometric Assay (면역방사계수측정법의 민감도 향상을 위한 변법의 평가)

  • Purpose The concentration of PSA (Prostate Specific Antigen) after radical prostatectomy in prostate cancer patients is a predictor of biochemical recurrence, and the AUA (American Urological Association) is defined as biochemical recurrence when the concentration of PSA is measured at 0.2 ng/mL or more, and when the concentration is measured at 0.2 ng/mL or more at the retest. This standard is also applied our hospital. In this laboratory, the PSA reagent using IRMA (Immunoradiometric Assay) is used, and the sensitivity at a very low value was not as good as the reagent used in the department of laboratory medicine. This study aims to increase the reliability of the results by improving the precision and sensitivity of very low values. Materials and Methods As a reagent for the study, PSA reagent using IRMA was used. As a method to improve the precision and sensitivity of very low values, a variation method on the serum volume(25 uL, 50 uL, 100 uL, 200 uL) was studied, and variation usefulness evaluation was conducted. The evaluation items were compared the results of precision, analytical sensitivity, recovery rate, dilution test, high-dose hook effect test, parallel test and very low concentration values(n = 20). Results The validation results were displayed in the order of 25 uL, 50 uL, 100 uL, 200 uL. As the serum volume increased, it was confirmed that CV (Coefficient of Variation)(%) improved. Analytical sensitivity(ng/mL) was 0.038, 0.041, 0.017, 0.015 and recovery rate(%) was 101±3, 101±3, 99±2, 97±4. very low concentration values(ng/mL) between each volume(n=20) were 0.135±0.068, 0.076±0.050, 0.048±0.034, 0.046±0.034. and high dose hook effect appeared as the serum volume increased. Conclusion Through the variation usefulness evaluation, it was confirmed that as the serum volume increased, the precision and sensitivity improved at very low concentration values. However, it is necessary to pay special attention to the occurrence of high-dose hook effect as the serum volume increases. In the case of tests that requires very low concentration values, it is thought that the reliability of the result will be increased if the variation method is properly used after the variation usefulness evaluation.

Antimicrobial Activity and Acute Toxicity of Natural Rutin (천연 Rutin의 항균효과와 급성독성에 미치는 영향)

  • As part of our search for less toxic antimicrobial agents from natural resources, antimicrobial activity of rutin isolated from Sophora japonica was tested in vitro against four kinds of gram positive bacteria and four kinds of gram negative bacteria by serial broth dilution method. Among eight kinds of bacteria tested, the antimicrobial activity of rutin was the most potent against Mycobacterium smegmatis showing MIC of $375\;{\mu}g/ml$. The acute toxicity of rutin was examined in mice and rats. and $LD_{50}$ value administered intraperitoneally in mice was 650 mg/kg(Confidence limit: 894-1,473 mg/kg). There were no significant changes in serum biochemical values and histopathological changes as compared with those of control after intraperitoneal administration with rutin in rats.

Reference Intervals from Hospital-Based Data for Hematologic and Serum Chemistry Values in Dogs (병원자료에 근거한 혈액 및 혈액화학 검사항목의 참고구간 설정)

  • Reference interval is critical for interpreting laboratory results, monitoring response to therapy and predicting the prognosis of the patients in clinical settings. The aim of the present study was to update established reference intervals for routine hematologic and serum chemistry values for a population of clinically healthy dogs (range, 1-8 years) seen in an animal hospital. Blood was obtained by venipuncture while animals were physically restrained, and samples were analyzed for 9 chemistries on MS9-5H (Melot Schloesing Lab, France) and 6 hematology on Vet Test 8008 (IDEXX, USA). Data from 105 dogs (52 males and 53 females) for hematology and 113 dogs (37 males and 76 females) for chemistry were used to determine reference intervals using the parametric, nonparametric and bootstrap methods. Prior to analysis, all parameters were tested for normal distribution using Anderson-Darling criterion. Of the 9 biochemical analytes, alkaline phosphatase, alanine aminotransferase, aspartate aminotransferase, creatinine, total protein, and glucose concentrations did not fit normal distribution for both original and transformed data. All but eosinophil count satisfied normal distribution for either original or transformed data. Parametric method can be used for original cholesterol concentrations, RBC, WBC, and neutrophil counts. This technique can also be used for power-transformed values of blood urea nitrogen concentrations and for logarithm of lymphocyte and monocyte counts. Non-parametric or bootstrap method was the preferred choice for the remaining 7 biochemical parameters and eosinophil count as they did not follow normal distributions. All three statistical techniques performed in similar reference intervals. When establishing reference intervals for clinical laboratory data, it is essential to assess the distribution of the original data to increase the accuracy of the interval, and non-parametric or bootstrap methods are of alternative for the data that do not fit normal distribution.

Toxicity Screening of Single Dose of Inorganic and Organic Arsenics on Hematological and Serum Biochemical Parameters in Male Cynomolgus Monkeys

  • A screening study of the acute toxicity of organic arsenics such as arsenobetaine and arsenocholine, a product of arsenic methylation metabolite, and inorganic arsenic was carried out to examine hematological and serum biochemical parameters in cynomolgus monkeys(Macaca fascicularis). We found soft and liquid feces, and vomiting in all treated groups with inorganic and organic arsenics. The monkeys in inorganic arsenic-treated group showed a significant increase in vomiting frequency compared with those in three organic arsenics-treated groups. These results suggest that inorganic arsenic might be more toxic than three other organic arsenics tested. The monkeys in inorganic arsenic-treated group showed a decrease in platelet and an increase in monocyte on day 4 and the monkeys in arsenocholine-treated group showed an increase in reticulocyte percentage on day 8. The monkeys in inorganic-treated group also showed decreases in AST and ALT values and the monkeys in arsenobetaine-treated group showed a decrease in AST value and an increase in T-CHO value. However, these hematological and biochemical changes were within the physiological ranges, showing that the single dose of inorganic and organic arsenics did not affect at least hematological and serum biochemical parameters. The present study of toxicity with single dose of arsenics provides valuable indicators for longer term study of toxicity of repeated doses of arsenics in primates.

Hematological analyses of Japanese macaque (Macaca fuscata) housed in Everland Zoological Gardens (국내 동물원에서 사육 중인 일본원숭이의 혈액상)

  • Hematological and serum biochemical values were assessed from 20 clinically healthy Japanese Macaques raised in Everland Zoological Gardens and compared to the International Species Information System (ISIS) reference range that is used internationally as standard for wildlife animals. Taking our standard on sexual maturation at age 4, tRBC values in Macaques under age 4 were significantly lower than those over age 4, but the Hb and PCV values were significantly higher. Compared to the ISIS standard, the tRBC values in Macaques under age 4 were significantly lower whereas the Hb and MCHC values were significantly higher. Moreover, in the samples of Macaques over age 4, the PCV and MCV values were significantly lower than the ISIS standard. On serum biochemistry values the creatinine and amylase values in the Macaques under age 4 were significantly lower than those over age 4. In comparison with the ISIS standard, the values of ALT, ALP, BUN, IP, $Ca^{2+}$ and $K^+$ in the Macaques under age 4 did have no significant difference. The values of TP, GGT, tBil, amylase, TG and UA were significantly higher than the ISIS standard, but the values of albumin, AST, glucose, creatinine, cholesterol, CPK, LDH, $Na^+$ and Clwere significantly lower. In contrast, the values of TP, albumin, ALT, ALP, creatinine, cholesterol, amylase, TG, IP and $Na^+$ in the Macaques over age 4 did have no significant difference, but the values of GGT, BUN, tBil, UA and $Ca^{2+}$ were significantly higher, while the values of AST, glucose, CPK, LDH, $K^+$ and $Cl^-$ were significantly lower. On the other hand, there was no significant difference in hematological and serum biochemical values between the groups of male and female.

Utility of Serum and Urine uPAR Levels for Diagnosis of Breast Cancer

  • Malignant tumors have a capacity to degrade the extracellular matrix by controlled proteolysis. One system involved in these processes is the urokinase-type plasminogen activator (uPA) system. uPAR levels are elevated in tumors from several types of cancer. Our study was planned to investigate serum and urine levels of uPAR in breast cancer patients (n=180) and healthy controls (n=60) by ELISA. Serum (p<0.001) and urine (p<0.001) uPAR values in the patients were both significantly elevated. High serum and urine levels of uPAR can be used as diagnostic tools in lymph node positive patients.

Comparison of Nutrient Intakes, Bone Metabolism and Risk Factors of Chronic Diseases between Postmenopausal Lacto-Ovo-Vegetarian and Omnivores in Kyunggi-Do (일부 폐경기여성의 채식군과 일반식군의 영양섭취상태, 골대사 및 만성 퇴행성 질환의 위험인자에 관한 비교연구)

  • The amount and type of dietary protein that effects affect bone mineral loss after menopause and different dietary patterns may influence anthropometric measurements, biochemical values, and bone metabolism. Objective : Our objective was to investigate bone metabolism and chronic disease factors in two different dietary patterns. Design: The sample consisted of 40 lacto-ove-vegetarians(LOV) and 49 omnivores aged from 47 to 85 years in good condition without any medical problems. Results : The average energy intake of LOV was 1414.0kcal and omnivores, 1403.2kcal. The difference between means was not statistically significant. The differences of means of body weight and BMI between the two groups were not significant but the systolic blood pressure of LOV group was significantly lower than omniovores(p<0.05). The values of serum cholesterol(p<0.05) and glucose(p<0.05) of LOV were signifcantly lower than that of omnivores. The serum albumin level(p<0.01) of LOV was lower than it'scounterpart but in a normal range. The intake of crude fiber(p<0.05) of LOV group was significantly higher. The urinary pH of LOV was 7.4 and that of omnivores was 6.3(p<0.001). Plant calcium(p>0.05) and plant iron(p<0.001) had positive correlations with the uninary pH but animal protein(p>0.05) and animal iron(p>0.05) had negative correlations with the urinary pH. The level of urinary biochemical marker of bone resorption, deoxypyridinoline(DPD) of LOV was significantly lower than the other group(p>0.05). The T-scores of bone mineral density for the two sites in the two groups were not significantly different. Conclusions : The DPD level and urinary pH value of LOV were more desirable for the bone metabolism that the omnivores. The diet of LOV had lower risk factors of chronic diseases than the other group.

Effect of Strategic Feed Supplementation during Gestation on Intake, Blood-biochemical Profile and Reproductive Performance of Goats

  • Effect of strategically supplemented pregnancy allowance was ascertained during different phases of gestation on the reproductive performance of does. Gravid does (18) were allotted to 3 dietary treatments HH, HL and LH in a completely randomized block design. All does were provided wheat straw ad libitum and supplemented with concentrate mixture at the rate of $20g/kgW^{0.75}$ from 0-60 days post-mating. Subsequently, HH group was given concentrate at the rate of $40g/kgW^{0.75}$ from 61 d post-mating to term. HL group was offered concentrate from 61-90 d post-mating and 121d post mating to term at the rate of $20g/kgW^{0.75}$ and from 91-120 d post-mating at the rate of $40g/kgW^{0.75}$. LH group was provided with concentrate from 61 to 120 d post-mating and 121 to term at the rate of 20 g and $40g/kgW^{0.75}$, respectively. Mean total dry matter and concentrate intake ($g/kgW^{0.75}$) was significantly higher on HH in comparison to comparable intake of HL and LH treatments, however, wheat straw followed the reverse trend. Haematological and biochemical parameters except serum glucose, total serum protein and A:G ratio did not differ significantly among dietary treatments. The gross gain in weight, products of pregnancy at 21 weeks of gestation, pregnant does average daily gain, birth weight of kids and survivability of kids were significantly lower in treatment HL in comparison to comparable values for HH and LH treatments. The net gain in gravid does was significantly higher in treatment HH than the comparable values obtained in HL and LH treatments. The last month of pregnancy was found to be nutritionally most sensitive period of gestation for native goats.

Clinico-biochemical Study on Experimental Partial and Complete Obstruction of the Common Bile Duct in Korean Goats (한국염소에서 실험적 총담관부분 및 완전폐쇄에 따른 임상생화학적 연구)

  • Clinical signs, serum chemical values and histological findings of hepatic tissue after partial and complete obstruction of common bile duct in Korean goats were investigated. Abnormal clinical signs were not observed in partial obstruction of common bile duct, but in complete obstruction clinical signs such as jaundice, urine color change, were observed. Serum total bilirubin, total cholesterol, aspartate aminotransferase, sorbitol dehydrogenase, gamma glutamyltranspeptidase, and total protein values increased on the 1-4th day and then gradually decreased to normal level in partial obstruction. However, they tend to increase persistently by the 24th day in complete obstruction of common bileduct. Histologic features of hepatic tissue in partial obstruction were not changed as compared with normal hepatic tissue. On the other hand, in complete obstruction of common bite duct there were moderate bile duct proliferation in a portal area, rupture of bile canaliculi, phagocytosis of bile pigment by Kupffer cells, periportal fibrosis, intrahepatic bile stasis and hepatic cell necrosis.
