Design of Secant Pattern Radiator with Septum Circular Polarizer (격벽 구조형 원형 편파기의 시컨트 패턴 방사체 설계)

  • In this paper, we design a secant pattern antenna combined with a septum circular polarizer to be used in the satellite data communications. To generate a radiation pattern with secant type, Fourier relationship between current distribution and space factor is applied and the circular symmetric structure of the aperture simplifies the design. The optimization process is performed by combining the radiator with the septum circular polarizer, and it is confirmed that the designed radiation pattern, axial ratio, and return loss characteristics satisfies the required performance of the antenna.

  • A complete septate uterus with cervical duplication and a longitudinal vaginal septum is a rare uterine malformation. The dissection of the septum can be difficult because it is difficult for hysteroscopists to find out initial point, direction and final point of a complete septum. This study aimed the introduction of more efficient surgical procedure using a balloon with methylene blue. We have experienced three cases with a complete septate uterus. We performed hysteroscopic dissection of a complete uterine septum using a balloon with methylene blue and obtained good reproductive outcomes. So we report three cases with a brief review of literatures.

Cor triatriatum(A case report) (삼중방심 치험 1예)

  • Cor triatriatum is a rare congenital malformation of the heart, in which a septum stretches in a transverse or oblique plane through the left atrium, thus separating it into two compartments. The upper one connects with the pulmonary veins, and the lower one connects with the left ventricle. Due to the rarity of, and great difficulty in-diagnosing, cor trlatrlatum, data On the surgery of this disease are of necessity very limited and so accurate pre-operative diagnosis was very difficult to make. `We experienced a case of the cyanotic congenital heart disease which was diagnosed as a large atrlal septal defect with streaming venous blood from inferior vena cava to left atrium through atrial septal defect in August, 1982. We found that there were transverse septum in the left atrium through atrlal septal defect, the pulmonary venous drainage were located in the upper chamber of the left atrium, and the lower chamber was connected with the left atrial appendage, mitral valve and `left ventricle. But our case had not any opening in this transverse septum and the right atrium was connected with the upper chamber of the left atrium through the upper part of the atrlal septal defect, and was communicated with the lower chamber of the left atrium through the lower part of the atrlal septal defect. We excised the transverse septum and repair this atrial septal defect with Woven Dacron patch accompanying with the drainage of coronary sinus to right atrium. The post-operative course was not eventful and he was discharged with good result on the post-operative 8th day, and has been in good .condition up to now for longer than 10 months.

Computerized Tomographic Study on the Anatomic Variation of the Paranasal Sinus (전산화단층사진을 이용한 부비동의 해부학적 변이에 관한 연구)

  • Objectives : To evaluate the anatomic variations of the paranasal sinuses on computed tomographs. Materials and Methods : The author examined the CT images of the paranasal sinuses retrospectively in 500 patients who visited Chonbuk National University Hospital between January 1996 and December 1997. Results : The highest incidence of anatomic variation of the paranasal sinuses in bilateral structures was agger nasi cel1(73.2%), followed by concha bullosa(31.1%), Onodi cell(24.0%), Haller ce1109.8%), maxillary sinus septum(3.0%), paradoxical middle turbinate(2.5%), pneumatized uncinate process(2.0%), and bent uncinate process. The highest incidence of anatomic variation in midline structures was nasal septum deviation(53.2%), followed by nasal septum aerated(29.4%), bulla galli(24.7%) asymmetric intersphenoid septum(22.3%), and nasal septum spur(13.8%). The correlation between anatomic variation and paranasal sinusitis was not found. Conclusions : The results of this study will aid in the diagnosis and treatment of paranasal sinus diseases, especially in the treatment planning before functional endoscopic surgery.

Correction of Deviated Nose: Importance of Rasping of Non-affected Side Nasal Bone Flap and Trimming of Non-affected Side Dorsal Septum ("C"자형 만곡비변형의 교정: 건측 비골판줄질(Rasping of Nasal Bone Flap)과 건측 비배측 비중격절제술(Trimming of Dorsal Septum)의 중요성)

  • A total of 21 patients were operated. Via a columellar labial incision, the upper lateral cartilages were separated from the septum. A submucous resection of the septal cartilage was carried out. After rasping the convex lateral nasal wall of the unaffected side, a low-to-low lateral nasal osteotomy was conducted. Along the deviated dorsal line at the bony vault passing the submucous tunnel, a paramedian nasal osteotomy was performed. The convex side of the nasal bone flap was contoured by rasping. The convex side of the "T"-shaped dorsal septum was trimmed. A total direct septal extension graft of the septal cartilage was done and the alar cartilages were suspended to it. The postoperative results were evaluated by photogrammetric analysis processed by a "neon glow" filter in Adobe Photoshop. The distance from the nasal midline to the most deflective point at 5 levels was measured, and the proportion indices were obtained in regard to intercanthal distance. The results revealed improvement in all levels (p < 0.05), although not perfect. In summary, this technique can result in a clinically good-looking in spite of a slight deflection that still exists

Identifying the more suitable nostril for nasotracheal intubation using radiographs

  • Background: One nostril must be selected for nasotracheal intubation. In some cases, structural anomalies within the nasal cavity hinder the insertion of the tube or complications, such as epistaxis, develop. This study examined the possibility of using radiography to select the nostril that would induce fewer complications. Methods: Four hundred and five patients who underwent nasotracheal intubation under general anesthesia were studied. A 7.0-mm internal diameter nasal right angle endotracheal (RAE) tube and 6.5-mm internal diameter nasal RAE tube were inserted into men and women, respectively. Complications were considered to have developed in cases in which insertion of the tube into the nasal cavity failed or epistaxis occurred. The tube was inserted into the other nostril for insertion failures and hemostasis was performed in cases of epistaxis. The degree of nasal septal deviation was determined from posteroanterior skull radiographs or panoramic radiographs; the incidence of complications was compared depending on the direction of the septal deviation and the intubated nostril. Results: The radiographs of 390 patients were readable; 94 had nasal septum deviation. The incidence of complications for cases without nasal septum deviation was 16.9%, that for cases in which the tube was inserted into the nostril on the opposite side of the deviation was 18.5%, and that for cases in which the tube was inserted into the nostril with the deviation was 35.0%, showing a high incidence of complications when intubation is performed through the nostril with septum deviation (chi-square test, P < 0.05 ). Conclusions: Although there were no differences in the incidence rates of complications between intubation through the left nostril and that through the right nostril, radiological findings indicated that incidence of complications significantly increased when the tube was inserted into the nostril with the septum deviation.

Computed Tomographic Evaluation of the Nasal Septum Deviation in Clinically Normal Dogs (정상 개에서 컴퓨터단층촬영을 이용한 비중격 변위 평가)

  • This study was performed to determine a comprehensive anatomic variation and deviation of the nasal septum in clinically normal dogs by use of computed tomography (CT). The nasal septum was evaluated in 36 shihtzu dogs and 16 beagle dogs. A degree of a septal deviation was evaluated by measuring a maximum angle and a distance of the septal deviation. The angle and distance of the septal deviation were significantly higher in shih-tzu dogs than in beagle dogs. There was no significant relationship between body weight and age and all measurements (angle and distance) in both breed dogs. Results of this study suggest that nasal septum deviation and its defect are observed on CT images of clinically normal dogs, especially in shih-tzu dogs.

Myoepithelioma of the Nasal Septum: A Case Report (비중격에 발생한 근상피종 1례)

  • Myoepithelioma is a rare benign neoplasm that mostly arises in the major salivary glands and sometimes in the minor salivary glands, which account only for less than 1% of all salivary glands neoplasms. However, its extra-salivary involvement is even rarer and only a few cases of nasal cavity myoepithelioma were reported in the English-language literature so far. In this case report, we present a 40-year-old female with unilateral nasal obstruction diagnosed as myoepithelioma of the nasal septum and treated with endoscopic sinus surgery.

  • 본 논문에서는 T형 분기 도파관 내 Septum의 최적설계를 위하여 상용 전자파해석 소프트웨어의 API(Application Program Interface) 기능을 활용하는 VBA(Visual Basic Application) 기반 설계 인터페이스를 개발 하였다. 최적설계의 설계 변수는 도파관 내에서 Septum의 위치와 크기로 하였고, 목적 함수는 도파관의 반사계수($S_{11}$)를 가장 작게 하면서 T형 분기 도파관에서 출력되는 에너지를 2:1로 분배하도록 설정하였다. 목적함수는 본 논문에서 정한 함수이며 설계 목표에 따라 인터페이스 내에서 변경이 가능하다. 설계 인터페이스는 마이크로소프트사의 엑셀을 이용하였고 인터페이스 내에서는 사용자가 원하는 T형 분기 도파관의 크기 설계가 가능하며, 입력 port의 입력 모드를 설정하고, 해석 주파수를 선택하여 사용자가 정한 목적 함수에 맞는 최적화된 Septum의 크기와 도파관 내에서의 위치를 찾을 수 있게 하였다. 또한 사용자가 원하는 위치에서의 E Field, H Field, 전류 밀도를 크기 또는 복소수 형태의 수치 데이터로 획득할 수 있도록 설계하였다.

A Giant Hydatid Cyst in the Interventricular Septum with Papillary Muscle Involvement

  • We present here a 44-year-old male patient with hydatid disease who was referred to our hospital due to dyspnea and chest pain for the last 2 month before admission. Using echocardiography and contrast-enhanced computed tomography the heart hydatid was diagnosed. However, hydatid disease of the interventricular septum is rare; particularly, the involvement of mitral apparatus with mitral regurgitation (MR) is an exceptionally rare presentation. Early diagnosis and an integrated treatment strategy are crucial. Surgical excision was performed and the patient had an uneventful recovery and follow-up at 3 months.
