Regional Differences in Access to Clinical Trials for Cancer in Korea

  • Purpose: The ability to access clinical trials for cancer treatment is important. This study investigated whether regional differences exist in oncologic clinical trial protocols conducted in South Korea. Methods: Records of all approved oncologic clinical trials conducted in 2019 were downloaded from the Republic of Korea Ministry of Food and Drug Safety. The study covered Seoul, the capital area, other metropolitan cities, and provincial areas. Descriptive statistics summarized the distribution patterns of clinical trials by region. Results: A total of 202 oncologic clinical trials were conducted in 63 institutions in 2019. Of these protocols, 186 (92%) were available in Seoul, 120 (59%) in the capital area, 64 (32%) in metropolitan cities, and 66 (33%) in provincial areas. More regional differences in protocol availability were observed in domestic trials, investigator-initiated trials, phase 1 and 2 trials, and smaller-scale trials. Conclusion: Most oncologic clinical trials were conducted in medical institutions located in Seoul, with the rest conducted in the capital area, metropolitan cities, and provincial areas. The findings reveal clear differences in protocol availability between Seoul and the other regions. Measures designed to improve geographical access to oncologic clinical trials may be needed given their growing importance in cancer treatment.

지방 대도시권 광역전철 구축 전략 (A Strategy to Build a Local Wlde-city Metropolitan Transportation Electric Railway System)

  • Now, the population and economical power is deeply concentrated to Seoul-metropolitan region, therefoere the provinces is so lagging behind and the country cannot get balance development. And, like metropolitan region, the more outer the living area of local wide-area city is spreading, the more serious wide-region transportation problems are. But the railroads near wide-area city fun seldom commuter train, so they cannot absorb the transportation demand of wide-area city metropolitan region. To solve above transportation problem, local wide- area cities try to make a subway or a light rail transit. But it is so difficult because the limit of public finance and original small capacity of light rail. In the paper, I propose 'Local city wide-area electric railways' system of the consortium of 'Local wide-city and province transportation union', Korean National Railroad (KNR) and local capital. A 'Local wide-city and province transportation union' operates commuter railways directly using the facilities of KNR, and local capital invests to the stations of above railways system. Therefore the commuter railway system around local wide-city region will be constructed quickly and the stations will be the strong point of local development and investment.

사회적 자본과 자살충동 (Social Capital and Suicidal Impulse)

  • Objectives: This study examines the relationship between social capital and suicidal impulse for the Metropolitan Seoul residents, paying particular attention on individual and area level risk factors using multilevel analysis techniques. Methods: Data are obtained from the 2005 Seoul Health Indicators Survey and the Seoul Survey. At the individual level, we analyze demographic variables including sex, age, marital status, educational attainments, family income, field of economic activities, having close friends or relatives, and having participating groups or organization. To measure the area level social capital, happiness index, attitude toward minority, the percent of volunteer activity, the percent of having close friends or relatives, and the percent of participating groups or organization are considered. A multilevel logistic regression analysis is conducted utilizing the SAS 9.1. Result: Results show that non-aged, female, not currently married, unemployed individuals tend to have relatively high motivation of suicide. Controlling for individual level covariates, the relationship between social capital and suicidal impulse is neither significant nor substantial. Further, the consideration of area social capital does not explain the variances in suicidal impulse across 25 Kus. Conclusion: Suicidal impulse among Seoul residents is mainly attributable to the composition of individual characteristics but little to the contextual social capital.

한국사회 빈곤구조의 지역 편차 분석 - 수도권과 지방의 빈곤 격차를 중심으로 - (Regional Variations of Poverty in Korea -How are Capital and Metropolitan Area Different from Non-Capital and Non-Metropolitan Area?-)

  • 빈곤은 개인 및 가구의 특성 뿐 아니라 거주 지역의 특성에 의해서도 규정되는 현상이라는 점에 주목하여, 본 연구는 수도권과 지방의 빈곤 격차의 양상 및 빈곤 결정에의 지역(수도권과 지방) 효과를 실증 분석하였다. 한국복지패널 2차년도 자료를 활용하여 수도권과 지방의 빈곤격차를 분석한 결과 다음과 같은 사실을 확인할 수 있었다. 첫째, 절대 빈곤 뿐 아니라 상대 빈곤 측면에서 빈곤은 수도권보다 지방에 편중되어 있었다. 지방을 보다 세분화하여 분석한 결과에서는 지방 대도시보다는 지방 중소도시에서, 지방 중소도시보다는 지방 군 지역에서 빈곤이 더욱 심각한 것으로 나타나, 지역 빈곤은 지역 발전 정도와 밀접한 관련이 있었다. 둘째, 수도권과 지방의 빈곤 분포를 보다 상세하게 분석한 결과, 동일한 인구학적 특성을 가진 경우라 할지라도 지방에 거주할 경우 빈곤율이 현저히 높게 나타나, 빈곤 구성에서 지역간 차이가 두드러짐을 확인할 수 있었다. 셋째, 빈곤결정에 거주지역(수도권/지방)의 영향을 분석한 결과, 지방 거주가 빈곤 확률을 높이는 지역 효과가 있음을 확인할 수 있었다. 넷째, 수도권과 지방의 빈곤 격차를 거주자의 특성 효과와 지역 효과(계수효과)로 분해한 결과, 빈곤 격차의 $49.2{\sim}73.7%$는 지역 효과에 의거한 것으로 확인되었다. 수도권과 지방의 빈곤 격차에 대한 본 연구의 분석결과는 한국사회의 빈곤문제 해소를 위해서는 지역적으로 차별화된 빈곤대책이 필요하다는 것을 보여주고 있다.

토지환경성평가의 이론 및 기준·지도작성에 관한 연구 (Theoretical Review of Environment-Oriented Land Suitability Analysis and Setting of EOLSA Criteria and Classification System)

  • The objectives of this study are to build up the concept of Environment-Oriented Land Suitability Assessment(EOLSA) and to develop the EOLSA mapping system by applying the EOLSA criteria to the case study area. In order to draw out the EOLSA critera, this study adopted the Delphi method including the experts' awareness survey to urban planners as well as environmental researchers in May and June 2001. As a result, the concept of EPLSA was defined as a process of land use planning to scientifically assess the physical and environmental value of land and to classify conservation aptitude into several grades for the sustainable management of environmental resources. With an outcome of applying the EOLSA criteria with five degrees to the Seoul Metropolitan Area (SMA), Grade I, indicating the highest conservation value, accounted for 57.76% of the SMA. Then, Grade II reached up to 15.06%, Grade III 3.12%, Grade IV 15.92%, and Grade V, the lowest conservation value, 7.99% respectively. And also, the case analysis showed that the share of Grade I area was the highest in Gapyong county and Yangpyong county, Pochon county, Yeonchon county, Yongin city in the order and the lowest in Kwangmyong city, Osan city, Kunpo city, Kuri city, and Buchon city.

서울 近郊 傳統 農村의 變化 (The industrialization and the change of social structure of the traditional rural villiage in the Seoul Metropolitan suburbs)

  • This study intends to describe the characteristics of the transformation of the rural society by tracing the changing social and economic processes of traditional clan villiage (Yeolmi-ri, Silchon-myeon, Kwangju-gun, Kyunggi-province) in the suburbs of Seoul Metropolitan Area. Rural society has experienced serious change of the social structure by the ever expanding urbanization and industrialization since 1960's. The suburbs of Seoul Metropolitan Area is the representative case area of such transformation. This study analyzed various chracteristics of population structure of a villiage, the processes of residential movement and the occupational shift of each household of the villiage in terms of household's life-history collected by interviewing with each household. And the whole households of a villiage is divided by two groups of the native clan group and the non-dan in-migrant group. The results of this study are summarized as follows. 1) This villiage is located within a lineal distance of 40km and a time distance of about 30 to 40 minutes, adjoined dy the Joongbu(중부) national highway, the Kyungchoong (경충) industrial road, the local road between Yangpyuna(양평) and Gonjiam(곤지암) and having a advantageous iocational linkage to Seoul capital city. 2) It is a typical traditional dan villiae constituted of dan family Gu(구) one of whose ancestors had appointed to the prime minister of Chosun Dynasty, and until 1979, 56 out of 60 families of the villiage households were clan families. 3) Since 1979, as the villiage invaded by the out-movement of the small scale manufacturing industries from Seoul capital city, labours moved in this vallige from outside and increased villiage size up to 203 households in 1993, consequently the constitutional proportion between the native clan group and the non-clan in-migrant group was reversed up to 28% vs 72% deepening the sociological heterogeneity. 4) Because of the small scale of industries in this villiage and the vicinity with the city of Seoul, the population turnover of this villiage is very high. The turnover frequency is firstly high within same administrative district of gun(군) level and secondly high between this villiage and the city of Seoul. Thus short-distance movement is identified. 5) There is a close correlation between the residential movement and the occupational shift. The most numbers of non-farm native group have the experiences of migration while the members of native farming group don't shift both the job and the place of residence. The inmigrant group had several migration experiences but few occupational shifts. Thus occupatioanl shift is high for the native group while residential shift is high for the in-migrant group. It is concluded that the change proceeded in Metropolitan suburbs was not directed to form the traditional rural village as 'gesund Gebiet' with respect to the ballance among the economic, cultural and environmental adequency.

서울대도시권역의 설정과 지역구조에 관한 연구 (A study on the establishment and regional strunture of Seoul metropolitan region)

  • 본 논문은 지속적인 서울시의 성장이 그 주변지역의 성장을 수반하면서 서울대도시 권화되어가는 과정을 밝히고 서울대도시권역을 설정한 후, 서울대도시권역 내에서 기능적 특성에 따라 지역을 분류하려는 것이 그 목적이었다. 1990년 시점에서의 서울대도시권역의 경계가 어느 정도 확대되었는가를 측정하기 위하여 도시화 정도를 측정하는 지표와 서울과 의 기능적 연계성을 측정하는 지표를 토대로 하여 그 권역을 설정하였다. 그 결과 서울대도 시권역은 수도권의 거의 전 지역을 포함하고 있는 것으로 나타났다. 또한 서울대도시권역에 포함된 지역들은 서울이 갖고 있는 기능을 일부 분담하면서 기능적인 면에서 특화되거나 전 문화되어 기능적 특성에 따라서 신종주거기능 탁월지역, 제조업기능 탁월지역, 서비스기능 탁월지역, 잠재적 개발가능지역의 4개 그룹으로 지역을 분류할 수 있었다.

신 행정수도 건설의 논거와 과제 (Arguments and Some Issues to be Considered for Building the New Administration Capital City in Korea)

  • 신 행정수도 건설은 지난 16대 대통령선거에서 노무현 후보가 당선됨으로써 참여정부의 핵심 국정과제로 부상했다. 그러나 신 행정수도 건설에 대한 회의론 내지 반대론을 제기하는 사람들이 적지 않다. 신 행정수도 건설의 실현가능성에 의문을 품는 사람들, 통일 이후 신 행정수도 건설을 고려하자는 신중론자들, 그리고 신 행정수도 건설이 수도권 문제 해결과 지역균형발전을 위한 적절한 정책수단이 아니라는 반대론자들이 있다. 이런 상황에서 참여정부의 일차적 당면과제는 회의론자들과 반대론자들을 설득해 신 행정수도 건설에 대한 국민적 지지기반을 넓히는 일이다. 이를 위해 본 논문은 신 행정수도 건설의 논거를 제시하고 이 공약의 성공적 이행을 위해 고려해야 할 몇 가지 과제들에 관해 논의한다. 먼저, 수도권 집중의 실태와 폐해를 살펴보고, '전국이 고루 잘 사는 나라'의 비전을 스위스 사례를 들어 제시한 다음. 역대 정부의 지역균형발전정책 및 지방분권정책의 성과를 점검한다. 이어 신 행정수도 건설의 의의가 '서울 존(尊) -지방 비(卑)' 의 서울주의 차별문화를 청산하고, 서울을 하나의 지방으로 인식시키는 국토평등화의 계기를 마련하며, '분권분산형 국가'와 '동북아 허브(Hub)국가' 건설을 추동하는 데 있음을 강조한다. 더불어 신 행정수도가 통일한국의 항구적 행정수도로 건설되어야 할 이유를 논변한다. 마지막으로, 신 행정수도 건설에 수반된 몇 가지 과제들. 즉. 국민의 동의 절차. 신 행정수도의 인구규모, 신 행정수도의 법적 지위, 신 행정수도 건설과 지방분권 및 지방혁신의 관계 등에 대한 필자의 견해를 밝힌다.

수평적 형평화 기금에 의한 지역균형발전전략 연구(II) (A Study on the Balanced Regional Development Strategy through the Horizontal Equalization Development Fund (II))

  • 본 논문에서는 지금까지 대부분의 지방재정 논의가 중앙정부를 중심축으로 하는 지방재정의 재정분권과 형평화 효과를 중심으로 이루어지고 있다고 보고 대안적으로 수도권과 비수도권의 상생발전을 위한 수평적 형평화 기금 조성방안을 제시한다. 먼저 지역 간 재정기반의 불균등을 분석하고, 형평화 기금 조성을 위한 부동산관련 조세적 방법, 개발이익 및 개발부담금 등을 활용하는 비조세적 방법, 공동세 방법, 수도권 규제조정 및 용도지역 변경에 따른 이익활용방법을 분석한다. 마지막으로 형평화 기금의 운용을 위한 사회적 협약과 계약이론에 기반 하는 거버넌스 전략을 제시한다.

The Longitudinal Trend of Cardiac Surgery in Korea from 2003 to 2013

  • Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.
