The Story of Serratia Marcescens: Pathologic Risk Factors in Breast Implant Surgery

  • Serratia marcescens (S. marcescens) emerged as an opportunist in the setting of immunodeficiency in the 1970s, when serious infections occurred in San Francisco hospitals after USA. Navy experiments had aerosolized the bacteria to study biologic warfare. We investigate the risks of S. marcescens in San Franciscans who undergo mastectomy with implant reconstruction. From 2007 to 2011, the senior author took breast capsule cultures for all patients at the time of tissue expander exchange/explant. Of the 142 women who had reconstruction, 23 had positive cultures. Only the two patients who were positive for S. marcescens developed clinical infections that required explantation. Both had postoperative chemotherapy with transient neutropenia, and both had close ties to San Francisco. Clinical signs of infection emerged for both patients months after initial surgery, despite having previously well healed incisions. Other patients were culture positive for Pseudomonas, Proteus, Enterococcus and MRSA and did not develop require explant. While the link between San Francisco and S. marcescens is controversial, a patient's geography is a simple screening tool when considering postoperative risks, especially in the immunocompromised. Closer monitoring for neutropenia during chemotherapy, and a lower threshold to administer S. marcescens targeted antibiotics may be warranted in these patients.

학교시설개선을 위한 평가지표개발에 관한 연구 (A Study on Developing the Standardized Index of Evaluation for Upgrading the School Facilities)

  • The demographic and environmental changes such as the decrease of the population growth rate and the increase of the population of the senior citizens, which become issues of the modern society, are bringing numerous changes of many aspects like national economy, community, and culture. These social phenomena give impacts on the school facilities as well. Thus owing to the decrease of the number of the students, the number of the newly constructed schools has been decreased drastically, and moreover the merger of the schools cause many schools to be default. Meanwhile, the school facilities have been expanded into the functions of the local community centers, and the various educational activities demand the diversity and flexibility of the spaces. These factors add more burdens to the current schools. Therefore the school facilities are needed to adopt a flexible and relevant change necessary for using the spaces in accordance with the changed needs of the times, and are needed to reinvented continuously not to limit the educational and social activities because of the degradation of the school facilities. Yet because there are no proper criteria for doing the task, problems related to the relevance of the task and the unnecessary investment are raised. So through this study I tried to demonstrate the standardized index in terms of the sequential priority of upgrading the facilities among the schools.

전국 권역별, 연령별 여자 성인과 노인의 짠맛 미각판정치, 식태도 및 식행동 비교 (A Comparison of Salty Taste Assessment, Dietary Attitude and Dietary Behavior among Adult and Senior Women by Region and by Age in Korea)

  • Objectives: The purpose of this study was to compare and analyze the results of salty taste assessment, dietary attitudes, and dietary behaviors among adult and senior women by region and by age. The results generated from this was expected to provide fundamental data for implementing a nationwide salt reduction education program. Methods: The salty taste assessment tool was applied to 4,064 subjects from 15 areas in Korea. Also, a survey of dietary attitude and dietary behavior related to salt intake was conducted for all subjects participated in this study. Results: The salty taste assessment scores by region and by age were the lowest in capital (p < 0.01) and was highest among the 70+ year age group (p < 0.01). The dietary attitude scores and dietary behavior scores showed that Gyeongsang was the highest (p < 0.001) and the capital was the lowest. The dietary attitude scores were highest at 20&30's followed by 40's and 70's group (p < 0.001). Dietary behavior scores showed that 20's~30's and 40's groups were higher than the other age groups (p < 0.001). The score of 'I like kimchi' was $3.46{\pm}0.88$, which was the highest among 10 dietary behavior questions. The score of 'I eat a lot of kimchi' was $3.30{\pm}0.90$, which was the highest among 10 dietary behavior questions related to salt intake. The scores of salty taste assessment had significant positive correlations between the scores of dietary attitude (p < 0.001), dietary behavior (p < 0.001) and self-awareness (p < 0.001). Conclusions: Based on the results of salty taste assessment, we observed a tendency that older people and regions except the capital had higher preference for salty taste. Our results suggested the necessity for a nationwide salty reduction education program tailored for regions and for different age groups.

일개 상급종합병원 의사들의 환자안전문화에 대한 인식 분석 (Analysis of doctors' cognition of patient safety at general hospitals)

  • 본 연구에서는 의료기관의 환자안전문화를 이해하고, 의사들의 환자안전문화에 대한 인식분석을 위한 기초자료로 활용하고자 시도하였다. G시에 소재한 상급종합병원 의사를 대상으로 2011년 8월 1일 부터 9월 5일까지 설문조사하여 194부를 최종분석 하였다. 연구결과는 첫째, 대상자와 병동 및 병원안전문화 간의 관계는 직원배치가 성별, 연령별, 병원 근무년수, 환자 접촉여부, 1주 근무시간에 따라 인식의 차이가 있으며, 조직학습과 병동내 팀워크는 1주 근무시간에 따라, 병동 안전문화 모든 하부영역은 진료과별로 유의하게 나타났다. 둘째, 대상자와 의료사고보고체계, 환자 안전도 평가 및 전반적 의식수준과의 관계에서는 사고에 대한 피드백과 의사소통, 사고빈도보고, 전반적 안전의식이 진료과별로 유의하였으며, 전반적 안전의식은 환자접촉과 1주 근무시간에 따라 유의한 결과를 나타냈다. 셋째, 병동 및 병원 안전문화 인식 하부영역과 전반적 인식, 환자안전도 평가는 모두 정의 상관관계가 있는 것으로 나타났다. 의료사고 보고체계와는 직속상관/관리자의 태도와 병원경영진의 태도를 제외한 모든 영역에서 정의 상관관계가 있는 것으로 나타났다. 넷째, 환자안전도 영향을 미치는 환자안전문화 하부영역은 조직학습, 의사소통의 개방성, 전반적 안전인식, 부서간의 협조체계, 피드백과 의사소통, 비처벌적 대응에서 유의한 결과를 나타냈다. 결론적으로, 의사들의 병동 및 병원 환자안전문화 수준을 높이고 의료사고보고체계를 충실하게 하기 위해서는 적절한 직원배치와 근무시간을 바탕으로 병동 내 조직적 학습을 통한 팀워크을 활성화 시키고, 부서간 팀원간의 개방적 의사소통과 사고에 대한 피드백을 제공하여 환자 안전에 대한 병원경영진의 적극적인 지원과 진료과별 협조체계 구축이 필요하다.

간호사의 태움 개념분석 (Concept Analysis of Tae-um in Nurses)

  • 본 연구는 간호사의 태움 개념분석연구로 개념의 속성을 확인하고 명확한 개념 정의를 내리고자 시도되었다. Walker and Avant의 개념분석 방법을 사용하였다. 간호사의 태움의 속성은 조직문화, 피해자인 미숙한 신규 간호사, 가해자인 선임 간호사, 가르침의 변질, 언어적, 신체적 폭력경험, 심리적, 신체적 증상으로 도출되었다. 간호사의 태움의 선행요인은 간호업무 과다, 높은 노동 강도, 직무 스트레스, 인력 부족, 열악한 근무 환경, 신규 간호사의 업무 능력이 기대에 미치지 못함, 조직의 위계질서, 힘의 불균형이었으며 결과는 신체적, 심리적 증상, 간호업무 효율 저하, 불신 증가, 이직의도 증가, 환자 간호의 질 하락으로 나타났다. 간호사의 태움의 선행요인을 해결하기 위해 활동 간호인력 확충을 위한 근로조건 개선, 사회적지지 프로그램 제공, 의사소통과 상호협력 관계를 지향하는 공동체 문화가 조성되어야 한다. 이후 간호사의 태움에 대한 더 많은 질적 연구와 태움의 속성이 잘 반영된 도구개발 연구, 태움 문화 개선을 위한 프로그램 개발 연구를 제언한다. 본 연구는 태움의 선행요인을 제시하여 해결방안을 제안함으로써 신규간호사 적응 프로그램을 위한 이론적 근거를 제공했다는 점에서 의의가 있다.

MBC '백종문 녹취록' 사건으로 본 공영방송의 위기 정치권력의 언론 통제 기제를 중심으로 (A Crisis in Public Broadcasting of South Korea A Perspective from the Case of the So-called "Paik Jong-moon's Taped Conversation" at MBC with a Focus on the Press Control by Political Power)

  • '백종문 녹취록' 사건은 '언론 자유와 민주주의에 대한 중대하고 심각한 도전'이다. 그럼에도 불구하고 이 사건은 주류언론이 보도하지도 않고, MBC의 관리 감독기관인 방송문화진흥회나 방송통신위원회 뿐만 아니라 국회에서도 사실파악이나 진상규명도 되지 않은 채 잊혀가고 있다. 본 연구는 그 원인이 무엇인지를 규명하기 위해 언론사의 담당 기자, PD, 원로 언론인, 방송규제기관의 전 현직 위원들, 그리고 언론계 전문가들을 대상으로 심층인터뷰와 문헌 연구를 진행하였다. 그 결과 연구자는 첫째, 주류언론의 무보도 현상의 원인으로 1) 주류언론의 정파성, 2) 공영방송사의 권력 감시 기능의 무력화 등이 주요원인임을 확인할 수 있었다. 둘째, 방송통신위원회와 방송문화진흥회, 국회에서 여당 측 다수파나 여당의원들이 진영논리로 야당 측 위원들이나 의원들이 제기한 백종문 본부장의 '불법 해고', '불법 편성 제작 개입', '부당 채용', '부당 거래' 등의 의혹에 대한 사실 파악 진상규명 요청을 다수결의 원칙에 의해 기각시켜왔기 때문이었다. 셋째, 공영방송사 조직 내 대항 세력이 붕괴되었기 때문이다. 결론적으로 공영방송사와 방송규제기관의 지배구조에 대한 법적 제도적 개편 작업이 시급하다.

국립세종도서관 직무개발 및 조직개편 전략 연구 (A Study on the Job Development and Reorganization Strategy of the National Library of Korea, Sejong)

  • 2013년 12월 출범한 국립세종도서관은 한국 정책정보 특화도서관과 세종시 복합문화시설로 각광받아 왔다. 그럼에도 본질적 정체성, 조직적 위상, 사무분장과 핵심역량, 정책자료 수집과 서비스 등은 여전히 취약하고 부실하다. 이에 본 연구는 정체성 정립 및 고품질 정책정보서비스의 전국적 제공을 위한 직무모형을 개발하고, 전략적 조직개편 방안을 제시하였다. 직무모형은 기존 72개에 38개를 추가하여 총 110개로 제안하였다. 이를 기반으로 단기 조직개편 모형(2023-2024)은 관장 직급을 현재 4급(서기관)에서 고위공무원단(직무등급은 나급)으로, 과장을 5급(사무관)에서 4급으로 상향 조정하였다. 하부조직은 현재 3과(기획관리과, 정책자료과, 서비스이용과)를 4과(기획행정과, 자료수집개발과, 정책정보서비스과, 대중서비스문화과)로 개칭·확대하고 인력은 총 51명으로 구성하였다. 중장기 모형(2025-2032)은 하부조직을 4과 1실(기획행정과, 자료수집개발과, 정책정보서비스과, 대중서비스문화과, 정책정보연구실)로 확대하고, 소요인력을 총 61명으로 제시하였다. 이를 위해서는 거시적·전략적 목표인 '한국 정책정보센터'를 표방한다는 논지로 이해집단을 설득시키고, 전략적 조직개편 및 직제개정에 모든 역량을 집중해야 한다.

한국노인과 미국이민 한국노인의 건강상태지각과 건강증진행위 (A Survey on Perceived Health Status and Health Promoting Lifestyle Patterns between Korean Elderly and Korean-American Elderly)

  • This study was designed to explore and compare health-promoting behaviors and perceived health status between Korean elderly and Korean-American elderly. One hundred fifty Korean elderly and one ten Korean-American elderly recruited from senior centers respectively. Collected data were analyzed using SPSS program through which with a structured questionnaire. T-test, ANOVA, and Pearson correlation coefficients were tested. The results were followings : 1. In religion, 32.7% of Korean elderly were protestant. while 61.8% of Korean-American elderly were protestant. 61.3% of Korean elderly were living with their children, and only 17.3% of Korean-American elderly were living with their children. 2. Perceived health status of Korean elderly was 3.08 and Korean-American elderly was 3.01, there was no significant difference in those two groups. 3. The mean HPLP score of Korean elderly was 2.63, showed significant relationships with age, economic status and education, while Korean-American elderly was 2.54, showed significant relationships with education and economic status. 4. There was no significant difference in the mean HPLP score in two groups, but Korean elderly showed higher practices in health responsibility, exercise, and stress management than that of Korean-American elderly. Both two groups showed highest practices in nutrition(3.14, 3.01). and the lowest practices in exercise(2.14, 1.92). 5. The HPLP score of Korean and Korean-American elderly showed(r=.24, r=.20) positive correlations with perceived health status. To draw concrete resolution for health promotion of Korean-American elderly, this study suggests followings for future research: 1. Developing health promotion programs focused on exercise and stress management is also imperatively suggested not only for better health practices of Korean immigrant elderly population but also for enhancing their level of well-beings and life satisfaction. 2. Identifying the influences of culture on their practices of health-promoting lifestyle patterns among Korean, Korean-American and other racial elderly groups.

일부 여자대학교 학생들의 영양지식, 태도, 식행동 및 식품섭취상태와 그 관련요인에 관한 연구 - 1학년, 4학년 중심으로 - (A Study on the Nutritional Knowledge, Nutritional Attitude, Eating Practice and Food Intake of Womans University Student)

  • This study was conducted to analyze the relations among nutritional knowledge, nutrional attitude, eating practice and total nutritional diagnosis and also to find the influencing factors of them. The survey was done during the period from April 1990 to June 1990 and the target population were Ewha Womans University students (freshmen and senior) of whom 1,354 peoples were surveyed. The summarized results are as follows ; 1. For the teaming environment, the percentage of completion on nutritional knowledge course was 28.6% and among them the department of foods and nutrition had the highest score(100.0%) and the next was dept. of medicine (53.4%). 2. To find the relation which the nutritional knowledge, nutritional attitude, eating practice and the total nutritional diagnosis influence on one another, the multiple correlation analysis was done. The attitude and the practice were highly correlated with total nutritional diagnosis, but the correlation between nutritional knowledge and total nutrional diagnosis was not significant. The correlation between the practice and nutritional knowledge which were positively and significantly correlated with attitude respectively was positive but not significant. 3. The difference which were analyzed by department, grade and completion on knowledge course on the nutritional knowledge, attitude, eating practice were significant (p<0.01) by ANOVA. 4. The significant variables on nutritional diagnosis are the eating practice points, the grade, the knowledge course, the breakfast and the attitude ($r^2=10.3%$) by multiple regression analysis. This study has the limitation that it did not consider the environmental factors of dietary life such as dietary culture, family environment, cultural habit. Therefore important points of these dietary studies are to extend to the practical nutritional education and dietary improvement for the national health not restricted to the local area or local population.

편의성 및 환경 특성이 뉴시니어의 음식점 선택에 미치는 영향 (The Affects of Convenience and Atmosphere Attributes on New Seniors' Restaurant Selection)

  • The purpose of this research was to study the affects of atmosphere and convenience attributes on restaurant selection of customers aged between 50 to 60, referred to as new seniors. A survey was conducted with questions including convenience and atmosphere attributes (5 factors, 24 questions), respondents' dining-out behavior (6 questions), and general characteristics. For the analysis, SPSS 20.0 was used for the frequency analysis, factor analysis and ANOVA. The findings of this research are as follows : First, new seniors considered cleanliness (4.23) as the most important factor when choosing a restaurant. Convenient facility (3.89), ease of use (3.68), atmosphere (3.33) and children amenities (3.18) followed. Second, the importance of factors were compared according to the general characteristics. Based on gender, cleanliness and convenient facility showed a significant difference (p<0.05). Depending on whether they help raise their grandchildren and household type, the difference of ease of use and children amenities showed significance (p<0.05). Third, the importance of factors were compared based on their dining-out behavior. According to the average visits made to restaurants and the average payment per person, the difference of cleanliness and ease of use factors showed a significant difference (p<0.05). In addition, there were significant differences (p<0.05) on convenient facility and atmosphere factor depending on the average payment per person. The results of this research hopes to provide useful information to marketers targeting new seniors.
