국내 만성질환 노인환자의 한의과 진료서비스 이용과 사망률 및 파킨슨병 발생률의 관계연구 (The Relation of Korean Medicine Services Use on the All-cause Mortality and Incidence of Parkinson's disease and Elderly Patients with Chronic Disease in Korea)

  • Objectives : To investigate the effectiveness of Korean Medicine services use on the all-cause mortality and incidence of Parkinson's disease(PD) in elderly patients with chronic disease based on the National Health Insurance Service Corhort Database (elderly), called as the NHIS-senior. Methods : This study was a retrospective cohort analysis conducted using the NHIS-senior. Patients with chronic diseases over 65 years old who were not diagnosed PD during 2007-2009 were identified. The case group was defined as patients who used both Korean Medicine and Western Medicine services and the control group consisted of patients who used Western Medicine service only. The all-cause mortality and incidence of PD was analyzed using the Cox proportional hazard model after a propensity score matching(PSM) with a 1:1 ratio. Results : After PSM, the cohort included 47,546 subjects (23,773 in the case group, 23,773 in the control group). Sex, age, comorbidity, severity of disability, and neurology medical service utilization were adjusted in both groups. The mortality was 0.668 times (95% C.I.: 0.646-0.690) lower in the case group than the control group, which was statistically significant (p<0.001). The incidence of PD was 1.051 times (95% C.I.: 0.962-1.148) higher in the case group than the control group, which was not statistically significant(p=0.272). Conclusion : It was not obvious that the use of both Korean Medicine service and Western Medicine services for prevention of PD is benefitial than using only Western Medicine. But it would be possible that using both Korean Medicine and Western Medicine services decreases the mortality than using Western Medicine alone.

일개 경로당 이용 노인의 웃음치료 참여 경험 (An experience in the laughter therapy by elderly of senior citizen's center)

  • 본 연구는 일개 경로당 이용 노인의 웃음치료 경험의 본질과 의미를 이해하기 위하여 밴 매넌(Van Mannen)의 해석학적 현상학적 연구방법을 적용하였다. 연구참여자는 경로당에서 웃음치료를 경험했으며 연구참여에 동의한 총 12명(남 4명, 여 8명)이며, 연구참여자와의 면담과 문학 및 예술작품 등을 참조하여 자료를 수집하였다. 노인들의 웃음치료 경험에 대한 자료 분석을 통하여 노인들이 지향하는 삶은 건강한 삶(몸의 기운을 회복함, 기분이 맑아짐), 행복한 삶(스트레스를 해소함, 잡념을 잊게 함, 일상의 활력소가 생김, 함께 웃어서 배가되는 즐거움, 추억을 되살리며 젊어짐), 희망하는 삶(고통을 잊고 긍정적 마음을 가짐, 기대하는 일이 생김)의 구조로 의미화 됨을 알 수 있었다. 건강한 삶은 신체, 심리적으로 건강행위를 추구하는 삶의 근본적인 원동력에 대한 체험이고 행복한 삶은 타인과의 관계형성과 공감을 통해 기쁨을 창조하는 삶이며 마지막으로 희망하는 삶은 다양한 상실의 고통을 극복하고 삶을 긍정적으로 통합하는 체험이다. 결론적으로 노인들에게 웃음치료는 노인들의 건강, 대인관계 및 삶을 통합하도록 하였다. 따라서 노인들에게 웃음치료를 활용할 수 있다면 노인들의 독립심과 자존감을 향상하는데 도움이 될 것이다.

의료기관 환자안전과 환자중심성 간 관계 연구 (A Study on the Relationship between Patient Safety and Patient-Centeredness in Hospitals)

  • Purposes: This study purposed to analyze the relationship between patient safety and patient-centerendess. Methodology: The comprehensive scores from patient safety assessment program and patient experience survey conducted by Health Insurance Review & Assessment Service were used as independent variables and dependent variables. This study analyzed the relationship between 4 patient safety-related areas(i.e. risk standardized readmission ratio, intensive care unit, preventive antibiotic, the drug evaluation) and 6 patient experience areas(i.e. nurse services, doctor services, medication & treatment, hospital environment, patient's right, overall experience) by using robust regression analysis. Findings: According to results, the score in 'patient's right' and 'risk standardized readmission ratio' areas were found to have a significant relationship, and 'overall experience' and the 'preventive antibiotic' areas. The ratio of senior beds and specialists was a general characteristics of hospitals that had a significant relationship on patient experience assessment. Practical Implication: The relationships between patient safety and patient experience assessment were varied depending on areas. Further study is needed to make clear the supposed relationship.

지역사회 노인 고혈압 환자의 건강정보이해능력이 고혈압 자가관리행위에 미치는 영향 (The impact of health literacy on self-care behaviors among hypertensive elderly)

  • Objectives: This study thus set out to examine the health literacy and self-care behaviors of community-dwelling elderly patients with hypertension and investigate their relations and influential factors, thus providing basic data to develop a nursing intervention program to promote self-care behaviors among hypertension patients. Methods: The subjects include 180 hypertension patients using community health centers, public senior centers, and welfare centers in Jeju. Results: The subjects scored a mean of 49.8 (${\pm}12.4$) points on health literacy and 55.8 (${\pm}6.7$) points on self-care behaviors. In the results of hierarchical regression analysis, health literacy independently predicted self-care behaviors (${\beta}=.12$, p=.050) after controlling for general characteristics, hypertension-related knowledge and self-efficacy. Conclusions: These results suggest that interventions for improving health literacy are important to enhance elder's ability of self-care behaviors. It is necessary to understand the health literacy of elderly patients with hypertension before providing them with educational intervention and information and to develop appropriate educational materials and intervention programs.

  • This study delves into the unique transformation of South Korea's elderly population, distinctively termed 'Nomad Silver'. Characterized by individuals aged 65 and above who actively seek novel experiences and embrace new activities, this demographic shift signifies a departure from traditional perceptions of the elderly. The Nomad Silver cohort, distinguished by their significant economic influence and evolving needs, necessitates a tailored approach to healthcare services. This paper underscores the importance of comprehending both the fundamental biological needs and the personalized desires of the Nomad Silver, aiming to enhance their satisfaction and overall well-being. Hospitals, in response, should innovate their services to resonate with the emotional, psychological, and social facets of this age group. Consequently, the paper proposes a four-pronged strategy for hospitals to adapt: comprehensive healthcare provision, patient-centric service development, senior health education coupled with community engagement, and establishing a generational bridge hub. Furthermore, the paper posits that catering to the Nomad Silver not only promises substantial financial gains for hospitals but also fosters new business opportunities across various sectors.

농촌 지역 여성독거노인의 건강증진행위와 삶의 만족도 (Health Promotion Behaviors of Rural Elderly Women Living Alone and Their Life Satisfaction)

  • Purpose: This study was conducted to investigate the correlation between degrees of health promotion behaviors and life satisfaction and effects of health promotion behaviors on life satisfaction in rural elderly women living alone. Methods: A descriptive correlation study was conducted with 189 rural elderly women living alone aged 65 or older in four senior counties in Jeollanam-do. Descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation, and multiple regression. Results: The subjects' health promotion behaviors and life satisfaction were significantly positive. Among the factors influencing the subjects' life satisfaction, nutrition and diet showed the greatest positive effects, followed by exercise and activity, and drinking and smoking. Among them, drinking and smoking had significantly negative influence. Conclusion: Among the health promotion behaviors that influenced life satisfaction, nutrition and diet, exercise and activity, and drinking and smoking were most significant factors. Therefore, this study provided basic data for improving the life satisfaction among rural elderly women living alone.

지역사회 노인의 지각된 스트레스가 인지기능에 미치는 영향 (The Effects of Perceived Stress on Cognition in the Community Elderly)

  • Purpose: The Purpose of this study was to investigate the effects of perceived stress on cognition in the community elderly. Methods: This was a descriptive study. Data were collected using individual-based interviews from 40 senior residents at the hall for the elderly in S City. The period of data collection was from June 19 to July 7, 2006. The tools of data collection were Mini-Mental State Examination (Folstein, Folstein, & McHugh, 1975) and Perceived Stress Scale (Cohen, Kamarck, & Mermelstein, 1983). Results: Cognition showed significant differences according to gender education, and regular exercise. Cognition was significantly correlated to stress and age. Stress was significantly correlated to orientation, recall, and attention/calculation. In the results of stepwise multiple regression, factors affecting cognition were stress, age, and gender. Conclusion: It is necessary to prepare health promotion programs that can reduce stress level in the community elderly.

  • This study is intended to provide a basis for the planning of local governments to, analyzed in Gwangju Metropolitan City area D sixth regional health plans and Tertiary regional social welfare plans, establishing phase, the evaluation phase side. Based on the study results suggest the following implications. First, Specifically, it should be established to systematically planning process. Second, Plan's vision, key challenges, businesses are logical and details should be provided consistently. Third, It should be evaluated focusing on residents of the area change. Fourth, Change of plan indicators, performance indicators, etc. must be presented in detail.

노인의 우울 영향 요인 (Factors Influencing Depression of the Elderly)

  • Purpose: This study was to examine factors related to depression of the elderly, and using them, to lay the ground for the development of nursing intervention. Methods: This was a descriptive correlation study. The subjects were 581 elders aged over 65 from senior centers in Seoul and Gyeonggi-do. Data were analyzed by descriptive statistics, Pearson's correlation coefficients, and multiple regression with SAS. Results: The predictors of depression in the elderly were quality of life, moral, life satisfaction, self-esteem, loneliness, nonformal support, and perceived health state. Among them, quality of life was the most significant predictor in the elderly. These factors explained 72.2% of the total variance. Conclusion: These findings suggest the need to develop nursing strategies for decreasing depression in the elderly. To decrease the depression of the elderly, the above-mentioned major influencing factors should be considered.

가족생활주기에 따른 외식업소의 품질 요소별 중요성 차이 (Korean Family’s Perception of the Importance of the Quality of Restaurants according to Family Life Cycle)

  • The purpose of this study was to examine the customer’s perception of the importance of restaurant quality by using a developed SERVQUAL model. In particular, it was intended to provide the basic information for nutrition education for family’s eating out. the data were collected through the survey over 440 families who have lived around the apartments in Kyongju and Seoul. The structure questionnaire included 35 items evaluating the quality of restaurants, adapting SERVQUAL model and adding other factors related to restaurant and the general characteristics of the families. The data were analyzed using SPSS 10.0 WIN program for descriptive analysis, a chi-square test and Pearson’s correlation test. The major results were as follows: From the factor analysis, the 35 items related to the quality of restaurant have been categorized into 10 factors, ie., ‘Health’, ‘Reliability’, ‘Employee’, ‘Food’, ‘Facilities’, ‘Product’, ‘Empathy’, ‘Access’, ‘Comfort’, and ‘Safety’. From the analysis of the correlation between family life cycle of four steps and 10 factors, four factors such as ‘Health’, ‘Facilities’ (p<0.001), ‘Comfort’ (p<0.01) and ‘Product’ (p<0.05) showed statistical significance. ‘Health’ and ‘Product’ factors marked the highest score in the family with senior couple live without children or single, but the lowest score in the family with young couple without children or single. ‘Facilities’ and ‘Comfort’ factors showed the highest scores in the family with under elementary school children, and the lowest scores in the family with over elementary school children.
