반조절성교합기와 구강에서의 운동 오차에 대한 비교 연구 (A study on the difference of movement between Semi adjustable articulator and Oral in vivo)

  • The movement range on the semi adjustable articulator and the movement range in an oral were measured. And then I studied to analyze the gap. I got wax records by the movement on the semi adjustable articulator, the movement in an oral. I measured the distance of the cusp tips that are close to the mesial direction and the distal direction, the buccal direction and the lingual direction then I compared gaps. As I saw results on data, I knew that the semi adjustable articulator represented the range of mandibular movement restrictively. I could find the decisive contradiction that the sliding movement finished on the semi adjustable articulator although it did not finish in an oral. When the sliding movement does not reappear exactly, it brings a fatal failure to the dental prosthesis. In addition it is impossible that the semi adjustable articulator restores the movement in an oral because the lateral condyle inclination and the horizontal condyle inclination are appeared to be straight. Therefore dental prosthesisses were made by the semi adjustable articulator, they will interfere with a mastication. I have obtained the following results; 1. The distance of sliding movement on the semi adjustable articulator showed shorter than the distance of sliding movement in oral. This means the increase of cusp inclination of the dental prosthesis that was made on the semi adjustable articulator. Therefore, when the lateral movement occurs in oral, there is a possibility to become the premature as the increase of cusp inclination. 2. The difference in the range of movement is considered as the gap that is made because the movement only can be occurred as straight in the semi adjustable articulator. 3. When the dental technician understand mandibular movement and articulator deficiencies, they can attain proficiency in use of the articulator and reduce the gap.

반조절성교합기와 Axiograph를 이용한 전방시상과로각 차이에 관한 연구 (A STUDY ON THE DIFFERENCE OF THE SAGITTAL CONDYLAR GUIDANCE BY SEMI-ADJUSTABLE ARTICULATOR AND AXIOGRAPH)

  • Statement of problem: In the Protar articulator, the models are almost parallel with the Camper's plane. The ultrasonic-based ARCUSdigma system is basis for the determination of dynamic function parameters with so-called "articulator related registration". Purpose: The purpose of this study was to compare the sagittal condylar guidance angles found by use of the wax protrusive records in a semi-adjustable articulator(KaVo Protar 7) with those found by use of the Axiograph (ARCUSdigma). 83 volunteers with intact dentition participated in this study after obtainment of informed consent. Material and method: The sagittal condylar guidance angles were measured and estimated statistically by semi-articulator and Axiograph. All the readings were in degrees. No control was used in this project. To test whether there was a significant difference between the 2 independent samples, paired t-test and Kruskal-Wallis test were carried out(p=.05). Results: 1. The mean results for the wax protrusive records were as follow: right side (32.65 degrees, SD 16.48); left side (33.27 degrees, SD 17.49). 2. The mean results for the Axiograph were as follow: right side (32.26 degrees, SD 7.00); left side (33.07 degrees, SD 7.58). 3. There was no statistical difference on the wax protrusive records and Axiograph(p>0.05). Conclusion: Both methods of wax protrusive records and Axiograph are clinically acceptable for measuring the sagittal condylar guidance angles in semi-adjustable articulators.

A comparative study to measure the condylar guidance by the radiographic and clinical methods

  • PURPOSE. The study was conducted to compare the radiographic and clinical methods of measuring the horizontal condylar guidance (HCG) values. MATERIALS AND METHODS. The condylar guidance was measured using the radiographic (CT scan) and three clinical methods i.e. the wax protrusive records, Lucia jig record and intraoral central bearing device in 12 patients aged between 20-40 years irrespective of sex. The records were taken and transferred on the semi-adjustable articulator to record the HCG values. The CT scan was taken for 3D reconstruction of the mid facial region. Frankfort horizontal plane (FHP) and a line extending from the superior anterior most point on the glenoid fossa to the most convex point on the apex of articular eminence (AE) was marked on the CT scan. An angle between these two lines was measured on both right and left sides to obtain condylar inclination angle. Three interocclusal protrusive wax and jig records were taken and transferred to the semi adjustable articulator. Three readings were recorded on each side. Similarly the records were taken and transferred to the same articulator using the intra oral central bearing device to record the readings. RESULTS. The statistical analysis showed insignificant differences in the HCG values between the right and left sides [(P=.589 (CT), P=.928 (wax), P=.625 (jig), P=.886 (tracer)]. The clinical methods provided low Pearsons correlation values [(R = 0.423 (wax), R = 0.354 (jig), R = 0.265 (tracer)] for the right as well as the left sides when compared with the CT values. Among the clinical methods, jig and wax method showed strong level of association which is statistically significant while the intra-oral tracer showed weak association with the other two methods. CONCLUSION. The right and left HCG values were almost similar. The CT scan showed higher HCG values than the clinical methods and among the clinical methods, values obtained from all the methods were comparable.

A comparative study to measure the sagittal condylar inclination using mechanical articulator, virtual articulator and jaw tracking device

  • PURPOSE. To compare the sagittal condylar inclination (SCI) in dentate individuals measured by the different methods with mechanical articulator (MA), virtual articulator (VA), and a jaw tracking device (JTD) system. MATERIALS AND METHODS. A total of 22 healthy dentate participants were enrolled in this study. For MA workflow, the SCI was obtained by a semi-adjustable articulator with protrusive interocclusal records. The SCI was also set on a VA by aligning intraoral scan (IOS) with cone beam computed tomography (CBCT) and facial scan (FS), respectively. These virtual workflows were conducted in a dental design software, namely VAIOS-CBCT and VAIOS-FS. Meanwhile, a JTD system was also utilized to perform the measurement. Intraclass correlation was used to assess the repeatability within workflows. The bilateral SCI values were compared by Wilcoxon matched-pairs signed rank test for each workflow, and Kruskal-Wallis test and post hoc p-value Bonferroni correction were used to compare the differences among four workflows. The agreement of VAIOS-CBCT, VAIOS-FS, and JTD compared with MA was evaluated by Bland-Altman analysis. RESULTS. Intraclass correlation of the SCI revealed a high degree of repeatability for each workflow. There were no significant differences between the left and right sides (P > .05), except for VAIOS-CBCT (P = .028). Significant differences were not found between MA and VAIOS-FS (P > .05). Bland-Altman plots indicated VAIOS-CBCT, VAIOS-FS, and JTD were considered to substitute MA with high 95% limits of agreement. CONCLUSION. The workflow of VAIOS-FS provided an alternative approach to measure the SCI compared with MA.

Occlusal rehabilitation of posterior fixed prostheses: A clinical report

  • Masticatory function is more important than esthetics in posterior fixed restoration. The usual technique-mounting the casts on a semi-adjustable articulator, etc. -cannot make all patients satisfied with their restorations. For example, functionally generated path technique can be an easier and more satisfactory method in the restoration of group function. These clinical reports describe various approaches for occlusal restoration of relatively simple posterior fixed protheses according to patients occlusal patterns. The 3-unit bridge restoration is one of the most popular treatment options in prosthodontics. Because dentists have much experiences of it, they restore a missing span of one tooth mechanically, that is, without special consideration. While esthetics is important in making an anterior 3-unit fixed prostheses, mastication is more focused on in posterior 3-unit bridge restoration. Many dentists are concerned about various aspects in esthetics, such as morphology of the tooth, value, chroma, hue, translucency, surface texture, etc. But they do not usually consider various methods to restore occlusion. They treat one-tooth-missing area in a similar way in spite of patients having variety of occlusal patterns. Three cases are presented here in 3 or 4-unit bridge restoration. They show some methods to restore patients' occlusal patterns.

  • The purpose of this study is to evaluate suitable anterior reference point similar to esthetic reference plane of the patient while transferring the suitable facebow. So those who live in Pusan with the normal occlusal relationship were chosen as subjects. The esthetic reference planes determined by the participants themselves and the superior reference line (anterior reference point) in Hanau & Denar articulator were compared with F-H plane in the standard lateral cephalogram. The results were as follows. 1. The angle between the esthetic reference plane and F-H plane was $2.81{\pm}5.7^{\circ}$. 2. The reference line, 43mm from Beyron point to the edge of the upper incisor, was the reference plane that had the closest paralleled relationship with the esthetic reference plane. 3. Transferring with ear piece type face-bow, the anterior reference point that showed the esthetic reference plane was 17mm below orbitale or 43mm above, the edge of the upper incisor.

Recording maximal intercuspation and border positions of the mandible with intraoral scanner using the acquisition software's multi-occlusion function

  • PURPOSE. This in vitro study was conducted to investigate the accuracy of intraoral scanner (IOS) for recording maximal intercuspal position (MIP) and border positions of the mandible. MATERIALS AND METHODS. Maxillary and mandibular master casts were articulated in MIP, protrusive, and lateral interocclusal position sequentially on a semi-adjustable articulator. For each articulation relation, sites of occlusal contacts (SOCs) and sites of clearance (SCs) were identified on the master casts with articulating paper (reference sites). IOS was used to take full arch scans and nine virtual interocclusal records (VIRs) for virtual articulation of models. Virtual SOCs and SCs were detected with 3D processing software and compared to those identified with the articulating paper. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for each articulation relation. RESULTS. For MIP, IOS showed adequate sensitivity and NPV of 100%, and specificity and a PPV of 99%. For protrusive position, the IOS showed a sensitivity and a NPV of 100%, a high PPV of 86%, and a specificity of 83%. For lateral positions, the specificity and the PPV were high (93% and 79%, respectively), but the sensitivity and the NPV were below the clinically acceptable limits (28% and 56%, respectively). CONCLUSION. IOS displayed clinically acceptable accuracy for recording MIP and protrusive border mandibular position. However, IOS had less accuracy for lateral border mandibular position.

CBCT 기반 디지털 안궁이전과 아날로그 안궁이전의 위치 편차: 증례보고 (Positional deviation between CBCT-based digital facebow transfer and analog facebow transfer: case series)

  • 조절성 교합기에 모형을 부착하기 위하여 안궁이전이 필요하다. 아날로그 안궁이전에서는 장비의 정확도와 작업자의 숙련도가 모형 부착 결과에 영향을 미칠 가능성이 있다. 더불어 편차가 큰 방법으로 부착된 작업 모형에서는 정확한 치과 보철물의 제작이 어려우므로, 아날로그 안궁이전으로 부착된 작업 모형의 위치 편차를 파악하는 것이 중요하다. 본 증례에서는 아날로그 안궁이전으로 부착된 상악 모형의 위치와 cone beam computed tomography 데이터를 기반으로 위치된 상악 모형의 위치를 거리 편차와 교합 평면의 각도 편차를 평가하였다. 이를 토대로 아날로그 안궁이전으로 부착된 상악 모형의 편차를 보고하였다. 아날로그 안궁이전 방법은 3 - 16 mm의 선형 편차와 5 - 7도의 교합평면 각도 편차를 가지는 상악 모형의 부착 결과를 나타내었다. 아날로그 안궁이전은 환자별로 위치 편차가 다를 수 있음을 확인했으며, 치과 보철물 제작에서의 부정확성을 초래할 가능성이 있다. 이러한 결과는 임상에서 아날로그 안궁이전 방법이 상악 모형을 부착하는 과정에서 큰 편차가 있을 수 있음을 나타내었기에 이를 보고하는 바이다.

Comparison of condylar displacement between three biotypological facial groups by using mounted models and a mandibular position indicator

  • Objective: Facial-type-associated variations in diagnostic features have several implications in orthodontics. For example, in hyperdivergent craniofacial types, growth imbalances are compensated by displacement of the condyle. When diagnosis and treatment planning involves centric relation (CR), detailed knowledge of the condylar position is desirable. The present study aimed to measure condylar displacement (CD) between CR and maximum intercuspation in three facial types of an asymptomatic orthodontic population. Methods: The study was conducted in 108 patients classified into three groups of 36 individuals each (27 women and 9 men; mean age, 20.5 years), based on the following facial patterns: hyperdivergent, hypodivergent, and intermediate. To quantify CD along the horizontal and vertical axes, the condylar position was analyzed using mounted casts on a semi-adjustable articulator and a mandibular position indicator. The Student t-test was used to compare CD between the groups. Results: Vertical displacement was found to be significantly different between the hyperdivergent and hypodivergent groups (p < 0.0002) and between the hyperdivergent and intermediate groups (p < 0.0006). The differences in horizontal displacement were not significant between the groups. In each group, vertical CD was more evident than horizontal displacement was. Conclusions: All facial types, especially the hyperdivergent type, carried a significantly high risk of CD. Therefore, the possibility of CD should be carefully evaluated and considered in the assessment of all orthodontic cases in order to accurately assess jaw relationships and avoid possible misdiagnosis.
