Purpose: The purpose of this study was to investigate the level of self management of patients with poorly controlled type 2 diabetes, and to investigate the factors influencing self management. Methods: The subjects consisted of 117 diabetes patients who visited the outpatient department of a university hospital from March to August 2008. Data were collected by asking the subjects to answer a 54-item questionnaire and were analyzed using the SPSS/WIN 14.0 program. Results: The total mean score of the patients in self -management was 4.38 out of 7. Medication adherence obtained the highest score under self-management, and the self-monitoring of blood glucose obtained the lowest score. Family support, self-efficacy, severity, and depression were found to be significantly correlated with self-management. In stepwise multiple regression analysis a total of 44.5% of the variance in self management was accounted for by family support, self-efficacy, severity, and depression. Conclusion: Therefore, a diabetic intervention program should be designed and provided for increasing family support, self efficacy, and severity and for reducing depression of patients with poorly controlled type 2 diabetes.
Propose. An effective rehabilitation program had been developed for psychiatric patients' self management of medication and symptoms in Korea. The rehabilitation program was designed to allow the patients to understand their illness, cope with their medical regimen, and prevent a relapse by recognizing any of the symptoms when they recur. Methods. The developed program utilizes the self efficacy method reported by Bandura, it includes manuals and videotapes focusing on real life situations, small group discussions, and telephone coaching. This study investigated the effects of this program with respect to various predictable variables in psychiatric rehabilitation. Thirty eight patients were selected for this study, 18 in the experimental program and 20 as controls. Results. The results showed that the subjects who attended this educational program reported significantly more improvement in attitude toward medication compliance (p=0.033), and significantly less relapse warning symptom scores (p=0.000) than the controls. Conclusion. This program may be a useful psychoeducational resource for professionals in the field of clinical practice in psychiatry.

Purpose: This study was conducted to determine the effects of Tai-Chi exercise and self-help management program applying laughter therapy in patients with osteoarthritis. Methods: This quasi-experimental study recruited a sample of 61 patients with osteoarthritis (experimental group: 30, control group: 31) through the community healthcare centers located in Seoul and Kyung Gi area. The primary outcome measures included symptoms of osteoarthritis, basic physical performance, fatigue, and depression assessed before and after the study. The data were analyzed using SPSSWIN V. 12.0. Results: At the end of the six-week intervention, left shoulder flexibility (t=2.011, p=.049), 6m walking speed (t=3.639, p=.001), and right balance (t=-2.30, p=.025) were significantly improved in the control group. Fatigue (t=3.012, p=.004), stiffness (t=2,093, p=.041), right shoulder flexibility (t=2.138, p=.037), right balance (t=-2.065, p=.043), and 6m walking speed (t=2.683, p=.009) were significantly improved in the experimental group. Conclusion: A twice a week, 6-week tai-chi exercise self-help management program applying laughter therapy is effective in decreasing fatigue and stiffness. Osteoarthritis is one of common chronic diseases that the patients should be required to continue self management for the rest of their life. Therefore, there is a need to develop the more effective self-help management program and nursing intervention to motivate them to maintain their own self-help management.
The purpose of this study was to evaluate the effects of introductory clinical practice education program for nursing students. The nursing students of 94 participated the introductory education program of 80 hours. The questionnaire survey on self efficacy and clinical competency were conducted 3 times(before education, after education, 2 months after education) to the nursing students. The data were analyzed by the SPSS 17.0 program. The results were as follows; The self efficacy and the clinical competency score showed a significant change in the posttest. Self efficacy and clinical competency have significant relationships with college life satisfaction, interpersonal relationship and academic aptitude satisfaction. Also, self efficacy has significant relationship with clinical competency and clinical practice satisfaction. As a result, the introductory clinical practice education program can be helpful to adjustment for nursing students before clinical practice.
Purpose: To examine the effects of a case management program on self-efficacy, depression and anxiety in pregnant women with gestational diabetes mellitus. Methods: Thirty-seven patients who enrolled in a diabetes outpatient clinic were randomly assigned to either an experimental group (n=19) or a control group (n=18). The experimental group received a 2-week intervention composed of a series of one face-to-face interactive interview and five telephone interviews based on the National standards for diabetes self-management education and Bandura's self-efficacy resources of performance accomplishment, vicarious experience, verbal persuasion, and emotional arousal. The effects of the intervention were assessed by completion of a 9-item self-efficacy questionnaire, a 20-item depression questionnaire and a 20-item anxiety questionnaire prior to, and after the intervention. The statistical significances were examined using t-test. Results: The change in self-efficacy was significantly greater in the experimental group than in the control group. Depression and anxiety changes in the experimental group were significantly greater compared to those in the control group. Conclusion: The results of study suggest that a case management program improves self-efficacy, and decreases depression and anxiety in pregnant women with gestational diabetes mellitus.
In Korea, the institutional reform for the national medical insurance system is in process. Eventually, three kinds of the national medical insurance system, i.e., medical insurance program for the industrial workers, the govemment employees and the private school teachers, and the self-employeds, would be merged into an unifed system. In this study, I analyzed the annual trends of the finance in the medical insurance system in Korea, in which I found the financial instability especially in the medical insurance program for the self-employeds. The regression analysis was carried out to forecast the accumulated reserve at the end of this year for the medical insurance program for the self-employeds. I also analyzed the economic effect of the merge of the medical insurance program for the self-employeds by using the case of Japan and Korea. I found that the medical insurance for the self-employeds is expected to have financial deficit at the end of the year 1998 after the merge. In onclusion, it seems to be quite difficult to solve the financial instability in the medical insurance program for the self-employeds after it would be merged. That means that there would be a lot of problems on the way to the merge.
Purpose: This study aimed to develop a mobile-based self-management health alarm (MSHA) program for modifying obese children's lifestyle based on the information-motivation-behavioral skills (IMB) model and to test its feasibility. Methods: A methodological study for the development of the MSHA program and pilot study with a one-group pretest-posttest design for feasibility testing was conducted. The MSHA program was designed to provide obesity-related information (I), monitor daily diet and exercise, provide motivational text messages (M), and enhance healthy diet and exercise skills (B) via a mobile-based web platform. In the feasibility test, six obese children participated in the 4-week program, and the number of days per week that they achieved their goals and differences in metabolic components were assessed. Data were analyzed using descriptive statistics and the Wilcoxon signed-rank test. Results: Participants successfully achieved their diet and exercise goals≥5 days per week. Body mass index (z=-1.99, p=.046), waist circumference (z=-2.20, p=.028), and triglyceride levels (z=-2.21, p=.027) significantly decreased. Conclusion: The MSHA program showed positive effects on health behaviors and metabolic syndrome risk. The program may be effective in improving metabolic syndrome in obese children by promoting self-health management behaviors.
Purpose: The objective of this study was to evaluate the effects of a urinary incontinence management program for middle aged women living in a rural area. Method: The research design was a one-group pretest-posttest design. Forty eight women with urinary incontinence aged 40-64, who lived in a rural area were included in the study. The urinary incontinence management program was composed of urinary incontinence education, pelvic floor muscle exercise, behavior management using a voiding diary. The program had three weekly sessions. Knowledge about urinary incontinence, physical symptoms, self-efficacy, and urinary incontinence related quality of life were measured before and after the program. Results: After carrying the urinary incontinence management program, knowledge score improved significantly compared to before the program ($6.31{\pm}3.30$ vs $7.77{\pm}2.55$, p=.01). But the score for self-efficacy, and urinary incontinence related quality of life did not improve. Conclusion: The 3 week urinary incontinence program had an effect on knowledge related to urinary incontinence, but not self efficacy nor urinary incontinence related quality of life. Thus further study is required to better evaluate the effects of the urinary incontinence program.

The purpose of this study was to identify levels of activity of daily living, self-efficacy. stroke specific quality of life and need for self-help management program for patients with hemiplegia in the home. Data were collected from June to November, 2000 and subjects were 88 poststroke patients who lived in Seoul and Kyunggi-do. The questionnaire consisted of 5 scales: activities of daily living, self-efficacy, stroke specific qulaity of life and need for a self-help management program. Data were analyzed using frequencies, percent, paired t-test, and Pearson's correlation coefficient with the SAS(version 6.12) program. The results are as follows ; 1) Most of subjects were Partially independent in ADL, but they needed assist once to do dressing, bathing meal preparation and house keeping work. 2) The mean self-efficacy score was 54.89(range : 1 to 80) and the individual differences were large. 3) Subjects responded that they were satisfied on the stroke specific quality of life scale totaled 65.8%. This value is comparatively low, especially for social role(51.4%), family functioning(58.3%) and mood (62.2%). 4) The highest needs for self-help management programs were for physical therapy, stress management, and range of motion exercise and the lowest needs were for elimination management and training, family counseling, and speech therapy. 5) On the demographic variables, sex showed significant differences for the dependent variables. Females had higher scores than males for IADL, self-efficacy, stroke-specific quality of life, and need for self-help management. 6) Age had high negative correlation with ADL, self-efficacy and stroke specific quality of life. Age was also correlated with need for self-help management. In conclusion, there was a high correlation for ADL, Self-efficacy and Quality of life in poststroke patients of home. The patient with a stroke also had a strong need for self-help management programs especially physical therapy and stress management. Therefore rehabilitation programs based on self-efficacy enhancement need to be developed in order to promote independent living for patients with hemiplegia.

Nevertheless, most of literatures on six sigma have mainly focused on technical and tactical aspects. This paper introduces a self-assessment for six sigma initiative in a major Korean software company. The self-assessment gives a valuable clue to how to manage leadership and cultural aspects of six sigma initiative.
